THE EFFECT OF EARLY REMOVAL OF THE FIXATION PLATES AND ACTIVE MOUTH OPENING EXERCISE ON THE TEMPOROMANDIBULAR DYSFUNCTION AFTER MANDIBULAR SETBACK SURGERY (하악골 전돌증 환자에서 하악지 시상분할골절단술후 금속고정판의 조기제거 및 기능운동이 턱관절장애에 미치는 영향)

  • Purpose: The effect of orthognathic surgery on the temporomandibular dysfunction has been controversial. The purpose of this study is to prove statistically that early removal of fixation plate at postoperative 2 weeks with active exercise of mouth opening could relieve preoperative temporomandibular dysfunction and reposition of temporomandibular joint. Patients and Methods: All 28 subject patients underwent mandibular setback with BSSRO in Kyunghee medical center by one surgeon. The fixation plates used for rigid fixation were removed at postoperative 2 weeks and we had the patients excercise active mouth opening with intermaxillary rubber rings for the guiding proper postoperative occlusion. Temporomandibular symptoms were checked and radiographs were taken before surgery, within a month after surgery, six to twelve months after surgery respectively. Results: The temporomandibular dysfunction symptoms were relieved after the surgery and the condyle was displaced inferior-posteriorly immediate after surgery and repositioned toward its original position during follow-up periods. Conculusion: Orthognathic surgery may benefit temporomandibular joint dysfunction by obtaining a postoperative stable occlusion and more physiologic neuromuscular function. The early removal of fixation plates after BSSRO could reposition the temporomandibular joint to physiologic position and relieve the symptoms of temporomandibular dysfunction by permitting movement of proximal segment.

  • This paper presents numerical experiments on migration of synthetic seismograms using by $45^{\circ}$ wave equation. The seismograms used are zero-offset seismogram (corresponding to stacked section) on point reflectors, dipping plane reflector, faulted and folded layers. The seismograms are constructed by upward continuation of seismic source wavelets, exploading on subsurface reflection interfaces, to the earth surface. The synthetic seismograms are migrated by downward continuation and imaging. The upward and downward continuations are implemented by solving the $45^{\circ}$ wave equation with the finite-difference method. Migration of the synthetic data used in this study results in relatively accurate reposition of subsurface structures while the synthetic sections are quite different from the structures.

Effect of Mandibular Reposition on Airway Resistance (하악의 위치 변화가 기도저항에 미치는 영향)

  • This study evaluated whether substantial airflow resistance changes occurred by changing jaw position in normal and snoring subjects. A case-control design was utilized to assess group differences. Subjects included 11 snoring patients and 10 non-snoring subjects. Airway resistance was assessed using a whole body plethysmograph. Subjects in this study had their mouth opening standardized to a position of 7 mm of vertical separation and the resistance was measured under the following conditions; normal jaw position and 2/3 maximum protrusive jaw position. The results were as follows : 1. The airway resistance was higher in snoring group than in non-snoring group. 2. Both groups had a significant decrease in their airflow resistance upon jaw protrusion. In conclusion, these data document that airflow resistance can be significantly influenced by jaw positioning. Moving the jaw in a protrusive position produced reduction of resistance.

  • This paper describes a framework for multimodal object manipulation in virtual environments. The gist of the proposed framework is the semantic integration of multimodal input using spatial ontology and user context to integrate the interpretation results from the inputs into a single one. The spatial ontology, describing the spatial relationships between objects, is used together with the current user context to solve ambiguities coming from the user's commands. These commands are used to reposition the objects in the virtual environments. We discuss how the spatial ontology is defined and used to assist the user to perform object placements in the virtual environment as it will be in the real world.

  • In this machining process, variation in cutting forces results in relative displacements between the tool and the workpiece leading to tool vibration. Also there is a demand to change the depth of cut very frequently. A soluion for both cases is to develop a system which has the ability to reposition a cutting tool to a very small level, i.e., micron. This ppaper presents the development of a micropositioning system (MPS) using a magnetostrictive material. The deveoped MPS is implemented to a lathe and subjected to static and machining test. The results show that the MPS has good potential for machining application

  • A cerebrospinal fluid hydrothorax is a very rare complication following ventriculoperitoneal (VP) shunt and usually reported in children. We report a case of 47-year-old woman who developed massive hydrothorax and respiratory distress following intrathoracic migration of distal shunt catheter. After the confirmation of catheter in thoracic cavity using radionuclide shuntogram, the patient was successfully treated with laparoscopic shunt catheter reposition.

  • Tarsal tunnel syndrome is a complex of symptoms resulting from the compression of the posterior tibial nerve or its branches. Many studies have done on etiologic agents. We reported two cases of tarsal tunnel syndrome secondary to the varicosis of posterior tibial vein. Symptoms were relieved after excision of the varicosis, neurolysis and reposition of posterior tibial nerve.

  • Many advanced nations have been developing the next generation logistic systems to effectively deal with gradually increased logistic flows over the world. Particularly, dual mode trailer(DMT) systems are significantly focused as a new alternative transportation which basically provides integrating of the land and railroad services. This paper presents new driving mechanism for DMT systems using the hydraulic pressure cylinder systems. Such is mechanically composed of supporting and rotating parts independently, which are mostly important for whole system performance. We analyze feasible operational problem occurred in driving mechanism from the conventional DMT systems and compare our proposed deign specification with the Modalohr DMT system to demonstrate its reliability and practicability.

  • The framework of "blue ocean strategy (Kim and Mauborgne, 2005)" applies to not only business firm level but also region or regional industry level. The paper suggests an approach of survey-based blue ocean strategy formulation. The result from survey on customer group and non-customer group gives information on positions of value curves for regions on strategy canvas and hints on how to reposition the focal region's value curve for both increases in value for customers and decreases in cost for local government, i.e., value innovation. In the context of rafting industry in Korea, this study shows the process how blue ocean strategy can be formulated with results from the survey on customers and business people in rafting industry. Relative positions of value curves on strategy canvas and importance scores for strategy elements provides the local government in region A with implications on which elements to raise or to reduce to create "blue ocean" in rafting industry under fierce inter-region competition.

자기변형재료를 이용한 절삭공구용 마이크로포지쇼너의 개발

The results show that the micropositioner with a magnetostrictive actuator has good potential for machining application.
