Takayasu 동맥염에 병발한 관상동맥질환 치험 1예 (Surgical Correction of the Coronary Artery Disease in Takayasu`s Arteritis)

  • Coronary artery involvement and myocardial ischemic symptom in Takayasu`s arteritis is uncommon Its presentation as coronary artery narrowing is a potentially lethal but correctable problem. In this case report, a 17-year-old woman of Takayasu`s arteritis with unstable anginal and moderate heart failure is presented. Her coronary angiogram showed that the main trunk of the left coronary artery was moderately narrowed and the proximal portion of the circumflex branch was severely obstructed and the right coronary artery was also narrowed diffusely. Simultaneously the patient had the moderate degree of mitral regurgitation. In order to save her life, the coronary bypass surgery using the saphenous veins and the modified Wooler`s mitral annuloplasty were done urgently, Immediate recovery was uneventful and the postoperative exercise capacity was markedly improved. But the long-term prognosis seems to be uncertain because of 3 reasons: 1] natural progress and complication of Takayasu`s arthritis; 2] fate of the saphenous vein grafts in a relatively young patient with aortitis; 3] residual mitral regurgitation. So long-term follow-up should be needed.

  • Pectus excavatum is rare, but it is the most common type of sternal congenital disorder. There are many surgical methods to correct pectus excavatum such as the Ravitch method, Wada method, Silastic mold method, and the Nuss operation. We report a case of minimal invasive surgery for pectus excavatum using a polyvinyl alcohol sponge.

Inverted Maxillary Incisor의 치료 (TREATMENT OF INVERTED MAXILLARY INCISOR)

  • Inverted maxillary incisor is that maxillary incisor rotates to the counterclockwise direction. The incisal margin and root apex of the impacted incisor is palpated at the mucobuccal fold near the labial frenum and on the palate among the rugae, respectively. Orthodontists confront ectopically erupting teeth in various locations. In the past, extraction of impacted teeth that deviated from their normal course of eruption had been performed indiscriminately. But, if it has not any clearcut contraindications, effeort should be made to achieve optimal esthetic results by conservative means, combining the skills of oral surgeon and orthodontist. The present report provides an illustration of satisfactory correction of a inverted maxillary incisor with surgical intervention and many springs that correct the tooth axis. This technique provides the clinician with an additional means to avoid unnecessary extraction of inverted teeth in certain cases.

Screening for variations in anterior digastric musculature prior to correction of post-traumatic anterior open bite by injection of botulinum toxin type A: a technical note

  • It has recently been reported that long-standing post-traumatic open bite can be successfully corrected with botulinum toxin type A (BTX-A) injection into the anterior belly of the digastric muscle (ABDM). The report documented an individual with bilaterally symmetrical and otherwise unremarkable anterior digastric musculature. However, the existence of variant anterior digastric musculature is common and may complicate the management of anterior open bite with BTX-A injection. Screening for variant ABDM can be accomplished via ultrasound, computed tomography, and magnetic resonance imaging. Screening for variant ABDM should be performed prior to BTX-A injection in order to account for musculature that may exert undesired forces, such as inferolateral deviation, on the anterior mandible in patients with anterior open bite.

  • Osteomas are benign osteoblastic tumors that occur mainly in the fronto-ethmoid areas ofthe head and neck region. When they occasionally occur in the temporal bone, the external auditory canal is the most common site of origin; they rarely occur in the mastoid region. Moreover, mastoid osteoma with mastoiditis is an extremely rare entity in the temporal bone. Recently, the authors experienced a case of mastoid osteoma with mastoiditis in the left temporal bone. The mastoid osteoma was completely resected itself without a mastoidectomy, only for correction of the cosmetic deformity; the mastoiditis was not treated. Hence, the authors report the first case of a mastoid osteoma with mastoiditis in Korea, along with a review of the related literature.

한국형고속철도 열차제어시스템 하부구성요소 신뢰도예측에 관한 연구 (A Study on Reliability Prediction for Korea High Speed Train Control System)

  • In this paper we study on a method to predict and to demonstrate the reliability of the Korea high speed train control system in quantitative point of view. For the prediction of the reliability in train control system which is composed of electronic parts, Relax Software 7.7 automation tool is employed and MIL-HDBK-217 Handbook that is a standard for the prediction of the failure rate in electronic components is used. Mean Time Between Failure (MTBF) is predicted based on the failure rate of the subsystems, State Modeling and Markov Modeling method is used to express a reliability function of the train control system composed by hardware redundancy as a function of time. We propose a Reliability Test which is performed on the level of the subsystems and Failure Report, Analysing, Correction action system which use the test operation data to prove the predicted reliability.

  • Ebstein기형은 수술적 교정을 위한 여러 가지 변형된 방법들이 소개되고 있지만 수술 후 잔존하는 삼첨판막 역류와 우심부전에 대한 문제는 여전히 남아있다. 본원에서는 심방조동이 동반된 Ebstein기형을 가지고 있는 19세의 남자 환자에 있어서 주름 성형술 없이 간단한 방법으로 삼첨판막 성형술을 시행하였고 양 방향성 상대정맥 -폐동맥 단락술 및 하대정맥 삼첨판 협부의 냉동절제 등을 성공적으로 시행하였기에 문헌고찰과 더불어 증례 보고하는 바이다.

  • 일반적으로 폐질환과 심장 질환이 동시에 발견되었을 때 동시 수술은 술자가 고려해야 되는 사항이 많다. 59세 여자 환자가 우중엽절제술과 이중판막 재치환술과 삼첨판막륜성형술을 동시에 시행 받았다. 동시 수술은 폐와 심장의 병변을 일거에 해소하면서 2차적으로 시행해야 하는 폐절제술 또는 심장수술에 따르는 위험성을 피할 수 있는 장점이 있으며 안전하게 시행할 수 있는 술식이다.

  • Polyotia is an extremely rare type of the auricular malformation that is characterized by a large accessory ear. A 3-year-old girl presented to us with bilateral auricular abnormalities and underwent two-stage corrective operation for polyotia. In this report, we present the surgical details and postoperative outcomes of polyotia correction in the patient. Relevant literature is reviewed.

  • Face mask is an extraoral appliance which used to protract maxilla, and can help in the correction of moderately severe class IlI malocclusions by the anterior displacement of the maxilla and maxillary dentition, and possibly restricting or changing the direction of the growth of the mandible. In three cases the results were followed. 1. Anterior crossbite was corrected 2. Maxilla & maxillary dentition were displaced forwardly. 3. Lingual tipping of the mandibular incisors, and backward & downward rotation of mandible were performed. 4. Acceptable improvement in the class III profile was performed.
