Renal Subcapsular Hematoma after Percutaneous Transfemoral Angiography

  • Vascular complications after percutaneous angiography include hematoma, pseudoaneurysm, arteriovenous fistula, thromboembolism, arterial laceration and infection. Hematomas may occur in the groin, thigh, retroperitoneal, intraperitoneal, or abdominal wall. A 54-year-old female underwent percutaneous transfemoral angiography for the evaluation of cerebral aneurysm. Renal subcapsular hematoma developed 3 hours after the procedure. Renal subcapsular hematoma after percutaneous angiography is very rare. We investigated the possible causes of renal subcapsular hematoma. To avoid this rare complication, we need to perform guide-wire passage carefully from the beginning of the procedure under full visual monitoring.

Establishment of normal reference of radiological morphology of renal artery in mini-pigs by renal angiography

  • Mini-pigs have been widely employed in preclinical studies to explore new therapeutic strategies for diseases of the human urinary system; however, the normal reference of the renal artery has not been clearly investigated in the mini-pig model. Therefore, we aimed to establish a normal reference of the radiological morphology of the renal artery in mini-pigs by renal angiography via catheterization of the carotid artery. The renal angiographies obtained from 15 mini-pigs were evaluated to determine the orifice from the aorta, facing direction, size and the number of branches of renal arteries. Cranio-laterally facing renal arteries with 2 distal branches were mainly observed in the renal artery of mini-pigs. Both sides of the renal artery presented symmetrical sizes; however, the right renal artery orifice from the aorta was located more cranially than the left counterpart. The results of this study will contribute to radiological diagnosis of the renal artery as well as preclinical studies of mini-pigs.

토끼에서 젤폼을 이용한 신동맥 색전술의 혈관조영술 평가 (Angiographic Assessment of Transarterial Embolization of Renal Artery Using Gelfoam in Rabbit)

  • The embolic effect of Gelfoam was investigated for transarterial embolization of renal artery (TAE-RA) in four normal rabbits. The catheter was selectively introduced into the unilateral renal artery under fluoroscopy and the Gelfoam-iohexol mixture was infused through a catheter into the renal arteries of 4 rabbits. The immediate and delayed (8 weeks) embolic effects on the renal arteries was investigated with selective angiography. The Gelfoam-iohexol was visualized under fluorosxopy in four rabbits. Renal arteries were ablated immediately after TAE-RA in four rabbits, however, opacification of renal parenchyma was visualized by injected contrast agent in 3 rabbits at 8 weeks. The Gelfoam-iohexol can be used as a short-term embolic materials for TAE-RA, however it is not adequate for permanent embolization of renal artery.

신혈관성 고혈압의 진단에 있어서 캅토프릴 신스캔의 의의 (Captopril $^{99m}Tc-DTPA$ Renal Scintigraphy in Diagnosis of Renovascular Hypertension)

  • To evaluate the sensitivity and specificity of captopril renal scan for renovascular hypertension, we employed the captopril renal scan in conjunction with renal angiography in 81 patients, 159 kidneys, who were referred to evaluate the cause of hypertension. We defined the renovascular hypertension by the criteria of demonstration of renal artery stenosis by angiography, and improvement or cure of hypertension by revascularization. Visual and quantitative evaluation of $^{99m}Tc-DTPA$ renal scan was peformed pre and post captopril administration. The prevalence rate of renovascular hypertension was 40% in comparing with renal angiography, and 70% in confirmed cases. The causes of renovascular hypertension in 81 patients were Takayasu's arteritis, fibromuscular dysplasia, atherosclerosis, essential hypertension, chronic pyetonephritis etc. The sensitivity and specificity of captopril renal scan in comparing with renal angiography were 80%, 86.5%, respectively and also 84.2%, 72.6% in confirmed cases of renovascular hypertension, respectively. The causes of false negative cases were nonfunctioning kidney due to complete obstruction or long duration of disease in basal scan, segmental branch artery stenosis, unknown causes, and suspicious true negative cases without confirmation. The false positive cases were abdominal aortic stenosis or aneurysm, dehydration, unknown causes, and suspicious true positive cases. We conclude that captopril renal scintigraphy is highly sensitive, reasonably specific diagnostic method and comparable to other techniques very favorably.

양측성 신동맥 협착증에서 발생한 급성 비대상성 심부전과 급성 신손상 (Acute decompensated heart failure and acute kidney injury due to bilateral renal artery stenosis)

  • Atherosclerotic renal artery stenosis (RAS) may result in hypertension, azotemia, and acute pulmonary edema. We report on a renal angioplasty with stent placement for bilateral RAS in a patient with acute decompensated heart failure and acute kidney injury. A 67-year-old female patient was admitted to our hospital with acute shortness of breath and generalized edema. Echocardiography showed left ventricular wall motion abnormality and the follow up electrocardiography showed T wave inversion in the precordial leads. We performed a coronary angiography to differentiate ischemic heart disease from non-cardiac origin for the cause of the heart failure. The coronary angiography showed no significant luminal narrowing, but bilateral RAS was confirmed on the renal artery angiography, therefore, we performed renal artery revascularization. After the procedure, the pulmonary edema was improved and the serum creatinine was decreased. Two weeks later, an echocardiography showed improvement of the left ventricular systolic function.

  • Renal artery pseudoaneurysm after blunt renal trauma is an uncommon complication of delayed hemorrhage, and diagnostic difficulties are experienced due to its rarity. Delayed hemorrhage after renal trauma is a life-threatening complication. Angiography is considered the gold standard to diagnose a traumatic renal artery pseudoaneurysm. We report here a case of delayed bleeding from a renal artery pseudoaneurysm that was diagnosed at 17 days after the injury and that was managed successfully with selective renal artery embolization without medical complication.

  • Polyarteritis nodosa (PAN) is a systemic necrotizing vasculitis predominantly involving medium- or small-sized arteries, typically of the kidneys and other internal organs. Given the rarity of PAN and the variable clinical presentation, diagnosis is challenging and, to date, no definitive diagnostic marker has been identified. A patient diagnosed with immunoglobulin A nephropathy was observed to exhibit deterioration in renal function. To determine whether new structural abnormalities had developed, computed tomography scans of the kidneys, ureters, and bladder were obtained. Both kidneys exhibited multiple cortical defects, and a renal angiogram was performed to determine the cause. Angiography revealed partial obliteration of the left distal renal artery branches and multifocal extensive infarctions in both kidneys, and the patient was diagnosed with renal-limited PAN. Following steroid monotherapy, an improvement in renal function was observed. We believe that this case report may be helpful to physicians who assess and treat patients with suspected renal-limited PAN.

Renal artery stenosis presenting as congenital nephrotic syndrome with hyponatremic hypertensive syndrome in a 2-month-old infant: a case report

  • Here, we present the case of a 2-month-old male infant with hyponatremic hypertensive syndrome resulting from stenosis of the right proximal and mid-renal arteries. The patient exhibited nephrotic-range proteinuria, low serum albumin, increased serum creatinine, and elevated renin and aldosterone levels. Doppler ultrasonography and computed tomography angiography revealed decreased vascular flow in the small right renal artery. Following a successful percutaneous balloon angioplasty, the patient experienced a decrease in blood pressure and normalization of serum electrolyte levels within a few days. However, it took 3 months for the proteinuria to resolve completely. This case is significant as it represents the first reported instance of a neonate presenting with clinical features resembling congenital nephrotic syndrome caused by renal artery stenosis that was successfully treated with percutaneous renal angioplasty.

  • Azygos system, one of the main collateral vessels which communicates superior vena cava with inferior vena cava, is well visualized by Xray angiography. This system is rarely demonstrated by radioisotope study. We report a case whose azygos system was shown during $^{99m}Tc-DTPA$ renal scanning.

한우신동맥(韓牛腎動脈) 분포(分布)에 관하여 (Distribution of Renal Artery in the Kidney of Korean Native Cattle)

  • The distribution of renal artery of fifty Korean native cattles (100 kidneys) was observed. Vinylite solution was injected into renal artery of ninety specimens for cast preparation. The angiography was prepared in 10 specimens by injecting thirty percent of barium sulfate solution into renal arteries, and then radiographed on a soft X-ray apparatus (Shimadzu Waltes 60). 1. A. renalis arose from the each side of abdominal aorta in the Korean native cattles. 2. The renal arteries were bifurcated into Ramus cranialis and caudalis (91%), and Ramus cranialis, medius and caudalis(9%) which were ramified 1-4 segmental arteries, respectively. 3. The segmental arteries were originated from R. cranialis and R. caudalis (87%), R. medius (9%) and A. renalis (4%). 4. The kidney were divided separately into 5-7 arterial segments by running of the segmental artery into the parenchyma. Among them six segments were mostly frequent(53%). 5. The Arcus arteriosus renalis was observed at 44% of the left kidney and 14% of the right kidney.
