대황산(大黃酸)의 당뇨병쥐 신장조직섬유화 억제 효과에 관한 실험연구 (Inhibitory Effect of Rhein on Renal Fibrosis in Diabetic Nephropathy Rats)

  • Objectives : To investigate the therapeutic effect and underlying mechanisms of rhein on renal fibrosis in diabetic rats. Methods : Diabetic nephropathy (DN) was induced in adult Wistar rats via introperitoneal injection of streptozotocin (STZ) (20 mg/kg/d) for three consecutive days. Two days after the last dose of STZ, rhein was administered to the diabetic rats at a dose of 25 mg/kg or 50 mg/kg, twice a day by gavage, respectively. Following 28 days treatment with rhein, the plasma glucose and creatinine levels were measured, the renal levels of TGF-${\beta}1$ protein and mRNA were examined, and the fibronectin mRNA levels were also determined. Results : Rhein significantly inhibited the increased plasma glucose and creatinine levels of diabetic rats in a dose- and a time-dependent way. Immunohistochemical analysis showed both doses of rhein markedly attenuated elevated induction of renal TGF-${\beta}1$ protein expressions in diabetic rats. Additionally, the high dose of rhein improved both TGF-${\beta}1$ and fibronectin mRNA expressions, while the low dose of rhein only alleviated fibronectin mRNA expressions. Conclusions : Rhein can improve renal fibrosis in diabetic nephropathy rats, and which may be mediated through inhibition of the renal mRNA expressions of TGF-${\beta}1$ and fibronectin.

Renal mRNA Expression of Renin, $AT_1$ Receptor, TGF-${\beta}1$ and Fibronectin in Obstructive Nephropathy

  • The present study was designed to quantify the alterations of renal renin, angiotensin type I receptor ($AT_1$), $TGF-{\beta}1$, and fibronectin gene expression in rats with unilateral ureteral obstruction (UUO). We also investigated the change of $AT_1$ density during UUO. Reverse transcription-polymerase chain reaction (RT-PCR) technique and receptor binding assay were used to detect mRNA expression and receptor density, respectively. At one day after UUO, renin mRNA level of the obstructed kidneys was decreased transiently and then subsequently increased to the level of sham kidneys. In the contralateral kidneys of the same rats, on the contrary, renin mRNA level was gradually decreased. Then, at 9 days after UUO, it was significantly lower than that of sham kidneys. The expressions of both $AT_1$ subtypes, called $AT_{1A}$ and $AT_{1B}$, mRNAs did not change at any time. UUO led to a significant decrease in $AT_1$ density in the obstructed kidneys compared with the sham kidneys at 1 and 3 days $(66\;{\pm}\;11.6%\;(p<0.005)\;and\;73\;{\pm}\;4.0%$ (p<0.01), respectively). Thereafter, $AT_1$ density was gradually increased and at 9 days it showed a marked elevation in the obstructed kidneys compared to the sham kidneys. In contrast, in the contralateral kidneys $AT_1$ density was significantly reduced from 3 to 9 days after UUO. The $TGF-{\beta}$1 mRNA level of the obstructed kidneys was unexpectedly decreased at 6 days after UUO. Then, at 9 days it was followed by a significant increase in the obstructed kidneys, whereas it showed an obvious decrease in the contralateral kidneys. In addition, fibronectin mRNA level was also significantly increased in the obstructed kidneys after UUO compared to the sham or the contralateral kidneys of the same rats. These results suggest a differential regulation of renal renin, $AT_1$ receptor, $TGF-{\beta}$1 and fibronectin mRNA levels at different stages of UUO.

도인(挑仁)이 일측 신절제와 streptozotocin으로 유발된 당뇨병성 신증 Rat에 미치는 영향 (The Effects of Prunus on Diabetic Nephropathy Rats Induced by Unilateral Nephrectomy and Streptozotocin)

  • Objectives: Diabetic nephropathy is the most common cause of end stage renal disease. Transforming growth factor (TGF)-${\beta}1$, type IV collagen, advanced glycation end-products (AGEs), and angiotensin II type 1 receptor (AT1) are the main factors of diabetic nephropathy. We investigated the effects of Prunus on renal function and histopathological changes of diabetic nephropathy rat model induced by unilateral nephrectomy and streptozotocin. Methods: Diabetes was induced in male Sprague-Dawley rats ($290{\pm}10g$) by injecting streptozotocin (55 mg/kg) into the tail vein after unilateral nephrectomy. Rats were divided into 3 groups (n=6): normal, control, and Prunus. After 8 weeks of oral administration of Prunus extract on the Prunus group from 3 days after streptozotocin injection, we checked weight, 24 hrs urine, blood biochemistry and renal tissue to evaluate renal function and histopathological changes by examining parameters including albuminuria, BUN, creatinine, cholesterol, low density lipoprotein (LDL), triglyceride, TGF-${\beta}1$, type IV collagen, AGEs, and AT1. We also measured mRNA expression of TGF-${\beta}1$, type IV collagen, AGEs, and AT1 by Real Time polymerase chain reaction (RT-PCR). Results: Prunus decreased the amount of 24 hrs proteinuria, and inhibited histopathological changes of diabetic nephropathy including the expression and accumulation of TGF-${\beta}1$, type IV collagen and AGEs which could promote development of diabetic nephropathy. Prunus also inhibited mRNA expression of TGF-${\beta}1$, type IV collagen. Conclusions: These findings suggest that Prunus might protect the renal function and inhibit the development of renal injury by regulating factors including TGF-${\beta}1$, type IV collagen, AGEs, except AT1, so Prunus can be used for diabetic patients to prevent the progression of diabetic nephropathy.

모링가 뿌리 추출물에 대한 신장섬유화 억제 효과 (Anti-Fibrotic Effects by Moringa Root Extract in Rat Kidney Fibroblast)

  • 신장섬유화는 내 외부적인 요인들에 의해 발생하며, 그 요인들에 의해 염증이 생기고 지속적인 손상이 일어날 경우 신기능의 상실이 유발된다. 또한 신장섬유화는 세포 외 기질의 과다축적, TGF-${\beta}$나, TNF-${\alpha}$, IL-1과 같은 사이 토카인에 의해 발생하며, TGF-${\beta}$는 신장 섬유화의 과정과 Type I collagen과 fibronectin, PAI-1을 포함한 섬유화 관련 인자들의 발현 유도에 중요한 역할을 한다. 본 연구에서는 TGF-${\beta}$를 처리한 신장섬유화 유도 모델에서 Moringa oleifera Lam 추출물에 대한 섬유화 관련 인자들의 영향을 확인하였다. 실험 결과 TGF-${\beta}$로 유도된 신장 섬유화 세포에서 모링가 추출물이 fibronectin, Type I collagen과 PAI-1의 단백질 및 mRNA 발현을 저해하였으며, 모링가 추출물 중 모링가 뿌리추출물이 가장 영향이 있는 것으로 확인 되었다. 모링가 뿌리추출물이 어떠한 기전을 통하여 섬유화 관련 인자들의 발현을 조절하는지 알아보기 위한 TGF-${\beta}$로 유도된 $T{\beta}RII$ 및 그 하위 기전의 인산화 정도를 확인한 실험에서 모링가 뿌리추출물이 TGF-${\beta}$로 유도된 $T{\beta}RII$과 그 하위기전의 Smad4, ERK의 인산화를 저해하였다. 그러나 TGF-${\beta}$에 의해 유도된 JNK와 p38, PI3K/AKT의 인산화에는 영향이 없었다. 따라서 모링가 뿌리추출물이 TGF-${\beta}$로 유도된 신장 섬유아세포에서 $T{\beta}RII$와 그 하위 기전인 Smad4, ERK를 통해서 Type I collagen 과 fibronectin, PAI-1의 발현을 조절하여 섬유화를 저해 한다는 것을 예상할 수 있다. 결론적으로 모링가 뿌리추출물이 섬유화 치료 및 완화에 좋은 물질이 될 수 있을 것으로 생각된다.

급성 허혈성 신손상 후 여러 성장인자 발현의 변화 (Alteration of Growth Factor Expression after Acute Ischemic Renal Injury)

  • 목 적 : 급성 허혈성 신손상 후 손상된 세뇨관 상피세포의 재생은 순환하거나 국소적으로 생성된 성장인자나 cytokine, 생리적 인자 및 주위 환경들의 조절에 의해 매개되는 것으로 알려져 있다. 따라서 급성 허혈성 신손상 후 신기능 회복에 따른 혈중 IGF-I의 변화와 전체 신장조직과 각 부위에 따른 IGF-I, -II, VEGF, $TGF-{\beta}1$, CTGF의 mRNA 발현의 변화를 알아보는데 연구의 목적이 있다. 방 법 : 체중이 250-300 g의 백서를 대상으로 급성 허혈성 신손상을 시켜 유발된 급성 신부전 모델을 이용하여 급성 신부전의 진행과정에 따라 혈청 IGF-I치와 여러 성장인자 발현의 변화를 측정하였다. 급성 신부전 진행은 혈청 크레아티닌을 측정하여 평가하였고 혈청 IGF-I 농도는 iodinated IGF-I과 polyclonal anti-IGF-I 항체를 이용한 방사면역 측정법을 이용하여 측정하였다. 신장에서 성장인자의 발현은 RT-PCR을 이용하였고 신장조직에서의 RNA 추출은 통상적인 방법에 준하였다. 신장에서의 IGF-I과 CTGF의 분포는 anti-IGF-I 항체와 anti-CTGF 항체를 이용한 면역조직화학법을 이용하였다. 결 과 : 1) 양측 신동맥을 결찰한 후 1일째부터 체중 감소와 혈청 크레아티닌치의 증가가 관찰되었는데 이러한 변화는 3일째에 가장 현저하였다. 혈청 IGF-I은 신손상 후 1일 째 현저하게 감소하였고 신손상 3일째에 더 증가되었다가 5일째부터는 신손상 전으로 회복되었다. 2) 신기능 회복에 따른 성장인자의 변화는 거의 유사하였다. IGF-I, IGF-II, $TGF-{\beta}1$ 및 VEGF의 mRNA 발현은 허혈성 신손상 후 1일째에 현저하게 감소하였고 신손상 3일째에는 신손상 전에 비해 증가하였으며 7일째에는 거의 신손상 전의 수준으로 회복되었다. 3) IGF-I과 IGF-II는 정상 신장에서는 주로 수질부 특히 바 깥쪽 수질부에서 발현되었고 허혈성 신손상 1일째에 현저하게 감소하였다가 3일째에는 정상 수준으로 회복되었다. $TGF-{\beta}1$은 정상에서 신수질부에서 주로 발현되었고 1일째는 현저하게 감소하였다가 회복되는 소견을 보였다. CTGF와 VEGF는 정상 신장의 수질부와 피질부 모두에서 발현되었으며 신손상 1일째에 현저하게 감소되었다가 3일째부터 정상 수준으로 회복되는 소견을 보였다. 4) 정상 백서의 신장에서 IGF-I은 주로 피질부위의 신세뇨관에 분포하였고 사구체에서는 발견되지 않았는데 급성 허혈성 신손상 후 1일째 신장의 피질부위에서는 IGF-I가 현저히 감소되었다. CTGF는 정상 백서의 신장에서는 주로 혈관에 분포하는 소견을 보였는데 신손상 3일째에는 세뇨관에서 현저한 증가가 관찰되었다. 결 론: 이번의 연구로 저자들은 IGF-I, IGF-II, $TGF-{\beta}1$, CTGF, VEGF 등과 같은 성장인자가 급성 허혈성 신손상 후 신기능과 세뇨관세포의 회복과정에 발현의 변화가 초래된다는 사실을 알 수 있었다. 따라서 신기능 회복을 촉진시키기 위해 여러 성장인자를 급성신부전 치료제로 사용할 수 있을 것으로 생각되고 이에 대한 연구가 더 필요할 것으로 생각된다.

Aldosterone Up-regulates Production of Plasminogen Activator Inhibitor-1 by Renal Mesangial Cells

  • In vivo studies have demonstrated that aldosterone is an independent contributor to glomerulosclerosis. In the present study, we have investigated whether aldosterone itself mediated glomerulosclerosis, as angiotensin II (Ang II) did, by inducing cultured renal mesangial cells to produce plasminogen activator inhibitor-1 (PAI-1), and whether these effects were mediated by aldosterone-induced increase in transforming growth factor $\beta_1$ (TGF-$\beta_1$) expression and cellular reactive oxygen species (ROS) activity. Quiescent rat mesangial cells were treated by aldosterone alone or by combination of aldosterone and spironolactone, Ang II, neutralizing antibody to TGF-$\beta_1$ or antioxidant Nacetylcysteme (NAC). This study indicate that aldosterone can increase PAI-1 mRNA and protein expression by cultured mesangial cells alone, which is independent of aldosterone-induced increases in TGF-$\beta_1$ expression and cellular ROS. The effects on PAI-1, TGF-$\beta_1$ and ROS generation were markedly attenuated by spironolactone, a mineralocorticoid receptor antagonist, which demonstrate that mineralocorticoid receptor (MR) may play a role in mediating these effects of aldosterone.

Protective Effect of N-Acetylcysteine on Progression of Adriamycin-induced Nephyopathy

  • Effects of antioxidants on the established nephropathy were investigated. The experimental nephropathy was induced in rats by intravenous injection of adriamycin (2 mg/kg). Six weeks later, when proteinuria was apparent, the rats were supplemented with N-acetylcysteine (NAC, 1 g/kg/day) in drinking water for additional 6 weeks. Glomerulosclerosis score and tubulointerstitial injury index were determined by light microscopy. Expression of transforming growth factor (TGF) ${\beta}1$ and laminin ${\beta}1$ was determined in the renal cortex by reverse transcription-polymerase chain reaction, Western blotting, immunohistochemistry, and immunogold electron microscopy. The adriamycin-induced proteinuria as well as the glomerulosclerosis and tubulointerstitial injury was ameliorated by the treatment with NAC. Adriamycin increased the expression of TGF ${\beta}1$ mRNA and protein, which was ameliorated by NAC. Although the expression of laminin ${\beta}1$ mRNA was increased, adriamycin did not significantly alter that of its protein. These results indicate that antioxidants ameliorate the established nephropathy in association with normalization of overexpressed TGF ${\beta}1$.

복합한약제제 KIOM-79으I STZ 유도 당뇨 모델에서의 최종당화산물 생성 및 Type IV Collagen 및 $TGF-{\beta}1$ 발현 억제 효과 (Inhibitory Effect of KIOM-79, a New Herbal Prescription, on AGEs Formation and Expressions of Type IV Collagen and $TGF-{\beta}1$ in STZ-induced Diabetic Rats)

  • Advanced glycation end products (AGEs) formation plays an important role in the progression of diabetic complications. To develop effective herbal formulations with suppression of diabetic nephropathy, a common complication in diabetic patients, we evaluated inhibitory activities of KIOM-79, a new herbal prescription, on the formation of AGEs using in vitro and in vivo model systems. Effects of KIOM-79 on the expression of AGEs, RAGEs (receptor for Advanced glycation end products), type IV collagen and renal $TGF-{\beta}1$ mRNA was also examined in streptozotocon (STZ)-induced diabetic rats. STZ-induced diabetic rats were treated orally with KIOM-79 (250 and $500\;kg^{-1}$ once a day for 13 weeks). In vitro system KIOM-79 suppressed the formation of AGEs $(18.12\;{\mu}g/ml)$. In STZ-induced diabetic rats showing accumulation of AGE and RAGE, pathological examination revealed that KIOM-79 prevented AGE and RAGE deposition in the kidney. In STZ induced diabetic rats, the expansion of mesangial matrix and the glomerular tufts seemed to be larger than those in normal rats. Howεver, after administration with KIOM-79, mesangial metrix and glomerular volume were decreased, and overexpression of type IV collagen was also decreased. Overexpression of renal $TGF-{\beta}1$ mRNA was inhibited significantly. These results suggest that the KIOM-79 might be an effective herbal prescription to prevent or alleviate the progression of diabetic nephropathy.

Upregulation of Renal Renin-Angiotensin System in Rats with Adriamycin-Induced Nephrosis

  • The present study was aimed to investigate whether the adriamycin-induced nephrosis is associated with an altered regulation of local renin-angiotensin system (RAS) in the kidney. Rats were subjected to a single injection of adriamycin (2 mg/kg body weight, IV) and kept for 6 weeks to allow the development of nephrosis. They were then divided into two groups, and supplied with and without cilazapril, an angiotensin converting enzyme (ACE) inhibitor, in drinking water (100 mg/l) for additional 6 weeks. Another group without adriamycin-treatment served as control. The mRNA expression of renin, ACE, type 1 and type 2 angiotensin II receptors (AT1R, AT2R), and transforming growth factor $(TGF)-{\beta}1$ was determined in the cortex of the kidney by reverse transcription-polymerase chain reaction. Adriamycin treatment resulted in heavy proteinuria. Accordingly, the mRNA expression of renin, ACE, and AT1R was increased in the renal cortex, while that of AT2R was decreased. Co-treatment with cilazapril attenuated the degree of proteinuria. While not affecting the altered expression of renin, cilazapril decreased the expression of ACE to the control level. Cilazapril further increased the expression of AT1R, while it restored the decreased expression of AT2R. The expression of $(TGF)-{\beta}1$ was increased by the treatment with adriamycin, which was abolished by cilazapril. An altered expression of local RAS components may be causally related with the development of adriamycin-induced nephrosis, in which AT1R is for and AT2R is against the development of nephrosis.

Inhibitory Effects of an Aqueous Extract of Gynura procumbens on Human Mesangial Cell Proliferation

  • Gynura procumbens (Lour.) Merr. has been used in some parts of Southeast Asia as a folk medicine to treat kidney diseases, diabetes mellitus, and hyperlipidemia. The present work was undertaken to prove the mechanisms of G. procumbens in the management of glomerular diseases. We investigated the effect of an aqueous extract of G. procumbens on cell proliferation, DNA synthesis, and the expressions of $TGF-{\beta}1$, PDGF-BB, CDK1, CDK2, and CDK4 in fetal bovine serum-activated human mesangial cells (MCs). The G. procumbens extract inhibited proliferation, DNA synthesis, expressions of PDGF-BB, CDK1, and CDK2 mRNA, and expression of $TGF-{\beta}1$ protein in MCs. The inhibitory effect of G. procumbens on MC proliferation may be mediated by suppression of PDGF-BB and $TGF-{\beta}1$ expressions and the modulation of CDK1 and CDK2 expression. Therefore, G. procumbens shows promise as an adjunct therapy in preventing progressive renal diseases.
