목 적 : 관류 자기공명영상(perfusion MRI)을 이용하여 대뇌(cerebral)에서 혈액뇌장벽의 파괴로 인하여 조영제가 혈관내에서 조직으로 빠져 나가는 투과성과 상대적 뇌혈류량을 영상화 해보고, 이 영상을 이용하여 구한 투과성비와 상대적 뇌혈류량비가 종양의 악성등급 평가와 감별진단에 어떠한 유용성이 있는지 알아보고자 하였다. 대상 및 방법 : 영상의학진단과 병리조직검사로 진단된 29명을 대상으로 뇌종양이 포함된 550(11 silce $\times$ 50 image)관류 자기공명영상을 3T기기에 장착된 프로그램으로 상대적 뇌혈류량을 영상화 하였고, 다른 한 방법은 개인 컴퓨터에 영상을 전송 후 IDL 6.2 프로그램을 이용하여 상대적 뇌혈류량(relative cerebral blood volume-reformulated singular value decomposition, rCBV-rSVD)과 투과성을 영상화 하였다. 그 영상을 이용하여 동일한 관심영역으로 화소별 평균 신호강도를 정량적(quantitative analysis)으로 측정하여 비모수적 통계인 Kruskal-wallis test를 통해 뇌종양별로 평균비교 분석을 하였다. 결과 : 상대적 뇌혈류량영상과 투과성영상을 이용하여 동일한 관심영역으로 정상부위와 종양부위의 정량적으로 분석한 상대적 뇌혈류량 비 와 (3T 기기자체 분석한 값, IDL 6.2로 분석한 값) 투과성비에서 고등급 성상세포종(n=4)의 경우 (14.75, 19.25) 13.13, 저등급 성상세포종(n=5) (14.80, 15.90) 11.60, 아세포종(n=5) (10.90, 18.60) 22.00, 전이성 뇌종양(n=6) (11.00, 15.08) 22.33, 수막종(n=6) (18.58, 7.67) 5.58, 핍돌기 신경교종(n=3) (23.33, 16.33) 15.67로 나타났다. 결론 : 종양별로 상대적 혈류량영상을 이용하여 측정한 상대적 뇌혈류량 비는 등급을 분류하기에 용이하지 않았지만, 투과성영상으로 측정한 투과성비는 종양 악성정도가 높을수록 높은 것으로 나타나 종양의 등급 평가와 감별진단에 유용하였다.
최근 신경계 질환의 진단에 많이 이용되는 관류자기공명영상기법 중 상대적 뇌 혈류량(relative cerebral blood volume)의 분석에 의한 뇌교종의 악성도 구분(고등급과 저등급 종양의 구분)에 있어 자기공명영상의 뇌 혈류량 검사의 유용성에 대하여 알아보고자 하였다. 뇌교종 환자 17명(평균연령 57.5세, 남자 11명, 여자 6명)을 대상으로 모든 환자에게 수술 전 관류자기공명 영상과 고식적 뇌 자기공명영상을 시행하고, 조직의 병리학적 검사를 시행하였다. 국소적 뇌 혈류량과 상대적 뇌 혈류량의 분석은 지멘스사의 소프트웨어(PAT)와 자체 개발한 영상 후 처리 소프트웨어 Xmap 2.0을 이용하여 비교, 분석하였다. 대상자 17명 중 뇌교종이 저등급인 6명과 고등급인 11명의 관류자기공명영상의 분석 결과는 저등급 뇌교종의 상대적 뇌 혈류량의 백질에 대한 종양부위의 평균 뇌 혈류량(rCBVw)의 평균은 1.62, 피질에 대한 종양부위의 평균 뇌 혈류량(rCBVc)의 평균은 0.12이었다. 고등급 뇌교종의 상대적 뇌 혈류량의 rCBVw의 평균은 33.53, rCBVc의 평균은 0.96이었다. 뇌교종과 반대측 뇌 백질과의 통계적인 상관성은 0.01(p-value)이었고. 뇌교종과 반대측 피질과의 통계적인 상관성은 0.02(p-value)로 나타났다. 결과 중 뇌교종과 뇌 백질의 통계분석수치가 뇌교종과 피질과의 통계분석 결과보다 더 유의한 결과를 나타내었다. 결국 두 가지 모두에서 유의한 결과를 나타냄을 알 수 있었다(p<0.05). 임상적으로 기존의 자기공명영상과 병리학적 결과와 함께 관류자기공명영상은 뇌교종의 등급을 판단하는데 있어 유용할 것으로 사료된다.
The purpose of this study was to investigated the usefulness of MR perfusion image comparing with SPECT image. A total of pediatric 30 patients(average age : 7.8) with Moyamoya disease were performed MR Perfusion with 32 channel body coil at 3T from March 01, 2010 to June 10, 2010. The MRI sequences and parameters were as followed : gradient Echo-planar imaging(EPI), TR/TE : 2000ms/50ms, FA : $90^{\circ}$, FOV : $240{\times}240$, Matrix : $128{\times}128$, Thickness : 5mm, Gap : 1.5mm. Images were obtained contrast agent administrated at a rate of 1mL/sec after scan start 10s with a total of slice 1000 images(50 phase/1 slice). It was measured with visual color image and digitize data using MRDx software(IDL version 6.2) and also, it was compared of measurement with values of normal and abnormal ratio to analyze hemodynamic change, and a comparison between perfusion MR with technique using Warm Color at SPECT examination. On MR perfusion examination, the color images from abnormal region to the red collar with rCBV(relative cerebral blood volume) and rCBF(relative cerebral blood flow) caused by increase cerebral blood flow with brain vascular occlusion in surrounding collateral circulation advancement, the blood speed relatively was depicted slowly with blue in MTT(Mean Transit Time) and TTP(Time to Peak) images. The region which was visible abnormally from MR perfusion examination visually were detected as comparison with the same SPECT examination region, would be able to confirm the identical results in MMD(Moyamoya disease)judgments. Hymo-dynamic change in MR perfusion examination produced by increase and delay cerebral blood flow. This change with digitize data and being color imaging makes enable to distinguish between normal and abnormal area. Relatively, MR perfusion examination compared with SPECT examination could bring an excellent image with spatial resolution without radiation expose.
To measure regional cerebral blood volume (rCBV) with perfusion MR imaging of cerebral fat embolism by neutral fat and free fatty acids in cats. Triolein (group 1, n=15), oleic acid (group 2, n=9) and linoleic acid (group 3, n=11) were infused into unilateral internal carotid artery using microcatheter through the transfemoral approach. PVA particle was used as a non-fat embolic material in a control group (group 4, n=9). Perfusion-weighted MR image was obtained at 30 minutes and 2 hours postembolization, based on T2-and diffusion-weighted images. The data of lesion and contralateral normal area were transferred to personal computer, time-to-signal intensity curve was drawn and trans for used to △R2/sup */ curve in regular order. The process in the personal computer was done by using the author's developmental image processing program and interactive data language (IDL) softwares. Statistical significance was approved by paired t-test and ANOVA. rCBV of the lesion was decreased comparing to the normal area in all groups. The ratios of rCBV were as follows (group No, at 30 minutes, at 2 hours); group 1,32%, 51%; group 2, 30%, 44%; group 3, 39%, 61%; group 4, 21%, 36%. rCBVs of 2 hours was significantly increased compared to those of 30 minutes in all groups (P<0.005). rCBV was decreased at 30 minutes in cerebral fat embolism and recovered a little, but significantly at 2 hours. Perfusion-weighted images was useful method in offering hemodynamic information in cerebral fat embolism.

We report perfusion weighted imaging (PWI) findings of nonenhanced anaplastic astrocytoma in a 30-year-old woman. Brain magnetic resonance imaging showed a nonenhanced brain tumor with mild peritumoral edema on the right medial frontal lobe and right genu of corpus callosum, suggesting a low-grade glioma. However, PWI showed increased relative cerebral blood volume, relative cerebral blood flow, and permeability of nonenhanced brain tumor compared with contralateral normal brain parenchyma, suggesting a high-grade glioma. After surgery, final histopathological analysis revealed World Health Organization grade III anaplastic astrocytoma. This case demonstrates the importance of PWI for preoperative evaluation of nonenhanced brain tumors.
Objective: Classically, single photon emission tomography is known to be the reference standard for evaluating the hemodynamic status of patients with moyamoya disease. Recently, T2-weighted perfusion magnetic resonance(MR) imaging has been found to be effective in estimating cerebral hemodynamics in moyamoya disease. We aim to assess the utility of perfusion-weighted MR imaging for evaluating hemodynamic status of moyamoya disease. Methods: The subjects were fourteen moyamoya patients(mean age: 7.21 yrs) who were admitted at our hospital between Sep. 2001 to Sep 2003. Four normal children were used for control group. Perfusion MR imaging was performed before any treatment by using a T2-weighted contrast material-enhanced technique. Relative cerebral blood volume(rCBV) and time to peak enhancement(TTP) maps were calculated. Relative ratios of rCBV and TTP in the anterior cerebral artery(ACA), middle cerebral artery(MCA) and basal ganglia were measured and compared with those of the posterior cerebral artery(PCA) in each cerebral hemispheres. Using this data, we analysed the hemodynamic aspect of pediatric moyamoya disease patients in regarding to the age, Suzuki stage, signal change in FLAIR MR imaging, and hemispheres inducing symptoms. Results: The mean rCBV ratio of ACA, MCA did not differ between normal children and moyamoya patients. However the significant TTP delay was observed at ACA, MCA territories (mean = 2.3071 sec, 1.2089 see, respectively, p < 0.0001). As the Suzuki stage of patients is advanced, rCBV ratio is decreased and TTP differences increased. Conclusion: Perfusion MR can be applied for evaluating preoperative cerebral hemodynamic status of moyamoya patients. Furthermore, perfusion MR imaging can be used for determine which hemisphere should be treated, first.
뇌혈류 SPECT영상을 사용하여 뇌혈류를 간접적으로 평가하는 방법으로 횡단면 또는 관상면의 단층상을 이용하여 관류 결손 부위의 계수 분포를 평면적으로 조사하는 방법이 주로 사용되어 왔으나, 일반적으로 관류결손 부위의 크기는 다양하며 특히 임의의 형태를 갖는 관류 결손을 포함하는 영상에서는 평면 분석에 의한 결과는 많은 오차를 나타낼 수 있다. 따라서 본 연구는 체적 분석에 의한 평가 방법을 제안하고자 하였으며, 기초연구로서 동일한 뇌혈류 SPECT 영상에 대하여 평면 분석과 체적 분석 방법을 적용하여 비교하므로서 체적 분석 방법의 안정도와 정확도를 검토하였다. 평면 분석 방법에서는 관류 결손 부위의 크기와 형상에 따라 뇌혈류평가 지표인 계수 분포의 변화가 관찰되었으며, 합산에 사용된 단층상의 수에 따라 체적 분석과 상이한 결과를 보였다. 또한 평면 분석은 횡단면을 사용하는 경우와 관상면을 사용하는 경우 각각에서도 상이한 결과를 보여 체적 분석 방법이 뇌혈류 평가지표로서 신뢰도 높은 결과를 산출할 수 있음을 알았다.
Recently, there has been growing interest in the assessment of physiological parameters on brain perfusion that provide more information than pure morphologic diagnosis. Quantification of parameters that characterize cerebral micro-circulation with magnetic resonance imaging is of great relevance for clinical application. We determine the local tissue concentration by exponential relationship between the relative signal reduction S(t)/$S_0$ and local tissue concentration of contrast material $C_m(t)$ in dynamic susceptibility contrast enhanced MR imaging. And then we made relative regional blood volume map by calculating the area under the measured concentration-time curves $C_m(t)$ during first pass of paramagnetic contrast material as a preliminary step for perfusion map. These images make it possible to compare the rCBV in different brain regions in one individual at a time. We have it in contemplation to obtain arterial and brain signal time curves simultaneously to make absolute rCBV and perfusion (rCBF) map. These maps may provide the method of comparative investigations of different patients having strong variation in AIF.
목적: 대뇌 교종(glioma)에서 상대적 뇌혈액용적(relative cerebral blood volume, rCBV)과 종양의 조직학적 등급(grade)을 비교하고자 하였다. 대상 및 방법 대뇌 교종으로 진단된 총 16명의 환자에서 수술 전에 고식적 T1 및 T2 강조 자기 공명영상 및 조영제를 이용한 관류 자기공명영상을 시행하였다. 교종은 교아세포종(glioblas tora)이 7예, 역형성 성상세포종(anaplastic astrocytoma)이 2예, 역형성 핍지교세포종(anaplastic oligodendroglioma)이 1예, 저등급 성상세포종(low-grade astrocytoma)이 5예, 저등급 핍지교종(low-grade oligodendroglioma)이 1예 있었다. 관류 자기공명영상으로 얻은 교종의 최대 rCBV을 종양의 조직학적 진단 및 등급과 비교 하였다. 결과: 교아세포종의 최대 rCBV은 반대편 정상 백질과 비교하여 433%-1330% (펑균 790%)로 현저히 증가되어 있었다. 강한 조영증강을 보인 역형성 핍지교종 1예의 최대 rCBV은 502%로 증가되어 있었으나 조영증강되지 않는 역형성 성상세포종 2예의 최대 rCBV은 각각66%, 284%로 비교적 낮았다. 저등급 성상세포종의 최대 rCBV은 80%-369% (굉균 202 %)였다. 저등급 핍지교종 1예는 최대 rCBV이 1450%로 교아세포종 보다 높았다. 결론: 관류 자기공명영상을 이용한 rCBV수치는 교아세포종과 저등급 성상세포종은 분명히 구분되었으나 조영증강되지 않는 역형성 성상세포종파 저등급 성상세포종 간에는 차이가 없었다.

Objective: To determine the usefulness of perfusion MR imaging in assessing the histologic grade of cerebral gliomas. Materials and Methods: In order to determine relative cerebral blood volume (rCBV), 22 patients with pathologically proven gliomas (9 glioblastomas, 9 anaplastic gliomas and 4 low-grade gliomas) underwent dynamic contrast-enhanced T2*-weighted and conventional T1- and T2-weighted imaging. rCBV maps were obtained by fitting a gamma-variate function to the contrast material concentration versus time curve. rCBV ratios between tumor and normal white matter (maximum rCBV of tumor / rCBV of contralateral white matter) were calculated and compared between glioblastomas, anaplastic gliomas and low-grade gliomas. Results: Mean rCBV ratios were 4.90°±1.01 for glioblastomas, 3.97°±0.56 for anaplastic gliomas and 1.75°±1.51 for low-grade gliomas, and were thus significantly different; p < .05 between glioblastomas and anaplastic gliomas, p < .05 between anaplastic gliomas and low-grade gliomas, p < .01 between glioblastomas and low-grade gliomas. The rCBV ratio cutoff value which permitted discrimination between high-grade (glioblastomas and anaplastic gliomas) and low-grade gliomas was 2.60, and the sensitivity and specificity of this value were 100% and 75%, respectively. Conclusion: Perfusion MR imaging is a useful and reliable technique for estimating the histologic grade of gliomas.
