하수관거정비사업의 침입수/유입수 산정 및 활용방법 개선방안에 관한 연구 (A Study on Improvement of Inflow/Infiltration Computation and Application Method in Sewer Rehabilitation Project)

  • In this study, current sewer infiltration/Inflow(I/I) computation and application method was examined about improvement and adequacy relating to the main issues raised by the field for practitioners. The results of review about infiltration calculation method were considered to be in need of improvement at 'standards of minimum sewage calculation'. Furthermore, the results of review about I/I application method were considered to be in need of improvement at 'standards of seasonal infiltration application' and 'the relative decrease in the Annual evaluation standards'. In addition, annual I/I analysis at JC County for the four years(2009~2012) in respect of operation flow and rainfall data was conducted. The result of annual infiltration analysis, compared average daily sewage generated average infiltration rate was found in 21.95 %, infiltration by unit was found in $0.31m^3/day/cm/km$ and $0.12m^3/day/day$, respectively. The result of annual inflow analysis, average rainfall - Inflow equations was found $y=5.499{\times}$($R^2$ 0.793), and the average Inflow quantity by sewer extension was predicted to $0.66m^3/mm-km$.

한국의 전통적 물리치료법에 관한 조사 (A Survey of Korean Traditional Physical Therapy)

  • The physical therapy that is in use nowadays, especially thermotherapy, aquatherapy and manual therapy, has been around since humans started to live on the earth. In primitive times, when men were injured while hunting, they washed the wound in clean water and dried it out in the sunshine. They also soaked their bodies in hot springs to decrease physical fatigue. This primitive Korean traditional physical therapy was in common use among people throughout the Koguryo, Koryo and Chosun dynasties. Historical records show that some kings in those days recommended to their men these methods to treat any chronic disease or illness. This was often thought of as a positive means of treatment. Korean traditional physical therapy has been developed, and the scientific effects of therapy have been proven. Korean traditional physical therapy is used extensively in hospitals and clinics to treat many kinds of diseases and illnesses. Unfortunately some methods that have not been medically substantiated as an effective treatment are being used by practitioners. In this situation there is an urgent need for scientifical research and data regarding any unproven physical therapy techniques.

비만치료 및 체중감량에서의 적절한 마황 사용에 대한 임상 진료지침 개발 (A Clinical Practice Guideline for Ma-huang(Ephedra sinica) Prescription in Obesity)

  • Background Clinical practice guidelines (CPG) are systematically developed statements to assist practitioners and patients on healthcare decisions. They provide recommendations for the average patient, which should take into account individual clinical judgment and the patient's values and expectations. Ephedra has sympathomimetic effect and has been used for weight loss worldwide. However, its safety is controversial especially in autonomic and cardiovascular systems. Therefore, the need of appropriate CPG for ephedra prescription in obesity was advocated in Korean Traditional Medicine. Methods The committee comprised of specialists of obesity, oriental herbology, oriental cardiology, constitutional medicine. The committee collected all relevant references about adverse effect and safety of ephedra in the forms of meta-analysis, systematic review, randomized controlled trial, case-control study and observational study from international and domestic databases and paper journals. 11 English- and 5 Korean-language references were gathered and categorized by PICO (Patient, Intervention, Comparison, Outcome) method. We could not complete strength of recommendation which should be clarified according to the evidence grade estimation. Result The first version of CPG for ephedra prescription in obesity was issued by Korean Oriental Association for Study of Obesity. It includes topics of introduction, pharmacokinetics, side effects and adverse events, constitutional aspect and recommendations for dose, indication and contraindication. Conclusion There should be periodic upgrade of this CPG from now on. Although there are some drawbacks in this version of CPG, it has significance as the first CPG in Korean Traditional Medicine.

만응환(萬應丸) 에탄올 추출물의 메티실린 내성 포도상구균에 대한 항균활성 및 내성억제 효과 (Antibacterial Activity and Inhibition of Resistance in Methicillin-resistant Staphylococcus aureus by Maneung-hwan Ethanol Extract)

  • Objectives In this study, we investigated the antimicrobial activity of a 70% ethanol extract of Maneung-hwan (MEH), which is prescribed by practitioners of oriental medicine for use against methicillin-resistant Staphylococcus aureus (MRSA). Methods The antibacterial activity of MEH against MRSA strains was evaluated using the disc diffusion method, broth microdilution method (minimal inhibitory concentration, MIC), checkerboard dilution test, and time-kill test. The mechanism of action of MEH was investigated by bacteriolysis using detergents or ATPase inhibitors Additionally, mRNA and protein expression were investigated by quantitative reverse transcription-polymerase chain reaction and western blot assay, respectively. Results The MIC of MEH was 25~1,600 ㎍/mL against all the tested bacterial strains. We showed that MEH extract exerts strong antibacterial activity. In the checkerboard dilution test, the fractional inhibitory concentration index of MEH in combination with antibiotics indicated synergism or partial synergism against S. aureus. The time-kill study indicated that the growth of the tested bacteria was considerably inhibited after a 24-h treatment with MEH and selected antibiotics. To measure the cell membrane permeability, MEH (3.9 ㎍/mL) was combined with Triton X-100 (TX) at various concentrations N,N-dicyclohexylcarbodimide (DCCD) was also tested as an ATPase inhibitor. TX and DCCD cooperation against S. aureus exhibited synergistic action. Accordingly, the antimicrobial activity of MEH in the context of cell membrane rupture and ATPase inhibition was assessed. Additionally, the expression of genes and proteins associated with resistance was reduced after exposing MRSA to MEH. Conclusions These results suggest that MEH possesses antibacterial activity and acts as a potential natural antibiotic against MRSA.

  • Cranio sacral therapy is a refinend and subtle type of osteopathic treatment that encourages the realease of stresses and tentions throughout the body, including the head. Cranio sacral therapy is trained to feel a very subtle, rhythmical shape change that is present in all tissue. This is called Involuntary Motion or Cranial Rhythm. The movement is of very small amplitude therefore it takes practitioners with a very finely developed sense of touch to feel it. This Rhythm was first described in the early 1900's by Dr. William G. Sutherland and its existence was confirmed in laboratory tests in the 1960's and 1970's. Tension in the body disrupts the cranial rhythm. This shows them what stresses and strains your body is under a present and what tentions it may be carrying as a result of its past history.

Complementary and alternative medicine for neck pain: Focus on manipulative therapies-chiropractic and osteopathic techniques

  • Neck pain is an extremely common symptom with a variety of potential etiologies. A significant number of patients are turning to complementary and alternative medicine therapies. In particular, chiropractic and osteopathic manipulation techniques are discussed. "Low quality evidence", as per the GRADE system criteria used by the Cochrane Review, supports the beneficial effects of these treatments Complications are generally benign and self-limited although occasional catastrophic consequences have been documented. Medical practitioners should familiarize themselves and their patients with the risks and benefits of complementary and alternative medicine in order to make informed decisions.

한국에서의 모바일인터넷활용에 관한 연구 (The Study on the Mobile internet proliferation in Korea)

  • The purpose of this paper is to investigate the diffusion process of mobile internet use in Korea and to explore the determinants driving MIU. A survey was conducted to collect data to empirically assess the research model. In total, 543 usable responses were collected using a questionnaire derived from previous research. Both the structural equation model and partial least squares were used to study the model concerning different user groups. Findings, The results indicate that there are significant differences in the users' perceptions of mobile internet usage during its different innovation diffusion stages. Of the determinants, perceived enjoyment is the most important predictor of mobile internet use. In addition to motivating users by making services more enjoyable, the findings suggest that practitioners should take the differences of adopter groups into account. Making the mobile internet easy to use and compatible with users' lifestyles would promote the use of technology as well.

  • To evaluate the fields of human anatomy and physiology important in clinical practice of physical therapy, a questionnaire survey was performed for 70 physical therapists who were practicing in general hospitals, clinics, public health centers, and rehabilitation centers in several provinces including Seoul and Chungcheongdo. Fields of anatomy and physiology were divided into 13 chapters and subdivided into 52 items. And the preferred ranks were statistically analysed according to the career of physical therapists, nature of the institute and the post of duty. As results joint system, muscular system, skeletal system and nervous system were ranked in the fields of great importance. The relative importance of practical knowledge in anatomy and physiology should be considered in educating students as well as reeducation of practitioners.

  • 아름다운 미소는 한 사람의 삶의 질에 큰 영향을 준다. 사람들을 만날 때 자신 있게 웃을 수 있다면, 항상 긍정적인 태도를 유지할 수 있고이를 통해 매일의 삶을 보다 즐겁고 행복하게 영위할 수 있을 것이다. 치과 의사로서 우리는 환자들에게 아름다운 미소를 되찾아 주어야 할 소명이 있다. 치과 의사로서 환자의 저작 기능을 회복 시켜 주는 것도물론 중요하지만, 치과 보철을 통해 심미적인 치아 외형 및 안모를 회복 시켜 주는 것이 최우선적인 과제임을 늘 명심해야 한다. 복잡한 전악 수복 케이스를 접할 때 치료 계획을 세우는 과정이야말로 어떤 치료 과정에 앞서 가장 중요하다고 할 수 있겠다. 가장 이상적인치료 계획을 세움에 있어 근간은 바로 심미성이 되어야 하며, 이 목표를 달성하기 위해 우리는 각 과정을 신중하게 접근해야 한다. 지난 몇 년간 "디지털 스마일 디자인" 컨셉이 치과계에 화두가 되어 왔다. 이 컨셉은 다양한 방법으로 임상에 적용될 수 있는데, 본 논고에서는 가장 기본적인 2차원적인 스마일 디자인을 통해 전악 구강 재건 케이스를 접근하는 방법에 대해 소개하고 간단한 임상 사진 및 포토샵을이용해 치료 계획을 세우는 방법에 대해 논해 보고자 한다.

중풍환자에 대한 일차 한의임상진료 가이드라인 (Clinical Practice Guideline of Korean Medicine for Stroke : Preliminary Guideline and Recommendation)

  • The aim of this new statement is to provide comprehensive and timely evidence-based recommendations on stroke management for clinical practitioners. Many countries are already well engaged in developing and releasing their own clinical practice guidelines, whereas Korean Medicine (KM) is still beginning. It will take time and effort to develop evidence-based guidelines and recommendations of KM or other traditional medicine because they are weak in the area of scientific evidence. The clinical practice guideline of Korean medicine for stroke was formulated through extensive review of published literature and consensus meeting of Korean medicine specialists. This project was supported by a grant of the Oriental Medicine R&D Project, the Ministry of Health and Welfare. Referring to guidelines developed in other countries, the experts in the subject tried to organize and develop guidelines and recommendations adequate for domestic medical circumstances. In December, 2008, a multi-disciplinary team called the Evidence Based Clinical Practice Guidelines Development Group (EBCPGsDG) for Stroke was organized. The writing committee was comprised of experts in internal medicine, acupuncture, rehabilitation, and Sasang constitution. Outside specialists and associated panels were invited for consultation. The scope of the guideline encompasses acupuncture, moxibustion and herbal medicine (including Korean medicine, traditional Chinese medicine, Kampo medicine) as interventions for stroke patients. It includes statements about ischemic stroke (I63), stroke not specified as hemorrhage or infarction (I64), and sequelae of cerebrovascular disease (I69) according to the International Classification of Disease (ICD). The committee subdivided the description of herbal medications into acute stroke management, subacute stroke management, post-stroke management, and secondary prevention of stroke. Guidelines on the practice of acupuncture and moxibustion were described in order for acute stroke management, subacute stroke management, chronic stroke management, and post-stroke rehabilitation. Clinicians who are working in the field of stroke care can adopt this guideline for their practice.
