Treatment outcomes of extended-field radiation therapy for thoracic superficial esophageal cancer

  • Purpose: To evaluate the efficacy and safety of extended-field radiation therapy for patients with thoracic superficial esophageal cancer (SEC). Materials and Methods: From May 2007 to October 2016, a total of 24 patients with thoracic SEC (T1a and T1b) who underwent definitive radiotherapy and were analyzed retrospectively. The median total radiotherapy dose was 64 Gy (range, 54 to 66 Gy) in conventional fractionation. All 24 patients received radiotherapy to whole thoracic esophagus and 23 patients received elective nodal irradiation. The supraclavicular lymph nodes, the celiac lymph nodes, and both of those nodal areas were included in 11, 3, and 9 patients, respectively. Results: The median follow-up duration was 28.7 months (range 7.9 to 108.0 months). The 3-year overall survival, local control, and progression-free survival rates were 95.2%, 89.7%, and 78.7%, respectively. There were 5 patients (20.8%) with progression of disease, 2 local failures (8.3%) and 3 (12.5%) regional failures. Three patients also experienced distant metastasis and had died of disease progression. There were no treatment-related toxicities of grade 3 or higher. Conclusion: Definitive extended-field radiotherapy for thoracic SEC showed durable disease control rates in medically inoperable and endoscopically unfit patients. Even extended-field radiotherapy with elective nodal irradiation was safe without grade 3 or 4 toxicities.

Moderately Hypofractionated Conformal Radiation Treatment of Thoracic Esophageal Carcinoma

  • Aims: To prospectively assess the efficacy and safety of moderately hypofractionated conformal radiotherapy in patients with thoracic esophageal cancer. Methods and Materials: From Sept. 2002 to Oct, 2005, 150 eligible patients with T2-4N0-1M0 stage thoracic esophageal squamous cell cancers were enrolled to receive either conventional fractionated radiation (CFR) or moderately hypofractionated radiation (MHR) with a three-dimensional conformal radiation technique. Of the total, 74 received moderately hypofractionated radiation with total dose of 54-60Gy/18-20fractions for 3.5-4 weeks in the MHR arm, and 76 received conventional radiation with total dose of 60Gy/30 fractions for 6 weeks in the CFR arm. Concurrent chemotherapy comprised of paclitaxel and cisplatin. Safety was evaluated, and local control and overall survival rates were calculated. Results: Statistically significant differences between the CFR versus MHR arms were observed in local/regional failure rate (47.3% v 27.0%, P=0.034) and the percentage of patients with persistent local disease (26.3% v 10.8%, P=0.012). But 3 and 5-year overall survival rates (43.2%, 38.8% v 38.2%, 28.0%, respectively) were not different between the two arms (P=0.268). There were no significant differences in the incidences of grade 3 or higher acute toxicities (66.3% v 50.0%) and late complications rates (27.0% v 22.4%) between the MHR and CFR arms. Conclusions: Moderately hypofractionated, three-dimensional radiation treatment could improve the local control rate of esophageal cancer and potentially increase patients' survival.

Long-term results of ipsilateral radiotherapy for tonsil cancer

  • Purpose: We evaluated the effectiveness and safety of ipsilateral radiotherapy for the patient with well lateralized tonsil cancer: not cross midline and <1 cm of tumor invasion into the soft palate or base of tongue. Materials and Methods: From 2003 to 2011, twenty patients with well lateralized tonsil cancer underwent ipsilateral radiotherapy. Nineteen patients had T1-T2 tumors, and one patient had T3 tumor; twelve patients had N0-N2a disease and eight patients had N2b disease. Primary surgery followed by radiotherapy was performed in fourteen patients: four of these patients received chemotherapy. Four patients underwent induction chemotherapy followed by concurrent chemoradiotherapy (CCRT). The remaining two patients received induction chemotherapy followed by radiotherapy and definitive CCRT, respectively. No patient underwent radiotherapy alone. We analyzed the pattern of failure and complications. Results: The median follow-up time was 64 months (range, 11 to 106 months) for surviving patients. One patient had local failure at tumor bed. There was no regional failure in contralateral neck, even in N2b disease. At five-year, local progression-free survival, distant metastasis-free survival, and progression-free survival rates were 95%, 100%, and 95%, respectively. One patient with treatment failure died, and the five-year overall survival rate was 95%. Radiation Therapy Oncology Group grade 2 xerostomia was found in one patient at least 6 months after the completion of radiotherapy. Conclusion: Ipsilateral radiotherapy is a reasonable treatment option for well lateralized tonsil cancer. Low rate of chronic xerostomia can be expected by sparing contralateral major salivary glands.

지방부 다차로 도로구간에서의 사고 예측모형 개발 (대도시권 외곽 및 구릉지 특성의 도로구간 중심으로) (Development of a Accident Frequency Prediction Model at Rural Multi-Lane Highways)

  • 도로구간에서의 주행조건은 연속적인 도로축 상에서 구간별로 변하게 되고 이에 따라 도로에서의 교통사고는 도로 기하구조 변수뿐만 아니라 도로주변 환경변수, 교통조건 변수 그리고 기타 다양한 변수들에 의해서 발생하게 된다. 따라서 본 연구는 현장조사를 통해 얻어진 다양한 도로기하구조 요소를 고려하여 동질성을 갖춘 구간 분할 후에 도로를 구성하는 도로 기하구조, 교통조건, 도로주변 환경 그리고 기타 다양한 요소들을 복합적으로 반영하고자 한다. 이를 반영하기 위해 본 연구에서는 도로구간의 주행조건을 결정짓는 주요인들에 의해서 주행조건 동질구간을 결정하고, 각 동질구간에서의 도로 및 교통조건 등을 고려하여 사고예측 모형을 개발하였다. 모형 개발을 위해 사용된 자료는 대도시권 외곽과 평지 및 구릉지를 대표할 수 있는 수도권 외곽내에 지방부 도로구간과 전라북도 지방부 도로구간에서 수집되었다. 본 연구에서는 연속된 도로구간에서 사고건수가 "0"인 구간수가 매우 높게 나타나므로 이에 대한 과대 예측을 방지하기 위해 ZIP(Zero Inflated Poisson) 모형을 이용하였다. 사고예측모형 개발 결과 지방부 다차로 도로구간에서 교통사고에 영향을 미치는 변수로는 교통량과 도로구간 길이를 포함한 EXPO($365{\times}ADT{\times}Length{\times}Year/10^{-6}$), 곡선반경, 종단구배변화, 가드레일, 지형(산악지), 횡단보도개수, 버스정류장 개수가 지방부 다차로 도로구간에서의 차대차 사고에 영향을 미치는 주요 설명변수로 나타났다.

절토사면 현황조사 자료와 위험도간의 상관분석에 관한 연구: 강원도, 충청도 일대 절토사면 (A Study on Correlation Analysis between Inventory Data and Danger Grade of Cut Slopes: Cut Slopes in Kangwondo and Chungcheongdo.)

  • 한국건설기술연구원(KICT)과 한국시설안전공단(KISTEC)에서는 전국 국도변에 분포하고 있는 절토사면에 대한 현황조사를 2006년부터 수행하고 있다. 절토사면 현황조사는 절토사면 정밀안전진단과는 달리, 현장에서 기본적인 육안조사를 통해 얻을 수 있는 여러가지 절토사면 특성에 대한 간단한 조사로 절토사면 유지관리의 기본이 되는 자료를 수집하는 것이다. 현황조사는 조사 대상 절토사면의 일반현황, 절토사면 특성, 조사자 소견으로 구성된다. 조사된 자료는 전국에 분포하고 있는 위험절토사면을 파악하고 정밀 안전진단의 조사순위를 결정하는데 활용된다. 본 논문에서는 SPSS (Statistical Package for the Social Sciences) 통계처리 프로그램을 이용하여 2006년부터 2008년까지 강원도와 충청도 지역에서 수집된 10,461개의 국도변 절토사면 현황조사 자료에 대하여 상관분석을 하였다. 현황조사 항목으로 부터 산출한 절토사면의 위험도 점수와 현황조사 항목간의 상관성을 분석하여 상관계수를 산출하였고 이를 통해 절토사면 위험도 점수에 보다 많은 영향을 미치는 현황조사 항목을 평가해보았다. 상관분석결과, 뜬돌 및 낙석 분포, 불연속면의 방향성 및 상부자연사면의 경사가 절토사면 위험도 점수에 영향을 크게 미치는 항목임을 알 수 있었다. 또한, 위험도 점수에 영향을 미치는 항목은 지역별로 약간의 차이가 나타남을 알 수 있었다. 추후, 절토사면 현황조사가 완료되면 우리나라에 분포하고 있는 절토사면의 지역별 특성을 파악할 수 있을 것으로 생각된다.
