  • Background: Few studies have examined the performance of the public long-term care insurance (LTCI) from the perspective of geographic equity. This study investigated regional variations and associated factors in the supply and utilization of nursing home care within and also between Japan and Korea. Methods: A comparative dataset was developed by extracting data from 2013-2015 LTCI statistics yearbooks and Organization for Economic Cooperation and Development regional statistics, as well as other comparable data in Japan and Korea. The unit of analysis was the prefecture in Japan and the province in Korea. We computed variation indices and conducted regression analyses for regional variations within each country and decomposition analyses to examine the variations between the countries. Results: The overall regional supply and use of nursing home care were higher in Japan, but the regional variations in Korea were larger than in Japan. In both countries, the nursing home supply was negatively associated with the proportion of older people with independent living. Nursing home use was also negatively associated with the supply of hospital beds and home care agencies in Korea; the relationship was the opposite in Japan, however. The country-based differences were more likely to be explained by differences in the distributions of the variables included in the analytical model than country-specific characteristics. Conclusion: Regional-level nursing home supply and use were unequal in both countries, and the contributing factors were not the same. Policy efforts are needed to advance regional equality in long-term care (LTC) and collaboration between health and LTC institutions for frail older people, especially in Korea.

  • The aim of this paper is to identify the components of the sustainable regional ecosystem of social economy enterprises (hereafter 'regional ecosystem'), and to explore the principles of reciprocity, regard and public policy, which have a fundamental impact on the development of such regional ecosystem. The regional ecosystem is an interactive system of the stakeholders who are related to the process of planning, producing, distributing and selling goods and services, and the agents who maintain and spread their social value orientation. This system operates through the interaction of 'local network (business relations and social relations)', 'intermediary organization' and 'public role.' On the basis of the conceptual framework on the regional ecosystem, the result of the in-depth analysis on the case of Gorang-erang Coop shows the interaction of reciprocity and public policy with respect to regard within the context of a characteristic of the regional ecosystem.

  • This study proposes measures to improve management performance of construction projects by re-designing the organizational structures of the Regional Construction Management Administrations of the Ministry of Land, Infrastructure and Transport. The re-designing measures are derived based on the three measures which were established by analyses on status of the Regional Construction Management Administrations, requirements from the current organizational structure, and domestic/overseas examples; the measures are: 1) organizational integration and scope expansion, 2) structural re-organization according to construction projects, and 3) building a knowledge management structure. In order to realize the re-designing measures, sixteen tasks are derived; a milestone is established according to importance, feasibility and lead-lag relationships of these tasks. If these tasks are to be carried out successfully, the management performance of the Regional Construction Management Administrations of the Ministry of Land, Infrastructure and Transport will be expected to improve. Ultimately, the Regional Construction Management Administrations can reduce cost and improve the quality of public service by outsourcing the non-core competence tasks and by concentrating on the construction management for the new infrastructure projects.

Methods: The authors analyzed the contents of USHK developed by the health promotion team at the Western Pacific Regional Office of World Health Organization (WHO WPRO) in collaboration with health promotion experts at University of the Philippine Open University, and by observing health classes using the USHK in Angono elementary school in Manila, Philippines. Results: The following are the characteristics of USHK: 1) The USHK was composed of a teacher's guide and six books targeted to two groups of children: ages 5~6 years and 10~12, and an integrated package containing materials that can be used to support health education and health promotion activities in elementary schools. 2) The USHK could be integrated in the curriculum to reduce the burden of teacher's class preparation time and help teachers conveying clear and accurate health messages in their classes. 3) Several evaluation tools such as pre-test post-test quizzes, self-evaluation tools, observer checklists, and evaluation interview protocols were developed to monitor and evaluate whether USHK was useful, helpful, and appropriate. Conclusions: We found that USHK was a practical tool for supporting health promotion in elementary schools and could be applicable to health promoting schools in Korea if it were modified to address Korean school health problems.

  • According to the library act, the regional central library is not only a public library but also a library representing the metropolitan municipalities in Korea. But the status and role of current representative libraries are so weak that they are nothing more than large public libraries. Focusing on realistic vulnerability, this study analyzed the major weaknesses and limitations of regional central libraries and suggested ways to normalize legal positions and operations in terms of revising and supplementing the library act, restructuring and strengthening the tasks to be performed, and expanding the manpower and operating organization directly connected with core competencies.

  • Purpose: The Intensive Care Unit (ICU) is an important inpatient care area where critical patients are treated intensively with advanced medical technology. The level of care of ICU and the modernization of related facilities is an important indicator of health care quality. At the present time, when the Regional public hospitals are frequently expanding, the rational planning of the ICU has become an important part of the medical institutions treating the ICU. The purpose of this study is to present basic data with net area which can be used in the architectural planning of the ICU. Methods: The investigation and analysis of the ICUs were conducted on 26 medical facilities, based on theoretical analysis through relevant guidelines, articles, and documents, and on the basis of the actual space composition and net area analysis through the architectural drawings. Results: This study provides basic data such as zone division, spatial composition, relationship between main activities and zones, composition of facilities in the zone and area ratio within each zone. Implications: The results of this paper are expected to be effective reference materials for future research for rational spatial organization and efficient operation of the Intensive Care Unit in regional public hospitals.
