Peripheral nerve injuries in the oral and maxillofacial regions require nerve repairs for the recovery of sensory and/or motor functions. Primary indications for the peripheral nerve grafts are injuries or continuity defects due to trauma, pathologic conditions, ablation surgery, or other diseases, that cannot regain normal functions without surgical interventions, including microneurosurgery. For the autogenous nerve graft, sural nerve and greater auricular nerve are the most common donor nerves in the oral and maxillofacial regions. The sural nerve has been widely used for this purpose, due to the ease of harvest, available nerve graft up to 30 to 40 cm in length, high fascicular density, a width of 1.5 to 3.0 mm, which is similar to that of the trigeminal nerve, and minimal branching and donor sity morbidity. Many different surgical techniques have been designed for the sural nerve harvesting, such as a single longitudinal incision, multiple stair-step incisions, use of nerve extractor or tendon stripper, and endoscopic approach. For a better understanding of the sural nerve graft and in avoiding of uneventful complications during these procedures as an oral and maxillofacial surgeon, the related surgical anatomies with their harvesting tips are summarized in this review article.

The patient had bruxism and epilepsy tendency. Inadequate or unstable posterior support was identified due to severe anterior attrition and decreased occlusal vertical dimension. Prematurities of posterior occlusal and wear facets increased the function of anterior teeth, resulting in severe wear. Wear facets displayed sharply defined peripheries, which are matched on articulated diagnostic casts. Also the patient showed C III malocclusion tendency, and lost some facial contour with drooping corners on the mouth. In this case, the alteration of OVD (Occlusal Vertical Dimention) may provide a biologically compatible adjunct to the treatment such as dentofacial esthetics, improved visual proportion in facial heightand mechanical solutions to the force-management of the masticatory system. The patient requires extensive restorative treatment to regain appropriate function, esthetics and comfort. According to the report by Farhad Fays, the average vertical distance from the maxillary to the mandibular mucolabial reflection in the region of the central incisors is approximately 34mm. However, the vertical distance of this patient was found to be 32mm, which was necessary to add gauge 20-sheets to apply vertical dimension. A removable occlusal overlay splint, which restores OVD to the estimated optimalposition, is the general first trial. The patient was observed periodically for 6 weeks, while appropriate adjustments were made vertical dimension to function. When patient felt comfortable with the splint, the teeth were prepared, and provisional restorations are placed for 3 months. The provisional restoration was fabricated by a diagnostic wax-up. When the patient felt comfortable with the provisional restoration, the final restoration mimics OVD, function, and esthetics that have been developed in the treatment restorations. Restoration of the extremely worn dentition presents a substantial challenge to thedentists. Therefore, careful evaluation of the etiology, history, and factors associated with occlusal vertical dimension should be preceded prior to the appropriate treatment planning.

A high assurance IP encryption (HAIPE) compliant protocol accelerator is proposed for military networks consisting of red (or classified) networks and black (or unclassified) networks. The boundary between red and black sides is assumed to be protected via a HAIPE device. However, the IP layer encryption introduces challenges for bandwidth on demand satellite communication. The problems experienced by transmission control protocol (TCP) over satellites are well understood: While standard modems (on the black side) employ TCP performance enhancing proxy (PEP) which has been shown to work well, the HAIPE encryption of TCP headers renders the onboard modem's PEP ineffective. This is attributed to the fact that under the bandwidth-on-demand environment, PEP must use traditional TCP mechanisms such as slow start to probe for the available bandwidth of the link (which eliminates the usefulness of the PEP). Most implementations recommend disabling the PEP when a HAIPE device is used. In this paper, we propose a novel solution, namely broadband HAIPE-embeddable satellite communications terminal (BHeST), which utilizes dynamic network performance enhancement algorithms for high latency bandwidth-on-demand satellite links protected by HAIPE. By moving the PEP into the red network and exploiting the explicit congestion notification bypass mechanism allowed by the latest HAIPE standard, we have been able to regain PEP's desired network enhancement that was lost due to HAIPE encryption (even though the idea of deploying PEP at the modem side is not new). Our BHeST solution employs direct video broadcast-return channel service (DVB-RCS), an open standard as a means of providing bandwidth-on-demand satellite links. Another issue we address is the estimation of current satellite bandwidth allocated to a remote terminal which is not available in DVBRCS. Simulation results show that the improvement of our solution over FIX PEP is significant and could reach up to 100%. The improvement over the original TCP is even more (up to 500% for certain configurations).
In order to investigate the degradation behavior of PET fabrics, sodium diethylene glycolate (SDEG)-diethylene glycol (DEG) solutions were prepared and PET fabrics were treated in the solution. The dissolution rate constant and apparent activation energy of the PET fabrics were calculated by Eyring's and Arrhenius's equation respectively and measured dyeing properties, moisture and antistatic properties. Then compared SDEG-treated fabrics with NaOH-treated. The results were as follows; 1. PET fabrics decreased their weight in SDEG-DEG solution, and the decreasing rate showed a linear relationship to the treating time at constant temperature and concentration of SDEG-DEG solution. 2. The dissolution rate constant showed a linear relationship to the concentration of SDEGDEG solution and an exponential relationship to treating temperature. 3. Apparent activation energy of dissolution was 23.45 kcal/mol. 4. The K/S values and the ΔL values of fabrics treated with SDEG-DEG solution are higher and lower respectively than fabrics treated with NaOH. 5. SDEG-DEG solution treatment improved fabric's moisture regain and it reached almost maximum at about 40% weight loss. 6. In the both reagent the light, wet and sublimation fastness of fabrics are similar. 7. SDEG-DEG solution gave more electrical discharge effect to the fabrics than that of NaOH. 8. NaOH treated PET microfiber have crater-like surface, while SDEG-DEG solution give bathochromic effect to the PET microfiber because which has wrinkles on the surface.

Objectives: Recently, bioceramic sealers like EndoSequence BC Sealer (BC Sealer) have been introduced and are being used in endodontic practice. However, this sealer has limited research related to its retreatability. Hence, the aim of this study was to evaluate the retreatability of two sealers, BC Sealer as compared with AH Plus using micro-computed tomographic (micro-CT) analysis. Materials and Methods: Fifty-six extracted human maxillary incisors were instrumented and randomly divided into 4 groups of 14 teeth: 1A, gutta-percha, AH Plus retreated with chloroform; 1B, gutta-percha, AH Plus retreated without chloroform; 2A, gutta-percha, EndoSequence BC Sealer retreated with chloroform; 2B, gutta-percha, EndoSequence BC Sealer retreated without chloroform. Micro-CT scans were taken before and after obturation and retreatment and analyzed for the volume of residual material. The specimens were longitudinally sectioned and digitized images were taken with the dental operating microscope. Data was analyzed using an ANOVA and a post-hoc Tukey test. Fisher exact tests were performed to analyze the ability to regain patency. Results: There was significantly less residual root canal filling material in the AH Plus groups retreated with chloroform as compared to the others. The BC Sealer samples retreated with chloroform had better results than those retreated without chloroform. Furthermore, patency could be re-established in only 14% of teeth in the BC Sealer without chloroform group. Conclusion: The results of this study demonstrate that the BC Sealer group had significantly more residual filling material than the AH Plus group regardless of whether or not both sealers were retreated with chloroform.

Many image and video compression algorithms work by splitting the input image into blocks and producing variable-length coded bits for each block data. If variable-length coded data are transmitted consecutively, then the resulting coder is highly sensitive to channel errors. Therefore, most image and video techniques for providing some protection to the stream against channel errors usually involve adding a controlled amount of redundancy back into the stream. Such redundancy might take the form of resynchronization markers, which enable the decoder to restart the decoding process from the known state, in the event of transmission errors. The Error Resilient Entropy Code (EREC) is a well known method which can regain synchronization without any redundant information to convert from variable-length code to fixed-length code. This paper proposes an enhancement to EREC, which greatly improves its transmission ability for the compressed image quality without any redundant bits in the event of errors. The simulation result shows that the both objective and subjective quality of transmitted image is enhanced compared with the existing EREC at the same BER(Bit Error Rate).
This study's purpose is to investigate the effects on leg muscle activity caused by perturbation, using a trapdoor system during the support phase of gait for healthy adults (n = 6, height $177.5{\pm}5.5cm$, weight $81.0{\pm}9.5kg$, age $30.0{\pm}3.3yrs$). The trapdoor had the functional ability of causing inversion or eversion. The release time for the trapdoor was specified for two times, 0.3 and 0.5 seconds after heel contact. While altering these variables, EMG was recorded for the leg muscles (rectus femoris, biceps femoris, vastus lateralis, tibialis anterior, gastrocnemius, soleus). The following conclusions were derived. The steptime was longer for the 0.5s eversion than 0.3s inversion condition. So in order to regain stability after the perturbation the unsupporting leg reached forward rapidly. This quick reflex can be observed through the center of pressure (COP) and its rapid change in direction. The gastrocnemius was activated throughout the total experiment. There was a low amount of activity recorded in the rectus femoris, vastus lateralis and tibialis anterior except for the condition of inversion 0.3s. For most of the conditions, the highest average EMG peak values were recorded during the condition of inversion 0.3s. The iEMG patterns were similar for the conditions of inversion 0.3s and eversion 0.3s. To cope with the rapid change in these conditions, the biceps femoris was activated. During the experiment except for the condition of normal gait, the activity of the soleus and gastrocnemius was relatively high. Therefore, to prevent injury from perturbation of the lower leg strengthening of the soleus and gastrocnemius is required. Likewise to prevent injury to the thigh strengthening for the biceps femoris.
Poly(vinyl alcohol)(PVA)/chitosan blend films with non-toxicity, biodegradability, and biocom-patibility were prepared by solution casting. Variation of the physicochemical properties of the blend films was investigated through to several analysis methods. Examination of antibacterial properties revealed that bacterio-static ratios of all blend samples containing chitosan more than 10 wt% were greater than 99.9%. Moisture regain was increased with increasing chitosan content but the degree of swelling was decreased. Up to chitosan content 15 wt%t, the melting and crystallization temperature of blend films was increased with chitosan content. The blends containing chitosan content 10 and 15 wt% gave melting temperature 229 and 228$^{\circ}C$, respectively. However, the melting temperature was decreased if chitosan content exceeded 20 wt%. The mechanical properties of the blend films were increased with increasing chitosan content in both dry and wet states. The blend film including 15 wt% chitosan exhibited unusually high tensile strength.
Objectives : The mortality rate of subarachnoid hemorrhage(SAH) has been reduced recently due to refinement of microsurgical technique and improved perioperative management. Also, many survivors of SAH show excellent neurological recoveries. However, we found that a high proportion of the survivors do not fully regain their premorbid status in cognitive and memory function. Object of this study is to evaluate which factors might influence on cognitive and memory impairment in ruptured aneurysmal SAH patients. Methods : In this prospective study, a series of 66 patients with aneurysmal subarachnoid hemorrhage(SAH) from 1996 to 1998, most of whom had a "good" or "fair" neurological outcome, were assessed with various tests of cognition and memory function. All patients underwent clipping operation by pterional approach. Right side approach was performed in 16 case and left 21 cases. K-WAIS(Korean-Wechsler Adult Intelligence Scale) was used as method of cognition and memory function test. The time interval between SAH and assessment varied between 4 months and 8 months, averaging 6.2 months. Statistical analyses were carried out for each test score to see whether aneurysm site(A-com : non A-com), route of approach, age and sex, vasospasm, Hunt-Hess grade and Fisher CT group at admission, Glasgow Outcome Scale(GOS) at discharge affect cognitive and memory function. Results : Aneurysm site was not shown to be associated with performance on any test, and the initial grade (Hunt-Hess grade, Fisher CT group) of SAH and vasospasm had only minimal predictive values. The grade at discharge( GOS) was proved to be the best predictor of impairment of cognition and memory function within 1 year after operation. Conclusion : The authors conclude that the diffuse effects of SAH are more important than focal neuropathology in relation to cognitive impairment in this group of patients.
Human cardiomyocytes (CMs) cease to proliferate and remain terminally differentiated thereafter, when humans reach the mid-20s. Thus, any damages sustained by myocardium tissue are irreversible, and they require medical interventions to regain functionality. To date, new surgical procedures and drugs have been developed, albeit with limited success, to treat various heart diseases including myocardial infarction. Hence, there is a pressing need to develop more effective treatment methods to address the increasing mortality rate of the heart diseases. Functional CMs are not only an important in vitro cellular tool to model various types of heart diseases for drug development, but they are also a promising therapeutic agent for cell therapy. However, the limited proliferative capacity entails difficulties in acquiring functional CMs in the scale that is required for pathological studies and cell therapy development. Stem cells, human pluripotent stem cells (hPSCs) in particular, have been considered as an unlimited cellular source for providing functional CMs for various applications. Notable progress has already been made: the first clinical trials of hPSCs derived CMs (hPSC-CMs) for treating myocardial infarction was approved in 2015, and their potential use in disease modeling and drug discovery is being fully explored. This concise review gives an account of current development of differentiation, purification and maturation techniques for hPSC-CMs, and their application in cell therapy development and pharmaceutical industries will be discussed with the latest experimental evidence.
