Effects of Heat Treatment and Extraction Method on Antioxidant Activity of Several Medicinal Plants (열처리와 추출방법에 따른 몇 가지 약초의 항산화 활성)

  • We investigated the effects of heat treatment and extraction method on the antioxidant activities of five medicinal plants: Cyperus rotundus, Eucommia ulmoides, Bupleurum falcatum, Achyranthes japonica Nakai, and Akebia quinata. Extraction was performed with only ultrasound, ultrasound after heating at $130^{\circ}C$ for 2 hours, and reflux extraction with distilled water. The phenolic contents of reflux extraction and ultrasound extraction after heating were higher than only ultrasound extraction, and ultrasound extraction after heating samples was higher than reflux extraction except for Eucommia ulmoides and Cyperus rotundus. Total flavonoid content was higher in reflux and ultrasound extraction after heating samples than only ultrasound extraction, except for Cyperus rotundus. ABTS radical scavenging activity was higher in reflux extraction and ultrasound extraction after heating a sample, than only ultrasound extraction. DPPH radical scavenging activity was higher in reflux extraction except for Achyranthes japonica Nakai and Akebia quinata. The reducing power of ultrasound extraction after heating was higher with Achyranthes japonica Nakai. From the results of this study, we can expect to increase the antioxidant activity of medicinal plant extracts by applying suitable extraction and pretreatment conditions on the type of medicinal plant.

Incidence of Vesicoureteral Reflux in Asymptomatic Siblings of Patients with Reflux (방광요관역류 환아의 무증상 형제 자매의 역류 빈도 조사)

  • Purpose : Vesicoureteral reflux(VUR) is defined as a retrograde flow of urine from the bladder into the upper urinary tract. It has been shown to predispose patients to hypertension, renal scarring, and end-stage renal failure if not recognized and treated. The observation that VUR occurs in siblings of children with reflux at a significantly higher rate than the general pediatric population has been recognized for many years and VUR was detected in 26% to 51% of siblings of patients with VUR. The purpose of this study is to determine the incidence of VUR in asymptomatic siblings of children with VUR and to see if this form of screening would be practical. Methods : We retrospectively reviewed the records of 28 patients with VUR and their siblings. The total number of asymptomatic siblings investigating VUR were 28 persons. All patients and siblings were evaluated for VUR by a voiding cystourethrography and all patients and siblings with VUR were performed $^{99m}Tc$ 2,3-dimercaptosuccinic acid renal scan. Results : A total of 28 patients(14 boys, 14 girls) with VUR were studied; the mean patient age was 2.7 years(range 1 month to 8.4 years). The total number of asymptomatic siblings investigating VUR were 28 persons(17 boys, 11 girls) and the mean age was 3.3 years(range 2 months to 7.4 years). Renal scar was detected in 20 of 28(71.4%) patients with VUR. VUR was noted in three of 28(10.7%) siblings and renal scar was detected in one of three siblings with VUR. Conclusion : In this study, the predictive value of a positive family history alone in identifying VUR was 10.7%. This incidence suggests more investigation of asymptomatic siblings and continued study of this group of patients at risk is needed for clarifying the family screening of patients with VUR.

  • This study was carried out to investigate the antioxidant activity of the ethanol extract from $d\bar{o}d\bar{o}k$ compared with that from ginseng. The peroxide values and thiobarbituric acid values were examined in order to estimate the antioxidant activity of the extract in soybean oil and lard. The antioxidant activity of the extract in soybean oil increased in the order of BHA$d\bar{o}d\bar{o}k$ cold extract(WDCE)$d\bar{o}d\bar{o}k$ reflux extract(WDRE)$d\bar{o}d\bar{o}k$ cold extract(CDCE)$d\bar{o}d\bar{o}k$ reflux extract(CDRE) $d\bar{o}d\bar{o}k$ showed significantly stronger antioxidant activity than that from ginseng.

Endoscopic radiofrequency Stretta therapy reduces proton pump inhibitor dependency and the need for anti-reflux surgery for refractory gastroesophageal reflux disease

  • Background/Aims: Radiofrequency treatment of the gastroesophageal junction using the Stretta procedure for treating gastroesophageal reflux disease (GERD) is observed to improve the symptoms and proton pump inhibitor (PPI) dependence and reduce the need for anti-reflux operations. As one of the largest studies in Europe, we evaluated the clinical outcomes of Stretta in patients with medically refractory GERD. Methods: A tertiary UK center evaluated all patients with refractory GERD who underwent Stretta between 2014 and 2022. Patients and primary care professionals were contacted to obtain information regarding the initiation of PPI and reintervention after Stretta. Results: Of the 195 patients (median age, 55 years; 116 women [59.5%]) who underwent Stretta, PPI-free period (PFP) data were available for 144 (73.8%) patients. Overall, 66 patients (45.8%) did not receive PPI after a median follow-up of 55 months. Six patients (3.1%) underwent further interventions. The median PFP after Stretta was 41 months. There was a significant negative correlation between PFP and age (p=0.007), with no differences between sexes (p=0.96). Patients younger than 55 years of age had a longer PFP than their older counterparts (p=0.005). Younger males had a significantly longer PFP than older males (p=0.021). However, this was not observed in the female cohort (p=0.09) or between the younger men and women (p=0.66). Conclusions: Our findings suggest that Stretta is a safe and feasible option for treating refractory GERD, especially in younger patients. It prevents further anti-reflux interventions in most patients and increases the lead-time to surgery in patients with refractory GERD.

  • Two-phase transient phenomena in the noncondensable gas-filled closed loop was investigated numerically using the RELAP5/MOD3 version 3.1 computer code. The condensation heat transfer correlation for noncondensable gases was studied in detail. Two modes of the reflux condensation which can be characterized by countercurrent flow of steam and its condensed water and the oscillatory between reflux condensation and natural circulation were predicted well. However, the natural circulation mode which the condensed water carried over the U-bend concurrently with steam was failed to predict.

Surgical Management of Esophageal Achalasia (식도 아칼라지아의 외과적 치료)

  • 30 case of achalasia treated by modified Heller`s myotomy at the department of thoracic surgery of Seoul National University Hospital from 1965 to 1987 were analyzed. Preoperative Symptoms were alleviated completely or partially in 90% of the 30 patients who were available for follow up study [average: 3.6 years]. Excellent or good results were achieved in 83.3% of patients. Only two of the poor results were due to reflux esophagitis. It is concluded that an antireflux procedure is not routinely necessary to prevent postoperative reflux esophagitis if the technique of espohagomyotomy is used properly.

  • The prevalence of gastroesophageal reflux disease(GERD) has been increased recently in Korea. Most patients who have GERD are well treated by medication. But patients who have suboptimal disease control under medical therapy are needed further treatment. Nowadays minimally invasive surgery using laparoscopy and thoracoscopy has become popular in the operation of GERD although antireflux surgery is vary. Antireflux surgery is a well-documented, effective therapeutic alternative to control GERD.

  • Objective: Case report on the improvement of intestinal metaplasia through ortho-cellular nutrition therapy. Methods: A Korean woman in her 50s who is suffering from bloating, abdominal distension and frequent eructation after meals. Results: Symptoms improved one year after the introduction of nutrition therapy, and patient was diagnosed as fully recovered from reflux esophagitis, atrophic gastritis, and intestinal metaplasia through gastroscopy. Conclusion: In patients with complex gastrointestinal problems, the application of nutrition therapy may help improve and treat symptoms.
