세계보건기구의 마스크 사용 지침에 대한 한국 보건당국의 이행 분석과 함의: 코로나 바이러스 감염병을 중심으로 (Analysis and Implications of Korean Health Authorities' Fulfillment on World Health Organization's Recommendations for Mask Use: Focused on COVID-19)

  • 이 연구는 코로나 19 확산에 따른 WHO의 마스크 사용 지침에 대한 한국 보건당국의 이행 및 불이행 내용을 분석하고 그것의 원인과 대안 선택의 결과를 도출하여 전염병 예방에서 지침의 한계와 연구의 함의를 제시하는데 목표가 있다. 이를 위해 국내·외 마스크 관련 문헌들을 고찰하고 WHO의 마스크 지침과 한국 보건당국의 마스크 권고사항을 분석대상으로 한다. 분석 결과는 다음과 같다. 첫째, 한국의 보건당국은 두 차례의 권고 사항에서 WHO의 "제한된 마스크 사용자 관점"을 기본적으로 수용하고 적용해왔다. 둘째, 한국의 보건당국은 마스크 수급이 부족해지면서 WHO 마스크 지침과는 상이한 이행을 하였다. 셋째, 한국의 보건당국은 보편적 마스크의 필요성에 따른 마스크 공급 안정화를 기반으로 코로나 19 초기 방역에 성공할 수 있었다. 넷째, WHO 마스크 지침에서 마스크 사용자 제한은 결과론적으로 지침으로서의 부분적 오류를 드러냈다. 분석에서 도출된 함의는 의학적 근거에 따른 국제 지침일지라도 일단 감염병 팬데믹 상황에선 국제 규칙의 엄격한 준수보다 각국의 방역상황 및 역량에 따라 자율적인 지침을 따르도록 허용할 필요가 있다는 점이다.

국제학술대회 참가자들을 위한 정보추천 서비스 (Recommending Talks at International Research Conferences)

  • 본 논문에서는 국제학술대회 참가자를 위한 개인화 된 정보추천서비스를 제안한다. 국제학술대회에서는 많은 논문들이 동시에 여러 세션으로 구성되어 발표되고 여러 연구관련 활동들이(예를 들어, 튜토리얼, 산업계토론, 공동연구논의 등) 짧은 기간 동안 이루어지므로 발표되는 논문들을 일일이 확인하고 그 발표에 참여하기가 쉽지 않다. 또한 학술대회의 정보 추천은, 기존의 영화, 책, 음악 등의 상품추천과 달리, 이미 정해진 해당 연구관련 커뮤니티가 대회 참가자들 및 발표자들을 중심으로 구성되어 있으므로 보다 명확한 소셜네트워크 기반추천 서비스가 가능하다. 본 논문에서는 각 학술대회에서 발표되는 논문들의 내용은 무엇인지, 참가자들이 어떤 논문에 관심을 가지는지, 그리고 각 참가자들이 다른 참가자들과의 맺은 소셜네트워크 등의 정보를 통해 발표에 참여할 만한 논문들을 추천하였다. 특히, 실제 운용되고 있는 국제학술대회 정보시스템, Conference Navigator를 이용하여, 여러 학술논문 관련 추천서비스를 비교 실험하였다. 기존의 Collaborative filtering 추천 알고리듬뿐만 아니라 학술대회참가자들의 소셜네트워크 기반 추천 서비스를 제공하였으며 연구결과 Cold-start 사용자들에게 특히 소셜네트워크 기반추천이 가장 좋은 결과를 보여주었다.

수용개작을 통한 뇌실외배액 간호 실무지침 개발 (Development of Nursing Practice Guideline for External Ventricular Drainage by Adaptation Process)

  • Purpose: This study was done to develop an evidence-based external ventricular drainage (EVD) nursing practice guideline in order to provide standardized nursing and prevent EVD related complications. Methods: We used the standardized methodology for nursing practice guideline adaptation developed by Korean Hospital Nurses Association for the guideline adaptation process in this study. Results: The newly developed EVD nursing practice guideline was adapted to the American Association of Neuroscience Nurses (AANN)'s clinical practice guideline which is 'Care of the patient undergoing intra-cranial pressure monitoring/external ventricular drainage of lumbar drainage.' There were 61 recommendations documented in the preliminary guideline all evaluated by 9 experts based on acceptability and applicability. The final practice guideline was composed of 3 domains with 57 recommendations. The three domains of nursing were the insertion, maintenance, and removal of the EVD. The number of recommendations in each domain was 8 in EVD insertions, 39 in EVD maintenance, and 10 in EVD removals. Of the 57 recommendations 3.5% were level 1, 31.5% were level 2, and 65% were level 3. Conclusion: The standardized practice guideline can improve nurses' performance and accuracy. It can also be used as the foundation for effective communication between all medical staff.

연약지반 TBM 터널 막장지보 계산방법 해설 (Commentary on Soft Ground TBM Tunnel Face Support Calculation Methods)

  • 최근에 독일터널위원회(German Tunnelling Committee DAUB)에서 TBM 막장지보의 새로운 계산법을 발표하였다. 이에 따라 보쿰시에 위치한 Ruhr 대학의 Zdenek Zizka와 Markus Thewes가 새로운 방법에 대해 활발한 논의를 하였다. 이러한 권유사항은 다양한 터널막장 지보 계산방법에서 지반조건에 따라 가장 적절한 계산법을 선정하는데 도움이 된다.또한, 막장에 적용되는 지반압 혹은 지하수압에 따른 막장안정 계산법들을 논하였다. 이 보고서에는 Zdenek Zizak and Markus Thewes의 터널 막장지보 계산법에 대한 기술적 논의를 구체적으로 설명하는 것이 목적이다.

한국의 항공로 설계의 법적 근거에 관한 연구 (A Study on Standards for Establishing ATS Routes Defined by VOR in Korea)

  • This paper addresses legislation status of air route design standard of Korea. Korea Ministry of Construction and Transportation's Order Number 300 (March 28, 2001) prescribes that firstly, the design of air routes in Korea should be in accord with International Civil Aviation Organization(ICAO)'s recommendations published in Annex 11: Air Traffic Service and Procedures for Air Navigation Services: Aircraft Operations (ICAO Doc 8168-OPS, 1986), and secondly, United States Federal Aviation Administration(FAA)'s Terminal Instrument Procedures (FAA, 1976). An investigation on the current air route specifications of Korea reveals that they are in accord with neither ICAO's recommendations nor FAA's standards in that the width of Korea's air routes are narrower that the international recommendations and standards. As a member state of ICAO, it is imperative for Korea to follow the international standards. In that purposes, Korea should legislate and regulate the detail standard for air route design. For the legislation, FAA regulations are to be followed because FAA rules contains more detail specifications while ICAO recommendations includes allows a range of policy decisions, which may result in a confusion in practice.

Recommendations for Improving Incentive Systems in the Building Sector of South Korea

  • Purpose: Reducing energy consumption and greenhouse gas emissions is a primary concern throughout the world, and the building sector is a particularly efficient area for making these reductions. In South Korea, the government has recently enacted policies for "Green Growth" that, among other things, enforce regulations in the building certification rating system (BCRS) and reorganize existing incentive systems. Method: In this study, we examined regulations and incentive systems used in the United Kingdom, Germany, and the United States that encourage the use of energy efficient technologies in construction and compared these policies to those used in South Korea. We also disseminated surveys to experts in the fields of architecture, planning and design, and engineering to better understand their knowledge and perception of the BCRS and its incentive systems. Additionally, we sought their recommendations for improving these incentive systems. Result: Based on our comparative case studies of regulations and incentives in other countries, alongside recommendations from experts in South Korea, we concluded that incentive systems in South Korea are limited and require improvement. We make recommendations for strengthening existing regulations and incentives and for implementing new incentive programs.

고혈압 환자의 건강 문해력과 질환관련 지식 및 치료지시 이행에 관한 연구 (A Study on Relationships between Health Literacy, Disease-related Knowledge and Compliance to Medical Recommendations in Patients with Hypertension)

  • Purpose: To survey relationships between health literacy, disease-related knowledge and compliance to medical recommendations in patients with hypertension. Methods: The subjects were 315 patients conveniently selected from public health centers in Gangwon province. Data were collected through a questionnaire (from January 10, 2013 to February 10, 2013) and analyzed by descriptive statistics, t-test, ANOVA, Duncan test and Pearson's correlation coefficient using the PASW statistics 18.0 program. Results: There were significant differences in health literacy according to age, marital status, monthly family income and education. In regard to disease-related knowledge, there were significant differences according to gender, marital status and monthly family income. There were positive correlations between health literacy and disease-related knowledge (r = .39, p = <.001), disease-related knowledge and compliance to medical recommendations (r = .28, p = <.001). However, there was no correlation between health literacy and compliance to medical recommendations. Conclusion: Health literacy and disease-related knowledge have a significantly positive correlation. Further studies are needed to confirm the effects of health literacy on chronic diseases.

비만치료 및 체중 감량에서 방풍통성산과 방기황기탕 사용에 대한 임상 권고안 (Clinical practice recommendations for Bangpungtongseong-san (Bofutsusho-san) and Bangkihwangki-tang(Boiogito) in obesity)

  • Objectives These prescribing recommendations have been written to guide clinicians on the appropriate use of Bangpungtongseong-san(BT) and Bangkihwangki-tang(BH) in the treatment of obesity. These recommendation are aimed at providing evidence based information concerning diagnosis and management of obesity. Methods We collected all relevant references about treatment effect of BT and BH on obesity in the forms of meta-analysis, systematic review, randomized controlled trial, case-control study, observational study and practice guideline from international and domestic databases and paper journals. We examined treatment effect, side effects, recommendations for dose, indication and contraindication of BT and BH. Results The treatment effect of BT and BH on obesity has been proved through clinical trial. BT is indicated for obese patients (Body mass index, $BMI{\geq}25$) with strong abdomen and a tendency to constipation, BH is indicated for obese patients ($BMI{\geq}25$) with a fair skinned, soft muscled, edematous and sweat easily. Conclusion We wish the information contained in theses recommendations will help clinicians reach a reasonable and beneficial decision with evidence-based results. Further studies are strongly needed to develop better treatment strategies for herbal medicines on obesity.

국민 건강증진을 위한 신체활동 지침 (Health-enhancing Physical Activity Guidelines for Koreans: the Status and Directions for Revision)

  • Objectives: There have been no specific as well as evidence-based physical activity guidelines issued on the governmental level. There just have been physical activity recommendations and guidelines released from governmental agencies or organizations as well as individual researchers and these are not consistent, thereby giving the public and even some professionals quite a confusion. As such, this study was aimed to suggest key factors to include when issuing national physical activity recommendations for Koreans. Methods: Since chronological evolution of modern physical activity recommendations reflects key factors to consider when establishing those recommendations and guidelines, that released for several decades was reviewed. Results: The evolution was found to be based on the basis of strong evidence from both epidemiological and exercise physiology studies and the key concepts of such evolution were feasibility, efficacy, and safety. Conclusion: On the basis of three key factors found in the review process, this study concluded that the upcoming national physical activity guidelines should include the following to maximize the outcomes of the efforts which have long been put by the division of physical activity under the 'New Health Plan 2010'. First, guidelines for achieving health benefits (ie, preventing chronic diseases such as cardiovascular disease, diabetes, and cancers or weight management) as well as improving cardiorespiratory fitness should be included to enhance efficacy. Second, safety measures should be included to enhance safety. Third, various alternative forms of physical activity should be included to enhance feasibility.
