Destructive Radiologic Development of Intravascular Papillary Endothelial Hyperplasia on Skull Bone

  • Intravascular papillary endothelial hyperplasia (IPEH) is a rare vascular benign lesion that rarely involves the central nervous system with or without skull invasion. We report a rare case of IPEH on the skull bone, which displayed destructive radiologic development associated with hemorrhage. A 14-year-old male presented with an incidentally detected a small enhancing, left frontal osteolytic lesion. Previously, he underwent operation and received adjuvant chemoradiation therapy for cerebellar medulloblastoma. Follow-up magnetic resonance imaging revealed a left frontal bone lesion, which expanded to an approximately 2 cm-sized well-circumscribed osteolytic lesion associated with hemorrhage for 20 months. Frontal craniectomy and cranioplasty were performed. Destructive change was detected on the inner table and diploic space of the skull. The mass had a cystic feature with hemorrhagic content without dural attachment. Pathologic examination showed the capsule consisted of parallel collagen lamellae representing a vascular wall, vascular lumen, which was pathognomonic for IPEH. Immunohistochemical staining revealed that the capsule was positive for CD34 and factor VIII, which favor the final diagnosis of IPEH. This was the first case of intracalvarial IPEH.

  • Highly sensitive $CaSO_4:Tm$ TLD is fabricated and their trap parameters are determined. The glow curve of $CaSO_4:Tm$ consists of three glow peaks and these peaks are isolated by the thermal bleaching method. The isolated glow peaks are fitted by a least squares method. The activation energies are 0.68, 0.82 and 1.03 eV. The frequency factors are $8.09{\times}10^8,\;9.14{\times}10^8$ and $1.03{\times}10^9/s$, and the kinetic orders are 1.37, 1.54 and 1.68, respectively. The optimum temperature range of the main peak for radiation dosimetry is between 220 and $290^{\circ}C$.

  • Purpose: To clarify the clinical and radiologic parameters that can be used to differentiate odontogenic keratocyst and ameloblastoma Materials and Methods: The records of 46 patients of ameloblastoma and 48 patients of odontogenic keratocyst at the Yonsei University Dental Hospital during the period of 1979 to 1995 were retrospectively reviewed. As a possible means for differentiating between the odontogenic keratocyst and ameloblastoma, the clinical parameters and the radiologic parameters were evaluated. Results: In the clinical parameters, there was no significant difference in age, sex, and sign and symptoms (p>0.05). In the radiologic parameters, there was significant difference in site, shape of the lesion, and external root resorption of adjacent teeth (p<0.05). Conclusion: The site, shape of the lesion, and external root resorption of adjacent teeth can be the parameters to differentiate odontogenic keratocyst and ameloblastoma, but a definite differentiation of these two lesions needs a more specialized imaging modality.

  • Spermiogenesis and mature spermatozoa of the Korean false minnow Pseudogobio esocinus (Cyprinidae) are described by means of scanning and transmission electron microscopy. Spermiogenesis is characterized by lateral development of the flagellum, absence of nuclear rotation, and eccentric nuclear fossa formation. The spermatozoa have a spherical head containing a nucleus with highly condensed chromatin and no acrosome. The nuclear fossa contains the proximal centriole and anterior part of the distal centriole. The midpiece is type A II and contains semi-fused mitochondria around the axoneme. However, the symmetrical distribution of 4 or more the mitochondria does not follow a general pattern of the cyprinid spermatozoa. Cytoplasmic vesicles in the midpiece are common in Cypriniformes and some of Siluriformes and Characiformes spermatozoa.

The Value of Facial Bone CAT Scan in the Diagnosis of the Nasal Bone Fracture (비골골절의 진단시 전산화단층촬영의 유용성)

  • Purpose: The nasal bone fracture is the most common type of facial bone fracture. In making a diagnosis, physical findings are much more important than the simple radiologic findings. Facial bone CAT scan can provide the accurate diagnosis and the correct location of nasal bone fractures, so it can be lessoned with proper intervention. The aim of this study was to evaluate the usefulness of facial bone CAT scan in the diagnosis of nasal bone fractures. Methods: The medical records and facial bone CAT scan of 45 patients clinically suspected nasal bone fracture but was not diagnosed on simple radiologic findings were analyzed. Results: All of the 45 patients were confirmed as nasal bone fractures in the facial bone CAT scan. The most common cause of fracture was assault. The mean age was 23.2 years. Physical findings were tenderness (100%), swelling(93.3%), epistaxis(66.6%), deviation (42.2 %), external wound(17.7%) and crepitus(4.4%) in order. Conclusions: It was concluded that the simple radiologic findings can not be conclusive, where the physical findings indicate a suspected nasal bone fracture. The facial bone CAT scan was more reliable for the correct diagnosis and follow-on treatment.

Difficult Intubation in Patients Undergone Oromaxillary Surgery: Retrospective Study (전신마취를 시행한 구강외과 환자에서 어려운 기관내삽관: 후향적 연구)

  • Background: This retrospective study aims to describe the airway management and to search predictive parameter for difficult intubation in 700 patients undergoing oromaxillary surgery. Methods: The medical records of 700 patients undergone oromaxillary surgery were reviewed for airway management during perioperative period. The cases of difficult intubation were selected and those radiologic findings were reviewed. The mandibular depth (MD), mandibular length (ML), thyromental distance (TMD) were measured. Results: In 41 cases difficult intubation were recorded in anesthetic record. The grade of Cormack and Lehane was III in 36 patients and IV in 5 cases. The MD of difficult intubation cases was $4.2{\pm}3.2\;cm$. The ML of difficult intubation cases was $10.1{\pm}3.8\;cm$. The TMD of difficult intubation cases was $5.9{\pm}4.3\;cm$. Under the fiberoptic guided awake intubation was undertaken in 75 patient. In none of the cases was failed nasotracheal intubation. Conclusions: The patients undergoing oromaxillar surgery have a potentially difficult airway but, if managed properly during perioperative preiod, morbidity and mortality can be reduced or avoided. The radiologic findings were poor predict for difficult intubation. The fiberoptic guided awake intubation is a safe alternative to direct laryngoscopic intubation.

Postoperative Evaluation of Funnel Chest - The role of radiologic indices -

  • There are many objective indices of operative outcome of funnel chest, but there has been only a few report about the postoperative change of these indices and which is best correlated with subjective outcome. From January 1989 to December 1991, 74 patients with funnel chest underwent Ravitch operation and radiologic evaluation was performed on the twenty patients whose operative outcome were satisfactory by Humphreys` criteria and postoperative chest X-ray were available. Variable radiologic chest idices such as deformity grade [DG], lower vertebral index [LVI], upper vertebral index [UVI] and configuration index [CI] were measured preoperatively and 1, 6, 12 and 18 months pos-toperatively. The mean value of preoperative DG, LVI, UVI and CI were 5.32, 0.3, 0.26 and 1.01 respectively, and postoperative chages were as follows; DG; 3.13, 3.66, 3, 72, 4.0, LVI: 0.22, 0, 24, 0.25, 0.25 UVI; 0.27, 0.27, 0.27, 0.27 CI; 0.69, 0.77, 0.76, 0.79. The increments of CI were within the 90 percent confidence limit of normogram in most cases, but it was not true for LVI and UVI. And the differences between the observed and expected values of CI did not change significantly through the postoperative periods, which was not the case in LVI. We concluded from these results that 1] DG, LVI and CI were increased slowly during the postoperative follow up periods but the increments of CI were usually within the normal limits and so 2] CI can be used as a objective index of satisfactory outcome.

Regional Hospital Medical Equipment Operation of Adequacy Assessment (지역 의원 의료장비운영에 따른 적정성 평가)

  • Due to the increasing quantity of health services demand a wide range of growing medical markets and hospital through advanced, diversification can meet the needs of the patient. This hospital is having the introduction of expensive medical equipment. Because of this, hospitals are struggling to operate the hospital management. Therefore, when operating at the hospital before and after introduction of the most important factor in the interests of medical equipment through the analysis is directly related to hospital operations. CT study of the local clinics, or CR 40 where members of the medical equipment with 54 points against selected practitioners and hospital visits and interviews to investigate the expected benefits of each of the members raised an average of seven months to identify revenue performance caused by interests. Raised against the expected benefits resulting benefits to the overall average 87.43% did not meet the expected benefits. The hospital plans to introduce future when reviewing the medical equipment in consideration of the characteristics of the equipment requires a more solid plan, and whether the plan is properly implemented will be needed for the institutional verification capabilities.

  • In this experiment, how DEXA(Dual-energy X-ray Absorptiometry) bone mineral density was measured using the equipment. In order to maintain the same measurement conditions, bone mineral density measurements of 10 cm thick phantom, with an actual patient at a point when examining the same conditions(100 kVp, 1 mA) and then out to the five doses of radiation and its average was calculated by dividing measured. X-ray dose rate measured at the Research Institute, Sword of the gamma survey meters calibrated MEDCOM Ltd. (Inspector GM counter tube) was used, calibration factor is 1.15. On a horizontal plane around the patient, depending on the distance was significantly reduced dose rate. In addition, orientation $0^{\circ}$ head end was higher in the direction of the highest dose rate, $0^{\circ}$ $180^{\circ}$ direction from the direction towards the higher dose rate reduced to some extent in the direction of all the $120^{\circ}$ were able to identify.

  • Multileaf collimators whose Pb leaves are moving in two-dimensional directions have been used. We propose a different concept three-dimensional (3D) collimator with 3D shape that is automatically changeable to modulate the radiation dose even for complex tumors in real time. A voxel collimator, including a hinged Pb plane and a 3D assembly of many voxel collimators, was used. In each frame rotation axis, a motor, which was controlled by a circuit with field-programmable gate array (FPGA) board connected with computer, was operated according to a predetermined plan. Simulations of that, which are generally used for planning, were performed and compared with experimental results.
