Tissue 02 tension is an important guide in detection of the general condition in critical patients. The tissue 0, is more difficult to measure with 0, sensor in skeletal muscle and subcutaneous tissues to present. But it is much easier to measure 0, tension in bladder urine with Censini catheter in Foley catheter than in tissue. We have measured 0, tension in bladder urine, main pulmonary artery and radial artery in 16 patients in chest surgical department of Yonsei University. College of Medicine from September 26 to December 22, 1981. Six patients were male and ten patients were female. Their ages ranged from 8 to 43 years. The correlation equation between the simultaneously measured PuO2 and PvO2 was found to be Ypvo2=4.04 + 0.88 Xpuo2 [r=0.88, p<0.0001] in regression curve with computer [HP/3,000, Program: SPSS] in the Yonsei University. Measurement of 0, tension in bladder urine and MPA will be rather simple, rapid and reproducible method than that of the 0, tension in tissues. But the speed of 0, consumption in urine is fast and so the 0, tensions in bladder urine were measured as soon as possible after they were collected. They were no complications or morbidity during measurement of 0, tension in these procedures except spontaneous removal of radial arterial cannulas in 2 patients.
Introduction: The hand and wrist are particularly susceptible to electrical burn. Skin defect with damage or exposure of underlying vital structure requires coverage by skin flap especially in case of the need for late reconstruction. We are reporting 4 cases of electrical burned hand treated by posterior tibial arterial free flap. The commonly used skin flaps such as scapular flap or groin flap are too bulky so that they are not satisfactory in function and cosmetic appearance. So we tried to cover them with a more thin skin flap. Materials and Method: From January 2002 to June 2003, four cases of hand and wrist electrical burn were covered using posterior tibial arterial free flap. All the cases were due to high voltage electrical burn. Age ranged from 31 years to 38 years old and all the cases were male patients. Recipient sites were 2 wrist, one thenar area and one knuckle of 2.3rd MP joint. Additional procedures were flexor tenolysis (simultaneous), FPL tenolysis and digital nerve graft (later) and extensor tendon reconstruction (later). Result: All the flap have survived totally without any complication including circulatory concern about the donar foot. Posterior tibail arterial free flap was so thin that debulking procedure was not required. Conclusion: For skin coverage of the hand & wrist region, posterior tibial arterial free flap have many advantages such as reliable anatomy, easy dissection and easy anastmosis with radial or ulnar artery and possibility of sensory flap. The most helpful advantage for hand coverage is its thinness. So we think this flap is one of the very useful armamentarium for reconstructive hand surgery.
Purpose: Deviations of arterial palmar arches in the hand can be explained on the embryological basis. The purpose of this study was to provide new information about palmar arches through cadaver's dissection. The values of the location and diameter in these vessels were analyzed in order to support anatomical research and clinical correlation in the hand. Methods: The present report is based on an analysis of dissections of fifty - three hands carried out in the laboratory of gross anatomy. A reference line was established on the distal wrist crease to serve as the X coordinate and a perpendicular line drawn through the midpoint between middle and ring fingers, which served as the Y coordinate. The coordinates of the x and y values were measured by a digimatic caliper, and statistically analyzed with Student's t - test. Results: Complete superficial palmar archs were seen in 96.2 % of specimens. In the most common type of males, the superficial arch was formed only by the ulnar artery. In the most common type of females, the superficial arch was formed anastomosis between the radial artery and the ulnar artery. The average length of the superficial and deep palmar arch is $110.3{\pm}33.0mm$ and $67.9{\pm}14.0mm$ respectively. Regarding the superficial palmar arch, ulnar artery starts $-16.1{\pm}5.1mm$ on X - line, and $2.5{\pm}24.5mm$ on Y - line. Radial artery appears on palmar side $7.7{\pm}3.2mm$ on X - line, and $20.9{\pm}10.9mm$ on Y - line. But radial artery starts on $6.3{\pm}3.6mm$ on X - line, and $3.4{\pm}5.1mm$ on Y - line. Digital arteries of superficial palmar arch starts on $6.1{\pm}3.7mm$, $33.9{\pm}8.8mm$ on index finger, $1.8{\pm}3.4mm$, $40.1{\pm}7.3mm$ on middle finger, $-3.2{\pm}4.9mm$, $42.6{\pm}7.0mm$ on ring finger, and $-8.9{\pm}5.1mm$, $42.5{\pm}80mm$ on little finger in respective X and Y coordinates. Radial artery of deep palmar arches measured at the palmar side perforating from the dorsum of hand. It's coordinates were $9.7{\pm}4.8mm$ on X - line, $21.7{\pm}10.2mm$ on Y - line. Ulnar artery was measured at hypothenar area, and it's coordinates were $-20.4{\pm}6.3mm$ on X - line, and $30.6{\pm}7.4mm$ on Y - line. Conclusions: Anatomically superficial palmar arch can be divided into a complete and an incomplete type. Each of them can be subdivided into 4 types. The deep palmar arch is less variable than the superficial palmar arch. We believe these values of the study will be used for the vascular surgery of the hand using the endoscope and robot in the future.
Extracellular recordings were made from the spinal neurons in the lumbar enlargement of 16 cats before and during electrical stimulation of the radial nerve ipsilaterally and contralaterally. Only neurons activated by remote nerve stimulation (RNS) were included in sample. All the cell classes of spinal neurons which received afferents message from the skin and/or muscles were activated by RNS except LT cells. Approximately three quaters of cells activated by RNS had an inhibitory receptive field (RF) on the ipsilateral hindlimb and two thirds of RNS-activated neurons showed spontaneous activity. The most of these RNS-activated cells seemed to be in deep dorsal horn and in ventral horn as well. Stimulation of contralateral radial nerve produced activation of spinal neurons almost same degree as by ipsilateral nerve stimulation. The optimal stimulation parameters of radial nerve for activation of spinal cells were 5Hz-0.5 msec-2V while threshold stimulus for activation was approximately 0.18 V. Following close intra-arterial injection of $K^+$ ion excitability of RNS-activated neuron was increased in 4 of 8 cells whereas it was decreased in 2 of 8 cells. The results indicate that there are some spinal neurons in the lumbar enlargement of cats that can be activated by forelimb afferent $(A{\beta}\;&\;A{\delta})$ inputs.
Objective : To investigate the efficacy of extracranial-intracranial (EC-IC) bypass surgery using a radial artery interposition graft (RAIG) for surgical management of cerebrovascular diseases. Methods : The study involved a retrospective analysis of 13 patients who underwent EC-IC bypass surgery using RAIG at a single neurosurgical institute between 2003 and 2009. The diseases comprised intracranial aneurysm (n=10), carotid artery occlusive disease (n=2), and delayed stenosis in the donor superficial temporal artery (STA) following previous STA-middle cerebral artery bypass surgery (n=1). Patients were followed clinically and radiographically. Results : Bypass surgery was successful in all patients. At a mean follow-up of 53.4 months, the short-term patency rate was 100%, and the long-term rate was 92.3%. Twelve patients had an excellent clinical outcome of Glasgow Outcome Scale (GOS) 5, and one case had GOS 3. Procedure-related complications were a temporary dysthesia on the graft harvest hand (n=1) and a hematoma at the graft harvest site (n=1), and these were treated successfully with no permanent sequelae. In one case, spasm occurred which was relieved with the introduction of mechanical dilators. Conclusion : EC-IC bypass using a RAIG appears to be an effective treatment for a variety of cerebrovascular diseases requiring proximal occlusion or trapping of the parent artery.

Introduction: This study aims to investigate the effects of acupuncture stimulation on the radial artery's pressure pulse wave, along with various hemodynamic parameters, and to explore the possible underlying mechanism of pulse diagnosis in healthy participants in their twenties. Methods and analysis: This study is a prospective, single-arm, exploratory clinical study. A total of 25 healthy participants, without regard to gender, in their twenties will be recruited by physicians. Written informed consent will be obtained from all participants. The participants will receive acupuncture once at ST36 on both sides. The radial arterial pulse waves will be measured on the left arm of the subjects by using an applicable pulse tonometric device (KIOM-PAS). On the right arm (appearing twice), electrocardiogram (ECG), photoplethysmogram (PPG), respiration and cardiac output (CO) signals, will be measured using a physiological data acquisition system (Biopac module), while the velocity of blood flow, and the diameter and the depth of the blood vessel will be measured using an ultrasonogram machine on the right arm (appearing twice). All measurements will be conducted before, during, and after acupuncture. The primary outcome will be the spectral energy at high frequencies above 10 Hz ($SE_{10-30Hz}$) calculated from the KIOM-PAS device signal. Secondary outcomes will be various variables obtained from the KIOM-PAS device, ECG, PPG, impedance cardiography modules, and an ultrasonogram machine. Discussion: The results of this trial will provide information regarding the physiological and the hemodynamic mechanisms underlying acupuncture stimulation and clinical evidence for the influence of acupuncture on the pressure pulse wave in the radial artery. Ethics and dissemination: This study was approved by the Institutional Review Board (IRB) of Kyung Hee University's Oriental Medical Center, Seoul, Korea (KOMCIRB-150818-HR-030). The study findings will be published in peer-reviewed journals and presented at national and international conferences. Trial registration number: This trial was registered with the Clinical Research Information Service (CRIS) at the Korea National Institute of Health (NIH), Republic of Korea (KCT0001663), which is a registry in the World Health Organization's (WHO's) Registry Network.
In this paper, we have proposed and demonstrated a tonometry sensor array for measuring arterial pulse pressure. A sensor module consists of 7 piezoresistive pressure sensor array. Wire-bonded connection was provided between silicon chip and lead frame. PDMS(poly-dimethylsiloxane) was coated on the sensor array to protect fragile sensor while faithfully transmitting the pressure of radial artery to the sensor. Tonometric pulse pressure can be measured by this packaged sensor array that provides the pressure value versus the output voltage.
Aortic AIx(augmentation index) has been used to measure aortic stiffness and evaluate ventricular load quantitatively. Algorithm for the detection of augmentation point gradually increases the differential order to detect inflection point rather than detects the distinctive point that appears after a specific time. Developed algorithm for AIx is proved to provide more accurate results than the ones developed by previous studies with the deviation from $-11.5{\pm}14.34$ points to $-3.75{\pm}1.26$ points. Results could provide the basis for the measurement of aortic stiffness using easily-measurable radial artery pulse waves, and could be extended to develop a system for early diagnosis of various vascular diseases.

Upper extremity vascular injuries occurring with acute compartment syndrome are very challenging to manage in an emergency context in resource-poor settings. The need to always recognize the likelihood of coexisting compartment syndrome guides surgeons to perform concomitant fasciotomy to ensure a better outcome. We managed three vascular injuries in the upper extremities in two patients with concomitant imminent compartment syndrome observed intraoperatively. The first injury was complete brachial artery disruption following blunt trauma, while the second and third injuries were radial and ulnar artery transection caused by sharp glass cuts. Both patients were treated with vascular repair and fasciotomy. Secondary wound coverage was applied with split-thickness skin grafting, and the outcomes were satisfactory. Concomitant fasciotomy potentially improves the outcomes of vascular repair in emergency vascular surgery and should be considered for all injuries with the potential for acute compartment syndrome.
배경: 관상동맥 우회술에서 요골동맥 이식편의 사용이 증가하는 추세이나 혈관조영술을 이용한 요골동맥의 개통률 분석은 내흉동맥이나 복재정맥에 비해 적다. 대상 및 방법: 요골동맥을 이용한 관상동맥 우회술 후 2001년 1월부터 2006년 6월 사이에 재입원하여 혈관조영술을 시행받은 132명의 환자를 대상으로 하였다. 남자 92명, 여자 40명이었으며 수술 후 평균 32개월($2{\sim}110$개월)에 혈관조영술을 시행하였다. 수술 전 관상동맥 병소의 분포는 1혈관 질환이 7명, 2혈관 질환이 38명, 3혈관 질환이 87명이었으며 좌 주관상동맥 질환 46명이 포함되어 있었다. 인공심폐기를 사용한 경우(on-pump)가 58명, 인공심페기를 사용하지 않은 경우(off-pump)가 74명이었다. 결과: 관상동맥 조영술 결과 이식혈관의 평균 개통률은 91.2% (376/412)였으며 on pump군과 off pump군 사이의 개통률에 대한 통계학적 차이는 없었다(p=0.125). 수술 전 관상동맥 협착의 정도에 따른 요골동맥의 개통은 90% 이상의 협착이 있던 110개소 중 완전 개통을 보인 곳이 98개소(89%), 폐쇄된 경우가 8개소(7.3%)였으며, 90% 이하의 협착이 있던 76개소의 경우는 완전 개통이 60개소(78.9%), 폐쇄된 경우가 9개소(11.8%)로 나타나 수술 전 관상동맥에 90% 이상 협착이 있었던 군이 90% 미만의 협착이 있었던 군보다 높은 경향을 보였다(p=0.057). 폐쇄된 요골동맥 이식편의 관상동맥 문합 부위는 좌전하행지가 7군데 중1개소(14.2%), 대각지가 40군데 중 1개소(2.5%), 회선지가 96군데 중 6개소(6.2%), 우 관상동맥이 43군데 중 9개소(20.9%)로 우 관상동맥의 개통률이 가장 저조하였다(p=0.027). 요골동맥 근위부 문합방법에 따른 요골동맥이식편의 개통률은 3군간에 차이가 없었다(p=0.078). 결론: 요골동맥은 관상동맥 우회술에서 중기 혈관 조영술 상 만족할 만한 이식편이라 생각하지만 우 관상동맥에 이식하거나, 90% 이하의 관상동맥 협착에 이식시 개통률이 저조하였다. 인공심폐기의 사용 유무나 근위부 문합방법에 따른 개통률의 차이는 없었다.
