Objectives: To investigate the different effects of Sa-Am acupuncture with Spleen Jung-gyuck and Spleen Seung-gyuck on the radial pulse in healthy subjects. Methods: Sixty healthy volunteers (30 males and 30 females) participated in this study. The participants were randomly divided into three groups: control (C), Sa-Am acupuncture with Spleen Jung-gyuck (SP+) and Sa-Am acupuncture with Spleen Seung-gyuck (SP-). The radial pulse was measured using a multi-step tonometry system before, and 0, 30 and 60 minutes after acupuncture. Results: The heart rate adjusted radial augmentation index (RAI/HR) and high-tensioned pulse area (W area) significantly increased in the SP- group compared to the C and SP+ groups in males. The systolic pulse period (T4) increased significantly in the SP- group compared to the C and SP+ groups in females. The RAI/HR had positive correlations with W area and T4. Conclusion: The effects of Sa-Am acupuncture with Spleen Jung-gyuck and Spleen Seung-gyuck are different. Sa-Am acupuncture with Spleen Seung-gyuck induces acute increases of radial pulse parameters related to arterial stiffness in healthy subjects.
동맥전단부를 연구할수있는 새로운 실험방법과 기계역학적 분석방법을 제시하였다. 지금까지동맥역학적 연구는 대부분이 동맥의 길이방향과 원주방향에 대한것이 이였다. 두께방향의 변형은 포아손비라든지 비압축성가정으로 이론적으로 결정되었다. 또한 두께에 걸친 변형의 변화도 무시되었다. 그러나 병리학적인 의미에서 동맥의 두께에 걸친 변형도와 변형의 분포는 중요한 의미를 가진다. 그러므로 본연구에서 제안된 실험방법과 장치는 두께전반에 걸친 변형을 측정할수 있게 했다. 또한 전단부의 부위별 변형도의 관찰이 가능하고 병리적인 동맥경화증에 대한 현상과 역학적현상을 상관시킬 수 있음에 중요한 의미를 들 수 있다.
Objective: Increased aortic and carotid arterial augmentation index (AI) is associated with the risk of cardiovascular disease. The most widely used approach for determining central arterial AI is by calculating the aortic pressure waveform from radial arterial waveforms using a transfer function. But how the change of waveform by applied pressure and the pattern of the change rely on subject's characteristics has not been recognized. In this study, we use a new method for measuring radial waveform and observe the change of waveform and the deviation of radial AI in the same position by applied pressure. Method: Forty-six non-patient volunteers (31 men and 15 women, age range 21-58 years) were enrolled for this study. Informed consent in a form approved by the institutional review board was obtained in all subjects. Blood pressure was measured on the left upper arm using an oscillometric method, radial pressure waves were recorded with the use of an improved automated tonometry device. DMP-3000(DAEYOMEDI Co., Ltd. Ansan, Korea) has robotics mechanism to scan and trace automatically. For each subject, we performed the procedure 5 times for each applied pressure level. We could thus obtain 5 different radial pulse waveforms for the same person's same position at different applied pressures. All these processes were repeated twice for test reproducibility. Result: Aortic AI, peripheral AI and radial AI were higher in women than in men (P<0.01), radial AI strongly correlated with aortic AI, and radial AI was consistently approximately 39% higher than aortic AI. Relationship between representative radial AI of DMP-3000 and peripheral AI of SphygmoCor had strongly correlation. And there were three patterns in change of pulse waveform. Conclusion: In this study, it is revealed the new device was sufficient to measure how radial AI and radial waveform from the same person at the same time change under applied pressure and it had inverse-proportion to applied pressure.
Purpose: An extensive knowledge of the arterial anatomy of the upper extremity and its variations is indispensable to the hand surgeon. We report a patient with anomalous radial artery, superficial course of two radial arteries, encountered during the excision of volar wrist ganglion. Methods: The patient was a 53-year-old man who had a painful mass on the left volar wrist for 1 year. Under general anesthesia, a curved incision was made around the mass. With the skin flaps retracted, the dome of the cyst was identified. Particular care was taken to identify and protect the radial artery, which was intimately attached to the wall of the ganglion. Two radial arteries completely encircled the ganglion. The pedicle was traced to the volar joint capsule, radiocarpal ligament. The joint was open and the capsular attachments were excised. Results: The patient made an uneventful recovery. There were two arterial pulsations at the volar side of the wrist joint. Compressing this site revealed that the major arterial contributor to blood supply in the hand was the ulnar artery. At angioCT, an anomaly of the radial artery was found with a duplication. The pathway of this aberrant artery was superficial to the original radial artery. It changed its course subcutaneously at the level of the tendon of the brachioradialis muscle, and crossing the wrist lateral to the original radial artery and ending in the deep palmar arch. Conclusion: Authors experienced a case of bifurcating radial artery encountered during the excision of ganglion on the volar of the wrist. Because these duplicated radial arteries make strong contributions to the thumb and index finger as well as to the deep palmar arch, when they are present there may be probably less blood supply to the hand from the ulnar artery. If the radial artery is palpated superficially on the brachioradialis muscle, it is important to remember the kind of anomaly.
광전용적맥파는 인체의 말초 부위에서 맥파를 검출하는데 널리 이용되고 있으며 심박동수 측정 및 혈액산소포화도 측정에 이용되고 있다. 본 연구는 940 nm의 적외선을 이용한 광전용적맥파를 이용하여 손목에서 요골동맥의 맥파를 기록하였으며 센서의 측정 위치에 따라 맥파의 모양이 다르게 나타나는 것을 관찰하였다. 요골동맥 바로 위의 피부부위에서 기록되는 광전용적파는 손가락 끝에서 기록되는 맥파에 비하여 반전되어있는 특성을 보였다. 반전된 맥파의 발생기전은 센서의 발광부에서 나오는 940 nm의 적외선이 조직에 입사하여 반사되어 나오는 경로의 용적 변화가 심장주기에 따라 일어나지만 손가락 끝부분에 비하여 반대 작용이 있음을 나타낸다. 요골동맥의 바로 위 피하 조직은 심장주기에 따라 동맥의 확장 또는 수축이 발생하면 동맥외벽과 피부 사이 조직에 혈류량의 변화가 일어나며 동맥의 확장 시에는 혈류량이 감소하고 이완시에는 증가되는 특성으로 인하여 반전맥파를 형성하는 것으로 추측된다. 이러한 결과는 요골동맥의 벽에서 적외선 반사가 있음을 시사한다.
Increasing interest in the use of arterial conduites is based on the better patency of the internal thoracic artery(ITA) than the saphenous vein graft and the hope that other arterial conuits will perform similarly over the long term. Material and Method: Between May 1997 and July 1998, 43 patients underwent coronary artery bypass grafting with ITA and the radial artery(RA). There were 28 men and 15 women with a mean age of 61.5 years(range, 35 to 78). In 43 patents, 30 bilateral ITA(including 7 diabetes mellitus, 5 more older 70 years), 8 bilateral ITA only, 2 left ITA and both RA, 11 left ITA and left RA and 22 both ITA and left RA were used. Result: There was 1 hospital mortality. Of the 42 patients alive, 39 patients are asymptomatic. Postoperative complications were postoperative bleeding in 1 patients, and low cardiac output syndrome in 3. Follow-up angiography was performed in 5 patients after the operation(mean 3 months), and all ITA & RA grafts showed excellent results. Conclusion: We conclude that complete arterial revascularization with internal thoracic artery and radial artery is technically feasiale with low mortality and morbidity, and but long term follow-up is needed.
This paper describes an idea of design and construction for a radial pulse wave detector, In acquiring arterial pulse signal noninvasively, a new combinational fiber-optic transducer was used which has a detecting part and a sensing part. The mechanism of detecting part is composed of special form of structure that can detect changing pulses in contact with skin, and transmits arterial wall movements to the sensing part. It consists of elastic reflector and optical fibers, which are arranged in a fiber pair. Then, the intensity of the reflected light will be proportional to the displacement changed by inclined reflector. Using this transducer, it expected to make a clinically useful tool for arterial pulse wave diagnosis, especially on the application of "MACK- CHIN" in the field of Korean traditional medicine, since : his transducer has a simple structure and has an easy and clear signal acquisition method.on method.
광용적맥파는 심박과 동맥 혈액의 산소포화도 등을 측정하기 위하여 널리 사용된다. 본 논문에서는 요골동맥에서 780nm와 940nm 적외선 LED를 사용하여 PPG 신호를 측정할 수 있는 모니터링 시스템을 구현하였다. 구현된 시스템은 여러 상황에서 특성이 서로 다른 8종의 LED와 2종의 광전 센서를 조합하여 요골동맥에서 광전용적맥파를 측정하였다. 요골동맥에서 측정된 파형들은 반전되어서 기록된다. 이렇게 반전된 파형은 수축기의 신호는 감소시키고, 확장기의 신호는 증가시킨다. 시스템을 통하여 투과형 방식과 반사형 방식에서 각각의 신호 검출이 적합한 환경과 최적의 센서 조합을 파악하여 모니터링 시스템을 구현하고, 각 조합별 그 결과 피검자의 요골동맥에서 안정적으로 사용할 수 있는 심박신호 측정 시스템을 개발하였다.
Background: The safety and efficacy of arterial composite grafts for total arterial revascularization have been demonstrated. The saphenous vein (SV) is a widely used graft because of its accessibility, sufficient length, and ease of manipulation. Our aim was to compare mid-term outcomes of saphenous vein Y-grafts with radial artery Y-grafts joined by anastomosis to the left internal thoracic artery. Materials and Methods: Records of off-pump coronary artery bypass grafting with composite Y-grafts based on the left internal thoracic artery technique in 552 patients were analyzed retrospectively. After propensity score matching, 79 radial arterial (RA) composite grafts (RA group) and 79 saphenous vein composite grafts (SV group) were compared. The duration of mean follow-up was $24.6{\pm}14.6$ months (range, 1 to 55 months). Results: There were no differences in surgical mortality, all-cause mortality, or morbidity among the groups. Rates of 4-year survival were 91.7% and 96.3% in the RA and SV groups, respectively (p=0.519). The coronary reintervention-free survival rate and freedom from major adverse cardiovascular or cerebrovascular events were similar in the two groups (p=0.685, p=0.564). Conclusion: Construction of composite Y-grafts using the radial artery or saphenous vein showed similar mid-term results. Long-term follow-up and randomized trials will be needed to confirm our present conclusions.

Generally, arterial pulse waves are measured at the radial arterial of wrist or carotid arterial of neck using a sensor such as pressure sensor, piezoelectric sensor or optic sensor. But in this paper, arterial pulse wave is measured at the temple using PZT piezo sensor which is attached on the temple in form of a hair-band. Arterial Pulse waves are generally measured when a reagent is in a static state. But in this paper, we implemented the arterial pulse wave measurement system, as a previous stage of the arterial pulse wave measurement system for running at outdoors or on a running machine, that measures arterial pulse waves at the temple, which is the least moving part when running. Thorough the continuous study, if the motion artifact when running is possible to be removed, the system will be able to perform monitoring of running men's states and especially emergency signals such as serious pulse waves of an/old and feeble persons and handicapped persons.
