The objective of this report is to assess clinically and radiographically the state of the primary molars pulpotomized with a 15.5% ferric sulfate solution. The subjects selected were healthy children treated at the pediatric dental clinic of the Seoul National University Hospital in Korea. Thirty teeth were pulpotomized with a ferric sulfate solution(FS). Another twenty-one teeth were pulpotomized with 20% dilute formocresol(FC). Clinical and radiographic data for the fifty-one primary molars were collected with a mean follow-up period of 34 months. The success rate for the FS group was 80.0%. The success rate for the FC group was 81.0%. The differences in the results between the two groups were analyzed statistically utilizing the chi square test. External root resorption was observed in four teeth of FS group and four of the FC group. Periapical bone destruction was observed in three of FS group and two of FC group. There were no significant statistical differences between the success rates for FS group and the FC group.
Arsenic trioxide is one of the 'tooth pulp devitalizing agents' used through the dental history when anaesthesia was not available. But owing to its capacity to kill cells in surrounding tissues, the use of arsenic trioxide in vital pulpotomy has been reduced. Arsenic trioxide can cause necrosis of gingiva, bone which can cause osteomyelitis of the jaws. But some dentists still continue to use arsenic trioxide in their endodontic practices. The purpose of this article is to present arsenic trioxide induced osteomyelitis on maxilla and treatment process.
Calcium hydroxide has been used not only as pulp capping and pulpotomy agents in the operative dentistry, but dressing and temporary filling materials in root canal treatment. Calcium hydroxide was known to stimulate odontoblast to produce new reparative dentin and to eliminate microorganims effectively in the infected root canals. The purpose of this study was to evaluate the effect of calcium hydroxide solution on cultured L929 cells and its antibacterial effect on several streptococci. Calcium hydroxide solution (0.121g/100ml) was added to L929 cells and cell viability was measured using 3-(4,5-dimethylthiazol-2-yl) -2,5-dimethyltetrazolium bromide (MTT) and neutral red (NR) dye. Calcium hydroxide solution (20, 40, 60, 80, 100 and $150{\mu}l$) was added to L929 cells in 96-well microplates for 1, 4 and 24 hours respectively. Cell viability was gradually decreased when the volume and exposure time of calcium hydroxide solution were increased. When $150{\mu}l$ of calcium hydroxide was applied to L929 cells for 24 hours, there was more than fifty percent reduction of cell viability. Calcium hydroxide solution (20g/100ml) showed antibacterial effect against S. uberis, S. intermedius and S. mitis after thirty-second exposure. But 0.121g/100ml concentration of cacium hydroxide solution exhibited no antibacterial effect on six streptococci after one-hour exposure.

This study was undertaken to evaluate the pulpal responses to the pulp-capping materials such as glutaraldehyde and formocresol in pulpotomy technique, especially in the primary dentition. Mandibular primary canines and molars of 5 dogs (aged about 8-9 weeks)were selected for this study. The intervals of observation for histologic study of pulpotomized primary teeth with 2% glutaraldehyde, formocresol and calcium hydroxide in the usual manner ranged from 2 hours, 1 week, 2 weeks, 3 weeks and 5 weeks after experiments respectively. Each specimens were fixed with 10% formalin and decalcified in 5% nitric acid. All slides were stained with Hematorylin-Eosin and examined histopathologically. The results were as follows; 1. In calcium hydroxide groups, formation of dentin bridge was initiated in 1 week after experiments and completed in 5 weeks after experiments. 2. Formation of dentin bridge was not seen, whereas necrosis of pulp tissue was noted, in formocresol and glutaraldehyde groups. 3. Duration of tissue reactions and tissue changes were similar, in formocresol and glutaraldehyde groups. 4. In formocresol and glutaraldehyde groups, amputation surfaces of the pulp were covered with blood clots, beneath which coagulation necrois was noted, but inflammatory cells were not prominent, in 2 hours and 1 week after experiments. But coagulation necrosis was proceeded to the apical portion, accompanied by infiltration of inflammatory cells, since 2 weeks after experiments. And suppuration or gangrene of the pulp tissue were noted in 3 weeks and 5 weeks groups. 5. Suppuration or gangrene of pulp seemed to provoke the resorption of dentin wall, and inflammatory changes and resorption of roots were noted in the periodontal membrane near the periapical region. 6. As compared with calcium hydroxide groups, resorption of the root was pronounced in form or cresol and glutaraldehyde groups. Effects of medicaments to the succedaneous tooth germ were not seen.

Triple tooth is rare in primary dentition; it is the abnormal fusion of three teeth. There are a few reports of double teeth, but triple teeth are rare. These multiple teeth create several clinical problems, not only esthetic problems but also a high caries susceptibility, congenital missing permanent tooth germ(s), orthodontic problems, and periodontal problems. They also make it difficult to perform pulp treatment because of the complex tooth structure. A 1-year 7-month-old male underwent a partial pulpotomy with mineral trioxide aggregate (MTA) when pulp exposure caused by deep caries occurred at a maxillary anterior triple tooth in the first case. The second case was a 1-year 9-month-old male presenting with a fracture line at a mandibular anterior triple tooth. After removing the fractured fragment, a pulpectomy was performed at the remaining primary lateral incisors. Specific complications were not observed during 24 and 6 months of follow-up, respectively.

The pulp treatment of primary and immature teeth in the children and adolescents is affected by the environment of the patients, clinical careers and dentists performing the techniques, as well as clinical modalities and pulpal diagnosis. The purpose of this study was to investigate the current status of pulp treatment performed in real clinical practice, as well as the reasons behind the choice of treatment methods. The study was conducted on a total of 153 members of the Korean Academy of Pediatric Dentistry, and statistical data from the Health Insurance Review and Assessment Service (HIRA) from 2010 to 2018 were analyzed together. Both the survey results and the statistics from the HIRA showed that 1-visit pulpectomies of primary teeth were most common, and the frequency of the use of the Ni-Ti file was increased in children with primary or mixed-dentition. In the treatment of immature permanent teeth, regenerative endodontic treatment by individual dentists was found to be less frequent than in university hospitals.

The conserative treatment of the necrotic and infected primary molars is often necessary in order to avoid extractions which may lead to inability to masticate or loss of space and subsequent problems concerning the development of the occlusion in the permanent dentition. The purpose of this study was to evaluate the clinical significance of the conservative treatment of the infected primary molars with necrotic pulps employing the formocresol pulpotomy technique same as in vital teeth and surgical intervention in the cases with a parulis, fistula or cellulitis. The materials consisted of 25 primary molars of 9 boys and 4 girls ranging in age from 4 to 7 years. This study included clinical and radiographic observation at 3-month intervals postoperatively. The following results were obtained. 1. Clinically, 20 cases of 25 were successful, exhibiting no clinical problems such as fistula, mobility, toothache and tenderness to percussion, etc. 5 cases were regarded as failures. 2. Radiographiclly, 14 cases revealed normal features, in 6 cases slight rarefaction was observed in the periapical and interradicular region, and marked periapical and interradicular rarefaction was observed in 5 cases. 3. Marked bony deposition was observed in the periapical and interradicular regions in all the remainder except for 5 failurese. 4. Pathologic root resorption was observed in 2 cases. 5. No alveolodental ankylosis was observed. 6. Regeneration of the dental sac of the permanent tooth was observed in 1 case of 2 cases which had revealed loss of the dental sac.
After a vital pulpotomy in dogs' teeth, the responses of the remaining pulp tissue under hydroxides (calcium hydroxide, magnesium hydroxide, aluminium hydroxide and zinc hydroxide) were studied histologically. The class V cavities were prepared on the teeth and the pulp was amputated. Each hydroxide was placed over the amputated tissue and the cavity was sealed with zinc oxide eugenol cement. Animals were sacrificed after 3 days, 1, 2, and 3 weeks following the operation. The teeth were decalcified, sectioned and stained with hematoxylin and eosin. Microscopic examination reveals as follows; 1. Calcium hydroxide: Inflammatory change was seen in the superficial portion of the remaining pulp tissue at the 3rd day and 1st week. The incompleted calicified material began to be deposited from the canal wall at the 2nd week, and the advanced calcified material was seen at the 3rd week. 2. Magnesium hydroxide: Severe inflammatory change was seen in the superficial portion of the remaining pulp from the 3rd day and the 1st week samples. Inflammatory change was decreased at the 2nd week and the slight calcified material was deposited from the root canal at the 3rd week. 3. Aluminium hydroxide: Severe inflammatory changes were seen in the remaining pulp tissue, the blood vessel was dilated, and the odontoblasts were destroyed at the 3rd day and 1st week. The fibrous degeneration spread to the apex at the 2nd week. There was no evidence of newly formed odontoblasts or deposition of calcified material underneath aluminium hydroxide. 4. Zinc hydroxide: The micrscopic picture was destructive. A thick necrotic layer was found under the amputated surface at the 3rd day and 1st week. Granulation tissue formation as well as chronic inflammatory changes extended to the apical area in the pulp tissue. Also there were no sign of odontoblastic formation or calcified material at the 2nd and 3rd week.

At one week, a thin layer of darker staining tissues just below the necrotic zone was presented in the pulps treated with formocresol. In this stage the tissues beneath the darker staining layer were normal. 3. At two weeks, the cells of the palest staining layer were showed indistinct nucleus which suggested the karyolysis and the karyorrhexis in the pulps treated with formocoresol. As reached to the middle third of the pulp, the odontoblasts were scarcely evident or missed in this stage. 4. At three weeks, the necrotic zone was reached to the middle third of the pulp canal. The cells beneath the zone showed massive infiltration of inflammatory cells in the pulps treated with formocresol. 5. Dentin bridge in the control group was deposited below the necrotic zone from the two. weeks later. 6. Normal tissues were observed ill the apical third of all. the dental pulps in all case of calcium hydroxide and formocresol.

The strain gage, holographic and photoelastic analysis etc. have been used for stress analysis of prosthesis, orthodontic or orthopedic appliances and filling materials. But these methods has some limitation in analyzing the internal stress. The Finite Element Analysis has been proved to compensate this defect and widely used in this area. The purpose of this study was to compare the stress distributions of the various temporary filling methods being used in pulpotomy procedure. Three different models were designed according to temporary filling material and method: amalgam filling with ZOE base(Model I), amalgam filling with ZPC sub-base and ZOE(Model II), IRM filling only(Model III). The results of the experiment were as follows: 1. In model I under the load case 6 and 1, the significant stress was shown to be concentrated on the buccal portion of crown. 2. Model II showed the similar pattern of stress distribution to Model I. 3. In model III under load case 2, the stress was mainly distributed on the buccal cusp tip and buccal margin of filling material. In same model under the load case 3, the stress was distributed on the lingual cusp tip. 4. Based on the above data, IRM can be assumed to have advantage over the other tested materials in reducing the incidence of crown fracture by localized the stress within the filling materials.
