Diffuse alveolar hemorrhage and recombinant factor VIIa treatment in pediatric patients

  • Diffuse alveolar hemorrhage (DAH) is a life-threatening pulmonary complication in patients with hematologic malignancies or autoimmune disorders. The current treatment options, which include corticosteroids, transfusions, extracorporeal membrane oxygenation (ECMO), and immunosuppressants, have been limited and largely unsuccessful. Recombinant activated factor VII (rFVIIa) has been successfully administered, either systemically or bronchoscopically, to adults for the treatment of DAH, but there are few data on its use in pediatric patients. The current literature in the PubMed database was reviewed to evaluate the efficacy and risk of rFVIIa treatment for DAH in pediatric patients. This review discusses the diagnosis and treatment of DAH, as well as a new treatment paradigm that includes rFVIIa. Additionally, the risks and benefits of off-label use of rFVIIa in pediatric patients are discussed.

  • The inferior vena cava (IVC) filter is known as an effective and safe method for preventing fatal pulmonary thromboembolism in patients with deep vein thrombosis. Usually, the remaining IVC filters are asymptomatic and do not cause clinical problems. We report a case of duodenal perforation caused by a remaining IVC filter.

  • Central venous catheterization via an internal jugular vein or subclavian vein has become a common procedure in monitoring CVP and managing severely ill patients. However, there have beennumerous reports of complications associated with central venous catheterization. These include vessel injury, pneumothorax, hemothorax, nerve injury, arrhythmias, arteriovenous thrombosis, pulmonary embolism, and infection at the insertion site. We report a case of hemothorax after subclavian vein catheterization failure, along with successful treatment.

Experiences of Abdominoplasty without Undermining (박리없는 복부성형술의 경험)

  • No one technique provides an optimal outcome for all body contouring patients. There are many surgical options for abdominoplasty. Among these, this abdominoplasty without undermining consists of liposuction around abdominal subcutaneous fatty tissue, excision of lower abdominal flap. The procedure allows aggressive thinning and sculpting of abdominal flap. This operation minimizes the dead space, which often leads to postoperative complications, and preserves neurovascular supply to the abdominal skin. From 1999 to 2004, 18 patients underwent the abdominoplasty without undermining, resulting in high satisfaction rates with no significant complications, such as, pulmonary embolism and deep vein thrombosis. Patients could return to normal activity within a week. This abdominoplasty without undermining is an effective and safe alternative with low complication rate and enhances aesthetic results compared to traditional abdominal surgery.

Surgical Treatment of Mitral Valvular Disease (승모판막질환의 수술)

  • During the 4 years period to be reported, 34 operations were performed on the mitral valve in the department of Thoracic and cardiovascular surgery, Korea University hospital, from Aug. 1975 to April 1979. At the first 1-year period, the closed technique was used in 12 patients. After that, open-heart surgery was used routinely; 8 patients had open mitral commissurotomy and 14 patients had valve replacement. There were 18 men and 16 women with sex ratio of 1.1: 1. The age of the patients varied widely from 18 years of the youngest to 46 years of the oldest-average aged of 32.5 years. All had symptoms and the mean duration of symptoms was 6 years and 1 month. Preoperative atrial fibrillation was 47% and embolizations were in 3 of 34 patients. The operative mortality was none for the closed and 14% for the open technique combined rate of 9 per cent which were valve thrombosis, brain embolism and left pulmonary vein rupture in deauriculization. But surviving patients undergoing open heart surgery enjoyed symptomatic benefits comparable to these of the patients of closed.

The Antithrombotic Effects of Green Tea Catechins (녹차 카테킨류의 항혈전 효과)

  • Green tea catechins(GTC) were studied for its inhibitory effect on human platelet aggregation in vitro, for its antithrombotic effect in mice in viro, and bleeding and clotting time in rats. The catechins were isolated and purified from green tea, which were composed of (-)-epigallocatechin gallate, (-)-epigallocatechin, (-)epicatechin gallate and (-)-epicatechin, GTC produced a potent inhibition of human platelet aggregation in a dose-dependent manner against the stimulants such as ADP, collagen, epinephrine and ristocetin n vitro. GTC also prevented death due to the formation of pulmonary thrombosis by platelet aggregates in mice in a dose-de-pendent manner in viro. GTC increased the bleeding time, whole blood clotting time and plasma clotting time in rats, too. These results suggest that GTC is a promising antithrombotic agent.

Antithrombotic Activity of Sunghyangjunggisan

  • As apart of our continuing search for antistroke agents from the herbal medicinal resources, we examined in vitro, ex vivo and in vivo the possibility of Sunghyangjunggisan and its ingradients as a novel antithrombotic agent. In vitro ADP- and collagen-induced rat platelet aggregations were potently inhibited by Arisaematis Rhizoma, Cinnamomi Cortex and Zingiberis Rhizoma in a dose-dependent manner, but not by Sunghyangjunggisan. However, Sunghyangjunggisan significantly inhibited ex vivo rat platelet aggregation. Arisaematis Rhizoma, Atractylodis Rhizoma Alba, and Pinelliae Rhizoma also significantly inhibited ex vivo rat platelet aggregation. Sunghyangjunggisan, Alpiniae Fructus and Zingiberis Rhizoma showed significant protection from death due to pulmonary thrombosis in mice. Therefore, Sunghyangjunggisan can express the antithrombotic action, when it is orally administered.

Mitral Valve Replacement: A Case Report (승모판막 이식수술 1례 보고)

  • 37 years old female was admitted with chief complaints of dyspnea on exertion and hemoptysis. Past history and family history were non-contributory. Physical examination showed Grade III systolic murmur at the apex, which transmitted to the back. E. K. G. and X-ray findings were compatible with the mitral insufficiency. With small size of Beall mitral valve, mitral valve replacement was done under the cardia-pulmonary bypass using hemodilution technic. Patient was tracheotomized after operation and assisted respiration was done for four weeks. Postoperatively, all signs were fine and patient walked around the ward without any difficulty, but she was in psychotic state. On postoperative 60th day, she complained of sudden dyspnea and on chest film, tracheal stenosis was found and recannulation of the tracheal tube was made. Thereafter, she was quite fine until postoperative 110th day when she, by berself, removed the tracheal cannula and died of asphyxia. Autopsy findings of the valve showed no thrombosis, no variance of the valve, and good endothelization of the valve cuffs. Asphyxia, due to removal of the tracheal connula by herself under psychotic state, was considered to be the cause of death in this patient who had tracheal stenosis after tracheostomy.

Evaluation of the Oral Absorption of Heparin Conjugated with Sodium Deoxycholate as a Facilitating Agent in GI Tract

  • The oral delivery of heparin is the preferred therapy in the treatment of patients with a high risk of deep vein thrombosis and pulmonary embolism. New conjugates of heparin and sodium deoxycholate were synthesized in order to enhance the heparin absorption in the GI tract. After oral administration of DOC-heparin, the concentration in anti-FXa assay was increased with increasing amount of coupled DOC. The maximum concentration of DOC-heparin VIII conjugate was $3.3{\pm}0.5\;IU/mL$ at an oral dose of 10 mg/kg, which was 3-fold higher than the baseline level. Finally, DOC coupled to heparin greatly enhanced the absorption of heparin in the GI tract, and this enhancing effect was not induced by changing the tissue structure of the GI wall.

  • A dead, female, 3 years old, squirrel monkey (Saimiri sciureus) was submitted and examined. Before death the monkey has showed lethargy, recumbency and inappetence since November 14, 2001 and died in November 17. Grossly much fibrin was deposited on the pleura of right lung, pericardium, and diaphragm(pleural part). And reddening of right lung was seen. Histopathologically lung showed severe fibrinous pleuritis, severe edema, thrombosis, and focal necrosis in parenchyma. Also much fibrin and mononuclear cells were deposited on the pericardium. In bacterial culture on the pleura and parenchyma of lung, and pericardium, B. bronchiseptica was isolated. Therefore we confirmed this case as the fatal case by B. bronchiseptica in squirrel monkey.
