Comparison of the effects of different-sized airways in inspiratory trainers on maximal inspiratory pressure and rating of perceived exertion scale in healthy young people

  • Objective: This study aimed to investigate the effect of different-sized airways of the inspiratory muscle trainer (IMT) on maximal inspiratory pressure (MIP) and the rating of perceived exertion, as measured by the modified Borg scale (m-Borg). Design: Cross-sectional study. Methods: Twenty healthy subjects (10 men, 10 women) volunteered for the study. The spirometry was used to measure MIP. The trial order of the three spirometry conditions was chosen randomly. After measuring the MIP and before taking the final break, each of the conditions were immediately measured using the m-Borg. All subjects used the IMT with an airway diameter of 5-, 6-, and 7-mm. Results: The MIP significantly decreased with the decreasing airway diameter of the IMT (p<0.001), and the differences in all three conditions (7- and 6-mm, 6- and 5-mm, and 7- and 5-mm airways) were significant (p<0.05). The RPE significantly increased with the decreasing airway diameter of the IMT (p<0.001), and the differences in all three conditions (7- and 6-mm, 6- and 5-mm, and 7- and 5-mm airways) were significant (p<0.05). Conclusions: Decreasing the airway diameter of the IMT decreased the MIP and increased the m-Borg. In regards to physical exercise within the clinical setting, the m-Borg could be a useful as a prior monitoring method to prevent dyspnea for patients with narrowed airways (blocked with secretion or sputum in pulmonary disease) and the different-sized airways of the IMT could be applied as a useful tool to improve MIP for prevention of pulmonary disease.

폐암 수술 후 운동의 효과: PubMed 내 연구에 대한 체계적 문헌고찰 (Effectiveness of Exercise after Lung Cancer Surgery: A Systematic Review in PubMed Database)

  • Objectives This study is to review the effectiveness of exercise after lung cancer surgery. Methods Relevant randomized controlled trials (RCTs) were searched in PubMed database. The systematic review was conducted through flow diagram. The risk of biases were assessed through the Cochrane guideline. Characteristics and outcomes were extracted from each study. Meta-analyses of forced expiratory volume in one second (FEV1), 6-minute walk test (6MWT), quality of life (QoL), pulmonary complications were conducted. Results 14 RCTs were selected. In meta-analysis, exercise improved FEV1 (mean difference [MD] 0.14; 95% confidence interval [CI] 0.04 to 0.25; p=0.009; I2=55%) and mean change of FEV1 (MD 0.11; 95% CI 0.02 to 0.20; p=0.02; I2=0%). Exercise increased the distance of 6MWT, but there was considerable heterogeneity (MD 45; 95% CI 21.16 to 68.83; p=0.0002, I2=89%). There was no differences in QoL scores by 2 questionnaires (European Organisation for Research and Treatment of Cancer quality of life questionnaire, short form-36). Exercise reduced the duration of hospital stay (MD -3.32; 95% CI -5.27 to -1.36; I2=0%; 2 studies) but not duration of chest tube intubation (MD -1.37; 95% CI -2.81 to 0.06; I2=0%) and incidence of pulmonary complications (pooled risk ratio 0.54; 95% CI 0.23 to 1.30; I2=0%). Conclusions Exercise might reduce the duration of hospital stay after lung surgery. There was not enough evidence to prove improvement of lung function, aerobic capacity, muscle strength, QoL, and decline of pulmonary complications. Low-quality risk of bias, different units or estimation of outcome, different exercise type and duration, heterogeneity among studies make the evidence of effectiveness weak. Future researches are required to redeem these defects.

The Effect of Neuromuscular Electrical Stimulation on Pulmonary Function, Gait Ability, and Quality of Life in Patients with Chronic Obstructive Pulmonary Disease

  • Purpose: This study aims to investigate the changes in pulmonary function, gait ability, and quality of life when NMES is applied along with CBE and to provide basic clinical data to be used in pulmonary rehabilitation guidelines for patients with COPD to treat patients with severe COPD. Methods: For this study, CBE and NMES of quadriceps femoris on both sides were applied to the experimental group (n=10), and only CBE was applied to the control group (n=10). For a pre-test, a 6-minute walk test was performed, and pulmonary function and health-related quality of life were measured. Moreover, an exercise program was applied to each group for 30 minutes per session, 5 times a week, for 6 weeks. After that, a post-test was conducted the same way as the pre-test. Results: In the within-group comparison, there were significant differences in forced expiratory volume in one second, 6-minute walk test and health-related quality of life between the experimental group and the control group (p<0.01)(p<0.05). In the between-group comparison, the experimental group showed an increase in forced expiratory volume in one second and 6-minute walk test (p<0.05) and showed a decline in health-related quality of life (p<0.05). Conclusion: The 6-weeks NMES program improved health-related quality in patients with severe COPD by increasing expiratory volume by reinforcing the function of quadriceps femoris. This finding implies that NMES could be an alternative mode for improving physical functions of patients with severe COPD, who cannot participate in a breathing exercise program or are reluctant to participate.

Clinical Importance of Peak Cough Flow in Dysphagia Evaluation of Patients Diagnosed With Ischemic Stroke

  • Objective To investigate the relationship between peak cough flow (PCF), pulmonary function tests (PFT), and severity of dysphagia in patients with ischemic stroke. Methods This study included patients diagnosed with ischemic stroke, who underwent videofluoroscopic swallowing study (VFSS), PCF and PFT from March 2016 to February 2017. The dysphagia severity was assessed using the videofluoroscopic dysphagia scale (VDS). Correlation analysis of VDS, PFT and PCF was performed. Patients were divided into three groups based on VDS score. One-way ANOVA of VDS was performed to analyze PCF, forced vital capacity (FVC), forced expiratory volume in one second (FEV1), and age among the different groups. Results The correlation coefficients of VDS and PCF, VDS and FVC, and VDS and FEV1 were -0.836, -0.508, and -0.430, respectively, all of which were statistically significant at the level of p<0.001. The one-way ANOVA indicated statistically significant differences in PCF, FVC, FEV1, and age among the VDS groups. Statistically significant differences in VDS and age were observed between aspiration pneumoia and non-aspiration pneumonia groups. Conclusion Coughing is a useful factor in evaluating the risk of aspiration in dysphagia patients. Evaluation of respiratory and coughing function should be conducted during the swallowing assessment of patients with ischemic stroke.

다제내성 결핵의 치료 중 역설적 반응으로 나타난 종골 골수염 (Calcaneal Osteomyelitis Presenting as a Paradoxical Reaction during Treatment of Multidrug-Resistant Tuberculosis)

  • Tuberculosis in the foot progresses gradually; thus, diagnosis is usually delayed, and early treatment is rarely provided. If osteomyelitis occurs due to delayed diagnosis and treatment, surgical treatment should be considered. We report the case of a 46-year-old man with osteomyelitis of the calcaneus who was diagnosed with multidrug-resistant pulmonary tuberculosis and he was treated with anti-tuberculosis drugs. Bilateral adrenal masses, abscess of both testes and a small wound in the left plantar heel were observed. Both adrenal masses and abscess were regarded as paradoxical reaction of anti-tuberculosis treatment. After 1 month, he developed a pain in the left plantar heel that was compatible with calcaneal osteomyelitis in radiological features. He underwent right orchiectomy for right scrotal abscess aggravation and surgical treatment for left calcaneal osteomyelitis. Mycobacterium tuberculosis was confirmed by polymerase chain reaction. The patient was immobilized by cast for 8 weeks and the heel pain gradually improved.

Rehabilitative goals for patients undergoing lung retransplantation

  • Lung retransplantation (LRT) involves a second or subsequent lung transplant (LT) in a patient whose first transplanted graft has failed. LRT is the only treatment option for irreversible lung allograft failure caused by acute graft failure, chronic lung allograft dysfunction, or postoperative complications of bronchial anastomosis. Prehabilitation (rehabilitation before LT), while patients are on the waiting list, is recognized as an essential component of the therapeutic regimen and should be offered throughout the waiting period from the moment of listing until transplantation. LRT is particularly fraught with challenges, and prehabilitation to reduce frailty is one of the few opportunities to address modifiable risk factors (such as functional and motor impairments) in a patient population in which there is clearly room to improve outcomes. Although rehabilitative outcomes and quality of life in patients receiving or awaiting LT have gained increased interest, there is a paucity of data on rehabilitation in patients undergoing LRT. Frailty is one of the few modifiable risk factors of retransplantation that is potentially preventable. As such, it is imperative that professionals involved in the field of retransplantation conduct research specifically exploring rehabilitative techniques and outcomes of value for patients receiving LRT, because this area remains unexplored.

Circuit training과 마황(麻黃) 복용이 태음인 여성의 심폐기능향상과 체지방감소에 미치는 상관성 연구 (Correlation between Cardiopulmonary System Function and Body Fat by Circuit Training and Ephedra Herba in Taeumin Women)

  • Objectives : This study was aimed to find out correlation of relation between cardiopulmonary function and body fat. Methods : We studied tendency of change of cardiopulmonary function and body fat for medication of Ephedrae Herba capsule by ergogenic aids with circuit training. We got the results for Exercise stress test and Segmental Bioelectrical Impedence Analysis. Results : 1. Sample Group of Ephedrae Herba medication and Circuit training generally showed the insignificant improvement of Body composition, but Control Group of Placebo and Circuit training significantly(p<.05) showed significant improvement of Body composition. 2. Sample Group of Ephedrae Herba medication and Circuit training generally showed the significant improvement of cardio-pulmonary function. Control Group of Placebo and Circuit training showed insignificant elevation of Cardiopulmonary function. 3. In the case of Sample Group, there wasn't closely correlation relationship of improvement of cardiopulmonary function and body composition, but in the case of Control Group, there was closely correlation relationship of improvement of cardiopulmonary function and body composition. Conclusions : It might be recognized that cardiopulmonary function has the correlation of body composition, and Ephedrae Herba might help the reduction of Body Fat by elevation of Cardiopulmonary function for ergogenic aids and it might be needed further study In various viewpoints.

비흡연자 만성폐쇄성폐질환의 유병률과 위험인자의 융합적 분석 (Convergence analysis of Prevalence and Risk Factors of chronic obstructive pulmonary disease among Non-smokers)

  • 본 연구는 국민건강영양조사 제6기(2013-2015) 자료를 이용하여, 비흡연자 만성폐쇄성폐질환의 유병률과 위험인자를 파악하고 비흡연자 만성폐쇄성 폐질환자의 질병악화예방과 관리방안을 모색하고자 시행되었다. 40세 이상 만성폐쇄성폐질환자 중 비흡연자 4, 911명을 대상으로 복합 표본 교차분석과 로지스틱 회귀분석을 실시하였다. 연구결과 65세 이상(p<.001), 남자(p<.001), 농촌(p=.044), 무 배우자(p<.001), 낮은 학력(p<.001), 농림어업종사자(p<.001)에서 유병률이 높았고, 폐결핵(p<.001)과 천식(p<.001)이 있는 경우 유병률이 높게 나타났다. 또한, 위험인자로는 연령(p<.001), 성별(p<.001), 학력(p=.022), 직업(p<.001), 폐결핵(p<.001), 천식(p<.001)으로 나타났다. 따라서 유병률, 위험인자와 관련된 변수를 고려한 간호사정과 이를 바탕으로 임상에서 적용 가능한 호흡재활 및 호흡중재 프로그램개발이 필요할 것으로 사료된다.

트레드밀 운동이 뇌졸중 환자의 폐기능과 보행능력에 미치는 효과 : 메타분석 (Effects of Treadmill Exercise on Pulmonary Function and Gait Capacity in Stroke Patients : A meta-analysis)

  • Purpose : This study aimed to conduct a comprehensive meta-analysis to evaluate the effect size for pulmonary function and gait capacity of treadmill exercise in stroke patients. In addition, we aimed to examine the current status of treadmill interventions and compare the effect sizes of treadmill training to provide evidence-based practice for future development and application. Methods : The meta-analysis study was conducted using research articles that applied treadmill exercise to stroke patients and were published between January 2005 and February 2020. For a systematic meta-analysis, we conducted a search using the PICOS framework and selected 25 domestic stroke- and treadmill-related studies. The Comprehensive Meta-Analysis program was used to calculate the effect sizes for lung function and gait capacity (6-minute walk test and 10 m walk test). As Cohen's d has a tendency to overestimate the effect size, we used Hedge's g to increase the accuracy in computing the effect size. Results : Based on the results of the meta-analysis, the total effect size of treadmill exercise was 0.535, which was statistically significant, with a medium effect size (p<0.05). The effects of treadmill exercise in stroke patients were divided into dependent variables, namely pulmonary function (0.372) and gait capacity (0.584). In addition, for gait capacity, the effect sizes were evaluated for both the 6-minute walk test (0.756) and the 10 m walk test (0.514). Conclusion : This study provides objective evidence of the effectiveness of treadmill-based gait training programs. We hope that the results of this study will help support the development and implementation of treadmill-based gait training in stroke patients. Treadmill training is expected to improve not only pulmonary function, but also the gait capacity of stroke patients. Long-term investigations on the effects of treadmill training in stroke patients are necessary.

시각적 되먹임을 이용한 배가로근 수축이 건강한 성인의 폐기능에 미치는 효과 (The Effect of Transverse Abdominal Muscle Contraction with Visual Feedback on Lung Function in Healthy Adults)

  • Purpose : The purpose of this study was to investigate the effects of transverse abdominal muscle contraction on pulmonary function in supine position. Methods : 10 female and 10 male college students were recruited this study. Pulmonary Function test was measured with QUARK SPIRO. To check the changes in lung function with transverse abdominal muscle contraction, we tested pulmonary function in the supine, with and without transverse abdominal muscle contraction. While the transverse abdominis was activating, we investigate in the change of forced expiration. All statistical analyses were performed using SPSS Statistics VER.12.0 for window version p-values less than 0.05 were used to identify significant differences. Statistical analysis was used Paired t-test to know difference between activated and non-activated TrA of forced expiration. Results : The subject with transverse abdominal muscle contraction showed significant increments in variable of FVC, FEV1, PEF compared to the pre-intervention results (p<0.01). Conclusion : These results indicate that spontaneous contraction of the TrA helps pulmonary function. And the selective contraction of the transverse abdominis at the end of forced inspiration makes increases in the lung capacity.
