Geographical Distribution of Physician Manpower under the Influence of Public Health Physician (의사인력의 지역간 분포양상 및 공중보건의사의 영향)

  • The purpose of this research is to assess the geographical distribution of physicians and dentists and the degree of maldistribution of the physician. Data were obtained form the Korean Medical Association's report on physicians registry and census for 1990. To assess the degree of disparity in the rural-urban distribution of physician manpower and to identify changes in the distribution pattern, the Gini index of concentration was used. Major findings are as follows; 1. Urban-rural disparity in the distribution of physician manpower exists in all categories of manpower, i.e. physician, dentist, oriental medical doctor, general practitioner, medical specialist, practitioner, public health physician and public health dentist. Urban area which had 74.4% of nation's population, accounted for over 90% of all physician manpower. 2. In terms of the ration of physician manpower per 10, 000 population, in urban area, they were 8.2 physicians, 2.7 general practitioners, 5.5 specialists, 3.0 practitioners, 1.8 dentists and 1.3 oriental medical doctors. In rural area, the ratios were 1.4 physicians, 0.6 general practitioners, 0.9 specialists, 1.0 practitioners, 0.4 dentists and 0.4 oriental medical doctors. 3. Gini indicies computed to measure inequality of physician manpower distribution were 0. 3675 for physicians, 0.3372 for general practitioners, 0.3338 for specialists, 0.2263 for practitioners, 0.3132 for dentists and 0.3293 for oriental medical doctors. 4. Inspite of increase in the number of physician manpower, urban concentration of physician manpower intensified from 1980 to 1990. However, the Gini index for all physician manpower fell by 18.3~36.7% from 1980 to 1990, indicating more even distribution. 5. In rural area, the public health physicians and dentists had increased the ratios of physicians, general practitioners, practitioners and dentists per 10, 000 population remarkebly, and had decreased the Gini indicies of physicians, general practitioners, practitioners and dentists. Thus, public health physicians and dentists contributed to improve the distribution of physician manpower in rural area. Based on the results of this study, long-term and rational manpower policies should be developed to solve the problem of geographical maldistribution of physician manpower as well as short-term policy for inducing physicians to the rural areas.

Relationship Between Pesticide Exposure Factors and Health Symptoms Among Chili Farmers in Northeast Thailand

  • Objectives: The unsafe use of pesticides in agriculture represents a major hazard to human health. This study was conducted to investigate the association between pesticide exposure and health symptoms among chili farmers in northeast Thailand. Methods: This cross-sectional analytic study included 141 chili farmers in Sakon Nakhon Province, in northeast Thailand. Data regarding health symptoms were gathered using a self-report questionnaire. A medical technician tested blood cholinesterase activity using an erythrocyte method, and an occupational medicine specialist at Ramathibodi Hospital in Bangkok, Thailand reviewed the results. Associations between personal characteristics, pesticide exposure factors, and health symptoms were analyzed using multiple logistic regression. Results: Of the 141 chili farmers studied, 66.7% experienced pesticide poisoning, as indicated by below-normal cholinesterase levels. Fatigue was the most frequently reported symptom associated with pesticide exposure, affecting 37.6% of participants. This was followed by nausea and vomiting (31.9%), dizziness (14.9%), and dry throat (14.9%). Multivariate logistic regression analysis revealed that several factors were significantly associated with adverse symptoms: amount of work experience, volume of pesticides used, use of chemical pesticides, use of leaking containers during spraying, direct pesticide exposure while working, contact with pesticide-soaked clothing, consumption of food and drinks in the fields, and blood cholinesterase level indicating risk. Conclusions: This study suggests potential health risks for chili farmers stemming from exposure to and contamination by pesticides used in agricultural practices. To mitigate these risks, it is essential to supply personal protective equipment and to implement educational programs aimed at improving protective behaviors among farmers.

The Determinants of Adolescent Smoking by Gender and Type of School in Korea (우리나라 중고등학교 청소년의 성별 및 학교 종류별 흡연 결정요인)

  • Objectives: This study assessed the influences of various factors that are related to youth smoking such as gender, age and type of school, and we wanted to provide supporting data for tailored and effective policy initiatives to reduce adolescent smoking. Methods: A self-report survey was conducted on 14,910 teen-age students who were selected based on the nationwide distribution of students in large and small cities and counties, the gender ratio of the students and the ratio of students attending various type of school at 38 middle and high schools in six representative areas of each province. The survey was handled and managed by a health education teacher at each school. Binary and multinomial logistic regression was used in the analyses. Results: Smoking by adolescents was associated with gender, age and even height. Male high school students tended to smoke more than female high school students, but this differences was not significant for middle school students. The older the adolescents were, the more likely that they smoked, except for the female high school students. Height was meaningful for all adolescents, except for the boys at the vocational high schools. Monthly allowance was significant for all adolescents. School factors such as type of school and the students' school performance were also crucial factors. Attending a vocational high school was strongly related to smoking, especially for girls. Students' school performance and the perceived level of stress were strongly associated with smoking, especially for boys. Home factors such as the relationship with parents and conversation time with family members were closely related to smoking behavior. Knowledge about the health hazard of smoking was also found to be strongly related to adolescent smoking. Conclusions: In conclusion, demographic factors, school factors, home surroundings and the perception on the harmfulness of smoking are strongly related to adolescent smoking behavior, but these differ from gender and type of school.

Early congenital syphilis presenting with skin eruption alone: a case report

  • Congenital syphilis is one of the most well-known congenital infections, yet it remains a worldwide public health problem. Congenital syphilis can involve any organ system and present with various symptoms. However, early diagnosis of congenital syphilis is difficult because more than half of the affected infants are asymptomatic, and the signs in symptomatic infants may be subtle and nonspecific. Here, we report a case of congenital syphilis with only a skin rash, which led to a delay in diagnosis. This case indicates that congenital syphilis should be considered throughout early childhood.

A Case Report of Panic disorder treated by Yonggyechulgam-tang based on Shanghanlun provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 령계출감탕을 투여한 대양병(大陽病) 공황장애 증례 1례)

  • Objective : This case report show the effect of Yonggyechulgam-tang on Panic disorder. Methods : The patient was diagnosed Panic disorder based on DSM-5. According to Shanghanlun six meridian patterns diagnostic system, the patient treated with Yonggyechulgam-tang for 4 months. The level of panic disorder was evaluated by VAS and BAI. Results : After the treatment, VAS and BAI decreased $10{\rightarrow}2$, $42{\rightarrow}4$ each. Conclusions : Yonggyechulgam-tang improved the symptoms of Panic disorder on this study. Although Yonggyechulgam-tang is not an usual treatment for Panic disorder, we could prescribe the medicine due to Shanghanlun six meridian patterns diagnostic system.

Measuring and Reporting Corporate Social Performance: An Exploratory Study for Practical Application of Double Bottom Line (기업의 사회적 성과 측정과 보고: 더블바텀라인의 실무 적용을 위한 탐색적 연구)

  • The purpose of this study is to suggest a practical way to measure and report corporate social performance as public interest in corporate social value pursuits increases. In particular, we will look for ways to measure and report on the financial and social performance of a company based on the concept of the Double Bottom Line (DBL), which has recently spread to big companies. For this purpose, this study analyzed the theoretical background and practical techniques related to measuring and reporting corporate social performance, and examined methods for measuring and reporting social performance in the existing financial performance measurement system. As a result, SROI was the most suitable method for measuring social performance of a company. It is recommended that social performance reporting follows the disclosure method of the accounting system, and the details of reporting suggest that using the standard of GRI Standard, an international standard related to sustainability reporting, is the most reasonable alternative to 'Double Bottom Line' performance reporting.

  • The revision of domestic laws is in progress according to the UN Convention on the Law of the Coastal Zone Management . Along with it, the governmental policies related to maritime environmental preservation, especially around coastal zones and public works have been promoted based on enviornment, swiftly changed from the old function-based style. Therefore, this report will point out the important aspects which should be considered whenever there is any project on the coastal zones, suggest and classify projects of reviving the nationwide coastal zones, and show how to planning , check and evaluate the necessary technologies for reviving the coastal zones. This research will track down the coastal zone management method based on the natural enviornment as one of the necessary process to check the coastal zone enviornment for effective operating of the coastal zone management equipment method, starting on August 9, 1999, and report the technologies related with it.

A Case Report of Rhabdomyolysis Resulting from Shock Caused by Overworking (과로로 인한 쇼크 후 발생한 횡문근 융해증 1례)

  • Objectives : The purpose of this study is to report a case of Rhabdomyolysis that occurred after shock related to overworking, presenting myalgia(especially in the neck), muscular weakness, and dark urine. Methods : A patient diagnosed with Rhabdomyolysis received Korean medical treatment from November $17_{th}$ to $24_{th}$. Clinical improvement was evaluated using a numerical rating scale(NRS), neck disability index(NDI), and laboratory tests which included complete blood count, kidney function, liver function, serum eletrolytes and Creatinine kinase(CK). Results : After treatment, myalgia, muscular weakness and dark urine each improved. Laboratory results, including CK, had decreased to within normal range. Conclusions : Korean medical treatment could be effective for Rhabdomyolysis patients. Further extensive studies should be carried out.

  • Following the evaluation of the irrigation & drainage structure, a suitable repair and reinforcing procedure can be selected based on this report. Successful procedures take into account the causes of the crack. This provides a survey of crack repair methods, including a summary of the characteristics of the cracks that may be repaired with each procedure, the type of irrigation & drainage structure that have been repaired and a summary of the procedures that are used. Readers are also directly contacted to the website(http:/rri.karico.co.kr/rricon). This report is intended to support the purposes, philosophy, and needs, using easily understood graphics and photos from the construction fields. The results of this study will be distributed to the public official in technical post, the operator in construction site and the member of Korea Agricultural & Rural Infrastructure Corporation and etc.

A Case Report on Improvement Mental Health of the Disabled by Acupuncture and Moxibustion (침구치료로 개선된 장애인의 정신건강 증례보고)

  • The purpose of this study is to report the case mental health of people with disabilities has improved through visit treatment at public health centers. From October 26, 2021 to January 04, 2022, 6 visits were conducted and Patient Health Questionnaire-9 (PHQ-9), Beck Depression Inventory (BDI), Korean version of Center for Epidemiological Studies-Depression Scale (K-CES-D), Visual Analog Scale (VAS), Euro Qol-Visual Analogue Scale (EQ-VAS), Modified Barthel Index (MBI) were used to check changes. After Korean Medicine treatments such as acupuncture, electroacupuncture, electromoxibustion, PHQ-9 decreased from 18 to 8, BDI decreased from 22 to 15, K-CES-D decreased from 42 to 21. And VAS decreased from 85 to 50, EQ-VAS increased from 30 to 50. But There was no change in the MBI. Confirmation of improvement in mental health-related indicators of disabled patients through Korean Medicine treatments.
