Treatment of Eyelid Ptosis due to Kearns-Sayre Syndrome Using Frontalis Suspension

  • Blepharoptosis is a common indication for surgery in plastic surgery units, yet its possible underlying pathology frequently remains unidentified. A 52-year-old man with a 20-year history of progressive bilateral ptosis (right>left) presented with recurrent ptosis of both eyes; he had undergone an operation on the levator aponeurosis 12 years prior. Due to the suspicion of an underlying disease, he was evaluated further. Chronic progressive external ophthalmoplegia in transition to the more severe syndromic variant Kearns-Sayre syndrome, a mitochondrial disorder causing myopathy, was diagnosed. The patient was treated with coenzyme Q10, and he underwent ptosis surgery on both eyes. This case illustrates a potentially multi-systemic disease that was diagnosed by a further evaluation of a common symptom, in this case worsening blepharoptosis. Awareness of myopathic symptoms is necessary to prevent overlooking serious yet improvable conditions.

Clinical Observation on Oculomotor Nerve Palsy Treated by Moxibustion

  • Objective : Moxibustion is one of the primary remedies in traditional oriental medicine. We successfully treated a one patient who complained of oculomotor nerve palsy related to diabetes mellitus with moxibustion. Methods : We performed moxibustion treatment on the protuberances surrounding the Mok-chang acupoint and adjacent frontal scalp of the patient's head at the same time every day and observed the recovery of eye movement and changes of ptosis. Results : After about I week of moxibustion treatment, there were some changes of ptosis and eyeball movement. The patient felt better opening his eyes than before and seemed to reduce his paralytic condition. After 14 days, ptosis was remarkably improved, although slight diplopia remained, and eye movement had recovered to almost normal. Furthermore, his blood glucose was on the decrease and revealed an average 120 mg/dl. Conclusions : Moxibustion treatment performed on the Mok-chang acupoint remarkably improved ptosis and limited eye movement arising from oculomotor nerve palsy related to diabetes mellitus. We hope moxibustion is used for treating nerve palsies and similar diseases in the future.

  • Oculomotor nerve palsy causes ptosis, limitation of eye movement, diplopia, and facial pain. Despite imaging investigation, the cause of the palsy cannot be established in most cases. We treated a patient with idiopathic oculomotor nerve palsy with acupuncture, electroacupuncture, pharmacopuncture, and Korean medical physical therapy. In this case, the symptoms remarkably improved after 28 days of treatment. This case report suggests that traditional Korean medicine is effective in treating ptosis and limitation of eye movement diagnosed as idiopathic oculomotor nerve palsy.

안구형 중증 근무력증 치험 1례 (Case Study on Ocular Myasthenia Gravis)

  • Objectives : The purpose of this study is to report the effectiveness of Korean medical treatment on ptosis in myasthenia gravis. Methods : We treated the patient who had suffered from rt. ptosis and was diagnosed with myasthenia gravis with acupuncture, electropuncture, herbal acupuncture, cupping therapy, herbal medicines and western medicine such as corticosteroids and acetylcholinesterase inhibitors. The effectiveness of treatment was evaluated through Relative Interpalpebral Fissure(IPF) and photography. Results : After the treatments, relative Interpalpebral Fissure(IPF) was increased and improvement remained for three months after the treatment was finished. Conclusions : The result indicates that combination therapy of Korean medical treatment and western medicine had an effect on treatment of ptosis with myasthenia gravis.

  • Kearns-Sayre syndrome (KSS) is a rare mitochondrial disease that affects young adults, due to a deletion of mitochondrial DNA and characterized by the triad: age of onset lower than 20 years, chronic progressive external ophthalmoplegia, and an atypical pigmentary retinopathy. It is also characterized by other endocrine, neurological, and especially cardiac impairment with a very high risk of cardiac complications during surgical procedures under all types of anesthesia. We report a case of KSS revealed by severe bilateral ptosis and confirmed by a muscle biopsy with "ragged red fibers." The ptosis was surgically managed by cautious Frontal suspension under local anesthesia "Frontal nerve block." Through this case, we discuss challenges in the management of KSS patients.

  • In a complete oculomotor nerve palsy, patients show ptosis(paresis of the levator), abduction(paresis of the medial rectus and compensatory overaction of the lateral rectus) and dilated pupils. In oculomotor nerve palsy, the functions of four of the six extraocular muscles are compromised and its treatment is the most difficult problem in the paralytic strabismus. Currently, if the condition of the oculomotor nerve palsy is not improved within a year, surgical correction can be attempted. We experienced an improved case of the oculomotor nerve palsy in a Benedikt's syndrome patient treated with oriental medicine. We used herbal medicine and acupuncture. Based on this experience, it is considered that oriental medicine can be applied to the treatment of the oculomotor nerve palsy. Oculomotor nerve, Palsy, Ptosis, Abduction, Benedikt's syndrome.

  • Orbital injuries are common with facial trauma. Direct injuries to the globe are not rare but it can result in complications such as chemosis, subconjunctival hemorrhage and hyphema. Periorbital trauma or injuries to the extraocular muscles and blow-out fracture may result in lid edema, ecchymosis & ptosis and diplopia or limitation of ocular motion respectively. Indirect injuries to the optic nerve come up without any injuries but its complication is irreversible and severe such as loss of vision. The aim of this study is to review the literature on blindness or ptosis following facial trauma and present the cases of blindness after facial trauma and ptosis after mandibular fracture without specific clinical findings.

  • Horner's syndrome may rarely accompany herpes zoster ophthalmicus (HZO). A 78-year-old woman suffered from HZO accompanied ipsilateral ptosis and miosis. Before skin eruption, she was diagnosed as a paratrigeminal syndrome of Raeder. She was treated with intravenous acyclovir and prednisone for 7 days. Ptosis and miosis was not completely improved after 5 months of follow-up.

  • Objective: Improvement of myasthenia gravis through Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 52-year-old patient with myasthenia gravis was prescribed 21 different OCNT treatments, including Cyaplex. Results: Following the administration of OCNT, symptoms such as eyelid ptosis, masticatory dysfunction, diplopia, and fatigue showed improvement. Conclusion: OCNT can assist in alleviating and mitigating symptoms for patients suffering from thymoma and myasthenia gravis.

  • Objective: We describe the case of an 84-year-old man with a midbrain infarction causing pupil sparing oculomotor nerve palsy, with ipsilateral cerebellar ataxia diagnosed as Claude's syndrome. Methods: The patient was treated with Korean medicine, including herbal medicine, acupuncture, and electro-acupuncture, during 45 days of hospitalization. Improvements in the patient's symptoms and changes in ptosis were evaluated using the Korean version of the Scale for the Assessment and Rating of Ataxia (SARA). Results: After 45 days of treatment with the Korean medicine, the patient's symptoms, including ataxia and ptosis, improved. Conclusions: This study suggests that the treatment with Korean medicine was effective in aiding the patient's recovery.
