Early menarche and its consequence in Korean female: reducing fructose intake could be one solution

  • The mean age at menarche (AAM) of Korean females has been rapidly decreasing over the last 50 years; currently, the prevalence of early menarche (<12 years) is 22.3%. Female adolescents who experience early menarche are known to be at greater risk of psychosocial and behavioral problems along with several physical health problems such as menstrual problems. They also tend to achieve a shorter final height and develop obesity. Population-based Korean studies have shown a strong association between early menarche and the risk of obesity, insulin resistance, metabolic syndrome, nonalcoholic fatty liver disease, diabetes, breast cancer, and cardiovascular disease in adulthood. Although the exact mechanism of how early menarche causes cardiometabolic derangement in later adulthood is unknown, childhood obesity and insulin resistance might be major contributors. Recent studies demonstrated that an excessive consumption of fructose might underlie the development of obesity and insulin resistance along with an earlier AAM. A positive association was observed between sugar-sweetened beverages (a major source of fructose) intake and obesity, metabolic syndrome, insulin resistance, and cardiometabolic risk in Korean females. In pediatrics, establishing risk factors is important in preventing disease in later life. In this regard, early menarche is a simple and good marker for the management of cardiometabolic diseases in adulthood. Decreasing one's fructose intake might prevent early menarche as well as the development of obesity, insulin resistance, and cardiometabolic diseases.

Psychosocial Factors Associated with Smoking Cessation Attempts in Korean High School Students Who Engage in Intermittent and Light Smoking

  • Purpose: This study aimed to identify psychosocial factors associated with smoking cessation attempts among Korean high school students who engage in intermittent and light smoking. Methods: Cross-sectional secondary data derived from the 2018 Korea Youth Risk Behavior Web-Based Survey were analyzed within the framework of the biopsychosocial model. The sample comprised 829 high school students who reported current intermittent and light smoking, which was defined as cigarette smoking on 1~29 days in a 30-day period and no more than 10 cigarettes per day. Purposeful selection logistic regression analysis was performed. Results: In total, 71.7% (n=586) of the respondents had tried to stop smoking during the past 12 months. The main result was that respondents who had viewed an anti-smoking advertisement in the past year significantly more smoking cessation attempts than those who had not (adjusted odds ratio=2.59; 95% confidence interval=1.45~4.62, p=.001). Conclusion: To encourage smoking cessation attempts among high school students who engage in intermittent and light smoking, healthcare providers, including school and community nurses, should develop effective interventions using anti-smoking advertisements tailored to adolescents' interests and developmental stage.

복부비만의 지표인 허리둘레에 영향을 미치는 건강행태요인 -"2001년 국민건강.영양조사"의 자료를 기반으로 (Health Behavior Factors Affecting Waist Circumference as an Indicator of Abdominal Obesity)

  • Objectives: This study was performed to identify the socioeconomic factors, the psychosocial factors and the heath behavior factors that have an influence on abdominal obesity, as measured by using the waist circumference. Methods: Data was obtained from individuals aged above 20 years who had their waist circumference measured on the Korean National Health and Nutrition Examination Survey 2001, which was a cross-sectional health survey. Results: Regression analysis of the factors that affect abdominal obesity showed that the education level, income, smoking, duration of smoking, drinking consumption, frequency of exercises and sleeping were the associated factors for abdominal obesity. For men, the duration of smoking, education level, income and drinking consumption were the associated factors for abdominal obesity. For woman, the education level, income, duration of smoking, drinking consumption and frequency of exercise were the associated factors for abdominal obesity. Conclusions: Abdominal obesity is a risk factor for morbidity and mortality, and it is associated with chronic diseases, including cardiovascular disease and diabetes. Based on the findings, it is essential to modify heath behaviors for preventing abdominal obesity, which is a condition associated with the incidence of chronic disease.

지역 저소득층 아동의 우울증상과 주의력결핍-과잉행동증상에서 사회경제적 요소의 관련성 (Association of Psychosocial Factors in Developing Childhood Depression and ADHD in a Community Low Income Family Children)

  • Ovjectives: As the attention devoted to children's mental health increases, medical costs and burdens mount as well. In the present study, we evaluated the association between socioeconomic status(SES) and major child psychiatric symptoms. Methods: The subjects of this study were children of recruited from a mental health screening program in the Seoul Sungbuk mental health center over the course of 3 months. To establish the SES of each child, we collected data about each child's medical insurance, years of parental education, household income, family structure and housing. 149 children & parents completed questionnaires including the Childhood Depression Inventory(CDI) and the Korean Attention Deficit Hyperactivity Disorder Rating Scale(K-ARS). Results: The mean K-ARS-P score was $12.1{\pm}11.1$ and the suspected prevalence of ADHD was 20.8%(n=31). The mean cm score was $12.9{\pm}7.9$, and the prevalence of suspected depression was 16.8%(25). Depressive symptoms and ADHD symptoms were both more severe than those observed in a previous epidemiologic study in Korea. Depressive symptoms were more closely associated with family SES status. Conclusion: SES status is one of the most important risk factors in the development of major child psychiatric symptoms. In our study we found that depressive symptoms in particular were most tightly associated with psychosocial factors. Evaluation of the risk factors, early screening and intervention for low SES children would be valuable mental health management tactics to implement in a community mental health system.

농촌지역 주민의 영양결핍 및 사회심리적 요인과 근감소증 가능군과의 관련성 (Nutrition and Psychosocial Factors were associated with Possible Sarcopenia in the Rural Elderly)

  • 연구의 목적은 지역사회 거주하고 있는 노인들의 근감소증 가능군과 영양결핍 그리고 사회심리적 요인 등과의 관련성을 파악하고자 실시되었다. 연구대상자는 농촌지역 주민 중 60세 이상인 1,019명이다. 근감소증 가능군과 관련성에 포함된 변수는 영양상태, 사회심리적 요인(낙상에 대한 두려움, 자기효능감, 사회적 고립, 사회적 자본)이다. 근감소증의 기준은 개정된 아시아 진단기준에 따라 종아리 둘레로 의심자를 찾은 다음, 악력으로 평가하였다. 연구 결과 영양은 근감소증 가능군과 관련이 있었으며 사회심리적 요인 중에서는 낙상에 대한 두려움, 자기효능감이 근감소증 가능군과 직, 간접적으로 관련이 있었으며, 사회적 자본 중 사회적 참여는 영양결핍에 직접적으로 관련이 있고, 근감소증 가능군과는 간접적으로 관련이 있었다. 결론적으로 근감소증 가능군과 관련 있는 요인으로 영양결핍과 낙상에 대한 두려움 그리고 자기효능감 저하 등이 있고 영양결핍에 사회적 참여도 관련이 있어 지역사회의 근감소증 대상 보건사업에서는 이들 변수를 고려하여 실시되어야 한다.

의류디자인 근로자의 근골격계질환에 관한 연구 (A Study on Musculoskeletal Disorders of Workers in Clothing Design)

  • This study analyzed the effect of workers' socio-psychological burden factors on musculoskeletal disorders. A survey was conducted targeting office and field workers in the Seoul metropolitan area. Analysis was performed using the results of 357 surveys out of 400 surveys. The analysis results were derived as follows. First, the socio-psychological burden factor did not show statistically significant results for musculoskeletal disorders. Second, it was found that job stress factors also had a positive effect on musculoskeletal disorders. Third, social psychological burden factors were found to be more positive than office workers than field workers. Fourth, there was a statistically significant correlation between job stress and musculoskeletal disease risk scores. From the above results, job stress factors affect musculoskeletal disease risk factors (work frequency, pain level, and duration) and the increase in musculoskeletal disorders and the number of accidents.

What is the disease burden from childhood and adolescent obesity?: a narrative review

  • The prevalence of childhood and adolescent obesity has increased and exacerbated during the coronavirus disease 2019 pandemic, both in Korea and globally. Childhood and adolescent obesity poses significant risks for premature morbidity and mortality. The development of serious comorbidities depends not only on the duration of obesity but also on the age of onset. Obesity in children and adolescents affects almost all organ systems, including the endocrine, cardiovascular, gastrointestinal, reproductive, nervous, and immune systems. Obesity in children and adolescents affects growth, cognitive function, and psychosocial interactions during development, in addition to aggravating known adult comorbidities such as type 2 diabetes mellitus, hypertension, dyslipidemia, nonalcoholic fatty liver disease, obstructive sleep apnea, and cancer. Childhood and adolescent obesity are highly associated with increased cardiometabolic risk factors and prevalence of metabolic syndrome. The risk of cardiovascular and metabolic diseases in later life can be considerably decreased by even a small weight loss before the onset of puberty. Childhood and adolescent obesity is a disease that requires treatment and is associated with many comorbidities and disease burdens. Therefore, early detection and therapeutic intervention are crucial.

Psychosocial Predictors of Breast Self-Examination among Female Students in Malaysia: A Study to Assess the Roles of Body Image, Self-efficacy and Perceived Barriers

  • Background: Early detection is a critical part of reducing the burden of breast cancer and breast self-examination (BSE) has been found to be an especially important early detection strategy in low and middle income countries such as Malaysia. Although reports indicate that Malaysian women report an increase in BSE activity in recent years, additional research is needed to explore factors that may help to increase this behavior among Southeastern Asian women. Objective: This study is the first of its kind to explore how the predicting variables of self-efficacy, perceived barriers, and body image factors correlate with self-reports of past BSE, and intention to conduct future breast self-exams among female students in Malaysia. Materials and Methods: Through the analysis of data collected from a prior study of female students from nine Malaysian universities (n=842), this study found that self-efficacy, perceived barriers and specific body image sub-constructs (MBSRQ-Appearance Scales) were correlated with, and at times predicted, both the likelihood of past BSE and the intention to conduct breast self-exams in the future. Results: Self-efficacy (SE) positively predicted the likelihood of past self-exam behavior, and intention to conduct future breast self-exams. Perceived barriers (BR) negatively predicted past behavior and future intention of breast self-exams. The body image sub-constructs of appearance evaluation (AE) and overweight preoccupation (OWP) predicted the likelihood of past behavior but did not predict intention for future behavior. Appearance orientation (AO) had a somewhat opposite effect: AO did not correlate with or predict past behavior but did correlate with intention to conduct breast self-exams in the future. The body image sub-constructs of body area satisfaction (BASS) and self-classified weight (SCW) showed no correlation with the subjects' past breast self-exam behavior nor with their intention to conduct breast self-exams in the future. Conclusions: Findings from this study indicate that both self-efficacy and perceived barriers to BSE are significant psychosocial factors that influence BSE behavior. These results suggest that health promotion interventions that help enhance self-efficacy and reduce perceived barriers have the potential to increase the intentions of Malaysian women to perform breast self-exams, which can promote early detection of breast cancers. Future research should evaluate targeted communication interventions for addressing self-efficacy and perceived barriers to breast self-exams with at-risk Malaysian women. and further explore the relationship between BSE and body image.

European Approaches to Work-Related Stress: A Critical Review on Risk Evaluation

  • In recent years, various international organizations have raised awareness regarding psychosocial risks and work-related stress. European stakeholders have also taken action on these issues by producing important documents, such as position papers and government regulations, which are reviewed in this article. In particular, 4 European models that have been developed for the assessment and management of work-related stress are considered here. Although important advances have been made in the understanding of work-related stress, there are still gaps in the translation of this knowledge into effective practice at the enterprise level. There are additional problems regarding the methodology in the evaluation of work-related stress. The European models described in this article are based on holistic, global and participatory approaches, where the active role of and involvement of workers are always emphasized. The limitations of these models are in the lack of clarity on preventive intervention and, for two of them, the lack of instrument standardization for risk evaluation. The comparison among the European models to approach work-related stress, although with limitations and socio-cultural differences, offers the possibility for the development of a social dialogue that is important in defining the correct and practical methodology for work stress evaluation and prevention.

치료저항성 우울증의 연구에서 패러다임의 전환 (Paradigm Shift in the Study of Treatment Resistant Depression)

  • Treatment-resistant depression (TRD) is a major public health problem. It is estimated that about 30% of patients with major depressive disorder do not show substantial clinical improvement to somatic or psychosocial treatment. Most of studies for TRD have focused on the subjects already known as TRD. Patients with unipolar depressive episodes that do not respond satisfactorily to numerous sequential treatment regimens were included in the TRD studies. Such post hoc experimental design can be regarded only as consequences of having TRD, rather than as causal risk factors for it. Although informative, data derived from such studies often do not allow a distinction to be made between cause and effect. So, we should shift paradigm toward examining the risk for developing TRD in untreated depressed patients. To deal with this problem, untreated depressed patients should be enrolled in the study to identify biological markers for treatment resistance. The peripheral or central biological markers should be explored before starting treatment. Subsequent systematic administration of treatments with appropriate monitoring in the subjects can determine the risk for developing treatment resistance in untreated individuals. Such information could give a cue to improve the initial diagnosis and provide more effective treatment for TRD.
