The Breast Feeding Adaptation Scale-Short Form: Development and Testing of Its Psychometric Properties and Measurement Invariance

  • Purpose: This study sought to develop the Breastfeeding Adaptation Scale-Short Form (BFAS-SF) for use at 4 weeks postpartum, to test its validity and reliability, and to examine its measurement invariance. The latent mean score of the BFAS-SF across multiple groups was also compared. Methods: This methodological research study was conducted to develop a short form of the BFAS and to test its psychometric properties and measurement invariance. Data were collected twice for measurement invariance testing. The sample included 431 and 272 breastfeeding mothers at 2 weeks and 4 weeks postpartum, respectively. Results: Confirmatory factor analysis supported six dimensions of the BFAS-SF at 4 weeks postpartum. Multi-group confirmatory factor analysis revealed evidence for invariance of the BFAS-SF according to employment status, parity, delivery mode, and the postpartum period. There were statistically significant latent mean differences. Mothers who were unemployed and who had a vaginal delivery showed significantly higher scores for breastfeeding confidence, sufficient breast milk, and baby's satisfaction with breastfeeding. Conclusion: The BFAS-SF is valid, reliable, and an appropriate instrument for assessing mothers' breastfeeding adaptation. It can be used to compare mean scores according to employment status and delivery mode.

국내 연구에서 당뇨병 환자 건강관련 삶의 질 측정에 관한 모니터링 (Monitoring the Use of Health-Related Quality of Life Measurements in Korean Studies of Patients with Diabetes)

  • Purpose: The purpose of this study was to monitor the use of health-related quality of life (HRQOL) instruments in Korean studies of patients with diabetes. Methods: Of 86 Korean studies initially identified, 17 studies met the inclusion criteria. For each study, a description of the instrument and its psychometric properties were monitored by the Instrument Review Criteria of the Scientific Advisory Committee. These criteria include conceptual definition, attributes, taxonomy, reliability, validity, responsiveness, administrative mode, and language adaptations. Results: Five generic and one diabetes specific type questionnaires were identified from the 17 studies. Of those studies, conceptual definitions with the attributes of multi-dimension and subjectiveness were provided for 11 studies (71%). In the analysis of conceptual taxonomy, only 6 studies were classified as HRQOL, while other studies were done as QOL or health status. In monitoring of psychometric properties, reliability, validity, and responsiveness were reported for 88.2%, 64.7%, and 29.4%, respectively. One generic instrument was developed with a Korean population, while the other instruments were developed for Western countries. However, language adaptations were performed for only a few of the instruments. Conclusion: The psychometric properties including responsiveness of most instruments warrants further research, and the development of diabetes-specific HRQOL measurements should be sought to facilitate intervention outcomes across Korean studies of patients with diabetes.

  • Objective : The PTSD Checklist (PCL) is a self-report screen for posttraumatic stress disorder (PTSD) that can be scored for both diagnostic assessment and symptom severity measurement. The most recent revision of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) contains a number of changes to the definition of PTSD, and the aim of this study was to assess the psychometric properties of the Korean version of the PCL for the DSM-5 (PCL-5-K). Methods : The participants were 204 Korean veterans of the Vietnam War who completed the PCL-5-K, the Mini Mental Status Examination (MMSE), PTSD module of Structured Clinical Interview for DSM-5, Research Version (SCID5-RV PTSD module), Korean version of Impact of Event Scale-Revised (IES-R-K) and Combat Exposure Scale (CES-K). Results : The PCL-5-K demonstrated good internal consistency (${\alpha}=0.972$) and test-retest reliability (r=0.96); the suggested cut-off score for PTSD diagnosis was ${\geq}37$ with 0.88 sensitivity and 0.96 specificity. The PCL-5-K scale correlated highly with the IES-R-K and CES-K. Factor analysis identified only one factor. Conclusion : Among elderly Korean veterans of the Vietnam War, the PCL-5-K demonstrated similar psychometric qualities to those of both the original PCL and subsequent versions. It is expected that the PCL-5-K will be a useful PTSD screening tool.

Psychometric Properties of the Persian Version of the Self-Assessed Support Needs Questionnaire for Breast Cancer Cases

  • Background: It has been found that support given to women with breast cancer has a positive effect upon their reactions to the illness and may even prolong their survival. Perceived support needs assessment in breast cancer women could be considered as a necessary part of nursing function. Aim: The purpose of this study was to translate and culturally adapt the self-assessed support needs (SASN) questionnaire into Persian language and to investigate its psychometric properties. Materials and Methods: After forward-backward translation of the questionnaire and making appropriate changes, we selected 160 women with breast cancer as our study sample. The psychometric properties of the SASN, including its internal consistency, test retest reliability, and construct validity were evaluated through the known-groups technique. Results: The calculated Kaiser Meyer Olkin was 0.756, indicating that the sample was sufficiently large to perform a satisfactory factor analysis. The six factors all together explained 50.7% of the variance; the first factor (diagnosing) explaining the biggest part of variance (10.9). Internal consistency reliability was 0.83 for the whole scale and the stability of test was 0.78. For the first factor, Cronbach's alpha was 0.90 and factor loadings of scale's items were found to deal with diagnosis subscale. The domains described patients' diagnosis, treatment, support, femininity and body image, family and friends and information. Conclusions: The reliability and validity of the adapted version of the SASN was shown to be satisfactory. Thus, it can be used to investigate self-assessed support needs of Iranian women suffering from breast cancer since the SASN is a multi-domain scale.

Translation and Psychometric Testing of the Korean Versions of the Spiritual Perspective Scale and the Self-transcendence Scale in Korean Elders

  • Purpose: The purpose of this study was to translate the Spiritual Perspective Scale (SPS) and Self-transcendence Scale (STS) into Korean and test the psychometric properties of the instruments with Korean elders. Methods: A cross-sectional survey design was used to implement the three stages of the study. Stage I consisted of translating and reviewing the scales by six experts. In Stage II, equivalence was tested by comparing the responses between the Korean and English versions among 71 bilingual adults. Stage III established the psychometric properties of the Korean versions SPS-K and STS-K among 154 Korean elders. Results: Cronbach's alpha of the SPS-K and the STS-K .97, and .85 respectively with Korean elders. Factor analysis showed that the SPS-K had one factor; the STS-K had four factors with one factor clearly representing self-transcendence as theorized. Both scales showed good reliability and validity for the translated Korean versions. However, continued study of the construct validity of the STS-K is needed. Conclusion: Study findings indicate that the SPS-K and the STS-K could be useful for nurses and geriatric researchers to assess a broadly defined spirituality, and to conduct research on spirituality and health among Korean elders. Use of these scales within a theory-based study may contribute to further knowledge about the role of spirituality in the health and well-being of Korean people facing health crises.

간호사의 범문화효능감 측정도구 개발 및 타당화 (Development and Psychometric Evaluation of the Transcultural Self-efficacy Scale for Nurses)

  • Purpose: This methodological study was conducted to develop and psychometrically test the Transcultural Self-efficacy scale (TCSE-scale) for nurses. Methods: Initial 41 items for the TCSE-scale were generated based on extensive literature reviews and in-depth interviews with 18 nurses who had experience in caring for foreign patients. Cultural Competence and Confidence model was used as a conceptual framework. Content validity was evaluated by an expert panel. Psychometric testing was performed with a convenience sample of 242 nurses recruited from four general hospitals in the Seoul metropolitan area and Gyeonggi-do province of South Korea. To evaluate the reliability of TCSE-scale, a test-retest reliability and an internal consistency reliability were analyzed. Construct validity, concurrent validity, criterion validity, convergent validity and discriminative validity were used to evaluate the validity. Results: The 25-item TCSE-scale was found to have three subscales-Cognitive, Practical, and Affective domain-explaining 91.5% of the total variance. TCSE-scale also demonstrated a concurrent validity with the Cultural Competence Scale. Criterion-related validity was supported by known-group comparison. Reliability analysis showed an acceptable-to-high Cronbach's alpha-.88 in total, and subscales ranged from .76 to .87. The ICC was .90, indicating that the TCSE-scale has internal consistency and stability of reliability. Conclusion: This preliminary evaluation of the psychometric scale properties demonstrated an acceptable validity and reliability. The TCSE-scale is able to contribute to building up empirical and evidence based on data collection regarding the transcultural self-efficacy of clinical nurses. We suggest further testing of the applicability of TCSE-scale in different settings and community contexts.

  • Purpose: Health-related quality of life (HRQOL) in patients with cardiovascular disease in Korea has rarely been studied, mostly due to the lack of a psychometrically validated disease-specific instrument. The purpose of this study was to develop and validate a cardiovascular specific-HRQOL questionnaire (CD-QOL). Method: The CD-QOL was developed and validated as follows; item generation, pilot study, and psychometric tests. Patients were recruited from three-university hospitals. The patients were asked to complete the preliminary questionnaire comprising the content-validated items, SF-36, and CES-D. The NYHA and KASI classifications were used to classify the functional performance of the patients. The data was analyzed using correlation, factor analysis, multidimensional scaling, multitrait/multi-item matrix, ANOVA, and Cronbach's alpha. Result: Preliminarily, thirty-nine items were generated. Factor analysisextracted a five-factor solution with a total of twenty-two items. One item was deleted based upon the MDS. The remaining items were moderately correlated with the subscales of the SF-36 and associated with depression measured with the CES-D. The mean scores of patients in NYHA and KASI class I were significantly higher than those in NYHA and KASI class II or/and III, which suggested patients with better functional performance were likely to have a better HRQOL. Cronbach's alphas of the total and subscales were all greater than 0.70. Conclusion: The CD-QOL is a easily applicable instrument with excellent psychometric properties of content, criterion, factorial, convergent, and known-groups validity, and internal consistency reliability in Korean patients with cardiovascular disease.

  • Objective : The 18-item Obsessive-Compulsive Inventory-Revised (OCI-R) is widely employed to assess symptoms of obsessive-compulsive disorder (OCD). However, this instrument's factor structure does not align with contemporary dimensional models of OCD. Therefore, the objective of this study was to examine psychometric properties of the 12-item Korean OCI (OCI-12) on four obsessive-compulsive symptom dimensions, in patients with OCD. Methods : A total of 157 patients with OCD and 51 healthy controls completed psychological measures, including the OCI-R, Dimensional Obsessive-Compulsive Scale (DOCS), and scales evaluating anxiety and depressive symptoms. Pychometric characteristics of the OCI-12 with three neutralizing and three hoarding items eliminated from the OCI-R, were analyzed. Results : All OCI-12 items registered excellent internal consistency at 0.83. Confirmatory factor analysis revealed strong association between individual items and their proposed latent factors (i.e., subscales). Convergent validity was appropriate. A high correlation was particularly observed for the DOCS score (r=0.71, p<0 .001). Moreover, the OCI-12 was as sensitive as the OCI-R for determining effects of empirically supported treatment for OCD. Conclusion : The OCI-12 is a 12-item measure that adheres to the prevailing 4-factor model of OCD dimensions. Like OCI-R, it possesses good to excellent psychometric properties, including reliability, validity, and sensitivity to treatment.

한국어판 알츠하이머병 지식 측정도구의 신뢰도와 타당도 (Psychometric Properties of the Alzheimer's Disease Knowledge Scale-Korean Version)

  • Purpose: The purpose of this study was to evaluate the psychometric properties of the Korean version of the Alzheimer's Disease Knowledge Scale (ADKS-K) to determine its applicability to Korean adults. Methods: Cross-cultural validity was performed according to Consensus-based Standards for the Selection of Health Measurement Instruments (COSMIN). The Kuder-Richardson Formula 20 for internal consistency and Intraclass Correlation Coefficient (ICC) for test-retest reliability were conducted. Content validity, criterion related validity and construct validity were evaluated. The Classical Test Theory (CTT) model and the Item Response Theory (IRT) model were applied in performing the item analysis. Results: The KR 20 was .71, and the ICC was .90, indicating that the ADKS-K has internal consistency and stability reliability. Thirty items of the ADKS-K had significant Content Validity Ratio (CVR) values, i.e., mean of 0.82 and range of 0.60~1.00. Mean item difficulty and discrimination indices calculated by TestAn program were 0.63 and 0.23, respectively. Mean item difficulty and discrimination indices calculated by BayesiAn program were -0.60 and 0.77, respectively. These tests indicate that ADKS-K has an acceptable level of difficulty and discriminating efficiency. Conclusion: Results suggest that ADKS-K has the potential to be a proper instrument for assessing AD knowledge in Korean adults.

한국어판 영성측정도구(SAS)의 신뢰도와 타당도 검증연구 (Spiritual Assessment Scale;Psychometric Evaluation of the Korean Version)

  • Spirituality has been found as a factor that may influence an individual's health and response to illness and dying. The purpose of this study was to assess the reliability and validity of the Korean version of the Howden's Spiritual Assessment Scale(SAS). The SAS was developed based on the attributes of spirituality and constructed with 28 items, 4 subscales. The translation involved four steps : translation into Korean, checking agreement, back translation into English, and arriving at a consensus. Psychometric evaluation was done on 222 nurses from five hospitals. All responses were voluntary and anonymous. The Cronbach's alpa coefficient for internal consistency was .928 for the total 28 items and .700${\sim}$.805 for subdimensions. Item- total correlations ranged from .36${\sim}$.68. Principal Component Factor Analysis with Varimax Rotation yielded four factors with four or more items each loading at .40 or higher. These factors explained 51.3% of the total variance. The items clustered in this study were almost identical with initial scale. There was positive correlation(r=.648, p=.0001) between SAS and Ellison & Paloutzian's Spiritual Wellbeing Scale and negative correlation(r=-.418, p=.000) with Pines' Burnout Scale which indicate convergent and discriminant validity. In conclusion, this tool can be effectively utilized for assessing spirituality in Korea.
