Gasserian Ganglion and Mandibular Nerve Block for the Patient with Mouth Floor Cancer -A case report- (구상암 환자에서의 갓세르 신경절 및 하악신경 차단 -증례 보고-)

  • Most neoplasm of the floor of the mouth are squamous cell carcinoma. They originate from anterior midline floor of the mouth and penetrate into the sublingual gland. Invasion of the mandible is a late manifestation. Lymphatic spread is the submaxillary and subdigastric nodes and advanced lesions of them produce severe pain, The initial step in managing patients with cancer pain is the oncology therapy in the form of radiotherapy, surgery, or chemotherapy, alone or combined. When oncologic therapy is ineffective, the pain must be treated by systemic analgesic, psychologic, neurostimulating, regional analgesic,and meuroablative techniques. We successfully treated with gasserian ganglion block on the left side and mandibular nerve block on the right side with pure alcohol in the patient having severe submandibular, lower lip and tongue pain.

  • Objective : It is a presentation of theoretical, clinical. historical foundation about using biofeedback in oriental medicine. Methods : We generalize definition, explanation, history and clinical application of biofeedback and study community with and clinical practice in oriental medical theory, referring to The neurofeedback book-An introduction to Basic Concepts in Applied Psychophysiology published by Michacl Thompson & Lynda Thompson Results : We acquire possibilities of application in oriental medicine as follows. 1. It is a view of treatment for the whole man not separating body and mind. 2. It make much of yin-ping-yang-bi(陰平陽秘) and spontaneous cure in the human body. 3. It make much of a curer's mental condition through dao(道). 4. It make much of a breathing corresponding to nature. 5. We make use of it in diagnosis and evaluation of disease. 6. It is psychologic treatment of self-leading. Conclusions : Biofeedback is expected that usefully applied to oriental psychotherapy of Kyungja-pyungji therapy(驚者平之療法). Oh-Ji-Sang-Seung therapy(五志相勝療法), Qigong therapy(氣功療法), Autogenic relaxation Training(自律弛緩療法) basing on oriental medical theory not separating body and mind.

Factors Influencing Activities-specific Balance Confidence in Community-dwelling Old Adults (지역사회 거주 노인의 활동 특이적 균형자신감에 영향을 미치는 요인)

  • Purpose: The purpose of this study is to identify factors influencing activities-specific balance confidence in community-dwelling older adults. Methods: This is secondary analysis of data from an intervention study for improving cognitive function. The data were collected from March 2 to September 30, 2017 at a senior center. Data of 131 older adults were included for this secondary analysis, and were analyzed by using t-test, ANOVA, and multiple regression. Results: The mean score of activities-specific balance confidence is 65.08 out of a possible range of 0-100. The significant factors affecting activities-specific balance confidence among old adults include 'more than 85 years old', 'waist circumference', 'depressive symptoms', 'activity restriction due to fear of falling', and 'self-rated health' which explained 52.8% of the variance. Conclusion: The study results indicate that psychologic factors as well as physical condition should be considered for interventions to increase activities-specific balance confidence.

Social Support and burden in mothers of children with cancer (암환아 어머니의 사회적지지와 부담감)

  • The purpose of the study was to identify the degree of social support and burden in mothers of children with cancer, and to determine a strategy of an effective social support utility for burden relief in these mothers. The subjects of this study were consisted of mothers of cancer children, registered at a'C'University affiliated hospital in Taejon. The data was collected from July 1 to August 31, 1994. Three instruments were used to collect the data : a semi-structured interview questionnaire which was developed by the researcher was used to identify the content of the mother's burden, a Visual Analogue Scale(VAS) that had a 10cm horizontal line was used to measure the degree of mother's burden and the PRQ part I & part Ⅱ were used to measure the degree of mother's social support. The content of mother's burden collected through in depth interviews was analyzed using content analysis. Also burden and social support data were analyzed by SAS program. The result of this study were summarized as follows : 1. The data on burden content from the interview were categorized as psychological, physiological, family interaction with maternal role, financial burden and personal interaction with social relational burden. 2. The degree of burden measured by VAS had a mean of 8.04(range from 0.5 to 10). 3. The score for social support measured by PRQ part Ⅱ had a mean of 88.9(range from 71 to 113). The highest scores was for reassurance of worth. The lowest score was for opportunity for nuturance. Most of the 15 mothers received the greatest amount of support from their sisters, spouse, friends, neighbors, relations, priest were perceived as eaningful resource person. 4. The correlation coefficient of burden and social support was somewhat negative correlation but no statistical significance(r=-.072). Therefore, a further study is necessary to repeat the qualitative research for exploring factor to be affected family caregiver's burden according to disease proceeding stage. On the basis of the results from this study, future research will be promoted valid and reliable tool development. Through this study, nurses understand and assess the individual psychologic burden and further it would be recommendated to produce professional education program for pediatric oncologic specialist nurse.

  • Bilateral cleft lip reconstruction can be performed in one-or two-stages. The choice depends on the surgeon's proficiency and experience. However the type and severity of the cleft must be considered. Complete or incomplete symmetric, bilateral clefts in which the premaxilla is within the alveolar arch or slightly protruded can be successfully treated with simultaneous lip reconstruction on both sides. There are some advantages that one-stage lip reconstruction facilitates creation of a symmetric and balanced lip, retraction of protruding premaxilla and the parent's psychologic satisfaction. However, there are some disadvantages such as micrognathia of the upper jaw by severe scar formation and poor adaptation of flap margins by severe tension. The surgeon must be able to judge and exploit the advantages offered by one-stage approach. We present the case report of one-stage reconstruction of primary bilateral cleft lip with literature reviews.

A Clinical Report about Hiccup and Eructation from Emotional Stress (칠정(七情) 자극 후 발생한 해역(咳逆).희기(噫氣) 환자 치혐 1례)

  • This case show that emotional stress and wekness of gastrointestinal function occur a chronic hiccup and eructation(during 3 months), oriental medical therapy can cure completely this symptom. Seven emotional stress(anger, delight, sadness, thinking, worry, fear, horror) occur various pathologic changes. Hiccup is caused by CNS, psychologic, thoracic, abdominal factor etc. In oriental medicine, GI trouble regarded as a very importent factor that occur hiccup and eructation. Besides, emotional stress is a very important factor as well. This patient have an introvert personality ordinary times. One day, she had a miscarriage by hers husband's assault. After that time, she suffered from very serious GI trouble(dyspepsia, epigastric pain, anorexia). And then hiccup, eructation, weight loss(10kg/3months) is occured. This patient diagnosed only gastritis. This symptom did not stop during 3 months, regardless of western medical therapy(some western drugs medication, fluid supply etc). After oriental medical therapy(herb medicine;Yukunjatang, acupuncture;CV12, CV6, S36, CV17, H7, SP6, P6, moxibustion;VI2, CV6, S36, negative therapy;back area and traditional oriental medical psychotherapy;ventilation, support, desensitization and family interview) put in operation during 15 days, patient completely recover from hiccup, eructation and anxiety disorders.

The Effect of School Performance and Stress on Depressive Symptoms Among Youths (서울시내 일부 고등학교 학생들에서 학업성취도가 우울 증상에 미치는 영향)

  • Depression is one of the most common psychologic disorder and dealt as a major public health concern. School students have heavy school work loads and stress from various evaluations representing school performance. In this study, to examine the effect of school performance and stress on depressive symptoms, we assessed the frequency of risk factors for depressive symptoms among 2,717 high school students. From April 2001 to May 2001, using a self-reported questionnaire, we collected data on the frequency of occurrence of depressive symptoms and potential risk factors for depressive symptoms. Depressive symptoms were measured using Center Epidemiologlc Studies Depression (DES-D) index. Thirty-seven percent of high school students reported having high depressive symptoms for a week Female students had a higher prevalence of depressive symptoms than males (OR=1.44, 95% CI 1.22-1.70). In a multiple logistic regression model, we found that good perceived school performance (OR=0.43, 95% CI 0.31-0.60) had a negative association with depressive symptoms. On the contrary, we also found that stress from school performance (OR=2.03, 95% CI 1.69-2.44) associated with high depressive symptoms. In conclusion, high school students had a relatively high prevalence of depressive symptoms. Students with stress from school performance had the higher risk for depressive symptoms.

An Analysis of Research Related to the Massage as the Nursing Intervention (간호중재로서의 마사지 적용연구 분석)

  • Purpose: The purpose of this study was to identified patterns of studies and to analyze the nursing research related to the massage intervention in Korea and to improve direction of nursing research in massage intervention in Korea. Methods: The research studies related to the massage were selected from the Korean Nurses Academic Society Journal and dissertation which were conducted experimental research design the last 7 years in Korea. The total numbers of the studies were 58. Result: The study subjects were post operational state in 34.5%, healthy adult in 17.2% studies. The others dealt with infant, cancer, dementia, hypertension, preterm neonates, elderly. Types of massage were whole body in 31.1%, foot in 22.6%, hand in 17.2%, back in 13.8% studies. And Meridian massage in 17.2%. Length of massage was below 5 minutes in 17.2%, 6~10 minutes in 29.4% and 11~20 minutes in 32.8% studies. Frequency of massage was only 1 time in 25.9%, 2~5 times in 31.1% studies. Massage produced psychological and physiological effects. Positive psychologic effects were anxiety relief, pain relief, mood enhancement and fatigue relief. Positive physiologic effects on body systems were blood pressure, heart rate, edema, skin temperature, immune system, blood glucose, excretion, PMS syndrome and growth and development. Conclusion: As a result, effects of massage were almost positive. Therefore massage is useful alternative nursing intervention. But more attention research is needed to identify the objectivity instrument to measure and repeat research.

  • Objectives : The symptom of torticollis caused by abnormal contraction in cervical muscles, is easily found among those who get much stress physically and mentally in modern society. Due to this disease, the patients suffer not only from psychologic distress for appearance but also from functional disorder. Therefore, this case is to look for the effective treatment and understandings on the spasmodic torticollis. Methods : Herb medicine is used Guibi-tang(歸脾湯), Heosu-san(回首散), Hyangso-san(香蘇散) and Samul-tang(四物湯). Manipulation therapy is used stretching and meassage technique on cervical muscles. Self exercise of cervical and lumbosacral muscles, physical therapy with ultra sound carried out. Result and Conclusion : Acquired spasmodic torticollis effectively is treated with herb medicine, manipulation, exercise and physical therapy.

Apparel Store Patronage Behaviors on Store Types (의류점포 유형별 점포애고행동에 관한 연구 -부산지역을 중심으로-)

  • The purpose of this study were to examine the types of apparel store, to investigate consumer characteristics related to the apparel store patronage behaviors, and to find out the differences of casual relationships on apparel store patronage behaviors in store types. Data were collected from 451 consumers living in Pusan and analysed by factor analysis, ANOVA and path analysis. The results were as followings : First, the types of apparel store recognized by consumers were composed by five factors such as Casual-wear store, Traditional market store, Neighboring store, Department store, and Small-sized department store. Second, apparel store patronage behaviors have significant differences recreational and economic shopping orientations, Fashionability in store evaluative creteria, social psychologic risks in perceived risks, and observation store information source. Third, there were differences in casual relationships an apparel store patronage behaviors across store types. Casual-wear store patronage behaviors were directly influenced by the information source, and traditional market store patronage behaviors did by the perceived risk. Apparel shopping orientations directly influenced Neighboring store, and department store patronage behaviors. Evaluative creteria of store directly did small-sized department store. That is, consumer characteristics directly influencing to patronage behaviors were different across apparel store types. Therefore, the marketers of apparel store should understand consumer characteristics related to patronage behaviors and affecting variable and then try to encourage consumer's store patronage behavior.
