Norepinephrine and Serotonin in the Patients with Psychogenic Impotence (심인성 발기부전 환자에서 Norepinephrine과 Serotonin에 관한 연구)

  • Various neurotransmitters have been proposed as possible mediators of penile erection. Especially, norepinephrine and serotonin might have a important role in sexual arousal and penile erection. And it could be hypothesized that the psychogenic impotence is associated with the depletion or imbalance of norepinephrine and serotonin from evidences, such as the symptomatic manifestation of depression and the antidepressantinduced sexual dysfunction. The authors investigates the association of norepienphrine and serotonin with psychogenic impotence. The psychogenic impotent group(PIG) consisted of twenty-three patients with psychogenic impotence and the controlled group(CG) consisted of twenty-seven patients without psychogenic impotence. PIG had no organic cause accounting for their erectile dysfunction. The Beck Depression Inventory(BDI) and the State-Trait Anxiety Inventory(STAI) were applied to each subject to assess mood, state anxiety(SA) and trait anxiety(TA). Plasma norepinephrine level from systemic blood and 5-hydroxyindoleacetic acid(HIAA) levels from 24-hours urine were measured in each subject. The mean score of BDI of PIG was significantly higher than that of CG(p=0.015). PIG had a tendency of higher TA compared with CG(p=0.054). And also SA was higher in PIG, but did not show significant difference(p=0.193). The level of norepinephrine was significantly lower in patient with psychogenic impotence(p=0.000). And the level of 24-hours urine 5-HIAA was lower in PIG but did not show significant difference(p=0.494). Although the authors did not exclude depressive disorders in PIG, the present findings suggest that psychogenic impotence might have higher depressive mood and trait anxiety, and be associated with the depletion of norepinephrine in systemic blood.

  • Objectives : Abnormal movements are among the most common psychogenic symptoms, and psychogenic movement disorders(PMD) are estimated to account for 2-3% of patients in movement disorders clinics. Psychogenic tremor is the most frequent symptoms among psychogenic movement disorders. There are no treatment studies specific to PMD and lack any generally accepted treatment strategies. Methods : In this case, a 40 years old women had been suffered from head tremor for 4-5 years without any evidence for other neurological disorders and medical problems. Her symptom caused by stress events and severity of symptom had been change the level of by stress. We treated her with korean traditional herbal medicine(Chengsimyeonjatang), Qigong-based Stress Reduction Program(BQSRP), Giungoroen(wise saying and lofty opinion) and Gyeongjapyeongji(desensitizing the unease) for 2 months. Results : After treatment, the severity and frequency of the head tremors were decreased. In addition, anxiety level also decreased. Conclusions : This result suggests that our oriental medical treatments was effective on psychogenic tremor.

  • Psychogenic vomiting (七情吐) is the disease of vomiting due to seven emotions that joy, anger, melancholy, anxiety, grief, fear and terror being the response of the mind to the environmental stimuli. We experienced a 21year-old man who had a psychogenic vomiting due to maladjustment in a military life, and whose condition was improved through oriental medical treatment. We treated the patient with Herbal medications and Giungoroen (至言高論)-wise saying and lofty opinion). Giungoroen is psychological therapy that promotes patient's recognition of disease and will to cure it through conversation. After being treated, the patient showed that symptoms (vomiting, nausea, abdominal discomfort, insomnia, a depressed mood, a feeling of uneasiness) was improved considerably. Tills result suggests that oriental medical treatment bas good effect on psychogenic vomiting due to adjustment disorder.

Psychogenic Dizziness Patients Treated with Korean Medicine Treatment Based on Yanshimtang-Gamibang: Three Case Reports (양심탕가미방(養心湯加味方)을 기본으로 한 한방치료로 호전된 심인성 어지럼증 치험 3례)

  • Objectives: To propose Korean medicine treatments based on effects of Yanshimtang-Gamibang on three patients diagnosed with psychogenic dizziness. Methods: Korean medicine treatments based on Yanshimtang-Gamibang were applied to three patients diagnosed with psychogenic dizziness after experiencing a traffic accident. Effects of Korean medicine treatments based on Yanshimtang-Gamibang were evaluated using DHI (Dizziness Handicap Inventory), BAI (Beck Anxiety Inventory), NRS (Numerical rating scale), NDI (Neck Disability Index), ODI (Oswestry Disability Index), and EQ-5D (EuroQol-Five Dimensions). Quality of sleep was assessed with total sleep time and number of times waking up during sleep. Results: After the treatment, there were significant decreases in DHI and BAI scores as well as values of NRS, NDI, ODI, EQ-5D. The sleep quality of three patients also showed improvement. Conclusions: Korean medicine treatments based on Yanshimtang-Gamibang might be effective in people with psychogenic dizziness.

  • Psychogenic cough is barky, croupy, and explosive and disappears during sleep, and is not affected by antitussive drugs. Physical and radiological examinations of the respiratory tract and other routine laboratory test are normal. It is helpful to recognize this condition early in its course so that appropriate treatment can be instituted before iatrogenic problems resulting in multiple hospitalizations and chronic steroid use occur. We experienced two child and adolescent cases that showing typical psychogenic cough so reported it with the literatures review.

  • A psychogenic seizure is a disorder which is occasionally met by neurologists and psychiatrists. The most important thing that clinicians should do is differential diagnosis with true epileptic seizures. Especially psychiatrists are expected to be able to diagnose psychogenic seizures through a thorough history taking and an observation of clinical features to some degree before the confirmatory diagnoses. We experienced a case of 11-year-old girl with psychogenic seizure which was characterized by obvious psychological and environmental stress, clinical features, and good outcome by prompt a loboratoy examination and psychiatric intervention. To diagnose and treat childhood psychogenic seizures, we suggest that it is very important to examine the familial background and take a thorough history, and perform psychoeducational intervention in addition to electroencephalogram.

Diagnosis and successful visual biofeedback therapy using fiberoptic endoscopic evaluation of swallowing in a young adult patient with psychogenic dysphagia: a case report

  • Psychogenic dysphagia is a deglutition disorder characterized by a fear of swallowing, with no structural or functional causes. This report presents the case of a young male patient who had severe malnutrition due to psychogenic dysphagia and was provided visual biofeedback using fiberoptic endoscopic evaluation of swallowing (FEES). A healthy 25-year-old man presented to our clinic with a complaint of throat discomfort when swallowing that had started 6 months prior. As the symptoms worsened, he became fearful of food spreading to his lungs after swallowing and the development of respiratory difficulties. His food intake gradually decreased, resulting in a weight loss of 20 kg within 2 months. Evaluation of organic and other functional causes of dysphagia was performed, but no abnormalities were detected. The sensation of a lump in his throat, fear of swallowing, and anxiety were transformed into somatic symptoms. The patient was diagnosed with psychogenic dysphagia. After visual biofeedback by a physician who performed FEES, the patient resumed eating normally and increased his food intake. If routine tests do not reveal structural or functional causes of dysphagia, assessment of a psychogenic swallowing disorder should be considered. FEES can help in the diagnosis and management of psychogenic dysphagia.

Psychogenic Dizziness for Psychiatrists in Korea (정신건강의학과 의사를 위한 심인성 어지럼)

  • Many patients with dizziness present with a symptom pattern that does not reveal the cause by neurotologic diagnostic approaches. In such cases, the physician frequently diagnoses psychogenic dizziness. Psychogenic dizziness is not characterized by true vertigo, and occurs in combination with other psychiatric symptom cluster. One out of two to four patients with dizziness are psychogenic dizziness. But there are few concern about this including clinical practice and study in Korea. I wrote this paper to increase concerning and attending to this for psychiatrists in Korea. I reviewed etiology including biological and psychological relations between dizziness and psychiatric disorder(especially anxiety), diagnostic approaches of, characteristics of dizziness of various psychiatric disorders related to, and the treatment of psychogenic dizziness. I also briefly reviewed the central and peripheral dizziness for psychiatrists. I suggest psychiatrists and clinicians in the psychosomatic field in Korea to acknowledge, concern, and attend to psychogenic dizziness. In turn, it will be helpful to well treat the patients with psychogenic dizziness.

A Case Report of the Herbal Medication Treatment and Psychotherapy for a Patient with Psychogenic Chronic Low Back Pain (심인성(心因性) 만성요통(慢性腰痛) 환자에 대해 한방치료(韓方治療)와 정신요법(精神療法)을 적용한 1례)

  • In this study, we treated a patient with psychogenic chronic low back pain for 2 years with herbal medication treatment and psychotherapy. The case was a 17 years old female patient complained chronic low back pain for a long time. She received the spine operation, epidural injection, many kind of oriental medicine treatment about low back pain, but there was no improvement. We diagnosed her pain as a psychogenic pain disorder, and treated her with herbal medication for decrease her stress, and did a psychotherapy such as the progressive muscle relaxation, and counseled about trouble with her family. After treatment, her chronic low back pain was improved so much. We think that the herbal medication treatment and psychotherapy can be helpful to treat patients who have psychic and physical disorders.

A Clinical report on psychogenic torticollis (심인성 사경증 환자 치험 1례)

  • Objectives : The purpose of this study was to investigate the clinical application of Korean Traditional treatment to psychogenic torticollis patient who has panic disorder as underlying disease. Methods : Herb medicine used in this study are Heosu-san(Huishou-san) which is previously used for torticollis in several studies, Guibi-tang(Guipi-tang) and Gamiondam-tang(Jiaweiwendan-tang) which are frequently used for phychological disease. Also, physical therapy and aroma therapy were practiced which basically stretches and relaxes muscles around cervical muscles. Self exercise of cervical muscles was carried out during acupuncture. Results : After the comprehensive treatment, the clinical symptoms of the patient were improved. Conclusions : It is suggested that oriental medical treatment are effective in psychogenic torticollis.
