N,N,N-Trimethylphytosphingosine (TMP)의 염증성 피부질환 치료제 가능성에 관한 연구 (Study for Possibility of N,N,N-Trimethylphytosphingosine (TMP) for Management of Chronic Skin Diseases)

  • Skin disease is one of the most common diseases and its incidence is increasing dramatically in modern society. Specially, many attempts have been made to treat chronic skin inflammation diseases, such as psoriasis and atopic dermatitis, but effective therapies for the immune cell-mediated skin diseases, including psoriasis and atopic dermatitis have not been developed. Until recently, several drug candidates which were claimed to be effective for skin diseases have been reported, but most of them are not used to treat chronic skin disease. Especially, Psoriasis is characterized by excessive growth and aberrant differentiation of keratinocytes, but is fully reversible with appropriate therapy. The trigger of the keratinocyte response is thought to be activation of the cellular immune system, with T cells and various immune-related cytokines. Formation of new blood vessels starts with early psoriatic changes and disappears with disease clearance. Several angiogenic mediators are up-regulated in psoriasis development. Contact- and mediator-dependent factors derived from keratinocytes, mast cells and immune cells may contribute to the strong blood vessel formation of psoriasis. New technologies and experimental models provide new insights into the role of angiogenesis in psoriasis pathogenesis. TMP and its derivatives themselves effectively inhibited in vitro cell migration, tube formation, and the expression of angiogenic factors. However, TMP and its derivatives induced side effects including hemolysis and local side effects. Therefore, in an attempt to reduce the toxicity and the undesirable side effects of TMP and derivatives, a liposomal formulation was prepared and tested for its effectiveness. TMP and derivatives liposomes retained the effectiveness of TMP in vitro while side effects were reduced. These results support the conclusion that TMP effectively inhibits in vitro angiogenesis, with the possibility that use as a psoriasis relief agent.

백호탕을 통한 건선의 치험 2례 (Two Cases of Psoriasis Patients Treated with Baekho-tang)

  • This paper confirmed the effect of Baekho-tang(白虎湯) on psoriasis patient diagnosed with Yangmyung-Yeoljeung(陽明熱證). We treated patients with psoriasis classified as Yangmyung-Yeoljeung(陽明熱證) according to Shanghanlun(傷寒論) with Baekho-tang(白虎湯). Two patients was treated with Baekho-tang(白虎湯) for 18 days and 75 days, respectively. Baekho-tang(白虎湯) used in the treatment consisted of Gypsum Fibrosum 16g, Oryza semen 12.5g, Anemarrhena Rhizoma 6g and Glycyrrhizae Radix 2g. The severity of psoriasis was evaluated by PASI(psoriasis area severity index) and pictures. Also we checked the change in Pruritus by VAS(visual analogue scale). After the Baekho-tang(白虎湯) treatment, the PASI of 2 patients decreased from 13 to 2.8 and from 10.8 to 0.8 respectively. In addition, VAS of 2 patients diminished from 10 to 3 and from 8 to 1 respectively. Moreover, no side effects were observed. We received effective results by giving Baekho-tang to Psoriasis patients with sweaty, dry mouth, and heating sensation. Anti-inflammatory, antipyretic effects and immunomodulatory functions of Baekho-tang(白虎湯) could lead to improvement of psoriasis.

전향적 전후비교환자군과 다중회귀분석방법을 활용한 변증연구 - 117명 건선환자의 양허증과 혈열증를 중심으로 - (A Symptom Differentiation based on Prospective pre-post intervention design and multiple regression analysis in Korean medicine - Focus on Yang Deficiency and Blood Heat Type Psoriasis -)

  • Objectives: To study symptom differentiation for yang deficiency and blood heat based on 117 psoriasis patients. Methods: Obtained frequency analysis (%), mean (%), and OR, CL(P-value) with prospective pre-post intervention design and multiple regression analysis. Results: There were no statistically significant differences as to sex, BMI, smoking and marital status, but there were statistically significant differences in variables of average age, family history, and alcohol consumption (P<0.05). There were no statistically significant differences in psoriasis symptoms as to initial age of onset, morbidity span, area of the initial onset, and the progression of psoriasis during the last three months of each patient. However, the type of psoriasis showed a statistically significant difference (p=0.011). While there were no statistically significant differences as to common cold, condition of psoriasis after recovering from the cold, skin condition, exercise, and seasonality, irregular perspiration showed significant difference (p<0.00). When confounding factors have been controlled the blood heat patient group as the comparison group, multiple regression analysis showed OR, CI(95%) of 1.06(0.31-3.63) for men, 0.28(0.08-1.06) for aged 30 to 49,0 and 0.18(0.04-0.80) for aged 50 and older. it was 0.06(0.01-0.7) for family history, 1.06(0.29-3.88) for drink alcohol, 19.90(2.53-156.7) for seasonality, and 10.28 (3.19-33.11) for perspiration problems. In these variables, Sex, age, smoking, and alcohol consumption showed no statistically significant results, but family history(p=0.049), seasonality(p=0.005), and irregular perspiration (p=0.017) were statistically significant. Conclusion: Family history, seasonality and irregular perspiration are the determining factors for yang deficiency and blood heat in psoriasis.

향기요법을 사용한 건선환자의 치험 1례 보고 (A Case of Psoriasis Treated with Aromatherapy)

  • Psoriasis is a common skin disease and the difficulty of its medical management is well known, A report was done on psoriaSis which were treated with aroma therapy to be consulted in Dept. surgery, opthalmology & otolarygology, college of Oriental Medicine, Dongeui University, from Dec. 24, 2003 to Mar. 5, 2004. We gave score and checked involvement in plaque, erythematous popules and invasiveness according to psoriasis area and severity index (PASI). After 75 days of treatment the patient showed that symptoms were disappeared and score of PASI changed from 54.7 to 0. These results that Aroma therapy has good effects on psoriasis in assistant therapy.

대양병(大陽病) 백호가인삼탕(白虎加人蔘湯)으로 호전된 건선 치험 1례 (A Case Report of Psoriasis treated by Bekhogainsam-tang)

  • Objective : This study reports the effect of Bekhogainsam-tang on psoriasis. Methods : This is a case report on a patient who has been suffering from psorasis. According to Diagnostic system based on Shanghanlun provisions, Bekhogainsam-tang was provided for 47 days. The severity of psoriasis was evaluated by PASI score and pictures. Results : After taking Bekhogainsam-tang for 47 days, PASI score decreased $1.4{\rightarrow}0.2$ Conclusions : This case report showed an effectiveness of using Bekhogainsam-tang according to Diagnostic system based on Shanghanlun provisions on psoriasis.

  • Objective : There was an improvement in a patient with psoriasis following the intake of Hoeyeok-tang for 317 days. By analyzing these cases, we reviewed the use of Hoeyeok-tang in skin diseases and attempted to interpret the related Shanghanlun articles. Methods : The diagnostic process was analyzed according to the disease pattern identification diagnostic system based on the Shanghanlun provisions. The progression of psoriasis, which was addressed, was assessed using the PASI score. Results : The patient was diagnosed with Soeum-byung 324, and the PASI score decreased from 13.6 to 0.4 after the intake of Hoeyeok-tang for 317 days. The patient's compliance and expectation from treatment were good, and no specific adverse effect was identified. Conclusions : This study suggests the clinical use of Hoeyeok-tang in psoriasis based on Shanghanlun.

전신성(全身性) 건선(乾癬) 치험 1례(治驗 1例) (A Case of the Systemic Psoriasis)

  • Objective : This study is designed to investigate the effect of Oriental medical therapy on psoriasis. Methods : We applied Aroma ointment to abnormal skin two times a day. For herbal medicine, Bangpungtongsungsan-gami was given. We checked involvement in scaly skin, erythematosus papules and infiltration according to psoriasis area and severity(PASI) Results : After treatment, the patient showed that itching symptoms was disappeared and score of PASI changed from 27 to 0.8 Conclusions : According to the result, Oriental medical therapy and Aroma therapy has a good effect on psoriasis. But further studies are required to definitely prove the effectiveness of Aroma therapy

Traditional topical herbal therapies in psoriasis

  • Psoriasis is a common, chronic, recurrent, immune-mediated inflammatory disorder of skin. Although conventional topical and systemic therapies yield remission for a while, they carry a risk of many side effects that limit the long term use of these agents and the disease recurs generally in a short period of time after cessation of therapy. An increasing number of psoriatic patients are using herbal products as a treatment to control their disease with less side effects. Clinicians should be aware of plants used in psoriasis and side effects related with them. The aim of this article is to review the different herbs used in psoriasis treatment.

전신성 건선 환자의 한방 치료 임상 보고 1례 (A Clinical Case of Systemic Psoriasis with Korean Medical Treatment)

  • Objectives : The purpose of this study is to report the Sik-yang-seng(食養生) therapy with Korean Medicine and acupuncture on a chronic psoriasis patient who is occurred to latrogenic cushing's syndrome. Methods : The patient who has been diagnosed psoriasis had symptoms with latrogenic Cushing's syndrome. The patient was treated with Sik-yang-seng(食養生) therapy including Korean Medicine and acupuncture. Evaluation was done by PASI score, itching score, statistics and pictures. Results : After treatment, the PASI score of patients with Sik-yang-seng(食養生) therapy including Korean Medicine and acupuncture was 1.8 which means 95.5% improvement. Conclusions : Korean medical treatment performed with Sik-yang-seng(食養生) therapy can help recovery of Biwi(脾胃)function therefore can increase effect of the treatment of psoriasis and latrogenic Cushing's syndrome.

Koebner Phenomenon Delays Therapeutic Effect of Pulsed Dye Laser on Refractory Psoriasis

  • Psoriasis is a common chronic inflammatory skin disease that is histopathologically characterized by abnormal differentiation and hyperproliferation of keratinocytes, infiltration of T lymphocytes, and alternations of the dermal vasculature. Pulsed dye laser (PDL) is commonly used to treat cutaneous vascular lesions, and it has been studied for the treatment of localized psoriatic lesions. A 30-year-old female patient with refractory psoriasis on her forehead was treated using PDL. After two sessions, the size of the psoriatic lesion increased, which might have occurred due to koebnerization. As the patient continued to receive PDL treatment, the lesion showed significant improvement with no recurrence on the one-year follow-up. We present here a case of refractory psoriasis treated with PDL and demonstrates an association between a delayed therapeutic effect and the Koebner phenomenon.
