Comparison of Continuous Epidural Infusion of Narcotic Analgesics and Local Anesthetics Using 2-day Infusor and Meperidine IM. on Postoperative Analgesia (2-day Infusor를 이용한 마약성 진통제와 국소마취제의 지속적인 경막외 투여와 Meperidine근주와의 통증 치료 비교)

  • Recently, continuous epidural infusion of narcotics and local anesthetics have been used for postoperative pain relief. This study was designed to compare the analgesic efficacy and side effects of continuous epidural infusion of narcotics and local anesthetics with those of intramuscular administration of meperidine, for postoperative pain relief after cesarean section. Forty patients were divided into 2 groups of 20 patients each ; Continuous epidural group and control (IM meperidine) group. Before each operation, the epidural group had an epidural catheter placed (L1-2) and following each operation, a bolus of 1%~8ml of lidocaine was injected, followed by continuous infusion of morphine 3 mg/day, fentanyl 300g, 2% mepivacaine 20 ml, 0.5% bupivacaine 20 ml and normal saline 40 ml. The control group received meperidine 50mg IM injection as needed. We evaluated analgesic efficacy with VAS (Visual analogue scale) and side effect at 1, 6, 12, 24, 36 and 48 hour intervals after the operation. The results were as follows: 1) Continuous epidural group was superior to the control group with respect to postoperative analgesia. 2) Side effects (pruritus, nausea & vomiting) were more frequent in the epidural group.

Cutaneous Manifestations Associated with Liver Diseases (간질환에 동반된 피부증상에 관한 연구)

  • It has been well recognized that the various cutaneous manifestations associated with the liver diseases. A clinical study was made of 255 patients (AVH 84, LC 70, HC 41, CAH 26, CPH 23, AH11) with the liver diseases at Yeungnam University Hospital during the periods from May to November, 1985. The authors classified the cutaneous manifestations into 7 groups according to pathogenesis, and compared them with other reports. The results were as follows; 1. In 255 patients with various liver diseases, 161 patients (63%) showed the various cutaneous manifestations. 2. The various cutaneous manifestations were jaundice and/or pruritus (43.1%), vascular changes (39.6%), allergic manifestations (10.6%), nail changes (5.1%), hormone-induced changes (4.3%), pigmentary changes (3.5%) and others (2.4%) in that order. 3. Cutaneous manifestations were associated most frequently with liver cirrhosis (1.6 groups) and the least with chronic active hepatitis (0.7 group). 4. Allergic manifestations were seen mainly in patients with acute viral hepatitis. Three patients showed the serum sickness-like prodrome. 5. The other cutaneous manifestations were seen mainly in patients with chronic liver diseases.

A Statistics Study of Surgery, Ophthalmology & Otolaryngology (외관과(外官科) 외래환자의 통계적 고찰)

  • We analyzed statistics study in 1695 patients, who had visited to the Dept. of surgery, ophthalmology & otolaryngology Hospital of Downtown Orirental Medicine Kyung Hee University from March, 1996 to July, 1998. The results were as follows. 1. Distribution of ophthalmology, otolaryngology and dermatology classification was $43.48\%\;in\;ophthalmology,\;16.70\%\;in\;rhinology,\;5.72\%\;in\;laryngology,\;17.94\%\;in\;dermatology\;and\;16.17\%$ in otiatrics. 2. Ophthalmology group were $31.93\%$ males and $47.90\%$ females in 10 years old within. In treatment term, opthalmology patients who had treated for 1 month within was $59.70\%$. Strabismus and visual disorder was the most common ophthalmo-disease. 3. Rhinology group were $30.00\%$ males in 10 years old within and $21.56\%$ females in 21-30years old. In treatmentm, rhinology patients who had treated for 1 month within was $53.71\%$. Rhinitis and sinusitis was the most common rhinio-disease. 4. Otiatrics group were $30.54\%$ males in 41-50 years old and $31.78\%$ females in 51-60 years old. In treatment term, otiatries patients who had treated for 1 month within was $53.28\%$. Tinnitus, hearing loss and otitis media was the most common otiatrics-disease. 5. Laryngology group were $29.27\%$ males in 31-40 years old and $28.57\%$ females in 51-60 years old. In treatment term. Laryngology patients who had treated for 1 month within was $91.75\%$ Epipharyngitis and abnormal sensation pharyngolarylgeal region was the most common laryngo-disease. 6. Dermatology group were $39.82\%$ males and $39.79\%$ females in 21-30 years old. In treatment term, dematology patients who had treated for 1 month within was $83.55\%$. Acne and Pruritus was the most common ophthalmo-disease.

The purpose of this study This study was performed on the basis of clinical consideration about rhinitis patients who suffered from sneeze, rhinorrhea, rhinocleisis and etc. And we checked the clinical inclination of this disease and the characteristic treatments of Sasang Constitutional Medicine. 2. The method of this study For 17 months from October in 1999 to April in 2001 we selected outpatients who had consulted for rhinitis more than twice in the department of the Sasang Constitution of the Bundang Oriental medical hospital, Dongguk University, and 31 of those were confirmed of their course of treatment. From their medical records and history, we learned the general characterisics and patterns of rhinitis, and the qualities and effects of its various treatment. 3. The result of this study The result were as follows ; 1) The rate of patients who have had the symptoms over the two years was 74.2%. 2) In the distribution of chief symptoms, sneeze occupied 80.6% of all appealed symptoms, rhinocleisis 77.4%, rhinorrhea 71.0%, intranasal pruritus 54.8%, postnasal drip 16.1%, orderly. 3) The treatment period was about 30days. The method of treatment was the Herb-med Tx, acupuncture Tx and etc. The herb-med ingestion was about two times in that period. 4) On the effect of treatment, Sneeze and rhinocleisis were improved. 5) The symptoms of rhinitis was more depravated by fatigue and stressful condition than weather condition. This result shows that the improvement of general condition has an effect on the rhinitis treatment.

Yangkyuksanhwa-tang effected to Atopic Dermatitis (양격산화탕(凉膈散火湯)이 Atopy 피부염(皮膚炎)에 미치는 영향(影響))

  • 1. Objectives Yangkyuksanhwa-tang is used mush in pruritus and dermatopathy of Soyangin. It is suggested this prescription is effective on atopy dermatitis. 2. Methods For observation of Yangkyuksanhwa-tang effected to atopic dermatitis, extract of Yangkyuksanhwa-tang has been dispensed to the stratum corneum of epithelium in dermatome of murine after making damage to its defense mechanism against fat and causing atopic dermatitis artificially. After that, the change in outer dermatome and minute mechanism of epidermis, the change of eosinophil, the change in distribution of soybean agglutinin, the change in distribution of fat and ceramide in stratum corneum, the change in inflammation in dermatome, the change of cell accrementition and apoptosis, and the effect on anaphylaxis and Staphylococcus aureus was observed. 3. Results After administration of Yangkyuksanhwa-tang, severe skin damage such as eczema and psoriasis, that was observed in the case of atopy dermatitis, was decreased and the increase of eosinophil in serum was suppressed. Lipid lamella was recovered, so epidermal demage was relieved. The distribution of HSP70 in the outer skin was decreased. Yangkyuksanhwa-tang suppressed activation of $NF-_{\kappa}B$ p50, induced CD11/18b not to be generated, and suppressed inflammatory response of skin. Anaphylaxis and groth of Staphylococcus aureus was suppressed. 4. Conclusions Yangkyuksanhwa-tang decreased skin damage of atopy dermatitis. It has antibiosis about Staphylococcus aureus, it can be medicinal substances on atopy dermatitis. In addition, it is possible that it can be medicinal substances on regional skin allergy.

A Clinical Analysis on 117 Patients with Urticaria Based on Sasang Constitutional Medicine (117명 두드러기 환자의 특성에 대한 사상체질적 임상분석)

  • Objectives The purpose of this study was to find out whether there were differences of urticaria's disease aspect between patients classified into Sasang Constitution Types(SCTs) and whether those differences could be associated with Ordinary symptoms. Methods Medical records and questionnaires about 117 patients who visited one Korean medicine hospital due to urticaria were collated and statistically analyzed. Results 1) Ages 20 to 30, women, Soeumin(SE) and patients in conditions of chronic urticaria over 6 weeks were the majority among 117 patients in this study. Food and stress were most chosen as the main cause of urticaria. 2) Soyangin(SY) showed more severe symptoms of urticaria than other SCTs. In particular, the severity of pruritus, distribution of lesions and vulnerability to stress was statistically significant compared to other SCTs(p<0.05). SE expressed urticaria's symptoms at the medium-level of SY's and Taeeumin(TE)'s. TE exhibited relatively weak symptoms but TE only had slightly higher number of patients with angioedema compared to other SCTs. Taeyangin(TY) was only one case so more researches are needed. 3) In dispositional symptoms, SY had low quality of sleep and defecation. SE could not digest oily food well, felt dizzy and cold well, and had cold hands and feet. TE could eat and sweat much and tended to snore well. Conclusions In this study, urticaria had common cause of both stress and food in all SCTs but onset and severity of symptoms were different between SCTs. It could interpreted that these differences between SCTs were associated with ordinary symptoms native to each SCT.

Continuous Intravenous Infusion of Morphine and Ketorolac for Postoperative Pain (Morphine과 Ketorolac의 지속적 정주에 의한 술후 통증 완화 효과)

  • Despite their sometimes fatal complications such as respiratory depression when used for postoperative pain control, intravenous and epidural narcotics remain the mainstay of treatment regimens. Because of the problems, anesthesiologists are seeking alternatives. We compared the analgesic effect and complications of continuous intravenous morphine with ketorolac. Ketorolac is a non-steroidal agent with potent analgesics and moderate anti-inflammatory activity. Forty ASA physical status I or II patients were given morphine(20 patients) or ketorolac(20 patients):In the morphine group, an initial bolus dose of 2 mg i.v. was given followed by continuous infusion at a rate of 1 mg/hr for 48 hours. The ketorolac group was given initial bolus of 30 mg i.v. This was followed by continuous infusion at a rate of 3.75 mg/hr for 48 hours using a Baxter Daymate Infuser. We checked systolic, diastolic and mean arterial pressure, heart rate, visual analogue scale(VAS) and the Prince Henry Score(PHS). This was done before the initial bolus, at 5, 15, 30 and 60 min, at 2, 6, 12, 24 and 48 hours after administration. We observed the side effects nausea and vomiting, pruritus, hypotension, somnolence, urinary retention and respiratory depression. From our study we believe ketorolac in selected patients, is as effective as morphine in alleviating postoperative pain without side effects of morphine.

A Comparison of the Epidural Nalbuphine to Epidural Morphine in Post-Cesarean Section Patients (제왕절개술후 통증치료를 위해 경막외강에 투입된 Morphine과 Nalbuphine의 비교 연구)

  • The clinical effects of epidural nalbuphine were compared to those of epidural morphine in sixty Cesarean delivery. They were physical status 1 or 2 by ASA classification and randomly divided into three groups. They were administered nalbuphine 5 mg(Group N5), nalbuphine 10 mg(Group N10) or morphine 3 mg(Group M3) through an indwelling epidural catheter at the time of peritoneal closure. During the first postoperative 24 hours, their analgesic effects were evaluated by visual analogue scale(0-10), respiration rates and Trieger dot test. The severity of side effects(0-2) was also evaluated. The results were as follows ; 1) The number of patients who needed additional epidural analgesics was least in group M3 (p<0.05). There was no significant difference between group N5 and group N10. 2) The duration between the first and second epidural administration was ; 19.2 hours in group M3, 8.6 hours in group N10 and 5.4 hours in group N5. There was a significant difference each group (p<0.05). 3) From the fourth post operative hour, both groups receiving nalbuphine showed a higher VAS score compared to group M3(p<0.05). 4) The incidence of pruritus, nausea, vomiting and voiding difficulty were more severe in group M3 compared with the other groups. However the severity did not increase with increasing nalbuphine dosage. 5) There were no patients showing objective sedation or low respiration rate(10 times/minute). We concluded that epidural administration of nalbuphine 5 mg or 10 mg is one way of post operative pain control. Its side effects were less than epidural morphine, but it is a less convenient in the method of analgesia.

A Comparison of Patient-Controlled Analgesia and Conventional Intramuscular Opioid Regimen in Relation to their Post-Operative Pain Control and Side Effects (수술후 통증 관리의 Patient-Controlled Analgesia와 마약류의 전통적인 근육내 주사와의 비교)

  • Using a visual analogue scale, we compare the effect of patient-controlled analgesia and conventional intramuscular opioid regimen in 68 patients undergoing lower abdominal or gynecological surgery. We also recorded the incidence of side effects. We checked visual analogue scale 4 hours interval for 30 cases managed by patient-controlled analgesia and 38 cases of conventional intramuscular opioid group managed by obstetrician. We maintained fentanyl $0.33{\mu}g/kg/hr$ and set self administrable bolus dose $5.0{\mu}g$(lockout interval: 15 min) in patient-controlled analgesia group. Conventional intramuscular bolus injection group were administered meperidine 50 mg for 4 hour interval. Mean visual analogue scale scores obtained by patient-controlled analgesia group and intramuscular bolus injection group were $2.49{\pm}0.67$ and $4.53{\pm}1.28$(p<0.05). Side effects such as; no significant incidence of respiratory depression, urinary retention, postural hypotension, nausea, vomiting and pruritus were developed by either group. These results suggest that patient-controlled analgesia was more effective method compared with conventional intraumuscular opioid injection regimen for post-operative pain management.

  • Purpose: Kimura's disease is an uncommon benign lymphoproliferative inflammatory disorder with an unknown etiology. The recurrence rate after surgical excision is relatively high and renal involvement is its only systemic manifestation. The condition mainly involves the head and neck, and peripheral involvement is extremely rare. Materials and Methods: We encountered the case of a 28-year-old man who had a non-tender mass with mild brownish skin color changes and pruritus around the medial side of the distal arm and elbow. Results: The peripheral blood investigation revealed peripheral eosinophilia and elevated serum IgE levels. Magnetic resonance imaging showed an isointensity signal relative to the muscle on the T1-weighted images and hyperintensity signal relative to the muscle on the T2-weighted images. Conclusion: A marginal resection of the lesion was performed and there was no recurrence at 2 years postoperatively.
