한우(韓牛) 송아지의 흉부(胸部)에 여분의 전지(前肢)가 부착(附着)된 다지기형(多肢畸形) (Polymelia with an extra forelimb at the thoracic regioo in Korean native calf)

  • An abnormal female Korean native calf at five months age with five legs was observed macroscopically and radiographically. External features included two normal forelimbs, and hindlimbs, and an abnormal forelimb which was underdeveloped. A rudimentary forelimb, which was attached to the pectoral region, was observed at the right axillary part. The extra forelimb was scapural and humeral meromelia. It consisted of an underdeveloped forearm (antebrachium), several carpal bones (three bones), large metacarpal bone (Mc. 3 + 4), and two digits with two hoofs. The proximal, middle and distal phalanges, proximal and distal sesamoid bones are normal.

좌쇄골하동맥 기시 근위부에 발생한 대동맥교약증1 치험 (Coarctation of the aorta: unuaual type: a case report)

  • Coarctation of the aorta usually occurs just distal to the origin of the left subclavian artery, but may involve proximal to this vessel. One unusual type of coarctation of the aorta which located proximal to the left subclavian artery is presented. The patient was 23 year old soldier whose primary complaints were occipital headache and dizziness. Examination showed a unilateral hypertension in the right arm. The aortogram demonstrated coarctation between the left common carotid artery and left subclavian artery. On Jun. 14, 1983, patch graft aortoplasty was performed but failed due to pliable poststenotic aortic wall. And bypass graft from origin of the left common carotid artery to the descending thoracic aorta was performed. Postoperative course was uneventful for 4 months follows up periods. We now report a unusual type of coarctation of the aorta and its surgical treatment.

삼중방심 치험 1례 (Cor Triatriatum (A case report))

  • Cor triatriatum is a rare congenital malformation of the heart in which a diaphragm stretches in a transverse plane through the left pulmonary venous chamber, thus creates two subchambers. The proximal chamber connects with the pulmonary veins, and the distal one has left auricle and the mitral valve. A 3 year old boy who had Cor triatriatum underwent surgical excision of the abnormal diaphragm in March, 1989 in Chungnam University Hospital. An obliquely oriented fibromuscular diaphragm divided the left atrium into a proximal chamber which was connected to the pulmonary veins and a distal chamber which had the atrial appendage and the mitral valve. The opening in the diaphragm was 5 mm in diameter. There were no associated abnormalities. The abnormal diaphragm was completely excised. The postoperative result was excellent.

  • 관상동맥 우회로술 때 보이지 않는 좌전하행지의 노출 방법에는 여러 가지가 있다. 특히 심근 내 위치하는 좌전하행지인 경우 보다 표면에 위치하는 근위 관상동맥을 노출하여 내흉동맥편을 문합함으로써 이식편의 우수한 장기 열림을 기대할 수 있다 육안 상 보이는 원위 좌전하행지의 절개창을 통해 삽입한 더듬자로 심장 외막에 가까운 근위 좌전하행지를 쉽게 노출할 수 있는 방법을 보고하고자 한다.

슬관절 주위 거대세포종의 치료 (Surgical treatment of Giant Cell Tumor in Knee Joint)

  • Giant cell tumors are primary bone tumors originating from non-osteoblastic connective tissue. The sites of involvement were commonly distal femur, proximal tibia, proximal humerus, distal radius and others (including os calcis, ilium and sacrum). Giant cell tumor located around knee joint has been difficult to treat because of local recurrence following curettage with or without bone graft. Although primary resections reduce recurrence of the lesion, the joint function will be markedly impaired. Marginal excision was very often complicated by a loss of joint integrity since all the giant cell tumors occupy juxtaarticular positions. Techniques involving physical adjuncts(high speed burr and electric cauterization) have been used in the hope of decreasing the rate of local recurrence and avoiding the morbidity of primary resection. A meticulous clinical, radiological and histological evaluation is needed to choose the correct treatment, keeping in mind the possibility of recurrence after each treatment modality.

대동맥폐동맥 중격결손증, 개방성 대동맥관 및 우측 쇄골하동맥 이상기시를 동반한 대동맥궁 결손증 (Interrupted Aortic Arch Associated with AP Window, PDA, and Aberrant Origin of the Right Subclavian Artery from Proximal Descending Aorta [A Case Report])

  • A case of complete interruption of aortic arch with aortopulmonary window, patent ductus arteriosus, and aberrantly originated right subclavian artery from proximal descending aorta, in a four year old boy is reported in detail. This is the only reported case in Korea, who has had a successful one-stage total anatomical correction of this combination of defects. Under deep hypothermia and total circulatory arrest, aortic continuity was established using patent ductus arteriosus and anterior wall of pulmonary artery, which was anastomosed obliquely to anteromedial side of ascending aorta. Aortopulmonary window was closed using Impra patch via pulmonary arteriotomy. Then pulmonary arteriotomy was reconstructed primarily except at the junction of right pulmonary artery and main pulmonary artery, where a small piece of pericardium was used to close the defect to prevent kinking and narrowing of right pulmonary artery. Postoperative cardiac catheterization demonstrated a good reconstruction.

다발성 섬유성 골이형성증 변형에 대한 나사못 맞물림 골수정을 이용한 치료 (Treatment of Deformity in Polyostotic Fibrous Dysplasia Using Interlocking Intramedullary Nailing)

  • The fibrous dysplasia is a progressive and disabling condition that lead to deformity, especially weight bearing bones. The morbidity that is associated with the polyostotic fibrous dysplasia is the recurrent fracture and deformity. Various methods of treatment had been failed to control this problem. We used osteotomy and reconstruction nailing for polyostotic fibrous dysplasia occured in the proximal part of right femur with varus deformity and reconstruction nailing in left femur without osteotomy, and interlocking intramedullary nailing in right tibia to prevent pathologic fracture. These methods brought a good result of bone union and full weight bearing ambulation after 1 year and 6 month follow up. We think these methods are useful methods to control refracture and deformity, so we reported this case with bibliographic reviews.

CT 지표를 이용한 두정-후두부 뇌실 도관의 정확한 삽입 (Accurate Placement of Parieto-occipital Ventricular Catheter Using CT Parameters)

  • Objective : placement of ventricular catheter is important to achieve long-term patency of the shunt system. We describe a method of calculating the insertion site, trajectory, and the length of the ventricular catheter using CT parameters in shunt surgery. Patients and Methods : These can be rapidly obtained using a caliper from a scout cut and two axial scans at lateral ventricular and upper 3rd ventricular level. To compare this technique with traditional one, we analyzed 40 consecutive patients who underwent shunt surgery. Results : Of 20 patients undergoing ventriculoperitoneal shunt insertion using this technique, none had poor location of the proximal catheter. In the counterpart, 9 patients had poor location(p=0.001). The number of patients who required revision surgery were also lower in the group using this technique, but it was not statistically significant (4 versus 2, p=0.422). Mean follow-up period was shorter in this technique group. Conclusion : This technique provides an accurate placement of the proximal catheter without special instrument or additional expenses.

Direct Carotid Cavernous Fistula of an Adult-Type Persistent Primitive Trigeminal Artery with Multiple Vascular Variations

  • We report a case of spontaneous right carotid-cavernous fistula (CCF) in a proximal segment of persistent primitive trigeminal artery (PPTA) and combined vascular anomalies such as left duplicated hypoplastic proximal posterior cerebral arteries and a variation of anterior choroidal artery supplying temporal and occipital lobe. A 45-year-old male presented with progressive right exophthalmos, diplopia, and ocular pain. With manual compression of the internal carotid artery, a cerebral angiography revealed a right CCF from a PPTA. Treatment involved the placement of detachable non-fibered and fibered coils, and use of a hyperglide balloon to protect against coil herniation into the internal carotid artery. A final angiograph revealed complete occlusion of PPTA resulted in no contrast filling of CCF.

Concomitant Dual Origin and Fenestration of the Left Vertebral Artery Resembling Dissection

  • Dual origin and fenestration of the vertebral artery (VA) are very rare anomalies. Understanding of these variations, however, is important because they can be misdiagnosed as a VA dissection. A 42-year-old woman presented with motor weakness and sensory disturbance of the right upper extremity. Radiologic evaluations showed ectatic change in the right VA and an arteriovenous fistula between the right VA and the vertebral vein. We decided on endovascular occlusion of the proximal right VA and its fistulous portion. During the endovascular procedure, we had misunderstood the dual origin and fenestration of the VA as a dissection. Thus, failure to recognize these anomalies might result in unnecessary anticoagulation or therapeutic intervention. Clinicians should be alert to such VA variations when making a diagnosis and when planning any intervention or surgery involving the proximal VA.
