  • Purpose: Concept analysis was performed on the behavioral concept of endotracheal suctioning (ETS), to identify the goal, to develop astandardized clinical protocol, to identify the antecedents and consequences, and to differentiate the improper use of ETS. Method: Walker & Avant's concept analysis was employed using clinical guidelines, books and review articles in which the procedures of ETS were written in detail and published in Pubmed within the last 20 years. Result: The macro-goal of ETS was to remove accumulated respiratory secretions. Three defining attributes of ETS were identified; catheter, suctioning and asepsis. Each attribute involved empirical referents, such as the size and depth of the catheter, the suction pressure, duration and method for suctioning. The antecedents of ETS were identical to the clinical evidences for the need of ETS such as the nursing assessment data. The consequences of ETS serve as an evaluation criteria on the effectsof ETS based on the goal of ETS. Conclusion: The concept analysis of ETS demonstrates an example of considering a specific nursing protocol of ETS as a behavioral concept, applying concept analysis to it to identify it's key behavioral components as defining attributes and empirical referents and then developing and applying the standard ETS protocol.

  • Purpose: Resuscitation rooms in emergency department are places for time-critical tasks. This study aimed 1) to develop a resuscitation protocol for dyspneic patients and 2) to evaluate the effect of the resuscitation protocol for dyspneic patients in improving clinical performances. Methods: A panel of experts including emergency physicians and nurses developed a resuscitation protocol for dyspneic patients which included a list of critical interventions and time-limits. The resuscitation protocol was implemented in a emergency medical center for two months. Clinical performances of health care providers were compared by analyzing video-recorded clinical performances. Results: The resuscitation protocol consisted of four steps: Initial treatments within 5 minutes, main treatments within 15 minutes, diagnostic tests within 30 minutes, and disposition within 60 minutes. A total of 43 dyspneic patients (23 patients experimental group, 20 patients control group) were included for the analysis of the clinical performance change. Clinical performance rates were improved significantly from control group (66.71%) to experimental group (82.41%) after implementing the resuscitation protocol (Z=-3.09, p=.004). Conclusion: The resuscitation protocol developed for dyspneic patients improved clinical performance rates of health care providers. Further development of clinical protocols for other emergent cases at resuscitation rooms are strongly recommended.

  • This paper propose a new wireless multi-hop routing protocol that takes the hierarchical mesh of the hybrid WMN into account. WMN that is possible to provide various applications of wireless networks still has many open issues that should be solved despite the studies carried out over a decade. Especially, in routing protocol area, a problem degrading the routing efficiency by applying one of the routing protocols, which are designed for the MANET, to the hybrid WMN be solved above all. For the improvement of the routing performance, both good routing protocol and metric are essential. However, the recent studies are only concentrated in routing metric by use of the cross-layer design. Therefore, this paper is dedicated to the routing protocol that is essential for the performance of the routing but needed more studies. The proposed protocol in this paper is reactive, and designed to reorganize the hybrid WMN with several pseudo domains, and carry out domain-based route decision. By the simulation result for the performance analysis of the proposed protocol, the average delay for the route decision was decreased by 43% compared to AODV that is the typical reactive protocol.

  • Purpose: The purpose of this study was to develop an evidence-based nursing protocol for peripheral intravenous catheter management in hospitalized children. Methods: The preliminary protocol of this study was developed based on domestic and foreign guidelines, research, a survey and an analysis of medical records in a university hospital. It is a methodological research to develop evidence-based nursing protocol of peripheral intravenous catheter management verified by content validity by a group of specialists and users. Results: The specialists' verification of validity in the preparatory protocol had a CVI level of 0.94 and the propriety and conveniency of users had an average of $3.0{\pm}0.52$. The final evidence-based nursing protocol was composed of 5 areas (education, dressing and fixation, maintenance and replacement, observation and record, and coping to complication) with 46 specific recommendations. Conclusion: Based on domestic and foreign research and guidelines verified by specialists and users, the findings in this study provide a simple, applicable and evidence-based nursing protocol for peripheral intravenous catheter management in hospitalized children. By the clinical application of this protocol, nurse tasks in managing peripheral intravenous catheter can be performed with more scientific evidence and be standardized.

  • SPDY is the new protocol proposed by Google to complement problems of HTTP(Hypertext Transfer Protocol)/1.1 and to improve web speed. In this paper, we evaluated the performance of SPDY protocol in a variety environment. By this evaluation, we examined the characteristics of the SPDY protocol and compared the differences between the existing protocol and SPDY protocol. And we took a closer look at the Flow Control of SPDY. Through this, we analyzed the problem of SPDY. It improved performance at 3G network environment, but failed to improve performance at high speed WLAN and mobility environments. We also verified that Flow Control does not work well. And finally we proposed directions for improvement of this protocol.

  • Background: Recently, a study comprising adult patients with sepsis admitted in the intensive care unit (ICU) was conducted. The patients were treated with high doses of intravenous ascorbic acid, thiamine, and hydrocortisone; the clinical outcomes demonstrated significant therapeutic benefits. The mortality rate in children with sepsis is approximately 25%. However, the effects of additional treatment with ascorbic acid and thiamine ("vitamin protocol") in children are rarely investigated. Methods: A retrospective analysis was performed using medical records of patients diagnosed with sepsis and admitted to the pediatric ICU (PICU) between September 2016 and June 2019. The control group received treatment only as per sepsis protocol, whereas the treated group received both sepsis protocol and the vitamin protocol. The primary endpoint was change in Vasoactive-Inotropic Score (VIS) for 5 days. The secondary endpoints included the length of stay in the PICU, duration of using mechanical ventilators and vasopressors, and mortality rate. Results: The number of patients in the treated and control groups was 33 and 24, respectively. The treated group showed greater decrease in their VIS for 5 days than the control group (44.4 vs 18.6); however, the difference was not statistically significant. The length of stay in the PICU was significantly longer for the treated group than for the control group [10.0 days (Interquartile range (IQR), 6-18) vs 4.5 days (IQR, 4-10.3); p=0.004]. Conclusions: No significant treatment benefits were observed following vitamin protocol administration to the pediatric patients with sepsis. Further studies are necessary for improving the efficacy and safety of the vitamin protocol.

  • Conventional 1-1-1 cooperative protocol offers path-loss gain as advantage of multi-hop and spatial diversity which is equivalent to MIMO system. This protocol is enable to get higher reliability and reduction of power consumption than those of the single-hop or multi-hop. But the 1-1-1 cooperative protocol get only the diversity order 2 and limited path-loss reduction gain because this protocol has a single cooperative relay. We propose 1-2-1 cooperative protocol using two cooperative relays R1, R2. The 1-2-1 cooperative protocol can improve path-loss reduction and increase diversity order 3. Moreover, the cooperative relay R2 attains diversity order 2. The signaling method in transmission uses DF (Decode and Forward) or DR (Decode and Reencode) and 1-2-1 DF/DR cooperative protocol are applied to clustering based wireless sensor networks (WSNs). Simulations are performed to evaluate the performance of the protocols under Rayleigh fading channel plus AWGN (Additive White Gaussian Noise).

  • Recently, together with the development of wireless communication technologies and the wide use of smart phones, the demand of the next generation mobile communication has been increased. To construct the next generation mobile communication platform efficiently for a short period from protocol development phase to protocol stability phase, an protocol analyzer is required to verify and analyze the protocol. In this paper, we propose the protocol analyzer of LTE network which is the leader of the next generation mobile communication platforms. The protocol analyzer is a software based agent-server architecture and uses XML metadata which defines intercommunication messages to analyze the protocol of the next generation mobile communications. We can analysis the encoding messages applied to LTE network using the loading of the decoding library. We verify the superiority of the proposed analyzer through an integrated test with LTE network.
