소아 급성골수성백혈병에서 관해유도 요법 중 Posaconazole의 예방적 항진균 치료 (Posaconazole for Prophylaxis of Fungal Infection in Pediatric Patients with Acute Myeloid Leukemia undergoing Induction Chemotherapy)

  • Background: Posaconazole is a broad-spectrum triazole antifungal agent and the most recommended prophylactic antifungal agent for patients with acute myeloid leukemia (AML) undergoing induction chemotherapy. In this study, we evaluated the status and effectiveness of posaconazole as a prophylactic antifungal agent in pediatric patients receiving induction chemotherapy for AML. Methods: We retrospectively reviewed the electronic medical records of 36 pediatric patients with AML (between January 2013 and September 2017) at the Yonsei University Health System. Invasive fungal disease (IFD) was assessed as the primary endpoint of prophylactic antifungal effect. The secondary endpoints were incidence of fever, persistent fever despite the use of broad-spectrum antibiotics for 72 h, alteration of antifungal agent, intensive care unit admission, and death within 100 days. Results: Among the 36 patients, 18 patients used posaconazole, 12 were treated with suspension formula, and 6 of them were treated with tablets. Eighteen patients did not use antifungal agents prophylactically. The mean number of days of posaconazole administration was $26.8{\pm}16days$. IFD occurred in 2/18 (11.1%) patients in the no prophylaxis group and in 1/18 (5.6%) patients in the posaconazole group (p=0.49). Conclusion: Posaconazole is expected to be useful for the prevention of IFD in pediatric patients with AML undergoing induction chemotherapy. Prospective studies of the effectiveness of posaconazole prophylaxis should be conducted in more pediatric patients in the future.

단일병원 신생아 환자의 메티실린내성 황색포도알균 보균율 (Colonization Rate of Methicillin-resistant Staphylococcus aureus in Neonates: A Single Center Experience)

  • 목 적 : 단일 병원 신생아입원실에 입원한 신생아를 대상으로 환자의 임상적 특징에 따른 MRSA 보균율을 알아보고, 그 기원을 추정해 보며, MRSA 보균에 영향을 미치는 요소들을 살펴보고자 하였다. 방 법 : 2008년 1월부터 2011년 12월까지 을지대학교 서울 을지병원 신생아 입원실에 입원하여 MRSA 감시배양검사를 시행받은 1,733명의 신생아를 대상으로 의무기록을 후향적으로 조사하였다. MRSA 감시배양검사는 비강, 서혜부, 직장에서 시행하였고, 퇴원 시까지 매주 반복 시행 하였다. MRSA 감시배양결과에 따라서 보균자와 비보균자로 나누었다. 결 과 : 대상환자 1,733명 중에 415명(23.9%)이 MRSA 보균자였다. 제태기간, 출생체중, 분만 방식, 분만전 산모에게 항생제 투여 여부, 출생장소, 입원전 체류 장소에 따라서 MRSA 보균율에 차이를 보였다(P<0.001). 다변량 검사에서 분만전 산모에게 예방적 항생제를 투여하지 않은 경우가 투여한 경우에 비해서 신생아가 MRSA 보균자가 될 위험도가 2.8배(OR=2.77; 95% CI, 1.88-4.07), 출생장소가 외부인 경우가 본원인 경우에 비해서 2.3배(OR=2.28; 95% CI, 1.17-4.42) 높음을 확인하였다. 결 론 : 신생아 입원환자를 대상으로 한 MRSA 보균율은 23.9%로 상대적으로 높은 보균율을 확인하였다. 환자특성을 고려하여 추정한 HA-MRSA 보균율은 51/511명(10%), CA-MRSA 보균율은 309/858명(36%) 이었다. 본병원 신생아에서 MRSA 보균과 연관된 요인은 산모의 예방적 항생제 사용여부와 출생장소임을 확인하였다.

임프란트에 부착하는 세균의 동정 및 효과적인 항생제 선택 (ORAL MICROBES ASSOCIATED WITH TITANIUM IMPLANT AND THEIR ANTIBIOTIC SUSCEPTIBILITY)

  • This study was done to examine adherence of oral bacteria to titanium dental implant and to know the effective prophylactic antibiotics using an in vivo model. Three samples each of the implant material were set in an acrylic resin flange and placed in the maxillary buccal sulcus of twenty volunteers. At 6- and 54-hour intervals, each sample was placed on blood agar plate (BAP) and chocolate agar, and then they were incubated and identified. Also antibiotic susceptibility test was performed. The results obtained mere as follows ; 1. The microorganisms were chain-like Gram positive cocci and staphyline Gram positive cocci, Gram positive bacilli in order of frequency were found at 6-hour and 54-hour samples by Gram staining. 2. Streptococci was found predominantly at both 6-hour and 54-hour samples, but number of streptococci was decreased as compared to 6-hour samples. 3. There was no difference in the bacterial species adherent to implant between 6-hour and 54-hour samples. 4. All the microbes were sensitive to AMC (amoxacillin clavulanic acid), chloramphenicol, quinolone and vancomycin in the antibiotic susceptibility test. Above results suggest that streptococcus are mainly adhered to titanium implant after implant was placed in the oral cavity and AMC is the most recommendable antibiotics to prevent the peri-implant inflammation.

  • Poultry industry has always been a dynamic and integral part of national economies in many countries. Economic losses incur especially in large-scale rearing facilities, often attributed to the deterioration of environmental conditions, poultry exposure to stressors and development of diseases. While antibiotics have been commonly used for prophylactic purposes and as growth stimulants, extensive documentation of antimicrobial resistance among pathogenic bacteria due to indiscriminate utilization of antibiotic in the industry has led to public and governmental outcries. Elimination of antibiotics from poultry production has thus encouraged intensive search for alternatives. In this review, we discuss the immense potential of probiotics to fill the gap as alternative growth promoters and evidences of beneficial effects of probiotic application in poultry production.

  • Total joint arthroplasty is a successful joint replacement treatment that improves joint function and overall quality of life and provides pain relief. However, the prevalence of periprosthetic joint infection (PJI) has become prevalent with the rise in the incidence of arthroplasty surgery. PJI occurs rarely following arthroplasty however presents with serious complications, including high morbidity. The identification of causative microorganisms is essential for the treatment of PJI. Managing PJI requires complex treatment strategies, including long-term antibacterial treatment, and significant medical costs can be incurred. The American Academy of Orthopedic Surgeons, the Centers for Disease Control and Prevention, and Surgical Care Improvement Project guidelines recommend that prophylactic antibiotics such as first-generation cephalosporins be infused completely 1 hour before surgical incision. However, these preventative antibiotics are very limited, therefore risk factors must be identified to diagnosis and treat patients effectively. Moreover, determining antimicrobial susceptibility during artificial joint surgery and choosing the most appropriate treatment strategy following an accurate diagnosis of microbial infections are essential. In the present review, we describe the management, including the etiology, diagnosis, and classification of PJI, and approaches to its diagnosis using the available novel molecular diagnostic methods.

예방적 항생제 사용 양상과 수술부위감염률의 관련성 (Association between the Pattern of Prophylactic Antibiotic Use and Surgical Site Infection Rate for Major Surgeries in Korea)

  • Objectives : The purpose of this study was to analyze the association between the pattern of prophylactic antibiotic use(PAU) and the surgical site infection(SSI) rate for major surgeries in Korea. Methods : We retrospectively reviewed the medical records of patients who underwent cardiac, colon and gastric surgery, hysterectomies and hip/knee replacements at 20 hospitals, and inclusive of over 500 beds. We randomly sampled 60 cases per surgery type for patients discharged between September and November, 2006. A total fo 2,924 cases were included in our analysis. Cox's proportional hazard analysis was conducted to evaluate the association between the pattern of PAU and SSI rate. Results : The proportion of patients who received their first prophylactic antibiotics(PA) 1 hour before incision was 65.5%, who received inappropriate PAs was 80.8%, and the proportion of patients whose PA was discontinued within 24 hours of surgery was 0.5%. The average duration of PAU after surgery was 9 days. The relative risk(RR) of SSI in patients who received their first PA more than 1 hour before incision was significantly higher than for those who received it within 1 hour prior to incision(RR=8.20, 95% CI=4.81-13.99). Inappropriate PA selection increased SSI rate, albeit with marginal significance(RR=1.97, 95% CI=0.96-4.03). Also, prolonged PAU following surgery had no effect on SSI rate. Conclusions : These results suggest that the pattern of PAU in the surgeries examined was not appropriate. Errors in the timing of PAU and of PA selection increase SSI rate. SSI rate remained unaltered following prolonged PAU after surgery.

Comparison of Inflammatory Markers Changes in Patients Who Used Postoperative Prophylactic Antibiotics within 24 Hours after Spine Surgery and 5 Days after Spine Surgery

  • Objective : C-reactive protein (CRP) level, erythrocyte sedimentation rate (ESR), and white blood cell (WBC) count are inflammatory markers used to evaluate postoperative infections. Although these markers are non-specific, understanding their normal kinetics after surgery may be helpful in the early detection of postoperative infections. To compliment the recent trend of reducing the duration of antibiotic use, this retrospective study investigated the inflammatory markers of patients who had received antibiotics within 24 hours after surgery according to the Health Insurance Review & Assessment Service guidelines and compared them with those of patients who had received antibiotics for 5 days, which was proven to be non-infectious. Methods : We enrolled 74 patients, divided into two groups. Patients underwent posterior lumbar interbody fusion (PLIF) at a single institution between 2019 and 2020. Group A included 37 patients who received antibiotics within 24 hours after the PLIF procedure, and group B comprised 37 patients who had used antibiotics for 5 days. A 1 : 1 nearest-neighbor propensity-matched analysis was used. The clinical variables included age, sex, medical history, body mass index, estimated blood loss, and operation time. Laboratory data included CRP, ESR, and WBC, which were measured preoperatively and on postoperative days (POD) 1, 3, 5, and 7. Results : CRP dynamics tended to decrease after peaking on POD 3, with a similar trend in both groups. The average CRP level in group B was slightly higher than that in group A; however, the difference was not statistically significant. Multiple linear regression analysis revealed operation time, number of fused levels, and estimated blood loss as significant predictors of a greater CRP peak value (r2=0.473, p<0.001) in patients. No trend (a tendency to decrease from the peak value) could be determined for ESR and WBC count on POD 7. Conclusion : Although slight differences were observed in numerical values and kinetics, sequential changes in inflammatory markers according to the duration of antibiotic administration showed similar patterns. Knowledge of CRP kinetics allows the assessment of the degree of difference between the clinical and expected values.

바다에서 건진 절단된 하지 원위부의 재접합 (Replantation of a Distal Lower Leg in an Elderly Patient After Retrieval of the Limb from Seawater Immersion)

  • A case of distal lower leg replantation, in an elderly patient, was successfully performed after retrieval of the amputated limb from 3 hours of immersion in seawater. Due to the patient's age, bony union was delayed and functional nerve recovery was unsatisfactory. After massive saline irrigation of the amputated part, employing a catheter through the main artery, as well as, prophylactic antibiotics to prevent infection by Aeromonas, there were no postoperative complications. Despite the issue of age, immersion of an amputated part into seawater is not a contraindication of replantation.

경막외 카테터의 장기간 거치시 말단부의 감염 조사 (Bacteriological Culture of Indwelling Epidural Catheters)

  • The incidence of contamination of epidural catheters used for pain control was investigated. To prevent epidural infection, all patients with epidural catheters had taken amoxacillin 1.5gm/day orally. Of the cultures of catheters catched from 303 patients undergoing continuous epidrual catheterization, 5 catheters (1.7%) were found to be contaminated; cervical 1/86 (1.2%), thoracic 1/27 (3.7%), and lumbar 3/190 (1.6%). Staphylococcus epidermidis was the most common etiologic agent (60%). To prevent epidural infection, sterilization of the skin around the epidural catheter and prophylactic use of broad-spectrum antibiotics are thought to be beneficial.

Postoperative care after lymphaticovenous anastomosis

  • In this report, we discuss the postoperative protocol for patients undergoing lymphaticovenous anastomosis (LVA) in our unit. Immediately after LVA, the incision site is closed over a small Penrose drain and a simple gauze dressing is applied without compression. In the first 5 days, ambulation is allowed, but limb elevation is actively encouraged to promote lymphatic flow across the newly formed anastomosis. Prophylactic antibiotics are routinely given to prevent infection because this patient group is susceptible to infections, which could trigger thrombosis in the anastomosis.
