우리나라 어린이 요로 감염의 치료 행태 (Treatment for Urinary Tract Infection of Children in Korea)

  • 목 적 : 요로감염은 어린이에서 중요한 신요로계 질환으로 빠르고 정확한 진단과 치료로 신장 손상의 위험을 줄이고 불필요한 검사와 치료를 피할 수 있다. 저자들은 우리나라 어린이 요로감염 치료의 개선을 도모하는 첫걸음으로 설문 조사를 통해 요로감염의 진단, 치료, 영상 검사와 예방에 관한 진료 행태를 알아보았다. 대상 및 방법 : 전국의 소아신장전문의와 비뇨기과 전문의를 대상으로 설문조사를 통하여 요로감염의 진단, 치료, 영상검사, 예방의 진료형태를 조사하였다. 결 과 : 응답자들은 대부분 신생아에서는 소변 채취용 백을, 이후에는 중간소변 채취를 이용하여 소변 배양 검사를 하였고, 소변 배양검사가 음성인 경우에는 농뇨, nitrite 양성, 세균뇨 등의 소견을 보일 때 요로감염이라고 판단하였다. 요로감염 후에는 80$\%$가 예방적 항생제를 사용하였다. 방광 요관 역류가 있는 환아에서는 1-2세 이상의 환아가 고도의 방광 요관 역류를 보이거나 예방 요법 중에도 요로감염이 발생할 때 수술로 치료하는 경우가 많았으나 그 기준에는 차이를 보였다. 대부분이 요로감염 진단시에 신장 초음파 검사를 시행하였고 과반수에서 각각 다양한 시기에 배뇨 방광 요도 조영 검사와 DMSA scintigraphy를 시행하였다. 포경 수술은 대개 권하지 않았으며 반수에서 방광 요관 역류 환아의 형제에 대한 선별 검사를 시행하였다. 결 론 : 이 보고에서 밝혀진 다양한 치료 행태로 보아, 어린이 요로감염 치료의 개선과 발전을 위해 체계적인 진료 방침의 고안이 필요할 것으로 보인다.

최근 국내발병 개 파보바이러스성 장염에 대한 역학적 조사 (Epidemiological Observation on Recent Outbreaks of Canine Parvoviral Enteritis in Korea)

  • 세계적으로 canine parvovirus(CPV) 고역가 백신이 사용된 후 CPV 감염이 효과적으로 관리되고 있지만 국내에서는 아직도 CPV 장염의 발생이 계속되고 있다. 본 연구에서는 2003년 6월부터 2005년 5월까지 CPV 장염으로 확진된 140두를 대상으로 CPV 장염의 역학적 조사와 CPV 장염에 이환된 환축의 생존에 영향을 미치는 역학적 요인들에 대하여 조사하였다. CPV 장염은 6-18주령의 예방접종을 실시하지 않았거나 불완전하게 실시한 개체에서 주로 발생하였다. CPV 장염에 이환된 개 중 암컷의 생존율이 숫컷보다 높았으며, 나이가 많을수록 생존율이 높았다(p<0.05). 그리고 HI 항체가가 낮을수록 폐사율이 높게 나타났다(p<0.05). 주요 임상증상으로 설사, 구토, 기면과 탈수증상이 대부분의 환축에서 나타났다. 최근 국내에서 발생하는 CPV 장염은 CPV에 대한 예방접종의 실패와 CPV 백신에 대한 면역반응이 부적당한 것에 기인하는 것으로 생각되므로 면역이 완전히 형성되기 전의 어린 개들은 CPV에 오염된 환경에 노출되지 않도록 예방관리 하는 것이 중요하다.

CDV 함유 혼합백신과 예방접종 스케줄에 따른 강아지의 면역반응 (Serological Response of Puppies to the Selected Canine Vaccines and Vaccination Schedules against Canine Distemper Virus)

  • 본 연구에서는 우리나라 실정에 맞는 개 디스템퍼에 대한 예방접종 프로토콜을 마련하기 위하여, 국내에서 사용 중인 4종류의 상업용 백신과 3 가지의 예방접종 스케줄에 따른 강아지의 면역형성 능력을 비교 평가하였다. 생후 6주령에 예방접종을 실시하기 위하여 동물병원에 내원한 120두의 강아지를 4종류의 백신[C, G, K, V (또는 V3)군]과 예방접종 스케줄(2, V4 군)에 따라 20두씩 임의배치 하여 C, G, K, V(또는 3)군은 3주 간격으로 3회(6, 9, 12주), V2 군은 5회(6, 8, 10, 12, 14주), V4 군은 3회(6, 10, 14주)에 피하로 예방접종을 실시하였다. 초유를 섭취한 7마리 강아지의 모체이행 항체의 소장상태를 확인한 결과 모체이행항체는 생후 6주령에 방어수준 이하로 떨어졌다. 백신에 따른 면역형성능에서 V 백신의 면역형성 능력이 다른 백신보다 우수하였으며 백신 종류에 따라 면역형성 능력에 차이가 인정되어 사용되는 백신의 효능을 주기적으로 평가하여야 할 것으로 판단되었다. 그리고 가장 효과적인 예방접종 스케줄은 6주령에 예방접종을 시작하여 3주 간격으로 3회 접종하는 것으로 판단되었다. 그러나 예방접종을 실시한 모든 군의 대부분의 강아지는 생후 9주령까지 항체가 수준이 방어수준 이하로 나타나 개 디스템퍼의 감염을 예방하기 위하여 이 시기까지는 감영위험성이 높은 곳에 노출되는 것을 피해야 할 것으로 생각되었다.

The Role of Radiosurgery in Patients with Brain Metastasis from Small Cell Lung Carcinoma

  • Objective : The purpose of this retrospective study was to evaluate the outcome of gamma knife radiosurgery (GKRS) and/or whole brain radiation therapy (WBRT) for the treatment of small cell lung carcinoma (SCLC) metastasis to the brain. Methods : From 2000 to 2010, 50 patients underwent GKRS for metastatic brain lesions originating from SCLC. Among these patients, 11 received prophylactic cranial irradiation (PCI) before the development of metastatic lesions (PCI group), and GKRS was performed as an initial treatment for newly diagnosed lesions in 12 patients who had not received PCI (primary GKRS group). In addition, GKRS was performed as a salvage treatment for progressive lesions after WBRT in 27 patients (salvage GKRS group). The medical records and imaging data of all patients were retrospectively analyzed. Results : The overall survival of the 50 patients was 20.8 months (range 1-53) after the diagnosis of primary tumor and 12.0 months (range 1-47) after the development of cerebral metastasis. Median survival after GKRS was 4.8 months (range 1-15) in the PCI group, 4.6 months (range 0-18) in the primary GKRS group, and 7.6 months (range 0-33) in the salvage GKRS group. Further treatment for progressive lesions after GKRS was necessary in 15 patients, after a mean interval of 3.8 months. Causes of death were systemic organ failure in 15 patients, deterioration of neurological state in 13 patients, and unknown or combined causes in 16 patients. The local control rate of the lesions treated with GKRS was 76.4% (decreased in 13 patients and stable in 16 patients at the final imaging follow-up (mean 5.60 months). Conclusion : GKRS is an effective local treatment for brain metastasis from SCLC both as an initial treatment for newly diagnosed lesions after PCI and as a salvage treatment for recurrent or progressive lesions. However, the survival benefit is not significant because most patients die of systemic multi-organ failure with a short life expectancy.

세균성 질병 예방을 위한 식물 경구 백신 연구 동향 (Recent Studies on the Edible Plant Vaccine for Prophylactic Medicine against Microorganism-Mediated Diseases)

  • Plants have considerable advantages for the production of antigenic proteins because they provide an inexpensive source of protein and an easy administration of vaccine. Since a publication describing edible plant vaccine of HBsAg in 1992, a number of laboratories around the world have studied the use of plants as the bioreactor to produce antigenic proteins of human or animal pathogens. Over the last ten years, these works have been mainly focused on three major strategies for the production of antigenic proteins in plants: stable genetic transformation of either the nuclear or plastid genome, or transient expression in plants using viral vectors. As many antigenic proteins have been expressed in tobacco, also several laboratories have succeeded to express genes encoding antigenic proteins in other crop plants: potato, tomato, maize, carrot, soybean and spinach. At present many works for the production of edible plant vaccine against bacteria-mediated diseases have mostly performed the studies of enterotoxins and adhesion proteins. Also the development of new-type antigens (pili, flagella, surface protein, other enterotoxin and exotoxin etc.) is required for various targets and more efficacy to immunize against microorganism pathogens. Many works mostly studied in experimental animals had good results, and phase I clinical trial of LTB clearly indicated its immunogenic ability. On the other hand, edible plant vaccines have still problems remained to be solved. In addition to the accumulation of sufficient antigen in plants, human health, environment and agriculture regulation should be proven. Also oral tolerance, the physiological response to food antigens and commensal flora is the induction of a state of specific immunological unresponsiveness, needs to be addressed before plant-derived vaccine becomes a therapeutic option.

송과선종의 방사선치료 성적 (Radiotherapy Results of Pineal Tumors)

  • 1979년부터 1985년까지 서울대학교병원 치료방사선과에서 치료를 받은 송과선종 환자 23명에 대한 후향적 분석을 시행하였다. 종양에 대한 조직학적 진단은 1명에서만 이루어졌고, 나머지 22명의 환자에 있어서의 진단은 임상적 소견 및 방사선학적 경사결과에 의거하였다. 방사선조사는 1명에 있어서는 종양부위에 국한하였고, 14명에 있어서는 전 뇌부위, 8명에 있어서는 전뇌-척수부위에 대하여 시행하였다. 전 환자의 5년 생존율은 $71.5\%$이었고, 전 뇌부위 치료 환자의 5년 생존율은$69.3\%$, 전뇌-척수부 위 치료 환자의 5년 생존율은 $73.3\%$이었다. 원발 부위 및 원격부위에서의 재발이 각각 한명씩 확인되었으며 척수부위에서의 치료실패는 확인된 경우가 없었다. 이상의 분석결과 및 기왕의 여러 보고를 바탕으로 하여 송과선종의 방사선치료에 있어서 모든 환자에서 전척수 조사를 시행하는 것은 타당치 않으며 전척수 조사는 뇌척수액내에서 종양세포가 발견된 경우나 또는 종양의 제거나 생검을 위하여 외과적 시술이 시행되었던 경우 등에 국한되어야 할 것으로 결론지었다

Knowledge Level of Working and Student Nurses on Cervical Cancer and Human Papilloma Virus Vaccines

  • Purpose: To determine knowledge levels of working and student nurses about cervical cancer and prophylactic cancer vaccines. Materials and Methods: This study was performed on 259 nursing students in the Department of Nursing and 137 nurses working in Health Research and Practice Center, approved to participate in the study between April-June 2012. The study was performed universally without selecting a sample. A questionnaire that was prepared for evaluating participants' knowledge and attitudes about human papilloma virus (HPV) vaccine was distributed to the nurses and data obtained from the forms were transferred to SPSS 15.00 program and statistically analyzed. Results: It was found that 54.8% of the student nurses were between 21-24 years old and 13.1% of working students were between 25-28 years old. When student nurses and working nurses were compared in terms of their knowledge about the causes of cervical cancer, their ideas about prevention from cervical cancer with HPV vaccine, their ideas about possible risks of HPV vaccine and conservation ratios of HPV vaccine, it was observed that there were no statistically significant differences (p>0.05). When student nurses and working nurses were compared in terms of the information-source about HPV, ways of HPV contamination, awareness about people who are susceptible to HPV contamination and age of HPV vaccination, it was determined that there was a statistically significant difference (p<0.05). Conclusions: It was found that all nurses had some knowledge about cervical cancer and HPV vaccine, but this was not sufficient. Therefore; it is recommended to use verbal, written and visual communication tools intensively in order to have topics on cervical cancer, early diagnosis and prevention in bachelor and master programs for nurses, to inform society about cervical cancer and HPV vaccine for public health and to teach precautions for its prevention.

두통의 치료 (Treatment of the Headache)

  • 두통은 인류의 가장 흔한 호소 중의 하나로 임상에서 흔히 보는 장애이다. 두통은 뇌막염, 뇌출혈, 또는 뇌종양과 같은 다른 질환의 증상일수 있으나, 또한 편두통이나 군발두통 등과 같은 질병 자체로 표현된다. 일차적으로 두통 장애의 역학이나 국제 두통학회의 진단기준을 이해하고 흔치 않으나 심각한 이차적인 두통장애와 감별에 관심을 둬야 한다. 환자가 일차 두통장애의 기준에 맞으면 신경학적 진단검사의 보충이 없어도 치료를 시작한다. 두통 유형, 표현 양상, 동통기간과 강도 등에 따라 진통소염제나 혼합진통제, 혈관작용의 항편두통 약물 또는 신경이완제나 corticosteroid등을 선택한다. 편두통의 빈도와 강도에 따라 예방치료가 보통 4~6개월간 조절한다. 긴장형 두통은 발작성과 만성두통으로 구분되나 치료적으로는 급성완화와 예방치료로 시도된다. 많은 만성매일두통 환자들이 진통제나 ergotamine을 과용하고 있으며 그들의 의존성과 내재된 갈등조절, 수면장애, 우울등으로 과용된 약물의 제한이 쉽지 않다. 치료의 첫단계는 약물을 끊고 조심스럽게 대치요법을 시행한다.

군리탕가감방(君理湯加減方)이 항종양(抗腫瘍) 면역반응(免疫反應)과 항암제로 유발(誘發)한 부작용(副作用)에 미치는 영향(影響) (Effects of Gunleetang Gagambang Extract on Antitumoral Immunological Response and the Side Effect Induced by Antitumoral Agents)

  • Even though appropriate immune response is necessary for the survival of the individual, excessive or insufficient immune Response might cause autoimmune or allergic disease. So the immune response must be controlled to the degree that is beneficial for the well being of the individual. This study was undertaken to know the effects of Gunleetang Gagambang on the immune system of the mouse. Gunleetang Gagambang has been used for cure of tumor as a traditional medicine without any experimental evidence to support the rational basis for its clinical use. This study was carried out to evaluate the possible therapeutic or antitumoral effects of Gunleetang Gagambang extract against tumor, and to carry out some mechanisms responsible for its effect. Some kinds of tumor were induced by the typical application of 3-methylcholanthrene(MCA) or by the implantation(s.c) of malignant tumor cells such as leukemia cells(3LL cells) or sarcoma cells(S180 cells). Treatment of the Gunleetang Gagambang on water-extract(dailly 1mg/mouse, i. p.) was continued for 7 days prior to tumor induction and after that the treatment was lasted for 20 days. Against squamous cell carcinoma induced by MCA, Gunleetang Gagambang decreased not only the frequency of tumor production but also the number and the weight of tumors per tumor bearing mice(TBM). Gunleetang Gagambang on also significantly suppressed the development of 3LL cell and S180 cell-implanted tumors in occurrence-frequency and their size. and some developed tumors were regressed by the continuous treatment of Gunleetang Gagambang extract into TBM. In vitro, treatment of Gunleetang Gagambang extract had no effect on the growth of some kinds of cell line such as FsaII, A431 strain but significantly inhibited the proliferation of 3LL, S180 cells and augmented the DNA synthesis of mitogen-activated lymphocytes. Gunleetang Gagambang also stimulated the migrative ability of leukocyte, the MIF and IL-2 production of T lymphocytes, but not IL 6 production of B cells. Gunleetang Gagambang administration to mice enhanced NK cells activities. These results demonstrated that Gunleetang Gagambang extract exhibited a significant prophylactic benefits against tumors and its antitumor activity was manifested depending on the type of tumor cells. And these results also suggested that effect of Gunleetang Gagambang might be chiefly due to nonspecitie enhancement of NK cell activities and cell-mediated immune responses.

A novel herbal formulation consisting of red ginseng extract and Epimedium koreanum Nakai-attenuated dextran sulfate sodium-induced colitis in mice

  • Background: Ulcerative colitis (UC) is a commonly encountered large intestine disease in the contemporary world that terminates into colorectal cancer; therefore, the timely treatment of UC is of major concern. Panax ginseng Meyer is an extensively consumed herbal commodity in South East Asian countries, especially Korea. It exhibits a wide range of biologically beneficial qualities for almost head-to-toe ailments in the body. Epimedium koreanum Nakai (EKN) is also a widely used traditional Korean herbal medicine used for treating infertility, rheumatism, and cardiovascular diseases. Materials and methods: Separately the anti-inflammatory activities of both red ginseng extracts (RGEs) and EKNs had been demonstrated in the past in various inflammatory models; however, we sought to unravel the anti-inflammatory activities of the combination of these two extracts in dextran sulfate sodium (DSS)-induced ulcerative colitis in mice model because the allopathic remedies for UC involve more side effects than benefits. Results: Our results have shown that the combination of RGE + EKN synergistically alleviated the macroscopic lesions in DSS-induced colitic mice such as colon shortening, hematochezia, and weight loss. Moreover, it restored the histopathological lesions in mice and decreased the levels of proinflammatory mediators and cytokines through the repression of nuclear factor kappa-light-chain-enhancer of activated B cells (NF-κB) and nucleotide-binding domain (NOD)-like receptor protein 3 (NLRP-3) expression. In vitro, this combination also reduced the magnitude of nitric acid (NO), proinflammatory mediators and cytokine through NF-κB and mitogen-activated protein kinase (MAPK) pathways in RAW 264.7 mouse macrophage cells. Conclusion: In the light of these findings, we can endorse this combination extract as a functional food for the prophylactic as well as therapeutic treatment of UC in humans together with allopathic remedies.
