Core nursing practice and educational requirements according to nursing unit and clinical career in tertiary hospital (국내 상급종합병원의 간호단위 및 임상경력별 핵심간호실무와 교육요구도)

  • Purpose: The purpose of this study was to provide fundamental data to develop an educational program for the development of nurses' core competences, and to improve the curriculum in nursing education institutions. Methods: This study, based on a descriptive survey, was conducted with 1,019 nurses working in a tertiary hospital in Seoul. Among 47 core nursing practice items, core nursing practices as perceived by the participants and relevant educational requirements were measured on a four-point scale. The collected data were analyzed using descriptive statistics for the mean and standard deviation, one way ANOVA, and Duncan test as post-hoc test. Results: Regarding the core nursing practice perceived as by the nurses, there were statistically significant differences in 43 out of 47 items according to nursing unit, and in 15 items according to clinical career. Regarding the educational requirements for core nursing practice, there were also statistically significant difference for the same items as perception differences according to nursing unit and clinical career. Conclusion: The results of this study suggest further study on the development of educational programs to advance the clinical expertise of nurses and for the development of curriculum reflecting the actual conditions and requirements in clinical nursing practice fields.

A Study on the Architectural Planning of Corridor Space in Nursing Homes for the Elderly with Dementia (치매노인전문요양시설의 복도공간에 관한 건축계획적 연구)

  • The purpose of this study is to build fundamental data related to corridor space for nursing home design. Elderly with dementia were observed in corridor space at two nursing homes in In-cheon City. Two types of behavior were found out through in-depth observation; Group and Personal behaviors. Group behaviors were varying according to the type of programs such as rest, care, diet, and therapeutic services. Personal behaviors could be classified into wandering and private seclusion. These two behaviors were influenced by management programs of facilities, services of care-givers and individual characteristics of residents. The result of this study was discussed on the following matters: 1) Optimum size of residential unit; 2) Working spaces of care-givers; 3) Relationship between bedroom and common space for gathering; 4) Planning for wandering path; 5) Familiarity of interior finishing materials

Factors Affecting on the Work-related Performance Level of TB Control Manpower (결핵관리전담요원의 업무수행정도와 영향요인)

  • Purpose: The purpose of this study was to investigate the degree of performance of TB control manpower in Korea and to identify the affecting factors associated with it. Methods: This study employed a quantitative observational with cross-sectional design and the subjects were consisted of 189 TB (Tuberculosis) control manpower who were working in both community health centers and private hospitals. Variables included in this study were demographic, social, work-related characteristics, aqhnd self-efficacy. A reliability of the instrument for the performance was Cronbach's ${\alpha}$=.91. Data were analyzed by t-test, ANOVA and multiple regression using SPSS (19th version). Results: The mean value of the degree of work-related performance of TB control manpower was $4.4{\pm}0.49$9 and it was higher than those of general nurses working in any other health care departments. The mean differences in the degree of work-related performance were in self-efficacy, workplace, the number of full charge manpower in TB, the number of participation in in-depth education, and the willingness to job maintenance. In multiple regression analysis, factors associated with TB control manpower' work-related performance were self-efficacy (${\beta}$=.164, p=.025), workplace (${\beta}$=-.186, p=.023), the number of participation in in-depth education(${\beta}$=.339, p<.002), and the willingness to job maintenance (${\beta}$=-.157, p=.036). Conclusion: Results of this study showed that work-related performances of manpower working in private hospital were higher than those of manpower in public health centers. So, it is necessary to enhance nation's active intervention to improve effectiveness and reduction of risk factors in TB policy.

A Study on the Changes of Living Activities and the Use of Spaces of the Recuperating Elderly with Dementia in Skilled Nursing Facilities in Rural Area (농촌지역 노인요양시설 치매노인의 생활행위와 공간이용 변화에 관한 연구)

  • The purpose of this study is to clarify the actual condition of the daily living activities of the elderly with dementia in skilled nursing facilities. The thorough investigation and observation works were made from the view points of daily living behaviors and behavioral places of the recuperating elderly with dementia in skilled nursing facilities. This article discusses about the basic characteristics of the elderly and the actual condition of the daily living activities of the elderly with dementia in 3 skilled nursing facilities. The results of this study are summarized as follows; There is no wide difference between skilled nursing facilities on the characteristic of living behaviors of the elderly with dementia. Daily living activities and the use of spaces are largely influenced by the daily program and the operation policy of each skilled nursing facilities in addition to human relationship of the elderly with dementia. Common spaces such as day room, dining room plays an important role for their daily living activities, because that most of the elderly with dementia stay long during daytime. Layout of each private room and common space seems very crucial point in designing these facilities, in order to enable demented people to live their lives more independently.

Parenting Stress in Mothers of Premature Infants (미숙아를 출산한 어머니의 양육스트레스)

  • Purpose: The purposes of this study were to assess the parenting stress in mothers of premature infants and stress related characteristics of mothers and infants. Methods: The methodology was a cross sectional survey study using self-report questionnaires. Participants in this study were 36 mothers of infants with corrected ages of 4 months to 12 months who were born prematurely. Data were analyzed using t-test, ANOVA, and Pearson correlation with the SPSS WIN 19.0 program. Results: The mean score for parenting stress in mothers of premature infants was $74.639{\pm}17.570$, indicating that the mothers actually experienced stress. When mothers were able to have some private time, parenting stress was statistically significantly lower. When the residential status was having one's own house and the local community provided informational support regarding child nurturing, there was a statistically significant decrease in the sub-category of particular infant temperament. There was a statistically significant positive correlation between mothers' depression and parenting stress. Conclusion: These results indicate that there is a need for nursing interventions to increase mothers' private time and to include informational support regarding health management of children including developmental status assessment from local communities.

Recognition and Performance of Patient Private Information Protection (PPIP) in Nursing Students (환자 개인정보보호에 대한 간호대학생의 인식과 실천)

  • In this research we surveyed level of textual recognition and of practice by nursing students regarding patient privacy protection. The subjects were 383 nursing students. The questionnaire developed by Lee and Park (2005) were modified and used. Data analysis was conducted through descriptive statistics, t-test, ANOVA, and Pearson correlation coefficients. The result shows that about 96.1% of the subjects received education on patient privacy protection in school or hospital but 48.0% of all received education of 10 minutes or less. Average score for recognition of patient privacy protection was a 4.13 but average practice score was 3.84. Among all areas, communication tasks showed the highest scores for both recognition and practice. There are differences according to grade in school and age of the subject, and experience with clinical practice, existence of regular educational program embedded in clinical practice at hospitals, and the length of each educational session. For both total score and score for every domain, there was a correlation between recognition and practice. The conclusions drawn from this research will help colleges of nursing and hospitals to design and develop contents of educational programs for nursing students.

The Lived Experience of Caregivers of Chronic Renal Failure Patient (만성신부전환자의 주보호자로 살기)

  • Purpose: The research aims to understand the lived experience of the caregivers of chronic renal failure(CRF) patients and its essential meaning. The results of the study can be used as basic materials for developing comprehensive intervention methods of care givers of CRF patients. Method: The research used van Manen's hermeneutic and phenomenological research methods in order to describe the lived experience and to understand its meaning. It concentrates on the understanding the essence of experience and consists of existential survey, hermeneutic and phenomenological reflection and hermeneutic writings. Participants in this research were five women care givers of CRF patients who had hemolysis at C university hospital in a metropolitan city, the period of data collection was from July 27 to Sep. 4, 2004 and major data of results in the following 5 essential themes. "sole responsibility for the patient enduring everything", "creating their own field", "heavy and painful life without hope of their private life", "wishing not to be inherited and consoling each other". Conclusion: The above findings point out that the experience of care givers of CRF patient affected and changed all parts of an individual life and his or her family life. Therefore, it suggests that total family nursing care must be considered in order to provide the holistic caring for CRF patients and their care givers.

A Study on the Depression, Somatic Symptom, Activities of Daily Living for, the Elderly Women in an Urban Area (도시지역 여성노인의 우울, 주관적 신체증상 일상생활 수행능력에 관한 연구)

  • Purpose: The purpose of this study was to investigate the relationships among depression, somatic symptoms, and activities of daily living of elderly women in urban areas. Method: After obtaining participant's consent forms, a one-time, face-to-face, and private interview was conducted with each participant from Sep, 2006 to Jan, 2007 by trained graduate-level students. The questionnaire consisted of K-GDS, PHQ-15, Barthel Index, and Instrumental Activity of Daily Living. The collected data was analyzed with the SPSS/PC 12.0 program, which was used for frequency, percentage, mean, standard deviation, Pearson correlation coefficients, and multiple regression. Results: The major findings of this study were as follows 1) 34.1% of participants belonged to the depression group. 2) There were significant relationships between depression and monthly income, somatic symptoms, ADL, IADL, and number of chronic disease. 3) Significant factors influencing depression were somatic symptoms, ADL, and monthly income. Conclusion: The results of this study give useful information for designing interventions and program development for appropriate depression management and care for elderly women.
