An Analysis of Infertility Patients (불임증(不姙症) 환자(患者)의 통계적(統計的) 고찰(考察);서울대학교병원(大學校病院) 불임상담실(不姙相談室) 1872 예(例)의 분석(分析))

  • This study was presented of the 1,872 cases of infertile couples who visited and examined at the sterility clinic of Department of Obstetrics & Gynecology, Seoul National University Hospital from Sept., 1980 to Dec., 1983. Age, duration of infertility, past medical history, and other general factors were analyzed, and the factors responsible for infertility were classified and discussed. Mode of treatment, outcome of pregnancy, pregnancy rate responsible for each factor were also presented. The results were as follows: 1) The infertility was primary in 1,128, or 60.3% and secondary in 744, or 39.7%. 2) The age between 26 and 30 years of age comprised about one half of the total patients. 3) The duration of infertility between 1 and 4 years comprised about three quarters of the total patients, and the mean duration was 3.8 years. 4) The most common medical history in primary infertility was tuberculous disease, and that in secondary infertility was history of previous laparotomy. 5) About two thirds of antecedent pregnancies were abortion. 6) The major etiologic factor of infertility were male factor in 12.3%, tubal factor in 38.8%, ovulatory failure in 25.4%, uterine factor in 8.8%, cervical factor in 5.2%, peritoneal factor in 9.5%, and no demonstrable cause in 11.3%. 7) The types of male factor were azoospermia in 61.6%, oligospermia in 25.8%, low motility in 11.6%, and other abnormality in 1.0%. 8) The types of ovulatory failure were ovarian failure in 7.4%, hypothalamo-pituitary failure in 8.1 %, hypothalamo-pituitary dysfunction (including Polycystic ovarian syndrome) in 30.2%, and hyperprolactinemia in 22.4%. 9) The types of uterine factor were endometrial tuberculosis in 27.5%, uterine synechia in 33.8%, uterine anomaly in 19.7%, myoma and polyp in 9.1 %, and luteal phase defect in 9.9%. 10) The types of peritoneal factor were pelvic adhesion in 80.9% and endometriosis in 19.6%. 11) Surgeries were done in 408 patients, and they were salpingolysis, lysis of extraadnexal adhesion, salpingostomy, fimbrioplasty, ovarian wedge resection for polycystic ovarian disease, tubo-tubal anastomosis, and tubo-uterine implantation in orders. 12) 243 pregnancies were achieved during the infertility work-up, of which livebirth was 46.5%, ectopic pregnancy was 7.4%, spontaneous abortion was 7.8%, and on-going pregnancy or lost to follow-up was 36.2%. 13) Pregnancy rates in various factors were male factor in 18.7%, ovulatory factor in 31.7%, tubal factor in 24.2%, uterine factor in 34.6%, cervical factor in 19.0%, peritoneal factor in 29.0%, combined factors in 10.5%, and unexplained infertility in 37.1%. Pregnancy rate in whole patients was 25.2%.

  • Purpose : To investigate clinical-epidemologic aspect of infertility patients in Conmaul Oriental Hospital. Methods : From 2003, 5 to 2004, 4, a total 1223(female 943, male 280) patients was recruited, and their medical records were reviewed retrospectively. Results : The mean age of female was $31.83{\pm}3.63$ yesrs, and $34.08{\pm}3.66$ years in male. The mean BMI of female was $20.83{\pm}2.74$, and $24.36{\pm}3.03$ in male. The most common occupation of female was profession, administration, management.The primary infertility was 52.9% and the secondary infertility 47.1%. The most common past history in female was laparotomy. The mean duration of infertility was $3.32{\pm}2.5$ years and $3.9{\pm}2.63$ years in female and male. 83.6% of secondary infertility women experienced abortion. The incidence of patients with both combinded infertility factors of female and male was the most highest, and the most common single factor was ovulatory. 92.4% of female patients took sterility tests, 71.0% of male patients took semen analysis. 50.8% female took ovulation induction, IUI, IVF before coming. 8.91% of female and 72.14% of male were accompanied by their spouse. 61.2% of female took sterility test with their spouse. Female's duration of treatment was longer than man's. The most reliable source of choices was encouragement of family members, acquaintance. Conclusion : In this study, we presented staus concerend with infertility and the characteristics of patients went to oriental hospital. Furthermore, the study about oriental treatments and the results of that treatments is required.

Beneficial effects of intraovarian injection of platelet-rich plasma in women with poor ovarian response

  • Objective: Infertility can result from a diminished ovarian reserve, but a potential remedy exists in the form of platelet-rich plasma (PRP) administration. This treatment involves both biological factors and tissue trauma mechanisms, which stimulate folliculogenesis, making it a promising and effective strategy. We assessed the impact of direct PRP injections into the ovaries on the fertility outcomes of women classified as poor responders. Methods: A quasi-experimental study was conducted from April 2021 to December 2022, focusing on patients classified as POSEIDON grade 3 or 4. PRP injections were administered into both ovaries. After 3 months, data were collected on anti-Mullerian hormone (AMH) level, follicle-stimulating hormone (FSH) level, and the numbers of oocytes, mature oocytes, and good-quality embryos following ovarian stimulation. We then compared the data from before and after PRP injection. Results: This study included 50 women, with a mean of 39 years (interquartile range [IQR], 35 to 43) and 4 years (IQR, 2 to 6) for age and infertility duration, respectively. FSH levels decreased after treatment, while AMH levels and the numbers of oocytes, metaphase II oocytes, and high-quality embryos increased. However, only the increase in high-quality embryos was significant. The pregnancy and spontaneous pregnancy rates were 20% and 14%, respectively. Notably, women with secondary infertility exhibited a significantly higher pregnancy rate than those with primary infertility. Conclusion: Ample evidence suggests that PRP can enhance ovarian function. However, further studies are needed to identify the appropriate candidates for this procedure, establish the optimal PRP preparation method, and standardize the procedure for its adjuvant use in assisted reproductive technology cycles.

Study on Status Changes in Female Infertility who Admitted to Oriental Hospital (한방병원에 내원한 여성 불임 환자의 실태 변화 연구)

  • Purpose: Examine changes in the aspect and state of patient with infertility who admitted to Korean oriental hospital. Look at the changes in last 8 years and seek how oriental medicine can approach to infertility patients who admitted to Korean oriental hospital in last 8 years. Methods: Total of 7434 new patients with infertility who admitted to Gynecology of Oriental Hospital, $\bigcirc\bigcirc$ University Medical Center, from October 2002 to December 2009 was compared to infertility patients based on bottom collected data in this study, total of 1061 infertility patients were targeted for this study. Results: 1.There is no distinct increases in age between women who admitted to oriental medicine and patient's average age, it has shown constantly increasing trend in their age though. However, the average age of infertility patients have shown significant increasing range than average age of all infertility patients. 2. After analysis the result of infertility patient's rate of overall patients, the rate of infertility patients are remaining the same or slowing down in 2005, as the starting year(Table 2). The significant relationship was found in diagnosis and treatment of gynecology(Table 4~5) for infertility patients. In 2005 as the starting year, as patients who admitted to oriental hospital after diagnosed or treated at gynecology are increasing, the rate of infertility patients of overall patients admitted to hospital is slowing down, which is decreasing. 3. There is increasing number of patient diagnosed with "unknown cause" who admitted to Gynecology of Oriental Hospital after year of 2005. 4. The increasing number of patient diagnosed with "unknown cause" who admitted to oriental hospital has a tendency to be important factor in infertility treatment of oriental medicine. Conclusion: Unknown causes of infertility patients who admitted to oriental hospital will continuously increase as senses of society, economic, and social climate change.

Review of Indicators and Tools used to Assess Korean Medicine Infertility Treatment (한의 난임 치료에 활용되는 평가지표와 평가도구 사용 현황 조사 및 고찰)

  • Objectives: To investigate and analyze the assessment indicators and tools used in clinical practice to assess Korean medicine (KM) treatment for infertility, and to establish a basis for assessment tools to diagnose and assess infertility. Methods: Relevant studies published until March 2023 were extracted from Pubmed, Research Information Sharing Service, and National Digital Science Library databases. Results: Sixty-four studies comprising 4,105 patients were included. We investigated pregnancy outcomes, and assessed pregnancy- and childbirth-related factors, overall health, reproductive health, and mental health. Pregnancy result was most common primary outcome. Ongoing pregnancy, stillbirth, miscarriage, and ectopic pregnancy rates were suggested as indicators of pregnancy and childbirth-related assessment. Overall health was most commonly assessed with Likert and Visual analogue scale (VAS). Among reproductive health variables, menstrual history was most frequently assessed. Moreover, indicators such as reproductive function, sexual intercourse, and gynecological disease were assessed. The Infertility Stress Scale and the Fertility Quality of Life tool (FertiQoL) were used to assess mental health. Conclusions: Subjective scales and objective assessment tools, such as the Likert scale and blood tests/ultrasonography, respectively, are used to assess KM infertility treatment. Inconsistent assessment tools make quantitative analyses more challenging. The development of a standardized mental and physical function assessment questionnaire with confirmed reliability and validity ensure the effectiveness of KM infertility treatment, and promote future studies on infertility treatment.

The Efficacy on the Immunotherapy with Patient Lymphocytes in Unexplained Infertility (원인불명의 불임환자에서 림파구 정맥주입에 의한 면역 치료의 효능에 관한 연구)

  • The aim of present study was to determine the efficacy of immunotherapy with paternal lymphocytes for unexplained infertility. It has been apparent that reproductive success may be affected by the presence of abnormal autoantibodies. Unexplained infertility and repeated pregnancy wastage has been reported in the presence of abnormal autoantibodies. These data suffest that abnormal immune function may be an important pathologic entity contributing subfertility in patients with unexplained infertility. Therefore, immunotherapy may be a possible treatment modality for patients with unexplained infertility. Some investigators have reported that a proportion of infertile couples with repeatedly unsuccessful ET showed close histocompatibility similar to those of spontaneous recurrent abortion. Recently, it has been noted that immunotherapy with paternal lymphocytes achieves a high efficacy in preventing subsequent abortion in women with primary recurrent abortion of unknown cause, which was mediated by immune reaction including blocking antibody. To substantiate the hypothesis, we applied immunotherapy preceding Peritoneal Oocyte and Sperme transfer (POST) to 43 patients, 47 cycles of 82 patients, 89 cycles with at least three previous IUI failure from April, 1993 to February, 1995. There were no significant differences between treatment and control group in clinical response and hormonal response to controlled hyperstimulation. there was no significant difference between treatment and control group in pregnant rates per cycles (42.6% versus 28.6%), but a significantly lower abortion rate per pregnancy in treatment group, with 10.0% (2/20) compared with 50.03% (6/12) in control group. This study may suggest that immune therapy for patients with unexplained infertility with paternal lymphocytes might be beneficial.

The Stress of the Infertile Women (불임여성의 불임스트레스 정도에 관한 연구)

  • This study was designed and carried out to describe the stress of the infertile women and identify its predictors. The subjects of this study, 131 infertile women, were in primary or secondary infertility. the were conveniently sampled out from the infertility clinics of K University Medical Center and C Hospital in Seoul. The data were collected by using the Infertility Stress Scale which consisted of 35 items with four dimensions(cognitive, affective, marital and social stress) from August to November 1994. The data were analyzed by using the pc-SAS program. The information was obtained of Mean, Standard Deviation, Frequencies, Percentile, t-test, ANOVA, Duncan's multiple comparison test and Multiple Regression. The results are as follows; 1. The Mean of the stress of the infertile women is 2.78. The Means of the stress in 4 dimensions are 3.81 in the cognitive dimension, 3.05 in the affective dimension, 2.06 in the marital adjustment dimension and 2.41 in the social adjustment dimension. 2. The predictors of the stress of the infertile women are their educational levels and subjective economic status. They explain 14.08% of total variance.

  • Adrenal rest tumors are a rare extra-adrenal complication of congenital adrenal hyperplasia (CAH) in women although they are more commonly found in the testes of male patients with CAH. An ovarian adrenal rest tumor (OART) may coexist with CAH or imitate its symptoms without CAH. In this case report, we present the case of a woman with OART without CAH, whose main complaint was infertility and who had a baby after successful surgical treatment.

  • A clinical investigation was undertaken on primary male infertility patients of recent 5 years. The results obtained were as follow: 1. Suspective etiologic factors were: 1) testicular failure, 36.1 %; 2) varicocele, 18.7%; 3) endocrine abnormality, 13.5%; 4) obstruction, 13.5%; 5) idiopathic, 10.9%; 6) cryptorchidism, 2.6%; 7) necrospermia, 0.9%. 2. On semen analyses, azoospermia was found in 55.8%, single abnormal parameter in 21.5 %, and multiple/all abnormal parameter in 22.7% of the 163 cases. 3. For the evaluation of the sensitivity and specificity of noninvasive variables in predict in obstruction as the cause of azoospermia in patient who had undergone testicular biopsy, the testicular size and serum follicle-stimulating hormone(FSH) level revealed 100% of sensitivity. 4. Among the 43 patients with a testicular biopsy confirmed diagnosis there was a significant difference in testicular size, ejaculate volume(p<0.0001) and serum FSH(p<0.0001) between patients with testicular failure and those with ductal obstruction. 5. Of 93 treated patients with primary male infertility, 42 were managed by medical treatment including endocrine treament, retrograde ejaculation treatment, infection treatment and observation; 29 were managed by surgical treatment including varicocelectomy, vasovasostomy, vasoepididymostomy and TUR of ejaculatory duct; 20 were managed by sperm preparation treatment including artificial insemination(AI), electroejaculation plus AI and vibration ejaculation plus AI ; 2 were managed by microscopic epididymal sperm aspiration plus IVF, repectively. 6. 42 patients who could be followed-up, 21 patients(50%) impregnated their wives.

A large advanced seminoma in an older woman with androgen insensitivity syndrome

  • A 58-year-old woman who presented with inguinal hernia for the first time was diagnosed as seminoma and complete androgen insensitivity syndrome (CAIS). The patient received a late diagnosis, and therefore she could not take a proper management. CAIS is a rare X-linked recessive disease with an XY karyotype that is caused by androgen receptor defects. It usually present with primary amenorrhea or inguinal hernia. The risk of malignant transformation of undescended testis increases with age, thus gonadectomy should be performed after puberty. We present a case of large advanced seminoma in a woman with CAIS who was neglected and diagnosed lately.
