  • Air abrasion technology can prepare enamel and dentin for bonding, similar to etching by acidic gels and solutions. Longer treatment can excavate pit and fissures, preparing the tooth for immediate placement of bonded resin materials. Although not appropriate for every clinical situation, the air abrasive technology minimizes heat, vibration and bone-conducted noise associated with conventional means of caries removal since the cutting is accomplished by air pressure. Also, patients treated with the air-abrasion technology rarely request anesthesia. Air abrasion technology was more effective in treating early carious lesions and stains compared to lesions where caries had already progressed to produce soft dentin and the strong air stream and noise caused by the evacuation system was a major discomfort to pediatric patients, and the experience and skillfulness of clinician should be required for accurate and proper tooth preparation.

THE ANNUAL COMPARISON OF DENTAL CARIES PREVALENCES OF "K" PRIMARY SCHOOL CHILDREN. ("K"국민학교(國民學校) 아동(兒童)의 치아우식증(齒牙齲蝕症) 이환추이(罹患推移)에 관(關)한 연구(硏究))

  • The dental caries survey of "K" primary school children was performed twice for one year interval and evaluated the transitional trend of dental caries prevalences. The number of children surveyed at the first year, 1973, was 819 boys and 653 girls; and the second year, 1974, was 815 boys and 661 girls aged 6 to 12. Under this survey, the following results were detected: 1. The average df person rate in deciduous teeth was increased by 7.52% in boy, 5.59% in girl and 6.65% of both. 2. The average df Index in deciduous teeth was increased by 0.13 in boy, 0.04 in girl and 0.1 of both. The average dt Index was decreased by 0.59 in boy, 0.64 in girl and 0.61 of both. The average ft Index was increased by 0.73 in boy, 0.68 in girl and 0.71 of both. 3. There showed no significant differences in DMF person rate between 1973 and 1974. 4. The DMFT Index of the permanent teeth was decreased by 0.19 in boy, and increased by 0.22 in girl. There showed no significant differences in both sexes between 1973 and 1974. The average DT Index was decreased by 0.43 in boy, 0.03 in girl and 0.31 of both. The average FT Index was increased by 0.23 in boy; 0.35 in girl and 0.29 of both. The average MT Index showed no significant differences in both sexes between 1973 and 1974.

  • The third year dental caries survey of "K" primary school children was performed in June 1975, and evaluated their transitional trends of three consecutive past years, statistically. 1,421 children (782 boys and 639 girls) 6 to 12 years of age was surveyed at the third year. The results were as follows: 1) The average df person rate was 89.97%(boy 88.19%, girl 92.42%). It showed significant increment(P<0.005) compare to 1973 and nonsignificant to 1974. 2) The average dft index was 3.82(boy 3.76, girl 3.90). It showed nonsignificant difference compare to 1973 and 1974. 3) The average DMF person rate was 66.57%(boy 63.84%, girl 69.89%). It showed significant decrement(P<0.005) compare to 1973 and 1974. 4) The average DMFT index was 2.03(boy 1.88, girl 2.20). It showed significant decrement(P<0.005) compare to 1973 and 1974. 5) The sexual difference of df and DMF person rates, female showed a little higher value than male. It showed significant difference(P<0.005). 6) The increment of df and DMF person rates, the 7 to 8 year group showed the most and the 7 to 8 year group showed the least.

Generation of antibodies against N-terminus fragment of AgI/II protein from Streptococcus mutans GS-5 (연쇄상구균(Streptococcus mutans GS-5)의 항원단백질 AgI/II의 N-terminus절편에 대한 항체형성)

  • Dental caries results from localized demineralization of tooth enamel by acids of bacterial origin produced from the fermentation of dietary sugars. A group of related oral bacteria, collectively known as mutans streptococci, are implicated as the primary etiological agents of human caries. Within this group, Streptococcus mutans has been known as a causative agent for dental caries. As well as acid production yielding the demineralization of tooth enamel, adherence and colonization of S. mutans to the teeth are also important for their virulence Cell-surface fibrillar proteins, which mediate adherence to the salivary pellicle are virulence components of mutans streptococci, and primary candidates for a human caries vaccine. Here we report that the AgI/II gene from S. mutans GS-5 were cloned by PCR amplification of the bacterial chromosomal DNA and the integrity of cloned genes were confirmed by nucleotide sequencing. Sequence analyses showed the sequence alignment of 280 nucleotides between the cloned AgI/II and the reported sequence of S. mutans GS-5 showed the perfect match The cloned genes which signal nucleotide was truncated, were transferred into bacterial expression vector and the recombinant proteins were purified as His-tag fusion proteins In order to generate polyclonal antibodies against the recombinant proteins, AgI/II mr, some $100{\mu}g$ of the proteins was injected into mice three times. It can be used for an effective vaccine production to prevent dental caries caused by pathogenic S. mutans.

Detection of Proximal Caries Lesions with Deep Learning Algorithm (심층학습 알고리즘을 활용한 인접면 우식 탐지)

  • This study aimed to evaluate the effectiveness of deep convolutional neural networks (CNNs) for diagnosis of interproximal caries in pediatric intraoral radiographs. A total of 500 intraoral radiographic images of first and second primary molars were used for the study. A CNN model (Resnet 50) was applied for the detection of proximal caries. The diagnostic accuracy, sensitivity, specificity, receiver operating characteristic (ROC) curve, and area under ROC curve (AUC) were calculated on the test dataset. The diagnostic accuracy was 0.84, sensitivity was 0.74, and specificity was 0.94. The trained CNN algorithm achieved AUC of 0.86. The diagnostic CNN model for pediatric intraoral radiographs showed good performance with high accuracy. Deep learning can assist dentists in diagnosis of proximal caries lesions in pediatric intraoral radiographs.

Assessment of the Object Detection Ability of Interproximal Caries on Primary Teeth in Periapical Radiographs Using Deep Learning Algorithms (유치의 치근단 방사선 사진에서 딥 러닝 알고리즘을 이용한 모델의 인접면 우식증 객체 탐지 능력의 평가)

  • The purpose of this study was to evaluate the performance of a model using You Only Look Once (YOLO) for object detection of proximal caries in periapical radiographs of children. A total of 2016 periapical radiographs in primary dentition were selected from the M6 database as a learning material group, of which 1143 were labeled as proximal caries by an experienced dentist using an annotation tool. After converting the annotations into a training dataset, YOLO was trained on the dataset using a single convolutional neural network (CNN) model. Accuracy, recall, specificity, precision, negative predictive value (NPV), F1-score, Precision-Recall curve, and AP (area under curve) were calculated for evaluation of the object detection model's performance in the 187 test datasets. The results showed that the CNN-based object detection model performed well in detecting proximal caries, with a diagnostic accuracy of 0.95, a recall of 0.94, a specificity of 0.97, a precision of 0.82, a NPV of 0.96, and an F1-score of 0.81. The AP was 0.83. This model could be a valuable tool for dentists in detecting carious lesions in periapical radiographs.

  • The authors had surveyed on the snack foods of 300 primary school children in Seoul in order to obtain the informations for the dietary counseling and suggestions as a method of dental caries prevention. The obtained results were as follows; 1. Primary school children had the intake of the snack foods in the order of the apple, the candy, the orange, the cake, the milk, the biscuit, the Ramyeontang, the regular gum, the persimon, the Tugpokie. 2. Of the primary school children, 5.6% had intaking no snack food, but 10.6% once, 21.6% twice, 29.0% three times, and 33.0% four times or over in a day, respectively.

  • The fusion is an anomalous union of two or more tooth germ at some stage in the development process resulting in a decreased number of teeth in the dental arch. Fusion is common odontogenic anomaly but triplication of primary teeth is a rare phenomenon. We report a case of a 3 - year - old girl who visited our clinic for the pain on maxillary anterior tooth. The patient was diagnosed by the fusion of a the maxillary primary right central and lateral incisor with a supernumerary tooth and caries of dentine. And then, Endodontic treatment and composite resin restoration were performed on the triple teeth. After follow-up of 6 months period, there was no marked complications.

A Study on Appraisal for School Oral Health Education (국민학교 학동을 대상으로 한 직접 및 간접 구강보건교육의 효과평가에 관한 조사연구)

  • The authors have examimed 349 primary school children with questionare in order to appraise the oral health educational levels, one month later after performing oral health education as direct education and indirect education with video film, and compared the data from 350 uneducated children. The obtained results were as followings ; 1. It was estimated that the direct and indirect oral health education for school children were effective for in creasing the knowledge levels of oral health. 2. It was revealed that such items of oral health education as preventive measure for caries, tooth brushing method, etiology of dental caries and etiology of malocclusion were more effective for increasing the knowledge levels, compared to uneducated group. 3. Tooth Brushing Method should be educated in practical, not only to school children but also school teachers. 4. It should be established the goals and items for oral health education in practical as national level.

  • 150 "K" primary school children, from 6 to 10 years old, have been examined both clinically and radiographically for proximal caries in deciduous canine and deciduous posterior teeth and permanent first molar teeth. 1. 598 proximal cavities were diagnosed 154 (25.75%) were revealed by both examinations. 39 (6.52%) appeared on clinical examination only, but 405 (67.73%) were revealed by radiographic examination only. 2. The dental caries occurance on distal surface of first deciduous molar and mesial surface of second deciduous molar were revealed much higher values in all ages. 3. There is no significant differences observed between right and left arch. 4. Proximal cavities were revealed 1.7 times more in mandible than in maxilla.
