Medication Prescribing Status in Pediatric Patients with Migraine (소아청소년에서 편두통 치료 약물 처방 현황)

  • Background: Migraine is one of the leading causes of poor quality of life and disability, and migraine incidences in pediatrics are increasing. Proper medication is important for the preventive and acute treatment of migraine. This study aimed to identify the current status of prescribed medication in pediatric patients with migraine. Methods: We used data from a sample of pediatric patients from the Health Insurance Review and Assessment Service (HIRA-PPS-2018) and analyzed the status of prescription drugs and frequency of visits to medical institutions with migraine diagnoses in pediatric patients. Results: A total of 12,228 pediatric patients diagnosed with migraine during 2018 were analyzed. Among these patients, 7,170 (58.64%) were girls and 9,510 (77.77%) were adolescents. Additionally, 9,157 patients (74.89%) received acute treatment, and 592 patients (4.84%) received combination therapy with analgesics and triptans. Acetaminophen for acute treatment and flunarizine for preventive treatment were the most commonly prescribed. In most children and adolescents, acute treatment drugs were prescribed for less than 14 days. Conclusion: Analgesics, such as acetaminophen or non-steroidal anti-inflammatory drugs, were prescribed frequently for acute treatment in pediatric patients with migraine. The drug prescription duration was within the recommended range, indicating a low risk of overdose. For preventive treatment, clinically studied medication for pediatric patients with migraine was used.

ICU Nurses' Work Experience for Attempted Suicide Patient by Drug Ingestion: A Focus Group Study (일 대학병원 중환자실 간호사의 음독자살시도 환자 간호 경험: 포커스 그룹 연구)

  • Purpose: This study to describe experiences of nurses who work in intensive care units (ICUs) where they frequently encounter patients with attempted suicide by drug ingestion. Methods: Data were collected by 2 focus group interviews with 9 ICU nurses. The interview were recorded and transcribed, and data were analyzed using qualitative content analysis. Results: The results of data analysis, five themes were drawn: 'Confusion about for the attempted suicidal patient care', 'Helplessness for dying with attempted suicidal patients', 'Guilty for insufficient care', 'Ambivalence for the attempted suicidal patients', 'Recognition of need for professional approach'. Conclusion: The results of this study help us to understand patients who attempted suicide through the ICU nurses' experience. It would be useful to develop effective education programs for ICU nurses to preventive strategies for patients who are high risk of recurrence of suicidal crisis situation.

An Investigation on Acute Drug Intoxication of Inhabitants in Sim-An Islands Area (일부낙도지역주민의 음독에 의한 급성약물중독발생에 대한 조사)

  • The epidemiological information on 156 cases of acute drug intoxication treated at emergency room of Sin An Dai-Woo Hospital in Bi-Gum island during the year of 1981-1982 were reviewed and analyzed. And the findings obtained was as follows : 1) Annual incidence rate of acute drug intoxication was 3.3 per 1,000 in habitants (male 3.5, female 3.1). 2) In both sexes the highest incidence was obsesved in the age group of 20~49 years with 7.6 in male and 6.2 in female per 1,000 inhabitants. 3) In both sexes the highest ease distribution was observed during summer season of July and August (male 33.2%, female 26.4%). 4) Eating pesticides was the most frequently used for suicide purpose in both sexes (male 89. 3%, female 82.2%). 5) In terms of foundation for conclusion to eating poison, "by guardion's explanation" was the highest rate in both groups of eating pesticides (97.8%) and eating rodentcides(77.8%). 6) The case fatality rate due to eating poison was 4.5% (male 5.9%, female 2.8%). 7) In haspitalization days, the most cases was observed at less than 3 days (male 73.8%, female 79.2%).

Measures against Security Safety and Hallucinogenic Drug Crimes (환각성 약물범죄와 경호안전상의 대응방안)

  • Troublesome problems are being caused by hallucinogenic drugs which come not only from illegal narcotics but also from unprohibited general drug-kinds such as cough-remedy agents of textromerupan, namely, Lamina, Rubi-king and the likes. Abuse and misuse of narcotics permeated into and threatens mental health of socially-known peoples, actor, actress, talents and businessmen as well. This study especially dealt with problems related with Mesampetamin, Rurajaepam, Texstrometropan and Karisofrodol which affects mental behaviour, and considered counter-measures through analysis and developed appropriate system of goods circulation for guidance to be complied by the medicine professionalists. The guidance sought here is recommended to the government ministries for further enforcement of crack-down of the drug crimes. Arquesa in method is recommend as a method of identifying behaviors and facial expressions of abusers and misusers of narcotics at the spot in place. This study is directed to curtailing hallucinogenic drugs use for absolute preventive security by detecting out black dealers of narcotics supplying to the abusers and misusers. There is a need of special education program to cut off the flow of narcotics or drugs crimes into official security force as well as private security personnel. This study introduced a priority and emphasis of real augmentation to theory and practice of the modern scientific security which can protect the mental health of the guarded persons as well especially in the light of security safety curriculum at the school.

Levothyroxine Dose and Fracture Risk According to the Osteoporosis Status in Elderly Women

  • Objectives: To evaluate the association between fracture risk and levothyroxine use in elderly women with hypothyroidism, according to previous osteoporosis history. Methods: We conducted a cohort study from the Korean Health Insurance Review and Assessment Service claims database from January 2005 to June 2006. The study population comprised women aged ${\geq}65$ years who had been diagnosed with hypothyroidism and prescribed levothyroxine monotherapy. We excluded patients who met any of the following criteria: previous fracture history, hyperthyroidism, thyroid cancer, or pituitary disorder; low levothyroxine adherence; or a follow-up period <90 days. We categorized the daily levothyroxine doses into 4 groups: ${\leq}50{\mu}g/d$, 51 to $100{\mu}g/d$, 101 to $150{\mu}g/d$, and > $150{\mu}g/d$. The hazard ratios (HRs) and 95% confidence intervals (CIs) were estimated with the Cox proportional hazard model, and subgroup analyses were performed according to the osteoporosis history and osteoporosis-specific drug prescription status. Results: Among 11 155 cohort participants, 35.6% had previous histories of osteoporosis. The adjusted HR of fracture for the > $150{\mu}g/d$ group, compared with the 51 to $100{\mu}g/d$ group, was 1.56 (95% CI, 1.03 to 2.37) in osteoporosis subgroup. In the highly probable osteoporosis subgroup, restricted to patients who were concurrently prescribed osteoporosis-specific drugs, the adjusted HR of fracture for the > $150{\mu}g/d$ group, compared with the 51 to 100 ${\mu}g/d$ group, was 1.93 (95% CI, 1.14 to 3.26). Conclusions: While further studies are needed, physicians should be concerned about potential levothyroxine overtreatment in elderly osteoporosis patients.

A Study of the Influence of 'the Separation of Prescribing and Dispensing Roles' Policy on Community Pharmacies (의약분업실시에 따른 약국부문 경영수지 변화추계)

  • This study estimated the influence of 'the separation of prescribing and dispensing roles' (SPD) policy, which implemented in July 2000, on the community pharmacies in Korea, by using data on the revenues and expenditures in the previous researches. We first assumed the rate of drug price differential was 20% with four different models which was based on various data of studies carried out in the similar period. Later, we applied different rates for the sensitivity analysis. According to the results, all community pharmacies made the net profit of 118.9 billion won (0.552 million won monthly per one pharmacy) after the SPD policy. The ratio of net profit to net sales dropped by 7.41%p from 22.19% to 14.78%. Additional sensitivity analysis indicated there were wide range of differences about whether net profit or net gain, and that extent which either existed.

An Analysis of Infectious Disease Research Trends in Medical Journals From North Korea

  • Objectives: This study aimed to investigate the current status of infectious disease research in North Korea by analyzing recent trends in medical journals from North Korea in comparison with research from South Korea. Methods: Three medical journals (Preventive Medicine, Basic Medicine, and Chosun Medicine) were analyzed from 2012 to 2016. Articles on tuberculosis (TB), malaria, and parasitic diseases were selected and classified by their subtopics and study areas. Two medical journals published in the South Korea were selected for a comparative analysis of research trends. Results: Of the 2792 articles that were reviewed, 93 were extracted from North Korea journals. TB research in North Korea was largely focused on multi-drug resistant TB and extrapulmonary TB, whereas research in South Korea more frequently investigated non-tuberculous mycobacteria. Research on parasitic diseases in North Korea was focused on protozoan and intestinal nematodes, while the corresponding South Korea research investigated various species of parasites. Additionally, the studies conducted in North Korea were more likely to investigate the application of traditional medicine to diagnosis and treatment than those conducted in South Korea. Conclusions: This study presents an analysis of research trends in preventive medicine in North Korea focusing on infectious diseases, in which clear differences were observed between South and North Korea. Trends in research topics suggest a high prevalence of certain parasitic diseases in North Korea that are no longer widespread in South Korea. The large proportion of studies examining traditional medicine implies a lack of affordable medicine in North Korea.

The Effect of Gunshot or Cannonade Training during Military Service on Hearing Threshold Levels (군복무시 사격 및 포격훈련에 의한 소음폭로력이 청력에 미치는 영향)

  • To test if exposure history to rifle fire or cannonade training during military duty can induce hearing loss, history of personal military service and histroy of gunshot exposure were asked to 228 male college students with self -administrative questionnaire. Otoscopic examination and Rinne's test were performed if any abnormal finding was detected by pure-tone audiometry. Average hearing threshold levels of 500 Hz, 1,000 Hz, 2,000 Hz, 4,000 Hz and threshold levels at 4,000 Hz were calculated for 112 students who were remained after exclusion of cases with history of ear disease, of ototoxic drug administration, and of neuropsychiatric disease, and mean of those were compared between group of students who have completed military duty (completed group) and group of those who have not (not-completed group), and between group exposed (exposed group) and group unexposed to gunshot sound (unexposed group). Mean of average hearing threshold level and mean of threshold levels at 4,000 Hz of completed group and those of exposed group were higher than those of not-completed group and unexposed group, respectively. Proportion of cases that average threshold level was greater than 40 dB or threshold levels at 4,000 Hz was greater than 50 dB were higher also in completed group and exposed group than in duty not-completed group and unexposed group, respectively Multiple linear regression analysis including age, duration of military service, degree of gunshot sound exposure as independant variables and average hearing threshold level as dependant variable, was performed in order to estimate the effect of age on hearing, and any considerable effect of age on hearing could not be found. In conclusion, hearing impairment can be induced by rifle fire or cannonade training.

Study on the awareness of school doctor of Korean medicine in Middle School (중학생, 교사, 학부모 대상 한의사 교의 사업 인식도 조사)

  • Objectives : The objective of this study was to establish the direction of school doctor of Korean Medicine. Methods : In 2017, 630 middle school students, 212 teachers, and 294 parents participated in the survey. Frequency analysis and Chi square were used for statistical analysis. Results : The students' perception was relatively high for school physicians, but parents and teachers were not as high as 71.6% and 72.3%, respectively. The students had the most education experience of 'drug abuse, smoking, alcohol prevention'. All students, teachers, and parents were selected as priority for "daily life and health" in the field of health education. 85.6% of students, 95.3% of parents, and 89.4% of teachers answered "helpful" and "very helpful" as health education by Korean medicine doctor. The students', teachers', and parents' wishes for health education by school doctor of Korean Medicine were 'good way to grow in health'. Conclusions : Middle school students, teachers, and parents are all interested in school doctor of Korean Medicine. They should find out what they want and need and reflect on the health education program of middle school students.

The Effect to the Hepatic Metabolic Enzume that Uses Human Liver Microsome Caused by a Bariety of Yigiyak (Human Liver Microsome을 이용한 수종 이기약의 간대사효소에 미치는 영향)

  • In this study, we experimented the influence of three herbal medicines, which are Saussurea lappa Clarke, Poncirus trifoliata Rafin, Citrus aurantium Linne, which are called 'Yigiyak(理氣藥)' on drug metabolizing enzyme cytochrome P450 3A4 in Human Liver Microsome. Above all, the reason for this study is that herbal medicines can be assumed that herbs might have interactions with drugs, other herbs, alcohol and chemicals whether those are much better synergy effects than expected effects when the medicine was treated alone or not. As a result, we showed that all of five traditional herbal medicines had no CYP 3A4 inhibition effect on 10, 20, 30, 40, $50{\mu}g/m{\ell}$ doses in Human Liver Microsome even Saussurea lappa Clarke showed a little inhibition as about 93% and 79% inhibition rate of control. However, this result are mostly not enough to prove that SLC has a CYP 3A4 inhibition effect. Moreover, it is not that those rates showed that those herbal medicines have CYP 3A4 induction effect. In conclusion, the result could support that those herbal medicines are more safe than chemical drugs even if this is the basic step to prove that result. Therefore, more specific studies to support this result, which are Kinetic study, cell and animal study then finally until clinical research, are required.
