  • Recently various pressure vessels like an atomic reactor and plant facilities become more larger and are needed to bear in both very high temperature and pressure condition. And in making such a high pressure vessels the amount of annual usage of 2 $\frac {1}{4}$ Cr-1Mo steels which are predominant to resist high temperature high pressure and corrosive circumstances are increasing. But despite of this advantage of 2 $\frac {1}{4}$Cr-1Mo steel. when PWHT(post welding heat treatment) is carried out lots of reheating cracks are occur. In this reason it is strongly needed to study and examine the mechanical behavior of welded joints through welding to PWHT process. So in this study welded nozzle of pressure vessel where reheat cracks are frequently occur are selected for analysis the crack-occurrence mechanism.

A Comparative Study of Methods of Measurement of Peripheral Pulse Waveform

  • Objective: Increased aortic and carotid arterial augmentation index (AI) is associated with the risk of cardiovascular disease. The most widely used approach for determining central arterial AI is by calculating the aortic pressure waveform from radial arterial waveforms using a transfer function. But how the change of waveform by applied pressure and the pattern of the change rely on subject's characteristics has not been recognized. In this study, we use a new method for measuring radial waveform and observe the change of waveform and the deviation of radial AI in the same position by applied pressure. Method: Forty-six non-patient volunteers (31 men and 15 women, age range 21-58 years) were enrolled for this study. Informed consent in a form approved by the institutional review board was obtained in all subjects. Blood pressure was measured on the left upper arm using an oscillometric method, radial pressure waves were recorded with the use of an improved automated tonometry device. DMP-3000(DAEYOMEDI Co., Ltd. Ansan, Korea) has robotics mechanism to scan and trace automatically. For each subject, we performed the procedure 5 times for each applied pressure level. We could thus obtain 5 different radial pulse waveforms for the same person's same position at different applied pressures. All these processes were repeated twice for test reproducibility. Result: Aortic AI, peripheral AI and radial AI were higher in women than in men (P<0.01), radial AI strongly correlated with aortic AI, and radial AI was consistently approximately 39% higher than aortic AI. Relationship between representative radial AI of DMP-3000 and peripheral AI of SphygmoCor had strongly correlation. And there were three patterns in change of pulse waveform. Conclusion: In this study, it is revealed the new device was sufficient to measure how radial AI and radial waveform from the same person at the same time change under applied pressure and it had inverse-proportion to applied pressure.

The Development of a Cuff for the Accuracy Enhancement of the Sphygmomanometer

  • The purpose of this study is to develop a new cuff to improve the accuracy of blood pressure measurement, and to evaluate the performance of the developed system. We added a small bladder to the normal cuff, which we refer to as the double bladder system. The system that we developed for blood pressure measurement was based on the oscillometric method using a double bladder. This system was developed in order to reduce the oscillation noise and to amplify the signal of pure blood pressure. An oscillometric signal database based on the developed system was evaluated according to the ANSI/AAMI/SP10-1992 standard. The correlation coefficients between the cuff of the double bladder and the normal cuff were 0.98 for systolic pressure and 0.94 for diastolic pressure. The mean differences and the standard deviations between the average blood pressure obtained from a mercury manometer and that obtained from an automated sphygmomanometer were -0.7mmHg and 4.9mmHg for systolic, and -1.4mmHg and 5.4mmHg for diastolic pressure. We conclude that the proposed double bladder-based cuff system improves the accuracy of oscillometric blood pressure measurement. The developed system reduces the range of error by about $44{\sim}62%$ for systolic pressure and about $6{\sim}21%$ for diastolic pressure compared to the most recently developed, commercially available sphygmomanometers.

  • An experimental and computational study has been performed for investigation of the jet interaction in supersonic flow with a ramp located behind a sonic, lateral jet. The experimental techniques include schlieren, pressure taps, and Pressure Sensitive Paint. The numerical solver used in this study is AeroSoft's structured flow solver GASP Version 4.0. A Mach 4 crossflow with a pressure ratio of 532, and the 3D ramp was designed by parametric study using GASP. The results showed that the ramp located downstream of the jet decrese the nose-down pitching moment by 70% without a force loss.

The Discriminant Analysis of Blood Pressure - Including the Risk Factors - (혈압 판별 분석 -위험요인을 중심으로-)

  • The purpose of this study was to evaluate the usefulness of variables which were known to be related to blood pressure for discriminating between hypertensive and normotensive groups. Variables were obesity, serum lipids, life style-related variables such as smoking, alcohol, exercise, and stress, and demographic variables such as age, economical status, and education. The data were collected from 400 male clients who visited one university hospital located in Incheon, Republic of Korea, from May 1996 to December 1996 for a regular physical examination. Variables which showed significance for discriminating systolic blood pressure in this study were age, serum lipids, education, HDL, exercise, total cholesterol, body fat percent, alcohol, stress, and smoking(in order of significance). By using the combination of these variables, the possibility of proper prediction for a high-systolic pressure group was 2%, predicting a normal-systolic pressure group was 70.3%, and total Hit Ratio was 70%. Variables which showed significance for discriminating diastolic blood pressure were exercise, triglyceride, alcohol, smoking, economical status, age, and BMI (in order of significance). By using the combination of these variables, the possibility of proper prediction for a high-diastolic pressure group was 71.2%, predicting a normal-diastolic pressure group was 71.3%, and total Hit Ratio was 71.3%. Multiple regression analysis was performed to examine the association of systolic blood pressure with life style-related variables after adjustment for obesity, serum lipids, and demographic variables. First, the effect of demographic variable alone on the systolic blood pressure was statistically significant (p=.000) and adjusted $R^2$was 0.09. Adding the variable obesity on demographic variables resulted in raising adjusted $R^2$to 0.11 (p=.000) : therefore, the contribution rate of obesity on the systolic blood pressure was 2.0%. On the next step, adding the variable serum lipids on the obesity and demographic variables resulted in raising adjusted R2 to 0.12(P=.000) : therefore, the contribution rate of serum lipid on the systolic pressure was 1.0%. Finally, adding life style-related variables on all other variables resulted in raising the adjusted $R^2$to 0.18(p=.000) ; therefore, the contribution rate of life style-related variables on the systolic blood pressure after adjustment for obesity, serum lipids, and demographic variables was 6.0%.   • A hybridized simulator for generating blood pressure waveform is proposed to study the remedy and/or evaluation of the conventional sphygmomanometer utilizing the oscillometric method which is widely applied. The blood pressure of a flowing fluid was controlled for the blood vessel's condition caused by a rhythmical and periodical contraction/relaxation because of the special excitatory and conductive system of the heart. In this study, a hybridized controller composed of the PI feedback controller and the feedforward controller. The inverse dynamics function is proposed to operating the control valve while the pressure is applied in an oil pressure tank. The proposed hybrid simulator reproducing the blood pressure waveform in an artificial blood vessel has kept the control performance consistent over all range. Based on these results, the proposed simulators could be applied to the development and compensation of the non invasive sphygmomanometer type as well as to study the characteristics of the blood pressure and blood vessel.

Hemodynamic effects of induced overtransfusion of blood in dogs (과량수혈이 혈역학에 미치는 영향)

  • Appreciation of the large volume deficits which may occur in surgical or trauma patients due to blood loss has led to vigorous transfusion techniques designed to overt hypovolemic shock and ischemic damage to vital organs which may develop in minutes during the hypovolemic state. In a significant proportion of patients treated with massive rapid blood or fluid transfusion, hypervolemia occurs and life threatening pulmonary edema may develop. Especially, hypervolemia may occur during transfusion for preventing development of the so-called low output syndrome following cardiac surgery. However, the most effective indicator which reveals the adequate level of transfusion is not settled yet. The present study was aimed to compare the effectiveness of the indicators suggested thus far and to determine the most sensitive one. Eight dogs were experimentally studied in terms of left atrial pressure, pulmonary arterial systolic pressure, central venous pressure, mean systemic arterial pressure and heart rate before and after induced hypervolemia with infusion of 600ml heparinized homologous blood. Immediately after induced overtransfusion of the blood, pulmonary arterial systolic pressure increased 75.0%, in omparison with the control before transfusion, left atrial pressure 58.8%, central venous pressure 44.6%, and mean systemic arterial pressure 10.1%, one hour after transfusion, pulmonary arterial systolic pressure 40.0%, left atrial pressure 21.2%, central venous pressure 14.5%, and mean systemic arterial pressure 3.2%, central venous pressure 14.5%, and mean systemic arterial pressure 3.2%, respectively. Heart rate showed no significant change throughout the experiment. These result suggested that the changes of the pulmonary arterial systolic pressure is the most sensitive indicator for detection of hypervolemia during blood transfusion.

A Numerical Study on Pressure Fluctuation and Air Exchange Volume of Door Opening and Closing Speeds in Negative Pressure Isolation Room (음압격리병실에서의 병실 문의 개폐속도에 따른 실간 압력변동 및 공기교환량에 대한 해석적 연구)

  • Purpose: In this study, through the comparison of the pressure fluctuation and air exchange volume in negative isolation room according to the type of the door and door opening/closing speeds, which is one of the main factors causing the cross contamination of the negative pressure isolation room, establishes standard operating procedures to prevent cross contamination in high risk infectious diseases and isolation room design. Methods: In this study, the air flow each of the room is analyzed using ANASYS CFX CODE for flow analysis. In addition, the grid configuration of the door is constructed by applying Immersed Solid Methods. Results: The pressure fluctuation due to the opening and closing of the hinged door was very large when the moment of the hinged door opened and closed. Especially, at the moment when the door is closed, a pressure reversal phenomenon occurs in which the pressure in the isolation room is larger than the pressure in the anteroom. On the other hand, the pressure fluctuation due to the opening and closing of the sliding door appeared only when the door was closed, but the pressure reversal phenomenon not occurred at the moment when the sliding door was closed, unlike the hinged door. As the opening and closing speed of the hinged door increases, the air exchange volume is increased. However, as the opening and closing speed of the sliding door is decreased, the air exchange volume is increased. Implications: According to the results of this study, it can be concluded that the pressure fluctuation due to the opening and closing of the hinged door is greater than the pressure fluctuation due to the opening and closing of the sliding door. In addition, it can be confirmed that the pressure reversal phenomenon, which may cause to reduce the containment effect in negative pressure isolation room, is caused by the closing of the hinged door. Therefore, it is recommended to install a sliding door to maintain a stable differential pressure in the negative isolation room. Also, as the opening and closing speed of the hinged door is slower and the opening and closing speed of the sliding door is faster, the possibility of cross contamination of the room can be reduced. It is therefore necessary to establish standard operating procedures for negative isolation room for door opening and closing speeds.

A Study of the Correlation between Plantar Pressure and Obses Index in obses women. (비만인의 족저부 압력 분포 차이와 비만지표와의 상관성 연구)

  • Objectives : The limited number of studies to date have mainly focused on the effect of obesity on the characteristics of plantar foot pressures. This study is designed to find the correlation between plantar pressure and obese index. Methods : This study assessed the body composition of 30 obese women using bioelectrical impidence analysis and Gaitview AFA-50. The static and dynamic plantar pressure was determined from electronic footprints captured using a capacitive pressure distribution platform during standing and walking. The data were analysed by independent t-test and Pearson Correlation. Results : Positive correlations were noted between body weight, body mass index(BMI), waist circumference(WC), waist-hip ratio(WHR) and difference of fore and rear plantar pressure. And negative correlations were noted between body weight, BMI, WC, WHR and difference of left and right plantar pressure. Conclusions : The findings of this pilot study suggest that body composition influences the waight distribution in overweight and obese subjects.

A Phenomenological Study on Illness Experience of Patients with Pressure Ulcer (욕창 환자들의 질병 체험에 관한 현상학적 연구)

  • Purpose: The purpose of this study was to describe the illness experience of patients with pressure ulcer. Methods: A phenomenological methodology was used for the study. The data were collected by individual in-depth interview with seven participants with pressure ulcer during 2013~2014. All interviews were audio-taped and verbatim transcripts were made for the analysis. The data were analyzed using Colaizzi's phenomenological method. Results: All participants had underlying disease, such as spinal paralysis and diabetes. Average period of having pressure ulcer was 18 months, ranged from 3 to 36 months. A total of seven theme clusters were derived from the analysis; unexpected wound, inherent vulnerability to infection, reversal of the treatment policy, unpleasant and strange feeling of wound, sweeping fear and helplessness, socioeconomic burden, and healing through specific actions and reflection. The participants faced various contradictory and paradoxical situations in managing their pressure ulcers as well as underlying diseases in their everyday life. However, they slowly overcome these situations by strictly practicing concrete action-oriented strategies that they have learned through suffering and appreciating miraculous wound healing. Conclusion: The results of this study can help developing a patient-specific intervention program with sufficient emotional support by providing insights of the paradoxical illness experience of patients with pressure ulcer.
