A Study on Application of Hwangnyeonhaedok-tang Blended Prescriptions in Dongeuibogam (동의보감(東醫寶鑑) 중(中) 황연해독탕(黃連解毒湯)의 활용(活用)에 대한 연구(硏究))

  • The results of documentary survey of 81 prescriptions of Hwangnyeonhaedok-tang and Hwangnyeonhaedok-tang blended ones introduced in Dongeuibogam in which there are 42 of prescriptions using Hwangnyeonhaedok-tang as main constituent and 39 of prescriptions using Hwangnyeonhaedok-tang as assistant constituent can be summarized as followings, 1. Hwangnyeonhaedok-tang blended prescriptions using Hwangnyeonhaedok-tang as main constituent are applied to 20 categories of disease such as 7 of external disease from the chapters of various dermatos is, various injury, tumor(16.6%), 5 of fire disease from the chapter of fire(12%), 5 of eye diseases from the chapter of eye(12%), 4 of disease originated from damage by coldness from the chapter of coldness(9%), etc. 2, It has been found that mainly considered causative agents in diseases, when making up Hwangnyeonhaedok-tang blended prescriptions using Hwangnyeonhaedok-tang as main constituent, are feverish toxin in internal organs, followed by exogenous damp and fever, feverish toxin from damage by coldness and more such as extravasated blood, damp toxin, lack of ki and blood, lack of yin, congestion, the seven passions, blood overflow and excess liquor and sex.

Review of the converted weights of Shanghanlun prescriptions in 『Herbal Formula Science in Korean Medicine』 (『한의방제학』의 『상한론』 환산 용량 검토)

  • Purpose : In 『Herbal Formula Science in Korean Medicine (HFSKM)』, the weight unit of Shanghanlun '1 Liǎng (一兩)' is mostly converted to 3 g. We tried to confirm whether the usages of the Shanghanlun prescriptions of HFSKM are valid in the view of safety and efficacy. Method : The toxicity of herbs in Shanghanlun were summarized. The prescriptions of HFSKM including herbs recorded by weight in Shanghanlun were compared with the pharmacopoeia usage. Domestic clinical case studies were analyzed to confirm the clinical evidence for safety and efficacy. Results : Glycyrrhizae Radix, Rhei Radix et Rhizoma, Persicae Semen, Ephedrae Herba, Pinelliae Tuber, Aconiti Lateralis Radix Preparata, Asiasari Radix et Rhizoma, Armeniacae Semen and Scutellariae Radix have been reported to have toxicity. As a result of analyzing the 39 Shanghanlun prescriptions of HFSKM, 26 prescriptions were identified when they were within the pharmacopoeia usage, and 13 prescriptions were found in excess. As a result of analyzing the domestic clinical research papers, symptoms considered to be side effects were not mentioned, and improvement of the diseases was confirmed. As a result of measuring the actual weight, it was confirmed that the usage greatly exceeded the pharmacopoeia usage, such as 57.23 g for Pinelliae Tuber and 45.77 g for Armeniacae Semen. Conclusion : The composition of the prescription for Shanghanlun proposed in HFSKM is considered to be a reasonable dose, but continuous review and standard supplementation are necessary.

Usage of Korean Herbal Medicine for Climacteric Symptoms - Analysis of Korean Clinical Studies (갱년기 증상에 대한 한약 사용 - 국내 한의 임상연구 분석)

  • Objective : To investigate the usage of Korean herbal medicine (KHM) for climacteric symptoms, this study reviewed clinical studies conducted in Korea. Method : Literature search was performed on three Korean database; OASIS, RISS and NDSL. Studies published before July 2023 were categorized and analyzed according to the study type and herbal prescriptions. Results : Fifty-eight studies including 7 controlled trial, 27 case series and 24 single case reports were included in our review and the total number of case patients was 420. 52 prescriptions based on syndrome identifications and 11 prescriptions of Sasang constitutions were used and the most frequently used prescriptions were Soyosan(逍遙散) and Guibitang(歸脾湯). Thirty-two prescriptions were also mentioned in previous delphi research or clinical practice guideline, and the rest were not. Thirty-five studies used acupuncture combined with KHM and 10 studies used pharmacopuncture and 5 studies used herbal hipbath. Conclusion : To increase the therapeutic effect of KHM and establish KHM as an alternative to hormone treatment, various prescriptions should be used according to syndrome identifications in the clinical field. High-quality RCTs on frequently used prescriptions and the development of various type of KHM are also necessary.

Guideline of Improvement and Evaluation of Prescribing Errors in Colorectal Chemotherapy (대장암 항암 화학요법의 처방 오류 평가 및 개선안 제시)

  • Background: Colorectal cancer shows a significant increase in South Korea due to westernization of diet, lack of dietary fiber, drinking and smoking, irregular defecation. There are surgery, chemotherapy, radiation therapy in treatment of colorectal cancer. There may be a medication errors in the process of chemotherapy because of its high toxicity, narrow therapeutic index and the health status of cancer patients. Consequently medication errors can cause increasing the risk of death, prolonging hospital stay and increasing the cost. Among medication errors on medication use process, prescribing errors are of particular concern due to higher risk of serious consequences. It is important for pharmacist to prevent the prescribing errors before reaching patient. Therefore we analyzed the prescriptions of colorectal cancer, classified prescribing errors, suggested guideline to reduce prescribing errors and verified the importance of pharmacist's role in prevention of medication errors activity. Methods: We collected the numbers of prescriptions of colorectal cancer(n=2,373) through anti cancer management program and EMR and analyzed the errors of prescriptions by categories from Oct 1st 2011 to Sep 30th 2012 at Chungbuk National University Hospital. We reviewed the prescriptions as follows - patients' characteristics, the result of test, previous prescriptions, characteristics of antineoplastic agents and patients' allergy, drug sensitivity, adverse events. Prescriptions are classified into inpatient and outpatient and analyzed the errors of prescriptions by categories (dosage form, dose, input, diluents, regimen, product). Results: Total prescription number of inpatient and outpatient of colorectal cancer was 1,193 and 1,180 and that of errors was 107(9%) and 22(1.9%), respectively. In case of errors of categories, the number of errors of dosage form is 69 and 8, errors of dose is 15 and 5, errors of input is 9 and 9 in inpatient and outpatient prescriptions, respectively. Errors of diluents is 8, errors of regimen is 3, errors of product is 3 in only inpatient prescriptions. In case of errors of categories by inpatient department, the number of errors of dosage form is 34 and 35, errors of dose is 7 and 8, errors of input is 6 and 3, errors of diluents is 4 and 4, errors of regimen is 2 and 1, errors of product is 2 and 1 in SG and HO, respectively. In case of outpatient department, the number of errors of dosage form is 8 in HO, errors of dose is 5 in HO, errors of input is 5 and 4 in SG and HO, respectively. Conclusions: The rate of errors of inpatient is higher than that of outpatient. Junior doctors are engaged in prescriptions of inpatient and pharmacist need to pay attention to review all prescriptions. If prescribing errors are discovered, pharmacist should contact the prescriber and correct the errors without delay. The guideline to reduce prescribing errors might be upgrading software of anti cancer management program, education for physicians as well as pharmacists and calling prescriber's attention to preventing recurrence of errors.

Network Analysis Using the Established Database (K-herb Network) on Herbal Medicines Used in Clinical Research on Heart Failure (심부전의 한약 임상연구에 활용된 한약재에 대한 기구축 DB(K-HERB NETWORK)를 활용한 네트워크 분석)

  • Objectives: Heart failure is a chronic disease with increasing prevalence rates despite advancements in medical technology. Korean medicine utilizes herbal prescriptions to treat heart failure, but little is known about the specific herbal medicines comprising the network of herbal prescriptions for heart failure. This study proposes a novel methodology that can efficiently develop prescriptions and facilitate experimental research on heart failure by utilizing existing databases. Methods: Herbal medicine prescriptions for heart failure were identified through a PubMed search and compiled into a Google Sheet database. NetMiner 4 was used for network analysis, and the individual networks were classified according to the herbal medicine classification system to identify trends. K-HERB NETWORK was utilized to derive related prescriptions. Results: Network analysis of heart failure prescriptions and herbal medicines using NetMiner 4 produced 16 individual networks. Uhwangcheongsim-won (牛黃淸心元), Gamiondam-tang (加味溫膽湯), Bangpungtongseong-san (防風通聖散), and Bunsimgi-eum (分心氣飮) were identified as prescriptions with high similarity in the entire network. A total of 16 individual networks utilized K-HERB NETWORK to present prescriptions that were most similar to existing prescriptions. The results provide 1) an indication of existing prescriptions with potential for use to treat heart failure and 2) a basis for developing new prescriptions for heart failure treatment. Conclusion: The proposed methodology presents an efficient approach to developing new heart failure prescriptions and facilitating experimental research. This study highlights the potential of network pharmacology methodology and its possible applications in other diseases. Further studies on network pharmacology methodology are recommended.

  • The purpose of this study is to find out effects of prescriptions according to the formula, manipulation of Rhei Rhizoma, configuration. The following results were reached through investigations on the prescriptions using Rhei Rhizoma as a main component. Objectives : Analysis of prescriptions According to the formula : Liquid Extract Prescriptions were used widely on three parts to treat fever and damp heat in the interior organs. Powder Prescriptions were taken with hot water, thin porridge, tea etc. to treat damp heat, congestion of phlegm, acute episodes. Liquid Mixed Pill treat congestion of QI, damp heat, phlegm. Honey Mixed Pill treat accumulated fever, distension, acute excessive fever. Paste Pill treat blood stagnation, excessive toxic-fever, epidemic diseases. External Application treat inflammation by injury, swelling due to severe fever by internal damage. Methods : Analysis of prescriptions According to the manipulation of Rhei Rhizoma : Prescriptions including Liquor processed Rhei Rhizoma treat excessive toxic-fever, congestion of phlegm, blocking orifices on the upper side. Steamed Rhei Rhizoma strengthen effects of making evacuate and cooling of heat. Processed Rhei Rhizoma with vinegar strengthen effects of removing blood stagnation by activating blood movement, releasing gathering. Results : Analysis of prescriptions According to the Composition of Rhei Rhizoma : 41% of the total prescriptions were on the area of less than 20%. In case of lower groups show increased frequency of combination with Pharbitidis Semen, Persicae Semen, Scutellariae Radix and manipulation of baking, steaming, roasting. In case of higher groups show increased frequency of treating excess syndrome, critical illness, acute severe illness, and using proccesed Rhei Rhizoma with vinegar. Treatment of damp heat on the liver and gallbladder, disorder of the spleen and stomach is done mostly by prescriptions on the area of less than 30%. Conclusions : Rhei Rhizoma-Coptidis Rhizoma pair treat damp heat, heat toxins in blood, and Constipation caused by excessive heat. Rhei Rhizoma-Glycyrrhizae Radix pair relieve effects of Rhei Rhizoma passing blocked feces, removing the poison, activating blood movement, releasing gathering with the effects of Glycyrrhizae Radix relaxing tension by harmonizing Middle. Rhei Rhizoma-Magnoliae Cortex pair are used to treat damp heat in middle area, excessive heat in the stomach and intestine. Rhei Rhizoma-Pharbitidis Semen pair act on both blood system and QI system treating edema, damp, stagnation, heat toxins, feces. Rhei Rhizoma-Persicae Semen pair treat blood stagnation with fever on blood system.

Applications of Prescriptions Including Anemarrhenae Rhizoma and Phellodendri Cortex in Dongeuibogam (동의보감(東醫寶鑑) 중(中) 지모(知母)와 황백(黃柏)이 배오(配伍)된 방제(方劑)의 활용(活用)에 대한 고찰(考察))

  • Objectives : This study was performed to investigate applications of 122 prescriptions including Anemarrhenae Rhizoma and Phellodendri Cortex in Dongeuibogam. Methods : 122 prescriptions including Anemarrhenae Rhizoma and Phellodendri Cortex which have been used separately or concurrently in Oriental Medicine for a long time as a treatment for various disease in Dongeuibogam were studied through order of frequency, symptoms, dosages, and etc. Results : 1. 19(15.57%) prescriptions are recorded in fatigue chapter, 11(9.02%) in mental or emotional disorder as a result of repressed anger or stress chapter, 9(7.38%) in urine and cough chapters respectively and 8(6.56%) in eyes, ears and glycosuria chapters respectively, which are arranged in order of frequency. 2. Burning stress resulted from Yin deficiency treat herbs are ranked top, Yin deficiency treat herbs, dim eyes, Yin-yang deficiency treat herbs, kidney function deficiency treat herbs are ranked in order of frequency among 78 symptoms in prescriptions including Anemarrhenae Rhizoma and Phellodendri Cortex. 3. The dosages of Anemarrhenae Rhizoma which is more used than Phellodendri Cortex are ranged from 5 puns:4 puns ~ 2 nyangs:1 nyang. 1 jeon:5 puns is recorded 3(30.00%), 1 jeon:7 puns is 2 times(20.00%), the others are 1 (10.00%) used among 10 prescriptions including Anemarrhenae Rhizoma and Phellodendri Cortex. 4. The dosages of Anemarrhenae Rhizoma which is used same amount Phellodendri Cortex are ranged from 3 puns each ~ 4 nyangs each, same amount each or unidentified dosages. Same amount each or unidentified dosages each is recorded 36 times(38.71%), 5 puns each is 15 times(16.85%), 1 nyang each is recorded 12 times(13.48%), 0.7 jeon each is recorded 7 times (7.87%) and 1 jeon each is recorded 6 times(6.74%) used among 89 prescriptions including Anemarrhenae Rhizoma and Phellodendri Cortex. 5. The dosages of Anemarrhenae Rhizoma which is less used than Phellodendri Cortex are ranged from 3 puns:1 jeon ~ 4 nyangs:8 nyangs. 1 nyang:2 nyangs is recorded 4(17.39%), 0.5 jeon:1 jeon is 3(13.40%) each, the others are 1 time(4.35%) used among 23 prescriptions including Anemarrhenae Rhizoma and Phellodendri Cortex. 6. The less using dosage of Anemarrhenae Rhizoma and Phellodendri Cortex has a more percentage of decoctions groups, The more using dosage of Anemarrhenae Rhizoma and Phellodendri Cortex has a more percentage of the other groups except decoctions. The less using prescriptions have an effects of controlling yin and blood deficiency, enhancing Qi, etc, as a whole. The more using prescriptions as a form of compound powder have an effects in incurableness disease, chronic diseases, and etc. Conclusions : The 122 prescriptions including Anemarrhenae Rhizoma and Phellodendri Cortex in Dongeuibogam are mainly composed of Gamree-hwan, Samool-tang, Naebo-hwan, Sagoonja-tang and Gobangpoong-tang, and etc.

The Study about The Discourse on The Medications and Prescriptions on The DongyiSooseBowon Gabobon (" 동의수세보원(東醫壽世保元) 갑오본(甲午本)의 약방(藥方)"에 관한 연구(硏究))

Background and Purpose At the year of 2000, the society of Sasang Constitutional Medicine acquired ${\ulcorner}$HamsanSachon DongyiSooseBowon GaboGubon${\lrcorner}$ which is seen as a written copy of ${\ulcorner}$GaboBon${\lrcorner}$. According to Lee Sung-su who is the great-grandson of DongMu's older brother, ${\ulcorner}$HamsanSachon DongyiSooseBowon GaboGubon${\lrcorner}$ was worked by Lee Jin-yoon who is the grandson of DongMu's older brother, and it was copied by Han Min-gab and now is owned by Lee Sung-su who is son of Lee Jin-yoon. 2. Methods This paper was written in order to understand of the character of the discourse on the medications and prescriptions of the ${\ulcorner}$HamsanSachon DongyiSooseBowon GaboGubon${\lrcorner}$. 3. Result and Conclusion 1) Newly created prescriptions of ${\ulcorner}$GaboBon${\lrcorner}$ is composed of the 125 medications of Soeumin 46, Soyangin 36, Taeumin 29, Taeyangin 14. Regarding with that prescriptions, the average number of the medications per prescription is 9.3 and the average capacity of that is 42.6gram. 2) The experimental prescription is from ${\ulcorner}$GaboBon${\lrcorner}$. All the 37 prescriptions of ${\ulcorner}$Sanghanlon${\lrcorner}$ is found at the text of ${\ulcorner}$GaboBon${\lrcorner}$, and only 12 of 37 prescriptions of Traditional Medicine excepting ${\ulcorner}$Sanghanlon${\lrcorner}$ is described at the text of it. Hardly any case of treatment by Traditional Prescription can be found at the discourse on constitunal symptoms and diseases. 3) Only 13 of 69 newly created prescriptions is not prescribed at ${\ulcorner}$GaboBon${\lrcorner}$, and 7 of the 13 prescriptions don't have the symptom of charge also. 4) 6 prescriptions which are supposed to newly created prescriptions of ${\ulcorner}$The Discourse on Constitutional Symptoms and Diseases${\lrcorner}$ are not describe to the composition of medications. 5) All the newly created prescriptions of ${\ulcorner}$The General Remarks on the Soeumin${\lrcorner}$ of ${\ulcorner}$SinChucBon${\lrcorner}$ are to be regarded as GaboBon's.

  • This report describes 94 prescriptions related to the use of Radix Angelicae Dahuricae main bl ended from Donguibogam. The following conclusions were reached through investigations on the prescriptions that use Radix Angelicae Dahuricae as a key component. 11.7% of wind. 10.6% of carbuncle and deep-rooted carbuncle. recorded the largest number of clinical frequency of the prescriptions in therapeutic use when Radix Angelicae Dahuricae was ta ken as a monarch drug in prescriptions, Prescriptions that utilize Radix Angelicae Dahuricae as t he main component are used in the treatment of apoplexy and carbuncle and deep-rooted carbuncle. headache. wound and they are also used for treating 26 different types of diseases. The prescriptions are compounded with Radix Angelicae Dahuricae as a monarch drug can ap ply to apoplexy, exogenous febrile disease, invasion by wind. wind-cold pathogen, wind-heat path ogen. epidemic disease, pestilence. bruise, bites, deficiency of liver and kidney, deficiency, phlegm-fire, phlegm-heat. The dosage of Radix Angelicae Dahuricae is 0.37g to 7.5g, however 3.75g has be en taken the most for clinical application. The function of Radix Angelicae Dahuricae is to expelling Wind and relieving pain, to expelling wound and forming muscle. to astrict and neutralizing poison. to expelling wind and to getting through body hole. to emit and relieving pain from the combination of drugs and prescriptions.

A Study About the Formative Process of So Yang-In(少陽人)'s prescriptions (소양인(少陽人) 신정처방(新定處方)의 형성과정(形成課程)에 대(對)한 고찰(考察))

  • After studying about the formative process and theory concerning So-Yang In(少陽人)'s prescriptions, Some principles were founded. Those were summarized as follows. 1. Lee Jae Ma(李濟馬) established the theory and remedy about the So Yang In's diseases, and the formation of So Yang In's prescriptions was based on those theories and remedies 2. As the emprical prescription of So Yang In's diseases, ten pre-scriptions of Discussion of Cold-Induced Disorder(傷寒論) and nine of Won Myung dynasty period(元明時代) were described in Dong Eui Su-Se Bo Won(東醫壽世保元), and I could confirm that the formation of So Yang In's presciptions was based on those emprical prescriptions. 3. The pathological characteristics of emprical prescriptions of So-Yang In's diseases in Discussion of Cold-Induced Disorder(傷寒論) were completely corresponded with the principle of So Yang In's diseases, and we could know that emprical presciptions of diseases that were the base of So Yang In's prescriptions were selected by the principle of So Yang In's diseases. 4. In the serious cases of So Yang In's diseases, So Yang In's pre-scriptions were mostly made up of tonifying and nourishing yin medicine(補陰藥物) that dirrectly rufill the damaged Eum Chung Zi Ki(陰淸之氣). According to the above results, it could be concluded that Lee Jae-Ma(李濟馬) made So Yang In's prescriptions with emprical pre scri-ptions that were based on the theory of So Yang In's diseases and concluded that So Yang In's prescriptions were made up of tonifying and nourishing yin medicine(補陰藥物) in the serious cases of diseases.
