Left Ventricular-Right Atrial Canal Three Cases Treated Surgically (좌심실-우심방 단락 치험 3예)

  • Left Ventricular-Right atrial canal, which permits a communication between the left ventricle and right atrium has been reported with increasing frequency. Kirby made the first successful surgical correction in 1957. Recently we experienced 3 cases of left ventricular-Right atrial canal, which were corrected surgically. In one patient the diagnosis of left ventricular-Right atrial canal had been suggested before operation. Preoperative diagnosis had been incorrect in 2 cases; one as an atrial septal defect and the other as combined atrial septal defect and ventricular septal defect. The entire patients were discharged with good results within 2 weeks after operation.

  • These two pulmonary actinomycosis cases had undergone right upper lobectomy and left lower lobectomy individually under the diagnosis of lung cancer and lung abscess. Recently pulmonary actinomycosis is very rare by the widespread use of antibiotics. In addition to clinical similarity to lung cancer and pulmonary tuberculosis, lack of suspicion and difficulty in obtaining adequate culture material also hamper the precise preoperative diagnosis. The purpose of this report is to review of our experiences and to enhance consideration of pulmonary actinomycosis.

Intrapulmonary and Endobronchial Hamartoma (폐실질내 및 기관지내 과오종)

  • The hamartoma is the commonest benign tumor of the lung and proved incidentally as asymptomatic coin lesion on routine chest radiologic examination, but has very low incidence, especially in endobronchial origin. The authors experienced a case of coincidental with intrapulmonary and endobronchial hamartoma. The patient, a 60-year-old man, a farmer, was admitted due to coughing and fever. Preoperative diagnosis was achieved by flexible bronchoscopic biopsy and managed by right middle lobectomy. Three lobulated masses were palpable in the right middle lobe. He was discharged on 15th postoperative day, without problem.

Clinical Evaluation of Esophageal Leiomyoma (식도 평활근종의 임상적 고찰)

  • The five patients with leiomyoma of esophagus were treated from 1976, to 1990. The patients were 4 men and 1 woman whose ages ranged from 28 to 53 years. One of them was asymptomatic, two had mainly dysphagia and the others complained indigestion and epigastric discomfort. There was no relationship between the severity of symptoms and the size of tumor. The preoperative diagnosis was made by esophagoscopy and esophagogram, and all of them were treated by thoracotomy and enucleation. There was no postoperative complication and the results were excellent.

Immature Teratoma at Anterior Mediatinum - Report of one case - (종격동에 발생한 미성숙 기형종: 1례 보고)

  • We experienced one case of immature teratoma at anterior mediastinum. the patient was Syears old female whose complain were cough and chest pain. Chest film showed mass density St the lower half of the Rt chest. Chest CT showed inhomogenous mass which have some calcified area. At the time of operation, 12x10x13cm sized mass have smooth surface and its stump elongated to the thymus. a-FP level, preoperative 22.5ng/ml, was decreased to 9.7ng/ml after operation. Postoperative adjuvant chemotherapy was performed with Vinblastin, Bleomycin and Cisplatin combination. The patient had an uneventful postoperative or postchemotherapy course and was discharged in a good condition.

A Case of Thymolipoma in a Child (소아에서 발생한 흉선지방종 1 예)

  • Thymolipoma is a rare benign mediastinal tumor, composed of mature fatty and thymic tissues. A 9-year-old boy was referred with a one-month history of neck swelling. Preoperative computed tomography scan and fine needle aspiration biopsy suggested thymolipoma. Despite it being rare, thymolipoma should be considered in the differential diagnosis of mediastinal tumors. Characteristics of its clinical feature and radiological findings that can differentiated it from other mediastinal tumors are discussed with a review of the literatures.

Evaluation of Pulmonary Function after Pneumonectomy (일측폐 적출술후의 폐기능의 평가)

  • Studies of pulmonary function using spirometry were performed before and after pneumonectomy for inflammatory lung diseases from 1985 to 1990 at the Pusan Paik Hospital, Inje Medical College. Fifty-two patients were evaluated ; 33 tuberculosis, 17 bronchiectasis, 2 abscess, and 1 actinomycosis. All patients had preoperative and postoperative FVC, FVC[% predicted], FEV1, %FEV1, MVV and MVV[%predicted] determinations. And above datas were compared each other statistically with applying of the paired t-test. The results were obtained as follows : there were significant decreased after surgery in the values of FVC, FVC[% predicted], MVV, and MVV[% predicted], but the values of FEV1, and %FEV1 were no significant changes after surgery.
