파킨슨병 조기 진단을 위한 안드로이드 기반의 앱 개발 (Android Smart Phone App for early diagnosis of Parkinson's Disease)

  • 파킨슨병은 3대 노인성 질환 중 하나로 인구 고령화에 따라 전 세계적으로 발병률이 증가하고 있으며, 질병이 진행될수록 시간적·경제적인 비용이 높아지기 때문에 병의 진행정도를 파악하는 것과 초기에 진단해내는 것이 매우 중요하다. 본 논문에서는 스마트폰을 이용하여 파킨슨병을 조기 진단(Pre-Screening)할 수 있는 방법을 제안하고자 하였다. 개발한 어플리케이션의 콘텐츠들은 탭핑(Tapping), 떨림(Tremor), 나선형 그리기(Spiral Drawing), 보행(Gait)으로 구성되어 있으며, 통합형 파킨슨병 평가척도(Unified Parkinson's Disease Rating Scale)의 Part III(운동성 검사)를 기반으로 하였다. 이를 활용하여 파킨슨병의 운동성 증상으로 알려져 있는 서동증(Bradykinesia), 진전(Tremor), 자세 불안정(Postural Instability)에 대한 정량적인 진단이 가능하며, 증상의 진행 정도에 대한 모니터링이 가능하다. 이러한 방법은 파킨슨병 진단 및 치료에 유용하게 사용될 수 있을 것으로 기대된다.

How Well Do U.S. Primary Care and Obstetrics and Gynecology Clinicians Screen for Pregnancy Complications at Well Woman Visits? A Retrospective Cohort Study

  • Objectives: Pregnancy complications, including pre-eclampsia, gestational diabetes (GDM), and perinatal mood and anxiety disorders (PMADs), impact long-term health. We compared the frequency of screening documentation for pregnancy complications versus a general medical history at well woman visits between providers in primary care and obstetrics and gynecology. Methods: We conducted a retrospective cohort study of subjects with at least 1 prior birth who presented for a well woman visit in 2019-2020. Charts were reviewed for documentation of a general medical history (hypertension, diabetes, and mood disorders) versus screening for comparable obstetric complications (pre-eclampsia, GDM, and PMADs). The results were compared using the McNemar and chi-square tests as appropriate. Results: In total, 472 encounters were identified, and 137 met the inclusion criteria. Across specialties, clinicians were significantly more likely to document general medical conditions than pregnancy complications, including hypertensive disorders (odds ratio [OR], 2.45; 95% confidence interval [CI], 1.18 to 5.48), diabetes (OR, 7.67; 95% CI, 3.27 to 22.0), and mood disorders (OR, 10.5; 95% CI, 3.81 to 40.3). Obstetrics and gynecology providers were more likely to document any pregnancy history (OR, 4.50; 95% CI, 1.24 to 16.27); however, they were not significantly more likely to screen for relevant obstetric complications (OR, 2.49; 95% CI, 0.90 to 6.89). Overall, the rate of pregnancy complication documentation was low in primary care and obstetrics and gynecology clinics (8.8 and 19.0%, respectively). Conclusions: Obstetrics and gynecology providers more frequently documented a pregnancy history than those in primary care; however, the rate was low across specialties, and providers reported screening for clinically relevant complications less frequently than for general medical conditions.

건강행위시행 변화단계에 따른 의사결정의 균형: 운동, 금연, 유방조영술 검진, 질회음근 강화운동을 중심으로 (A study of the Stage of Change and Decisional balance : Exercise Acquisition, Smoking Cessation, Mammography Screening and Kegel's Exercise Acquisition in Korea)

  • This study was carried out to assess the perception of decisional balance of Korean subjects about 4 health behaviors and to identify the influencing factor of decisional balance for exercise acquisition, smoking cessation, mammography screening and Kegel's exercise acquisition. All are representative health behaviors nurses can intervene in Korea based on the Transtheoretical model. Convenient samples of 2,484 subjects (191; exercise, 169; smoking cessation, 1903; mammography screening and 221; Kegel's exercise) were selected from cities and counties over 9 provinces throughout Korea, and the data was collected from January 1, 1999 to February 29, 2000. The research instrument were the Decisional Balance Measure for Exercise (Marcus & Owen., 1992), Smoking Cessation (Velicer et al., 1985), Mammography Screening (Rakowski et al.,1992) and Kegel Exercise (Lim, 1999) and Stage of Change Measure for Exercise (Marcus et al, 1992), Smoking Cessation (DiClemente et al., 1991), Mammography Screening (Rakowski et al.,1992) and Kegel's Exercise (Lim, 1999). The data was analyzed by the SAS Program. The results are as follows; 1. According to the stage of change measure, 2,484 subjects were distributed in each stage of change for four health behaviors: 1,233 subjects (49.8%), 745 subjects (30.2%), 113 subjects (4.7%), 156 subjects (6.5%), and 216 (8.7%) belonged to the pre- contemplation stage, contemplation stage, preparation stage, action stage and maintenance stage. They were all series of stages of change in their efforts to do health behavior. 2. Factor analysis identified 3 factors (1 of Pros, 2 of Cons) for the exercise, 4 factors for smoking cessation (2 of Pros, 2 of Cons), 2 factors (1 of Pros, 1 of Cons) for the mammogram screening and 2 factors (1 of Pros, 1 of Cons) for Kegel's exercise of decisional balance. 3. The analysis of variance and multiple comparison analysis showed that for all 4 samples, the Cons of changing the problem behaviors outweighed the Pros for subjects who were in the pre- contemplation stage, The opposite was true for subjects in action and maintenance stage. 4. Through the discriminant analysis, it was found that one factor of Pros for exercise, one factor of Cons for smoking cessation, 1 factor of Cons for mammogram screening and one factor of Cons for Kegel's exercise were the more influencing factors, than others in discriminating the stages of change. Results are consistent with the applications of the Transtheoretical model, which have been used to understand how people change health behaviors. This results provide some evidence that subject's report of his/her health behavior corresponds to beliefs about usefulness of related health behaviors. The results of this study have implications for patients' health education and health intervention strategies. The findings of this study give useful information for nursing educators for 4 health behaviors, especially the factors relating to decision making in the different stages of change.

통합적 접근이 필요한 지구과학 개념에 대한 예비 교사의 이해: 현상론적 초안과 국면 분석을 통한 식 현상에 대한 정신모형 탐색 (Pre-Service Teachers' Understandings on Earth Science Concept needed for an Integrated Approach: Exploring Mental Models about Eclipse Phenomena by Analyzing Phenomenological Primitives and Facets)

  • 본 연구에서는 통합교과적 접근이 필요한 지구과학 개념에 대한 예비 교사들의 이해를 알아보기 위하여 식 현상에 대한 예비 교사들의 정신 모형을 탐색하였다. 본 연구에는 30명의 중등 예비 지구과학 교사와 36명의 초등 예비교사들이 참여하였다. 정신 모형의 분석을 위해서 서로 다른 맥락의 질문지에 대한 예비 교사들의 응답 및 질문지를 토대로 한 심층 면담 과정에서 나타난 현상론적 초안과 전략적 국면을 분석하였다. 연구 결과, 식 현상에 대해 예비 교사들은 4개의 서로 다른 수준의 정신 모형을 가진 것으로 분석되었다: (1) 차폐 모형, (2) 공전궤도면 모형, (3) 혼성모형, 그리고 (4) 그림자 모형, 차폐 모형은 결함이 있는 모형이며, 공전궤도면 모형은 불완전한 모형, 그림자 모형은 과학적으로 옳은 모형이다. 혼성 모형은 두 개 이상의 정신 모형을 동시에 사용한다. 중등 예비 교사의 경우는 공전궤도면 모형이 40%로 가장 우세한 반면, 초등 예비 교사의 경우는 차폐 모형이 67%로 가장 많았다. 국면의 수준이 정신모형의 수준을 결정하며, 국면의 수준은 현상론적 초안에 의해 결정되는 것으로 분석되었다. 이 연구를 통해 정신 모형의 상황 의존성을 확인하였으며, 식 현상에 대한 낮은 수준의 정신 모형들이 개념적 모형으로 바뀌기 위해서는 광학과 기하학이 통합된 새로운 접근이 필요함을 알 수 있었다. 또한 정신 모형의 규명을 통한 예비 과학 교사의 주제 특정적 교과교육학 지식의 향상 가능성을 시사하였다.

The Association of Pulse Pressure and Pre-Metabolic Syndrome in Korean Middle Aged Men

  • Pulse pressure is an independent risk factor for cardiovascular disease. The aim of the present study was to assess the association between pulse pressure and metabolic syndrome in Korean men. The study subjects were 8,439 adults aged 40 to 64 years, who underwent health screening examination from January 2012 to December 2014 at the Health Promotion Center of one hospital in Gyeonggi-do for general health check-up. They include the metabolic syndrome absent group (Absent, n=3,078), the pre-metabolic syndrome group (Pre-MetS, n=4,242) and the metabolic syndrome group (MetS, n=1,119). Progressive increase in pulse pressure was demonstrated for increasing components of the metabolic syndrome (P<0.001). The pulse pressure according to the degree of the metabolic syndrome was higher in the pre-MetS and MetS groups compared to the Absent group (P<0.001). Systolic and diastolic blood pressure, total cholesterol, triglyceride, fasting glucose, and abdominal obesity positively correlated with pulse pressure (P<0.001).

  • Cancer, like any disease, is a pathologic biological process. Drugs are designed to interfere with the pathologic process and should therefore also be validated using a functional screening method directed at these processes. Screening for cancers at an appropriate time and also evaluating results is also very important. Volumetric measurement helps in better screening and evaluation of tumors. Volumetry is a process of quantification of the tumors by identification (pre-cancerous or target lesion) and measurement. Volumetric image analysis allows an accurate, precise, sensitive, and medically valuable assessment of tumor response. It also helps in identifying possible outcomes such disease progression (PD) or complete response as per Response Evaluation Criteria in Solid Tumors (RECIST).

Effect of Screening on the Risk Estimates of Socio Demographic Factors on Cervical Cancer - A Large Cohort Study from Rural India

  • Background: Prospective cohort studies to determine cofactors with oncogenic HPV-infections for cervical cancer are very rare from developing countries and such data are limited to the few screening trials. Large screening trials provide such data as a by product. Some of the cases are prevented by screening and do not surface as invasive cancers at all. Also, pre-invasive lesions are detected almost entirely by screening. Screening causes selection bias if attendance in or effectiveness of screening is correlated with the risk factors. The aim of this study was to quantify the influence of screening on risk factors for cervical cancer. Materials and Methods: Our material stems from a rural cohort of 80,000 women subjected to a randomised screening trial. The effect of screening on the incidence of cervix cancer was estimated with reference to socio-demographic and reproductive risk factors of cervical cancer. We compared these risks with the incidence of cancer in the randomised control population by the same determinants of risk. Results: The results in the screening arm compared to the control arm showed that the women of low SES and young age were benefitting more than those of high SES and old age. The relative risk by age (30-39 vs 50-59) was 0.33 in the control arm and 0.24 in the screening arm. The relative risk by education (not educated vs educated) was 2.8 in the control arm and 1.8 in the screening arm. The previously married women did not benefit (incidence 113 and 115 per 100,000 women years in control vs screening arms) whereas the effect was substantial in those married (86 vs 54). Conclusions: The results in controls were consistent with the general evidence, but results in attenders and nonattenders of the screening arm showed that screening itself and self-selection in attendance and effectiveness can influence the effect estimates of risk factors. The effect of cervical cancer screening programmes on the estimates of incidence of cervical cancer causes bias in the studies on etiology and, therefore, they should be interpreted with caution.

Influence of Mammographic Screening on Breast Cancer Incidence Trends in South Australia

  • Purpose: To examine breast cancer (BC) incidence trends in relation to mammographic screening and risk factor prevalence in South Australia (SA). Materials and Methods: Trends in annual BC incidence rates were calculated using direct standardisation and compared with projected incidence derived from Poisson regression analysis of pre-screening rates. Annual percentage change and change time points were estimated using Joinpoint software. Biennial mammography screening participation rates were calculated using data from BreastScreen SA. Trends in overweight/obesity, alcohol use and hormone replacement therapy (HRT) use were examined using 1991-2009 Health Omnibus Survey data. Trends in total fertility were examined using data from the Australian Bureau of Statistics. Results: BC incidence increased around the time BreastScreen commenced and then stabilised in the mid-1990s. However rates have remained higher than projected, even though the proportion and age distribution of first time screening attendees stabilised around 1998. A decrease in BC incidence was observed among women aged 50-59yrs from the late-1990's but not among older women. Obesity and alcohol use have increased steadily in all age groups, while HRT use declined sharply from the late-1990s. Conclusions: BC incidence has remained higher than projected since mammography screening began. The sustained elevation is likely to be due to lead time effects, though over-diagnosis cannot be excluded. Declining HRT use has also impacted incidence trends. Implications: Studies using individual level data, which can account for changes in risk factor prevalence and lead time effects, are required to evaluate 'over-diagnosis' due to screening.

  • Inherited metabolic diseases (IMD) comprise a large class of genetic diseases involving disorders of metabolism. The majorities are due to defects of single genes that code for enzymes that facilitate conversion of various substances into others. Because of the multiplicity of conditions, many different diagnostic tests are used for screening of IMD. Molecular genetic diagnosis is the detection of pathogenic mutations in DNA and/or RNA samples and is becoming a much more common practice in medicine today. The purpose of molecular genetic testing in IMD includes diagnostic testing, pre-symptomatic testing, carrier screening, prenatal diagnosis, preimplantation testing, and population screening. However, because of the complexity, difficulty in interpreting the result, and the ethical considerations, an understanding of technical, conceptual, and practical aspects of molecular genetic diagnosis is mandatory.

  • If private discipline replace the strict administrative regulation, fundamental rights of protective essence can be relativized while if may be beneficial for the individual freedom. Assigning the state power to the private organization is equivalent to giving up the practice of individual liberty to the organized authority. It is important to make use of public law mechanism, particularly under the imperfect juristic system. Regarding the prohibition of prior censorship, it is necessary to consider whether it is desirable to prioritize the freedom of speech over the rest of the legal benefit. Apart from this necessity, new advertisement screening mechanism was irreversibly - and controversially - introduced for the health functional food since the unconstitutional verdict of the prior screening of advertisements. With this, the state must secure the broad order through legal regulations and norms in order to freedom of speech and other legal benefit can be harmoniously and practically guaranteed.
