Clostridium perfringens enterotoxicosis in a lion of zoo (동물원 사자의 Clostridium perfringens에 의한 장독혈증 감염증례)

  • A 3-year-old male lion at Gwangju Uchi Zoo presented for acute onset of haemorrhagic diarrhea and died. The lion showed reddening of the anus as the cause of haemorrhagic enteritis. Necropsy revealed a severe haemorrhagic colitis. Grossly, lesions included icterus, excess pericardial fluid. dark kidneys, and an enlarged, friable liver. The intestines were flaccid, thin-walled, dilated, and 9as-filled. The spleen was enlarged and pulpy because of congestion. Most of organs were rapidly postmortem autolysis. Histopathologically, the intestines were edema and transient leukocyte infiltration of the lamina propria, followed by necrosis. Especially of the intestinal submucosa was edematous, haemorrhagic, or filled with leukocytes. The crypts remained intact or dilated. C perfringens was isolated from a lion at bloody feces, and identified C perfringens type A, confirming the presence of C perfringens $\alpha-toxin$ by PCR. These results were suggested that the case were diagnosed as enterotoxicosis in the lion. More studies are needed on lion enterotoxemia. especially of its etiopathogenesis, in order to develop more efficient prevention for this disease.

Clinical Experience with Esophageal Atresia Combined with Duodenal Atresia (십이지장 폐쇄를 동반한 선천성 식도 폐쇄에 대한 고찰)

  • There is significant morbidity and mortality associated with the combination of esophageal atresia (EA) and duodenal atresia (DA). Nevertheless, the management protocol for the combined anomalies is not well defined. The aim of this study is to review our experience with the combined anomalies of EA and DA. From May 1989 to August 2006, seven neonates were diagnosed as EA with DA at Asan Medical Center. In all cases, the type of EA was proximal EA and distal tracheoesophageal fistula (TEF). The diagnosis of DA was made in theprenatal period in 1, at birth in 4, 4 days after birth in 1 (2 days after EA repair) and at postmortem autopsy in 1. Except the one case where DA was missed initially, primary simultaneous repair was attempted. DA repair with gastrostomy followed by EA repair in 2, EA repair followed by DA repair without gastrostomy in 2, and TEF ligation followed by DA repair with gastrostomy in 1. There were two deaths. One baby had a large posterolateral diaphragmatic hernia, and operative repair was not attempted. The other infant who had a TEF ligation and DA repair with gastrostomy expired from cardiac failure due to a large patent ductus arteriosus. Simultaneous repair of EA and DA appears to be an acceptable management approach for the combined anomalies, but more experience would be required for the selection of the primary repair of both anomalies.

Cervical Stenotic Myelopathy in a Thoroughbred Colt

  • A 14-month-old Thoroughbred colt with hind limbs lameness was referred to Equine Hospital of Jeju National University. During physical examination, the patient could not walk in a straight line but wobbled especially with hind limbs. He hesitated when backing and crossed hind limbs while circling tightly by protracting the outer hind limb keeping the inner hind limb stationed. Stenosis was observed between the 2nd (C2) and 3rd (C3), and the C3 and 4th (C4) cervical vertebrae on radiographs. The colt diagnosed as cervical stenotic myelopathy (CSM) based on the clinical signs and radiological findings. To confirm the diagnosis, postmortem computed tomography (CT) and histopathologic examination were performed after the euthanasia. The CT images revealed severe compression of the spinal cavity and cord between C3 and C4 cervical vertebrae. Grossly, there was compression of the spinal cord between the C3 and C4 cervical vertebrae. Histopathologically, severe axonal swelling and demyelination characterized by vacuolation and cavity formation were observed in the white matter of the spinal cord, especially in C3 and C4. CSM is problematic in the horse racing industry because of abnormal walking. This case report of CSM may offer information for diagnosis of the disease in the equine veterinary fields in the Republic of Korea.

  • Invasive pulmonary aspergillosis is a disease occuring predominantly in patients with defects in immunity such as neutropenia, hematologic malignancies or with defects in cell-mediated immunity. The isolation of Aspergillus from respiratory tract of normal host usually signifies tracheobronchial colonization, making this diagnosis difficult. There are isolated case reports occuring in normal hosts but most of them were diagnosed postmortem at autopsies indicating that early diagnosis of invasive aspergillosis in normal host is difficult. We describe here a case of invasive aspergillosis in a immunocompetent host diagnosed by lung biopsy which was successfully treated with Amphotericin-B. Invasive pulmonary aspergillosis should be included as one of the differential diagnosis if a patient with pneumonic consolidation does not respond to usual therapy, even if the patient does not have any defect in immunity.

  • An 11-year-old intact female Maltese was referred because of 1 week history of cluster seizure episodes. Based on brain CT scan, brain tumor was strongly suspected. The patient was euthanized according to client's request and we performed necropsy after euthanasia. The gross findings of the postmortem coronal sections of the brain showed that the mass was relatively well-demarcated, reddish in colored, and was present inside the left lateral ventricle and compressed adjacent tissues. The tumor mass had 2 distinct histopathological features: perivascular pseudorosette-like structures and a whirl-like arrangement of fibrillary cells. The immunohistochemical profile showed strong GFAP positivity and moderate S-100 expression, sparsely dotted staining with Ki-67. Based on the histopathological and immunohistochemical findings, the present case diagnosed to ependymoma.

A Case of Cryoglobulinemia-induced Acute Respiratory Distress Syndrome (한냉글로불린혈증 환자에서 발생한 급성호흡곤란증후군 1예)

  • Cryoglobulinemia is the presence of globulins in the serum that precipitate on exposure to cold temperatures(cryoglobulins). Pulmonary complications of cryoglobulinemia include interstial infiltration, impaired gas exchange, small airway disease and pleurisy. Only one other acute respiratory distress syndrome(ARDS) case has been described in patients with cryoglobulinemia. A 55-years old man was admitted with dyspnea. He had been diagnosed as being a hepatitis B virus antigen carrier 15 years ago. On the first admission, chest radiography showed a bilateral pleural effusion and a patchy infiltration on both lungs. On protein- and immuno-electrophoresis, cryoglobulinemia was confirmed. The patient was treated with corticosteroid and plasmapheresis. Forty-five days after the diagnosis, the patient complained of progressive dyspnea and showed a diffuse bilateral pulmonary infiltration on chest radiography. Despite intensive care with mechanical ventilation, the patient died as consequence of hypoxemia and multiple systemic organ failure. On a pathologic examination of the postmortem lung biopsy, multiple necrotizing vasculitis and increased infiltration of the lymphocytes and monocytes were observed. In conclusion, ARDS developed as a result of pulmonary hemorrhage due to cryoglobulinemia-associated vasculitis.

Magnetic Resonance Imaging Features of Suspected Acute Spinal Cord Infarction in Two Cats (두 마리 고양이에서 발생한 급성 척수 경색의 자기 공명 영상학적 진단 증례)

  • Spinal cord infarction is becoming recognized as an important cause of acute myelopathy in cats. Although the definitive diagnosis is confirmed through postmortem histopathologic examination, MR imaging features provide valuable informations for the diagnosis of spinal cord infarction. The aim of this report is to describe MR findings of acute spinal cord infarction in two cats and to evaluate usefulness of low field MRI (0.3Tesla) as a potential diagnostic tool of acute spinal cord infarction. A cat (unknown age, neutered male mixed breed cat) was referred one day after the acute onset of non-ambulatory spastic tetraparesis and the other cat (a 9-year-old, neutered female domestic short hair cat) was presented due to the acute onset of non-ambulatory paraparesis and one day later paraplegia. The lesions of the MR images were shown on the spinal cord parenchyma over C2 to C6 in case 1 and L2 to L5 in case 2. The MR images in these two cases were characterized by focal intramedullary lesions, mainly involving grey matter which were hyperintense T2 weighted and FLAIR images and hyperintense on DWI and hypointense on ADC map. The MR findings in both cases were highly suggestive of acute spinal cord infarctions, based upon previous reported small animal cases and human cases. In conclusion, based on MR features, together with the history and clinical examination findings, MRI modality can be used as an antemortem tool for the diagnosis of acute spinal cord infarction in cats.

A Case of Feline Infectious Peritonitis with Intestinal Manifestation in a Cat (고양이에서 장 병변을 가진 고양이 전염성 복막염 감염 증례)

  • A 2-year-old, intact male, Persian exotic cat, weighing 2 kg with a history of a palpated abdominal mass was admitted to Veterinary Medical Teaching Hospital, Seoul National University. On routine complete blood count (CBC) and serum biochemical analyses, there were anemia and 0.45 of albumin to globulin ratio. A feline infectious peritonitis (FIP) virus antibody test kit was negative. Radiography, ultrasonography and computed tomography (CT) were performed. A markedly enlarged abdominal mass was found. On cytologic examination of this mass, it was diagnosed as alimentary lymphoma or pyogranulomatous inflammation. Chemotherapeutic treatment for intestinal lymphoma was provided for several weeks, but the mass size was increased and clinical signs were not improved. The cat died six days after discontinuing chemotherapy. On postmortem examination, the definitive diagnosis was FIP. This case describes a cat with FIP in which an abdominal tumor had been suspected clinically.

Discrimination of cows from heifers by antemortem and postmortem inspection in slaughter house (도축장의 생.해체검사를 통한 경산우 및 미경산우 감별법(I))

  • This study was performed to make helpful standard for discrimination of cows from heifers by uterine and ovarian section inspection. A total of 3,736 heifers and 249 cows were inspected from January to November 2006 in Karak Market Slaughter house. Firstly, it was obvious that non-corpus albicans were observed in almost heifers(3,597/3,736, 96.3%), but there were distinctive corpus albicans in all cows (249/249 100%). Secondly, another evident was that the length and diameter of the horn in uterus was below 30mm in 94.5% (3,267/3,736) heifers, but above 30mm in 90.4% (225/245) cows, and it was easy to distinguish between right and left horn by difference of size in 87.6% (218/249) cows. Thirdly the diameters of pelvic cavity was below 220mm in 96.8% (3,615/3,736) heifers, but was over 200mm in 85.1% (212/249) cows. Fourthly another difference was found in second sacrum union condition and 13th thoracic vertebral process's states. In case of heifers, it was distinctive five lines in each second sacrum, but it was non-distinctive or united in the same each lines of the sacrum in cows. Fifthly in the results of 13th thoracic vertebral colors, the ratio between red color and white color was 7 : 3 in heifers (3,385/3,736, 90.6%), but that was 2 : 8 in cows (199/249, 79.9%). Consequently, it was possible to use the method which discriminated cows from heifers by uterine and ovarian section inspection in large scaled field slaughter house.

Aroma Pattern Analysis of Hanwoo Beef (M. longissimus) using Electronic Nose during Refrigerated Storage (전자 코를 이용한 한우 등심육의 냉장저장 중 향기 패턴 분석)

  • This study was carried out to investigate aroma patterns of Hanwoo (Korean cattle) beef using electronic nose during refrigerated storage, and to compare these results with chemical quality (pH, TBARS). The M. longissimus muscle from Hanwoo carcasses after 24 hrs postmortem was obtained and stored at 5${\pm}$1$^{\circ}C$ for 7 days. Sensitivity (dR/RO) values among electronic nose data were changed differently during refrigerated storage, and showed significant difference on the 7th day of storage (p<0.01). The dR/RO from SY/G, SY/AA, SY/Gh, SY/gCTl, SY/gCT decreased but those from SY/LG, T30/1, P10/1, P10/2, P40/1, T70/2, PA2 increased during storage for 7 days. Mapping these data using PCA (principal component analysis) showed that the 1st day data were present in the middle of the right side, the 3rd day data were present in bottom part of this area and the 7th day data spread out more widely on the left side. In case of DFA (discriminant factor analysis), the flock clustered round and located in different side clearly comparing with PCA plot. In analysis of correlation coefficients among electronic nose data and chemical quality data, there was significant correlation among sensor data (p<0.001). But pH and TBARS were not significantly correlated with electronic nose data. Consequently, PCA and DFA plot by electronic nose data showed difference during refrigerated storage and there were significant correlations among sensors. Therefore it will be possible to detect separate aroma patterns of Hanwoo beef using electronic nose.
