Breast Density and Risk of Breast Cancer in Asian Women: A Meta-analysis of Observational Studies

  • Objectives: The established theory that breast density is an independent predictor of breast cancer risk is based on studies targeting white women in the West. More Asian women than Western women have dense breasts, but the incidence of breast cancer is lower among Asian women. This meta-analysis investigated the association between breast density in mammography and breast cancer risk in Asian women. Methods: PubMed and Scopus were searched, and the final date of publication was set as December 31, 2015. The effect size in each article was calculated using the interval-collapse method. Summary effect sizes (sESs) and 95% confidence intervals (CIs) were calculated by conducting a meta-analysis applying a random effect model. To investigate the dose-response relationship, random effect dose-response meta-regression (RE-DRMR) was conducted. Results: Six analytical epidemiology studies in total were selected, including one cohort study and five case-control studies. A total of 17 datasets were constructed by type of breast density index and menopausal status. In analyzing the subgroups of premenopausal vs. postmenopausal women, the percent density (PD) index was confirmed to be associated with a significantly elevated risk for breast cancer (sES, 2.21; 95% CI, 1.52 to 3.21; $I^2=50.0%$). The RE-DRMR results showed that the risk of breast cancer increased 1.73 times for each 25% increase in PD in postmenopausal women (95% CI, 1.20 to 2.47). Conclusions: In Asian women, breast cancer risk increased with breast density measured using the PD index, regardless of menopausal status. We propose the further development of a breast cancer risk prediction model based on the application of PD in Asian women.

한국 여성에서 혈청 ferritin과 25-hydroxyvitamin D 및 대사 증후군의 관련성 (The association of vitamin D and urine microalbumin/creatinine and obesity in Korean adults)

  • 본 연구는 한국 여성에서 혈청 ferritin과 25-hydroxyvitamin D [25 (OH) D]와 대사 증후군 (MetS)의 관련성을 평가하기 위해 시행되었다. 다섯 번째 국민 건강 영양 조사 (KNHANES V) (2,010- 2,012)에서 ≥ 20세 이상 성인 9,256명(4,196 폐경 전 여성과 폐경 후 여성 4,340)의 데이터를 분석 하였다. 주요 연구 결과는 다음과 같다. 첫째, MetS가 없는 여성의 경우 관련 변수 (연령, 흡연, 음주, 규칙적인 운동, SBP, DBP, WM, TC, TG, HDL-C, FPG, Hb, Hct)를 조정 한 후, 비타민 D 상태는 혈청 ferritin 수치와 관련이 있었다 (폐경 전, p <0.001, 폐경 후, p = 0.027). 둘째, MetS를 가진 여성에서 비타민 D 상태는 혈청 ferritin 수치와 관련이 없었다 (폐경 전, p = 0.739, 폐경 후, p = 0.278). 결론적으로, 비타민 D 상태는 MetS가 없는 여성에서는 혈청 ferritin 수준과 양의 상관 관계가 있었지만 MetS가 있는 여성에서는 그렇지 않았다.

안면홍조에 대한 임상시험 참가자의 증상 발현 양상에 관한 연구 (A Cross Section Study : Hot Flush of Postmenopausal Women Who Participated in a Clinical Trial)

  • Purpose : To identify factors related to the severity and frequency of hot flush of postmenopausal women who participated in a clinical trial. Methods : A cross-section study of 34 postmenopausal women between 40 and 60 years of age was done. VAS(visual analog scale) was used to evaluate the hot flush degree. Statistical analysis was performed with multiple linear regression analysis. Results : Hot flush VAS was proportional to the percent body fat with 1.467 regression coefficient (p-value= 0.010). It implies that the symptom of hot flush becomes worse as the percent body fat increases. Some hormones also have effects on hot flush. We found that LH might cause increment of hot flush VAS(i.e., the corresponding regression coefficient is 0.912). Sasang constitution was not relevant to the hot flush degree. Hot flush VAS seemed to increase as the vascular aging degree by SA-2000 getting worse, but it was not statistically significant (p-value= 0.594). Conclusion : This study is a part of a prospective clinical trial. For more accurate result, larger-scale cross-section study or clinical trial are necessary.

장기간 골다공증관리교육이 골밀도와 약물순응도에 미치는 효과 (The Effects of Long term Osteoporosis Management Education on BMD Level and Medication Compliance in Postmenopausal Women)

  • Purpose: The purpose of this study was to evaluate the effects of long term osteoporosis management education consist of exercise, drug therapy, and nutrition on bone mineral density (BMD) and medication compliance among postmenopausal women who have just been diagnosed with osteoporosis. Methods: The research design consisted of a pre-and post-test quasi-experimental design through a nonequivalent control group. The participants in this study were 60 individuals with osteoporosis, and who were classified according to two groups; the control group (n=30) who were given general clinical guidelines, and the experimental group (n=30) who received management education once for 60 minutes every three months using a standardized educational sheet. After one year, BMD was assessed using band dual energy X-ray absorptiometry (DXA) and medication possession ratio (MPR) for medication compliance. Analysis of collected data was performed using descriptive statistics t-test, chi-test, and paired t-test. Results: No significant difference in BMD (t=-1.02, p=.311) and MPR (t=-0.77, p=.440) was observed between the two groups. However, a significant increase in femur neck BMD was observed in the experimental group compared with the control group (t=-2.18, p=.033). Conclusion: Conduct of further study will be needed for examination of the effect of long-term osteoporosis management education on BMD Level and Medication Compliance in Postmenopausal Women.

대두 이소플라본 보충 섭취가 폐경 후 여성의 골대사 지표와 소변 무기질 배설량에 미치는 영향 (Effect of Soy Isoflavone Supplementation on Bone Metabolism Marker and Urinary Mineral Excretion in Postmenopausal Women)

  • We studied the effects of soy isoflavone supplements on bone metabolism marker (serum osteocalcin, urinary deoxypyridinoline) and urinary mineral excretion (urinary Ca, Mg, Zn) in 47 postmenopausal women. There were 24 participants in the treatment group and 23 in the control group. The treatment group consumed isoflavone extract capsules daily (which contained 90 mg of soy isoflavones) for 12 weeks. The study compared before and after isoflavone intake in the following areas: Physical examination, diet survey, bone metabolism marker and urinary mineral excretion. The average age of the treatment group was 64.6 years and that of the control group was 66.5 years. There were no significant differences between the two groups in terms of height, weight and body mass index. Both groups maintained a regular diet pattern in terms of their average daily nutrient intake. There were no significant differences between the treatment group (23.9 mg) and the control group (25.4 mg) in terms of daily isoflavone intake based on diet. The analysis of bone metabolism marker changes in the treatment group after 12 weeks of taking the isoflavone supplements demonstrated significant differences in the following: Serum osteocalcin (13.7 ng/mL in befor versus 6.8 ng/mL in after) and urinary deoxypyridinoline (5.9 nmol/mmol Cr in befor versus 4.5 nmol/mmol Cr in after). The subjects in the treatment group showed no significant difference in urinary Ca excretion. But the subjects showed a significant difference in urinary Mg (131.9 mg/day in befor versus 115.6 mg/day in after) and Zn (400.5 $\mu\textrm{g}$/day in befor versus 310.2 $\mu\textrm{g}$/day in after) excretion in the isoflavone treatment group at the levels of p<0.001, p<0.01, respectively. No changes were made in the intake of minerals. The composition of serum osteocalcin and urinary deoxypyridinoline, and indicators of bone metabolism, including the excretion Mg and Zn, significantly decreased. Breast Feeding and Breast Cancer Risk: A Case-control Study in Korea

  • We carried out a case-control study to investigate protective effect of lactating against breast cancer in Korea. Cases (n=108) were the newly histologically identified breast cancer between December 1997 and August 1999. Hospital-based controls were selected by frequency matching method with age ($\pm$4 age) and menopausal status from the patients at the same hospital in the plastic surgery, general surgery and opthalmology department. Interviews included information on general characteristics of subjects, disease history, family history of breast cancer, vitamin supplementation, alcohol intake, food intake, and reproductive factors as well as lactation history. Odds ratio (OR) and 95% Confidence Intervals (CI) were calculated by using unconditional logistic regression. Age distribution of case control subjects were similar. Late menarche age $\geq$ 17 in premenopausal women was related to the lower risk of breast cancer OR = 0.41, 95% CI = 0.28-0.91. Family history of breast cancer was related to the higher risk of breast cancer only in premenopausal women (OR = 2.07, 95% CI = 1.35-2.71). Higher body mass index mass index (> 30) were associated with higher risk of breast cancer in postmenopausal women. For premenopausal women, women who had lactated $\geq$ 12 months to the first child had a significantly lower risk (OR = 0.53, 95% CI = 0.24-0.97) than the women had no breast feeding experience. However, results from postmenopausal women did not show an association with decreased breast cancer risk. These findings suggest that lactation may be a protective factor of breast cancer in Korean women.

폐경후 여성들의 골밀도에 따른 영양섭취상태와 생활습관과의 관련성 연구 - 충남일부 지역을 중심으로 - (Association of Bone Mineral Density with Nutrient Intake and Lifestyles of Postmenopausal Women in Chungnam)

  • This study was peformed to assess the relationships among bone mineral density(BMD), nutrient intake and lifestyle factors in postmenopausal women. A trained nutritionist examined the interviewed usual dietary intake and physical activity of 85 subjects in Chungnam with a questionnaire interview, and BMDs of the lumbar spines($L_2-L_4$), femoral necks(FN), ward's triangles(WT) and trochanters(TR) were measured by dual energy X-ray absorptiometry (DEXA). The BMDs(T-score) of $L_2-L_4$, FN, and WT were $0.996g/cm^2(-1.601),\;0.697g/cm^2(-1.697)$, and $0.793g/cm^2(-1.512)$, respectively, which were assessed as osteopenia by T-score,. and TR was normal at $0.718g/cm^2(-0.675)$. Subjects included 85 in Chungnam were divided into three groups according to the BMD measurement of the $L_2-L_4$ and FN assessed by T-score. The percentages of the osteoporosis, osteopenia, and normal groups were $32.9\%,\;42.4\%$ and $24.7\%$ respectively. The average age was significantly the highest of the osteoporosis group than in the other two osteopenia or normal groups(P<0.001). Among lifestyle factors, the BMD of lifestyles was significantly related with exercise but it was not significant with medication., salt. intake, bone fracture, coffee consumption, drinking and smoking. The nutrient intake of the subjects was most nutrient intake adequate to the Korean RDA level for most nutrients excepting energy intakes and calcium. The BMD of lumbar spines $L_2-L_4$ showed significant positive association with the intake of most nutrients except carbohydrate, especially in the normal group. The Higher BMD levels were found for those with high intake of protein and vitamin B1. The BMDs of $L_2-L_4$, and FN were positively correlated with showed energy expenditure, exercise, miscellaneous activity, per weekend and week(P<0.05) in normal group but that was showed negatively correlated in the osteopenia group. Therefore, this study confirmed that one of the most effective ways to minimize bone less in postmenopausal women is to maintain an adequate intake of calcium and other nutrients and regular physical activity.

  • 본 연구는 우리나라 폐경기 여성의 신체활동 정도와 대사증후군과의 관계를 알아보고, 추후 대사증후군 환자의 건강증진을 위한 기초자료를 제공하기 위해 실시하였다. 국민건강영양조사 제4기 2차년도 자료 중 폐경기 여성 1,007명을 대상으로 알아본 결과 우리나라 폐경기 여성의 대사증후군은 연령이 증가할수록, 신체활동 정도가 낮을수록 유의하게 높았다는 결과를 얻었다. 폐경기 여성의 대사증후군을 관리 예방하기 위해서는 연령을 고려하여 환자의 운동 선호도, 운동제약 그리고 환자의 특성을 고려한 중간 신체활동프로그램 개발 및 프로그램 효과검증과 같은 연구가 매우 필요하다고 사료된다.

Relationship Among Body Mass Index, Nutrient Intake and Antioxidant Enzyme Activity of Postmenopausal Women

  • To elucidate the relationship among body mass index, nutrient intake and blood antioxidant capacity in the postmenopausal period, 60 women residing in Iksan area were recruited. Body mass index (BMI) was calculated base on height and weight, and food and nutrient intakes were estimated by 24-hour recalls of 3 non-consecutive days. Parameters of antioxidant capacity including the activities of superoxide dismutase (SOD), glutathione peroxidase (GPx), catalase (CAT) and total antioxidant capacity (TA) were measured in fasting blood samples from the subjects. The average age, height, weight and BMI of the subjects were 65 years, 151.1cm, 59.5 kg and 26.0 m/kg$^2$, respectively. The macronutrient intake rate of carbohydrate : protein : fat were 65:17.5:17.5; the mean intakes of energy and protein were 1532.7 kcal (86.3% of RDA) and 67.1 g (122.0% of RDA) respectively. The mean intakes of phosphorus, vitamin A, niacin and vitamin C were higher than Recommended Daily Allowance (RDA) for Koreans. On the other hand, calcium and riboflavin intakes were only 84.6% and 70.4% of RDA. Among the parameters of antioxidant capacity, SOD activity was significantly lower in lean subjects (BMI<20) than in the normal or overweight subjects (BMI${\geq}$20) (p<0.05). TAs of the subjects with the highest intakes of vegetables and fruits were significantly higher than those of subjects with lower intakes (p<0.05). Antioxidant capacity was compared among subjects according to 3 different nutrient intake levels according percentage of RDA for Koreans for selected nutrients with the following results: The high protein and niacin groups exhibited significantly lower TA status than those of the other intake groups (p<0.05). In conclusion, the low BMI was associated with lower SOD activity in postmenopausal women. Higher consumption of fruits and vegetables was associated with higher TA. When protein and niacin intakes were excessive, SOD activity and TA tended to be low. SOD and TA, among antioxidant indexes, seemed to be mostly influenced by other factors. Therefore, more studies on the effects of nutritional intake and the activity of antioxidant enzyme should be conducted.

Vitamin D Proliferates Vaginal Epithelium through RhoA Expression in Postmenopausal Atrophic Vagina tissue

  • Postmenopausal atrophic vagina (PAV) is the thinning of the walls of the vagina and decreased lugae of the vagina. PAV is caused by decreased estrogen levels in postmenopausal women. However, the harmful effects of hormone replacement therapy (HRT) have resulted in considerable caution in its use. Various estrogen agonist treatment options are available. Vitamin D is influences the regulation of differentiation and proliferation of various cells, especially tissues lining stratified squamous epithelium, such as the vaginal epithelium. In this study, we hypothesized that vitamin D could provide an alternative and a safe treatment option for PAV by promoting the proliferation and differentiation of the vaginal epithelium. Thirty six patients were enrolled in this case-control study. Vitamin D associated proteins in a vitamin D and sex hormone treated vaginal epithelial cell line as well as normal and PAV tissues were measured. To confirm of cell-to-cell junction protein expression, cell line and tissue studies included RT-PCR, immunohistochemistry staining, and immunoblot analyses. The expression of cell-to-cell junction proteins was higher in women with symptoms of atrophic vagina tissue compared to women without the symptoms. Vitamin D stimulated the proliferation of the vaginal epithelium by activating p-RhoA and Erzin through the vitamin D receptor (VDR). The results suggest that vitamin D positively regulates cell-to-cell junction by increasing the VDR/p-RhoA/p-Ezrin pathway. This is the first study to verify the relationship of the expression of RhoA and Ezrin proteins in vaginal tissue of PAV.
