음악을 통한 청각자극이 만성 뇌졸중 환자의 편측무시에 미치는 영향: 단일대상연구 (The Effects of Auditory Stimulus Using Music on Unilateral Neglect in Patients With Stroke: A single-subject design)

  • 목적 : 본 연구의 목적은 음악을 통한 청각자극이 뇌졸중 환자의 편측무시에 미치는 효과를 알아보는 것이다. 연구방법 : 본 연구는 편측무시를 보이는 뇌졸중 환자 1명을 대상으로 하였다. 본 연구는 ABA 설계를 사용하였으며, 연구기간 동안 기초선A 5회, 중재기B 10회, 기초선A'5회로 총 20회기 실시하였다. 각 중재기 동안에는 대상자가 좋아하는 음악 5곡을 선택하게 하여 회기 당 60분씩 음악을 감상하도록 하였다. 기초선과 중재기간에는 편측무시 평가를 위해 별 지우기 검사(Star cancellation test), 선 나누기 검사(Line bisection test)를 시행하였다. 또한 중재 전, 후로 일상생활에서의 편측무시를 평가하기 위해 Catherine Bergego Scale(CBS)와 정서 상태를 평가하기 위해 Positive Affect and Negative Affect Schedule(PANAS)를 시행하였다. 결과 : 중재기 동안 별 지우기 검사, 선 나누기 검사를 통해 편측무시가 감소되는 추세를 확인하였으며, 중재 후에도 중재효과가 유지되었다. 또한 중재 후, CBS를 통해 일상생활에서도 편측무시가 감소되었음을 알 수 있었으며 PANAS를 통해 긍정적 감정이 증가하였음을 알 수 있었다. 결론 : 본 연구 결과, 음악을 통한 청각자극 뇌졸중 환자의 편측무시 감소에 긍정적인 영향을 준 것으로 확인되었다.

리듬청각자극을 이용한 후방 보행 훈련이 뇌졸중 환자의 보행과 균형에 미치는 영향 (The Effects of Backward Walking with Rhythmic Auditory Stimulation on Gait and Balance in Patients with Stroke)

  • 본 연구는 리듬청각자극을 이용한 후방 보행 훈련이 뇌졸중 환자의 보행과 균형에 미치는 영향을 알아보기 위한 것으로 21명의 뇌졸중 환자를 무작위로 세 집단으로 나누었고, 실험군I은 전방 보행 훈련 군, 실험군II은 후방 보행 훈련군, 실험군III은 리듬청각자극을 이용한 후방 보행 훈련 군으로 집단마다 7명씩 실험하였다. 실험은 3주간 주 5회 30분씩 실시하였으며 실험 전과 3주간의 실험 후에 각 실험군의 10m 보행 검사, 일어나 걸어가기 검사, 기능적 팔 뻗기 검사를 하였고, biodex gait trainer 2를 사용하여 활보장과 보장비대칭 비를 측정하였다. 연구의 결과 각 군내의 실험 전 후 비교에서 보행속도, 보행대칭성, 균형에 유의한 차이를 보였고(p<.05), 변화량을 비교했을 때 모두 유의한 차이를 보였으며(p<.05), 보행속도, 보행대칭성과 균형에서는 실험군III, 실험군II, 실험군I 순으로 효과적이였고, 활보장에서는 실험군II와 실험군III이 실험군I보다 효과적이였다. 이 결과를 통하여 뇌졸중 환자를 대상으로 보행 운동을 실시할 때 리듬청각자극을 이용한 후방 보행 훈련이 보행속도, 보행대칭성과 균형의 향상에 있어서 효과적인 방법임을 알 수 있다.

비디오투시조영검사를 통한 침습/흡인에 따른 음성의 음향적 분석 (Acoustic Voice Analysis in Patients with Penetration/Aspiration Via Videofluoroscopic Swallowing Study)

  • Background and Objectives The present study aimed to investigate the effects of penetration/aspiration (P/A) on voice acoustic parameters. Subjects and Method Twenty-seven patients were analyzed with the videofluoroscopic swallowing study (VFSS) and then divided into two groups based on the modified Penetration and Aspiration Scale results. Ten patients (5 males and 5 females) were included in the Non-P/A group, and 17 patients (12 males and 5 females) in the P/A group. Stroke was the major cause of swallowing disorders. Three sustained /a/ vowels recorded in pre- and post-VFSS were analyzed. Mann-Whitney U-test was used to compare acoustic values before and after VFSS, and the receiver operating characteristics (ROC) curve with combination of significant parameters was also conducted. Results Among acoustic parameters, the length of analyzed sample (p=0.010), number of segments computed (p=0.018), total number detected pitch periods (p=0.017), and second formant (p=0.013) in pre- and post-VFSS were significantly different between Non-P/A and P/A groups. In the P/A group after VFSS, the means of these significant parameters decreased. According to ROC combined with four significant parameters, the probability of predicting P/A condition was 84% (p=0.005), the sensitivity was 80%, and the specificity was 80%. Conclusion Voice acoustic analysis can reflect voice changes by penetration/aspiration and the combination of significant parameters can also detect swallowing disorders. Therefore, voice analysis can be a reliable screening tool for patients with swallowing disorders.

  • Objectives: The aim of this study is to describe the effects of treatment with traditional Korean medicine on a patient with voiding dysfunction caused by cerebral infarction. Methods: The patient was treated with herbal medicine, moxibustion, acupuncture, and electroacupuncture. The effects of these treatments were evaluated using the Korean version of the Modified Barthel Index and a post-void residual volume. Results: In this study, the Korean medicine Dangguishaoyao-san-hap-Jeoryeong-tang resulted in improvements in this patient with post-stroke urinary retention and successful removal of the indwelling catheter. Conclusion: The results suggest that traditional Korean medicine may be effective in ameliorating dysfunction caused by cerebral infarction.

상호억제 기법에 의한 경직성 편마비 환자 보행의 공간적, 시간적 특성 변화 (Characteristics Change of Spatial and Temporal Parameters of Gait in Spastic Hemiplegic Patients by Reciprocal Inhibition)

  • The purpose of this study was to determined the effects of reciprocal inhibition on spatial-temporal gait parameters in spastic hemiplegic patients through GaitRite system. The subjects were consisted 45 patients who had spastic hemiplegia due to stroke. All subjects randomly assigned to 3 group : manual reciprocal inhibition program group(manual group), neuromuscular electrical stimulation group(NMES group) and control group. The manual group received voluntary isometric contraction of pre-tibia muscle. The NMES group received neuromuscular electrical stimulation on tibialis anterior. The control group was not recieved any therapeutic intervention. Before and after experiments, spatial-temporal gait parameters and functional ambulatory profile was measure in all patients. The data of 30 patients who complete experimental course were statistically analysed. The results of this study were as following : 1. The percentage of change of functional ambulatory profile were markedly increased in manual group but statistically non significant(p>.01). 2. The percentage of change of gait velocity and cadence were markedly increased in manual group but statistically non significant(p>.01). 3. Asymmetry ratio of gait elements were more improved in manual group but statistically non significant(p>.01). 4. There were no statistical difference between pre-test and post-test with functional ambulatory profile, gait velocity, cadence and asymmetry ratios in NMES group(p>.01). 5. There were no statistical difference between pre-test and post-test with unctional ambulatory profile, gait velocity, cadence and asymmetry ratios in control group(p>.01). In conclusion, the present results revealed that reciprocal inhibition which produced by voluntary isometric contraction of pre-tibia muscle can be improved spatial-temporal gait parameters including functional ambulatory profile in hemiplegic patients. Therefore, reciprocal inhibition is useful to improve functional activities in hemiplegic patient. Further study should be done to analyze the effects of intervention duration of reciprocal inhibition, appropriate muscle contraction, optimal time to apply the reciprocal inhibition in more long period.
