Compensatory Function of Remnant Kidney in Partially Nephrectomized Korean Black Goats -On the Sequential Changes of Some Clinicopathological Values- (한국 흑염소에서 신장부분제거에 따른 잔류신장의 보상적 기능에 관한 연구 -몇 가지 임상병리학치의 변동에 관하여-)

  • In order to study the compensatory renal function in 6 Korean black goats with 75% reduction in renal mass, some blood chemical values were determined and pathological changes were examined. The results were as follows : 1. BUN level increased significantly from post-operation day 1 through day 30(p<0.01) compared with pre-operation level. Serum creatinine level increased significantly from post-operation day 1 through day 26(p <0.01 ) compared with pre-operation level and remained higher than pre-operation level at day 30(p <0. 05). 2. Serum calcium level increased significantly from post-operation day 4 through day 7(p<0.01) compared with pre-operation level and remained higher than pre-operation level from post-operation day 8 through day 14(p<0.05). Serum phosphorus level decreased at postoperation day 4,5(p<0.05 ) compared with pre-operation level. 3. RBC counts decreased at post-operation day 2. 3, 5. to 30(p<0.05), PCV decreased at post-operation day 2, 3, 5, 7, to 30(p<0.05), hemoglobin level decreased at post-operation day 2, 8, 14, to 30(p<0.05), compared with pre - operation levels, respectively. 4. The ventral half of the right kidney was infarcted by the ligation of ventral branch of right renal artery.

Study on the effect of Post Open laser Lumbar Micro-discectomy on the Cross Section Area of Deep Muscles in Patients (요추부 미세 현미경 레이져 디스크 수술(OLM)이 환자의 심부근육 단면적 크기에 미치는 영향)

  • Purpose : The purpose of this study is to figure out the effects of Open Laser Microdiscectomy(OLM) on deep muscles by comparing multifidus and longissimus muscle size (cross section area; CSA) of pre and post operation. Methods : The subjects consisted of forty patients who had OLM. The data were analyzed with paired t-test comparing left and right deep muscle CSA of pre and post-operation, and both the deep muscle CSA of pre and post-operation, using SPSS ver. 15.0 program. Results : The results of this study showed a significant difference in deep muscle size (CSA) between pre and post operation (p<.05). Although there was not a meaningful difference between right and left deep muscle size (CSA) in pre operation (p>.05), there was a significant difference between both of them in post operation (p<.05). Conclusion : Therefore we made the conclusion that the operation causes decrease of muscle tone in deep muscles and muscle imbalance by causing muscle atrophy in the lumbar deep muscle after the operation.

THE EVALUATION OF REPRODUCIBILITY OF OPERATION PLAN WITH THE COMPARISON BETWEEN STO AND POST-OPERATIVE JAW POSITION IN ORTHOGNATHIC SURGERY (악교정 수술에서 STO와 술 후 악골위치 비교를 통한 이동량 재현성에 대한 평가)

  • The purpose of this study is to examine reproducibility of operation plan and 3-dimentional jaw movement patterns by comparing jaw position of STO with post-operative jaw position. Twenty patients with class III dental and skeletal malocclusion who were treated with Le-Fort I osteotomy and B-SSRO were reviewed. Lateral cephalometric radiographs were taken within two weeks before operation and two days after operation. Cephalometric radiographs were compared and analyzed with orthognathic computer program '$V-Ceph^{TM}$'. Post-operative maxillary advancement was insufficient compared to maxillary advancement through STO. Post-operative setback movement was over compared to mandibular setback movement through STO. But statistically this is not significant. Maxillary vertical location is insignificant on the whole. Especially post-operative maxillary clockwise rotation is significant compared to maxillary rotation through STO. Post-operative maxillary clockwise rotation tendency is generally observed in all patients. So surgeons and staffs must consider this tendency when operation plan is established ans operation is being performed. Using intra or extra oral marking points, face bow, and bite plate will make exact surgery possible.

  • We consider a production system with post-operation and early set-up. As soon as there are no more units(raw materials) to process and the machine becomes idle, it undergoes a post-operation. After the post operation, if the number of waiting units is greater than or equal to m, it begins a set-up. After the set-up, if the number of units is greater than or equal to N($\geq$m), it begins to process the units. We first analyze the system by employing the decomposition property of the vacation queue. We, then, propose an algorithm that finds the optimal threshold values ($m^*$, $N^*$). Finally we present some numerical examples and interprete the system behavior.

Development of a Critical Pathway for Patients with Coronary Artery Bypass Graft (관상동맥 우회술 환자를 위한 Critical Pathway개발)

  • The purpose of this study was to develop a critical pathway for case management for patients who have received Coronary Artery Bypass Graft (CABG) because of Ischemic Heart Disease(IHD) which is a factor of rising medical expenses. For this study. a conceptual framework was developed through a review of the literature including six critical pathways which are currently being used in USA. In order to identify the overall service contents required by these patients and to draw up a preliminary critical pathway, 30 cases of medical records of patients who had CABG because of IHD between January, 1995 to June. 1996 at the Cardiovascular Center of Yonsei Medical Center in Seoul were analyzed. An expert validity test was done for the preliminary critical pathway and clinical validity test was also done using seven IHD patients with CABG between November 11 and 23, 1996. After these processes. the final critical pathway was developed. The results of this study are summarized as follows : 1. The vertical axis of the critical pathway includes the following eight items : tests, nutrition, medications, consultations, activity, assessments, treatments, education discharge planning and the horizontal axis includes the time from the start of hospitalization to discharge. 2. Analysis of the 30 medical records indicated that the average length of stay was 20.2days with the average length of stay from hospitalization day to operation day being 6.2 days, and the average length of stay from operation day to discharge day was 13. 9 days. Analysis of the service contents showed that the horizontal axis of the preliminary critical pathway was set from hospitalization to the 14th post operation day and the vertical axis was set to include eight items, the contents which ought to have occurred, according to the time frames of the horizontal axis. 3. As a result of the experts validity, it was found that among the total of 571 items. there was over 83% agreement for 482 items, less than 83% for 89 items, which were then deleted and a revision of the critical pathway was done. 4. A clinical validity test was done using seven IHD patients with CABG. During the process, three patients were deleted because they were out of the criteria the investigator set. Finally, four patients were used. The result of study indicated that only one patient was discharged on the tenth post operation day, which was one day later than the expected day. Three patients were discharged later than the expected day from three days to nine days. All the cases progressed on schedule until the operation day and the first post operation day, but from the second post operation days, there were differences between the critical pathway and the actual practice. The differences came from tests, assessments, and treatments. 5. On the basis of the results of the clinical validity test. the following revisions in the final critical pathway were made : the transfer from ICU to step down ward would be the second post operation day, and the transfer to a general ward, the fifth post operation day, for patients who complained of lack of sleep from the fifth post operation day to discharge, a sleeping pill would be prescribed, skin observations would be performed routinely from immediately after the operation until the third post operation day, and would continue if there was a sign of skin injury on the fourth post operation day, and assessment of chest pain would be done from the third post operation day, and the “stairs climbing” item, expected to be done on the ninth post operation day would be deleted. In conclusion, this critical pathway is partially applicable to the care of patients with CABG but there are some parts needed to be further investigated.

Clinical Analysis of Post-traumatic Hydrocephalus

  • Objective : Post-traumatic hydrocephalus is a complication of head injury and can present with several different clinical symptoms. However, the developing factors of post-traumatic hydrocephalus and treatment are still not well known. The authors design the study to focus on incidence, causing diseases and treatment of post-traumatic hydrocephalus. Methods : The 789patients of traumatic head injury followed by admission treatment over 7days from Jan. 1997 to Dec. 2001, were divided shunt group and shunt free group. We analyzed age, sex, causing diseases, developing time of hydrocephalus and effects of shunt operation in post-traumatic hydrocephalus. Results : The incidence of post-traumatic hydrocephalus for requiring shunt was 9.2% [64cases]. Chronic hydrocephalus which developed after 14days of injury was higher incidence [51cases]. We found following variables were significantly related to shunt-dependent hydrocephalus : low GCS score at admission, initial CT finding of traumatic subarachnoid hemorrhage and intracerebral hemorrhage [including intraventricular hemorrhage]. The effect of shunt operation was not related with the spinal pressure, but had statistically significant correlation with the response of lumbar drainage. Conclusion : We conclude that development of hydrocephalus after head trauma is related to low GCS score, intracerebral hemorrhage [including intraventricular hemorrhage] and subarachnoid hemorrhage. The effect of preoperative lumbar drainage has a significant role in predicting the result of shunt operation in patient with post-traumatic hydrocephalus.

  • This paper presents a process and operation planning system and an NC code post-processor for effective machining of press dies for production of cars. Based on the machining feature, major parts of press dies are categorized into 15 groups and a standard process plan is defined for each group. The standard process plan consists of a series of processes where a process is defined as a group of operations that can be done with one setup. Details such as cutting tools, cutting conditions, and tool paths are decided at the operation planning stage. At the final stage of process and operation planning, the NC code post-processor adjusts feedrates along the tool path to reduce machining time while maintaining the quality. The adjustment rule is selected based on the machining load estimated by virtual machining.

  • The clinical and statistical evaluation on the patients of maxillofacial deformities who were operated by author were analyzed after several month or years or se. Pre and post operative cephalometric radiographs of 45 orthognathic surgery patients were compared. The post operative radiographs had been taken at least 9 month to several years. Measurements were made between constructed hard tissue and soft tissue points located on each before and after film tracing. The items studied and evaluated are as follows: 1) Classification and divid of patients 2) Operation technic adopted by operator. 3) Motives of patients for operation and their untowards. 4) Self satisfication of patients after operation. 5) Post operative changes of soft and hard tissue. 6) Side action during and after operation.

  • Post-tonsillectomy pain in children is a difficult problem to manage. We examined the effect of 10% aerosol lidocaine for the post-tonsillectomy analgesia. ASA physical status I or II, between five and thirteen years of age were assigned, in a randomized fashion, into two groups. The Group I, 30 patients, received a total dose of 4 mg/kg of ten percent aerosol lidocaine on the tonsillar beds. Lidocaine was administered at the end of the surgical procedure. The Group II, 30 patients, were not sprayed with lidocaine aerosol. The postoperative pain scores were assessed by a "red and white" visual analoge pain scales (VAPS). VAPS values were obtained at one and eight hours after the operation. Values of one hour after the operation in the group I and II were $20.83{\pm}10.01$ and $34.50{\pm}10.53$, values of eight hours after the operation were $17.33{\pm}9.07$ and $23.5{\pm}11.08$. The post-tonsilectomy pains were significantly lower in the group I compared with the group II at one and eight hours after the operation. In conclusion, 4 mg/kg of ten percent aerosol lidocaine applied directly on the tonsillar beds was showen a superior, immediate post-tonsillectomy analgesic technique.

The Effect of the Patient's Characteristics on Nursing Outcomes in Gastrointestinal Surgery Patients (간호결과에 대한 환자 특성의 영향 - 위장관계 수술 환자를 중심으로 -)

  • Purpose: This study was performed to identify the patient characteristics significantly affecting nursing outcomes and their predictability in gastrointestinal surgery patients. Method: The subjects were 149 abdominal surgery patients from 3 general surgical nursing units of 3 general hospitals. Two instruments were used to measure nursing outcomes and acuity of the subjects. The nursing outcomes were measured at post-operation 4 and 7days using review of patients' records, observation of patients, and interviews with patients by a trained nurse. For data analysis, T-test or ANOVA, Pearson Correlation and Stepwise Multiple Regression were done. Result: Age, severity score, diagnosis, cancer or not, operation site were the subjects' characteristics that were significantly related to the nursing outcomes in both post-operation 4 and 7days. Cancer or not, age, diagnosis and severity score were the significant predictors for the scores of nursing outcome in post-operation 4days and the predictability was 34.9%. The predictability of cancer or not was highest, 22.6%. Age, diagnosis and cancer or not were the significant predictors for the scores of nursing outcome in post-operation 7days and the predictability was 27.8%. The predictability of age was highest, 17.3%. Conclusions: The patient characteristics affecting nursing outcomes should be considered when nursing care is planned and provided. Especially, careful attention should be given to the patients with cancer and older age. And, these patient characteristics should be adjusted for correct estimation of the effectiveness of nursing interventions on nursing outcomes.
