소아에서 척수액 검사 후 발생한 두통에 대한 연구 (A study for post-diagnostic lumbar puncture headache in children)

  • 목 적 : 소아에서 진단 목적으로 척수액 검사를 시행한 환아에서 천자 후 부작용으로 발생하는 두통의 발생 빈도와 영향을 끼치는 인자들에 대해 알아보고자 본 연구를 하였다. 방 법 : 2005년 3월부터 2006년 2월까지 조선대학교 병원 소아과에서 진단 목적으로 요추천자를 시행한 4세에서 13세 사이의 환아 중 뇌수막염이 의심되었던 44명을 대상으로 하였다. 요추천자 후 두통의 발생빈도와 성별, 연령, 요추 천자 횟수, 척수액 양, 두통의 과거력, 척수액 압력, 척수액내 백혈구 수의 각 인자들과 두통 발생사이의 상관성, 두통의 발생 시작시간과 지속시간 및 치료에 대해서 조사하였다. 결 과 : 천자 후 두통의 발생빈도를 보면 전체 44명 중 16명(36.4%)에서 발생했으며, 남녀가 각 39.4%(13명), 27.2%(3명)로 성별에서는 유의한 차이는 없었다. 두통의 과거력이 있는 6명중 50%(3명)에서 두통이 발생하였고, 과거력이 없는 38명 중 36.1 %(13명)에서 발생하여 두통의 과거력이 있는 경우 두통이 의의 있게 많이 발생하였다(P=0.037). 척수액내 백혈구 세포수가 많을수록 요추 천자 후 두통의 발생은 통계학적으로 의의 있게 많았다(P=0.012). 그 외 연령, 척수액 검사 횟수, 척수액 양, 척수액 압력과 천자 후 두통 발생 사이에는 통계학적 상관성이 없었다. 결 론 : 요추 천자 후에 두통은 기존의 다른 연구에서 보다 발생 빈도가 더 높았으며 두통의 과거력이 있는 경우와 척수액내 백혈구 세포수는 소아에서 척수액 검사 후 발생한 두통에 영향을 주는 인자로 생각된다.

요추천자 후 발생한 두통 치험 1례 (A Case Report of Korean Medicine Treatment for Post-Lumbar Puncture Headache)

  • Lumbar puncture is an essential procedure for obtaining fluid from spinal meningeal spaces, and post-lumbar puncture headache is the most frequent adverse event. This case report evaluates the effects of treatment with traditional Korean medicine on a patient suffering post-lumbar puncture headache using the Headache Disability Inventory (HDI) and the visual analog scale (VAS). The patient's HDI score decreased from 46 to 6 after treatment, and the VAS score decreased from 62 to 5. The results suggest that traditional Korean medicine can be helpful in managing post-lumbar puncture headache.

침도 침술 후 나타나는 두통에 대한 임상적 고찰 (The Study on Post Acupotomy Headache)

  • Objectives : This study was designed to investigate incidence rate, clinical features and treatment of headache followed after acupotomy. And based on that, we study correlating between post acupotomy headache and post-lumbar puncture headache(PLPH). Methods : From September 1st, 2007 to December 31st, 2007, the 73 inpatients who admitted to Daejeon Oriental hospital and underwent acupotomy on lumbar spine and cervical spine etc. more than once were observed. Then 9 inpatients who occurred headache after acupotomy was analyzed according to incidence rate, gender, age, clinical features. Results : We investigated clinical features of post acupotomy headache and made comparative study of post acupotomy headache and PLPH. The average frequency of post acupotomy headache was about 12.3%. The risk factors are undergoing acupotomy on lumbar spine, young age. Gender does not affect the incidence of post acupotomy headache. Patients complain of headache which worsen when the patients is upright and improve after the patients lie back down in a few minutes. Those clinical features are similar to PLPH. Conclusions : Although two type of headache have significant similarities, there is no enough evidence to support this study. In addition, the cause and treatment of post acupotomy headache are still unclear. Thus, based on the above study, more studies have to be investigated.

요부경막천자후 발생된 두통치료 -자가혈액봉합 37예- (Autologous Epidural Blood Patch for the Treatment of Headache in Post-Dural Puncture -Report of 37 cases-)

  • We performed the autologous epidural blood patch (AEBP) for the relief of headache and other related symptoms following dural punctures of the lumbar region during 8 years from 1981 to 1988. The total of 37 patients with the patching consisted of 9 cases in 3007 of spinal anesthesia, 12 accidental dural punctures in 4283 cases of lumbar epidural anesthesia, 12 cases in 4747 of epidural analgesia for back pain control, 3 cases of myelography and a case of diagnostic lumbar puncture. The headaches were relieved completely in 35 cases following the first AEBP and the remaining two were also relieved following the second AEBP. We think that the AEBP for postdural-puncture headache is the treatment of choice.

침도(도침)침술과 경막외 신경성형술의 비교 연구 (The Comparison between Acupotomy Therapy and Epidural Neuroplasty(Lumbar Vertebra))

  • Objectives : This study will broaden understanding of acupotomy therapy through comparison of side effects and complication which can be caused by the mechanism of treatment, surgical methods, and procedure by acupotomy therapy and epidural neuroplasty. Moreover, through an in-depth analysis of headache affected by two procedures, this research is supposed to find prospective cures for headache after acupotomy therapy. Methods : To compare acupotomy therapy with epidural neuroplasty this research was done using a comparative analysis eight theses about acupotomy therapy since 1995, as well as eleven theses about epidural neuroplasty since 2000. Other theses and data were used as references in the process of comparative analysis. Results : Acupotomy therapy and epidural neuroplasty, new treatments of damaged discs in the spine and stenosis made in 1990s, have the mechanism of treatment in common in that adhesion, a node or scar caused by the soft tissue damage is removed by putting catheter or acupuncture into the lesions. Epidural neuroplasty has additional injection into the lesions, which is different from acupotomy therapy in the process of surgical procedure. There are various reports of positive effects about curative effect in these two treatments. The two procedures may cause various complications. Headache may be a complication after surgery. The headache after acupotomy therapy is characterized as being an ache in the body, which is similar to that of post-dural puncture headache in the outbreaks and symptom. Headache after percutaneous epidural neuroplasty appears in general, which is similar to a headache as a result of the increased pressure of the brain spinal cord regardless of posture. Conclusions : Although they are alike in the mechanism of treatment, surgical methods and side effects, and complication after they are carried out as a result of analyzing theses related to acupotomy therapy and percutaneous epidural neuroplasty, there is a difference in aspects and mechanism of headaches experienced after the procedure.
