두경부 굴곡 운동이 만성 경부통 근로자의 통증 및 경장근 단면적에 미치는 영향 (Effect of Craniocervical Flexion Exercise on Pain and Cross Sectional Area of Longus Colli Muscle in Workers with Chronic Neck Pain)

  • This study compared the effects of craniocervical flexion exercise with conservative cervical flexion exercise in workers with chronic neck pain. Subjects were randomly allocated to two groups: control (n=20) and experimental group (n=20), respectively. The conservative exercise for the control group consisted of cervical flexion exercise, whereas the experimental group performed a craniocervical flexion exercise. To compare the two groups, the visual analogue scale (VAS) for a pain, neck disability index (NDI) for a neck disability scale, and cross sectional area (CSA) of longus colli muscle using ultrasound image were assessed on pre-intervention, post-intervention, and 2 weeks follow-up. 2-way repeated ANOVA was used with Bonferroni post-hoc test. (1) There were significant main effects (within and between groups) (p<0.05) and interaction effect (p<0.05) in VAS. Post-hoc test revealed that there were significant differences in all pair-wise comparisons. (2) There were significant main effects (within and between groups) (p<0.05) and interaction effect (p<0.05) in NDI. Post-hoc test revealed that there were significant differences between pre-intervention and post-intervention, pre-intervention and 2 weeks follow-up. (3) There were significant main effects (within and between groups) (p<0.05) and interaction effect (p<0.05) in CSA of longus colli muscle. Post-hoc test revealed that there were significant differences in all pair-wise comparisons. The findings indicate that craniocervical flexion exercise are more effective for improving pain, neck disability, and CSA of longus colli muscle than cervical flexion exercise in workers with chronic neck pain.

편마비 환자의 팔 뻗기 과제 수행 시 목표거리와 건·환측 사용에 따른 운동시간과 체간의 움직임 분석 (Analysis of Movement Time and Trunk Motions According to Target Distances and Use of Sound and Affected Side During Upper Limb Reaching Task in Patients With Hemiplegia)

  • The aim of this study was to investigate effects of reaching distance on movement time and trunk kinematics in hemiplegic patients. Eight hemiplegic patients participated in this study. The independent variables were side (sound side vs. affected side) and target distance (70%, 90%, 110%, and 130% of upper limb). The dependent variables were movement time measured by pressure switch and trunk kinematics measured by motion analysis device. Two-way analysis of variance with repeated measures was used with Bonferroni post-hoc test. (1) There were significant main effects in side and reaching distance for movement time (p=.01, p=.02). Post-hoc test revealed that there was a significant difference between 110% and 130% of reaching distance (p=.01). (2) There was a significant main effect in side and reaching distance for trunk flexion (p=.01, p=.00). Post-hoc test revealed that there were significant differences in all pair-wise reaching distance comparison. (3) There was a significant side by target distance interaction for trunk rotation (p=.04). There was a significant main effect in target distance (p=.00). Post-hoc test revealed that there were significant differences between 70% and 110%, 70% and 130%, 90% and 110%, 90% and 130% of target distance. It was known that trunk flexion is used more than trunk rotation during reaching task in hemiplegic patients from the findings of this study. It is also recommended that reaching training is performed with limiting trunk movement within 90% of target distance whereas reaching training is performed incorporating with trunk movement beyond 90% of target distance in patients with hemiplegia.

Stroke Recovery Can be Enhanced by using Repetitive Transcranial Magnetic Stimulation Combined with Mirror Therapy

  • The aim of the present study was to examine whether mirror therapy, in conjunction with repetitive transcranial magnetic stimulation (rTMS), can improve the upper extremity function of stroke patient. This study was conducted with 35 subjects, who were diagnosed as a hemiparesis by stroke. The Mirror plus rTMS group was of 12 members who undertook mirror therapy in conjunction with rTMS, the Mirror group was of 11 members who undertook mirror therapy, and the control group was of 12 members who undertook sham therapy. A motor cortex excitability was performed by motor evoked potential, and upper limb function was evaluated by Fugl-Meyer Assessment, and Box and Block Test. Significant difference was shown after the experiment, in comparison of the groups in terms of latency, and as the result of post hoc test, significant difference was shown between the Mirror plus rTMS group and control group, and between the Mirror group and control group, respectively. Significant difference was shown after the experiment in comparison of the groups in amplitude, and as the result of post hoc test, significant difference was shown between the Mirror plus rTMS group and Mirror group, and between the Mirror plus rTMS group and control group. Significant difference was shown after the experiment, in comparison of the groups in FMA and BBT, and as the result of post hoc test, significant difference was shown between the Mirror plus rTMS group and Mirror group, and between the Mirror group and control group. The study showed that mirror therapy in conjunction with rTMS is more effective to improve upper extremity function, than mirror therapy and sham therapy.

Effects of Mulligan's Mobilization with Movement on Talofibular Interval in Subjects with Chronic Ankle Instability

  • Purpose: This study was conducted to determine the effects of Mulligan's mobilization with movement (MWM) on changes in the talofibular interval in the sagittal plane in subjects with chronic ankle instability (CAI). Methods: Sixteen subjects with chronic ankle instability participated in this study. The talofibular intervals were measured from US images, and the weight-bearing lunge test was used to assess dorsiflexion of the ankle joint. Each dependent variable were measured on the both affected side and sound side in three trials in pre- and post-MWM. Dependent variables were examined with a two-way mixed-design analysis of variance (ANOVA). The two factors were side (sound side versus affected side) and intervention (pre- versus post-intervention). For post hoc analysis, paired t-tests were performed to compare the dependent variables. A p<0.05 was considered to indicate significance. Results: Dorsiflexion and talofibular interval differed significantly pre- and post-intervention (p<0.05). Post-hoc analysis revealed that the talofibular interval post-MWM was significantly less than that pre-MWM on the both the affected and sound side (p<0.05). The ankle dorsiflexion in the post-MWM group was significantly greater than that in the pre-MWM group on the affected side and the sound side (p<0.05). Conclusion: The Mulligan's MWM decreased the talofibular interval in subjects with CAI. These findings suggest that the MWM technique can change the position of the talus relative to the fibular in the weight bearing position.

NCPX를 이용한 도로 표층 유형별 노면 소음 비교 연구 (A Study for Comparing Road Noise by Surface Types using NCPX)

  • PURPOSES : The purpose of this study is to study the noise reducing effect of Micro Surfacing by comparing general asphalt, longitudinal tining and Slurry Seal. METHODS : This study measures vehicles' noise of each section by the NCPX method that can measure noise between the road surface and the tire at the field. Total sound pressure and sound pressure level by the 1/3 octave band frequency are calculated through the field data of each section. Total sound pressure level is compared by ANOVA test statistically. After ANOVA test, post-hoc test is conducted to know mean difference of surface type by Tukey. RESULTS : As the result of CPB analysis to confirm sound pressure levels by frequency, it was shown that sound pressure levels by frequency are totally similar except for those of frequency bands between 100Hz and 500Hz. The result of ANOVA test and post-hoc test, it was shown that sections of surface type have a difference. The result of Micro Surfacing was lower 2~5dB(A) than other surface type. CONCLUSIONS : It is found that the noise reduction performance of Micro Surfacing was better than other surface type.

벽 미끄러짐 쪼그려 앉기 방법에 따른 넙다리네갈래근의 근활성도 비교 (Comparison of Quadriceps Femoris Muscle Activations during Wall Slide Squats)

  • PURPOSE: The purpose of this study was compare quadriceps femoris muscle activity while performing wall slide squats of four methods. METHODS: Forty subjects, with no history of patellofemoral pain, quadriceps injury, or other knee injury volunteered for this study. Muscle activation of the vastus medialis obliquus, rectus femoris, vastus lateralis muscles were recorded while subjects performed 10 consecutive wall slide squats. Subjects performed the wall slide squats during four different methods: (1) basic wall slide squat, (2) keep back upright against fitness ball, (3) standing of unstable surface, (4) squeezing ball between both knees. Statistical analysis were accomplished by utilizing the one-way ANOVA(Bonferroni's post-hoc test) by SPSS 20.0 program. Significance level was set at p<.05. RESULTS: Muscle activations induced wall slide squats of four methods compared and results showed that there was significant difference only in vastus medialis obliquus and rectus femoris but there was no significant difference in vastus lateralis. The vastus medialis obliquus was significantly different only keep back upright against fitness ball at post-hoc test. The rectus femoris was significantly different keep back upright against fitness ball and standing of unstable surface at post-hoc test. CONCLUSION: Based on these results, we conclude that quadriceps femoris muscle activations are differenced by performing wall slide squats of four different methods in healthy subjects. These data suggest that for quadriceps muscle strengthening, exercise professionals can perform the wall slide squats by altering several task variables. Further research is needed to determine the exact mechanism by which quadriceps function is altered.

Effects of Different Knee Flexion Angles According to Three Positions on Abdominal and Pelvic Muscle Activity During Supine Bridging

  • This study analyzes how different knee flexion angles affect the abdominal and pelvic muscle activity during supine bridging. Twenty healthy subjects participated in the study. We used surface electromyography (EMG) to measure how three different knee flexion angles ($100^{\circ}$, $70^{\circ}$, and $40^{\circ}$) affected the activity of the transverse abdominis/internal oblique (TrA/IO), external oblique (EO), biceps femoris (BF), rectus femoris (RF), and gluteus maximus (GM) muscles on the dominant side during supine bridging. The one-way repeated analysis of variance (ANOVA) was used to determine the statistical significance of TrA/IO, EO, BF, RF and GM muscle activity and the GM/BF activity ratio. For the TrA/IO, EO, BF, and GM muscles, supine bridging with different knee flexion angles resulted in significant differences in abdominal and pelvic muscle activity. For the TrA/IO muscles, the post-hoc test demonstrated that muscle activity significantly increased at $40^{\circ}$ compared to $70^{\circ}$; however, there were no significant differences between $100^{\circ}$ and $70^{\circ}$ or $100^{\circ}$ and $40^{\circ}$. For the EO muscle, the post-hoc test demonstrated that muscle activity significantly increased at $40^{\circ}$ compared to $100^{\circ}$ and $70^{\circ}$; no significant difference was observed between angles $100^{\circ}$ and $70^{\circ}$. For the BF muscle, the post-hoc test demonstrated that muscle activity significantly increased according to the knee flexion angle ($40^{\circ}$ > $70^{\circ}$ > $100^{\circ}$). For the GM muscle, the post-hoc test demonstrated that muscle activity significantly increased according to the knee flexion angle ($100^{\circ}$ > $70^{\circ}$ > $40^{\circ}$). However, for the RF muscle, there was no significant difference. Additionally, the GM/BF activity ratio significantly increased according to the knee flexion angle ($100^{\circ}$ > $70^{\circ}$ > $40^{\circ}$). From these results, we can conclude that bridging with a knee flexion of $100^{\circ}$ can strengthen the GM muscle, whereas bridging with a knee flexion of $40^{\circ}$ is recommended to strengthen the IO, EO, and BF muscles. We can also conclude that knee flexion angles should be modified during supine bridging to increase the muscle activity of different target muscles.

초음파치료를 동반한 뒤통수밑근 이완운동이 긴장성 두통 환자의 근육의 기계적인 특성과 목 장애 지수, 두통 영향 검사에 미치는 영향 (The Effect of Combined Suboccipitalis Release Exercise and Therapeutic Ultrasound on Mechanical Properties of Cervical Muscles and Neck Disability Index, Headache Impact Test in Tension-Type Headache Patients)

  • Purpose : This study aimed to apply muscle relaxation exercise and ultrasound therapy as an effective treatment method for tension headache patients by comparatively analyzing the muscle mechanical properties, neck disorder index, and headache impact test. Method : The subjects were classified into three groups based on the intervention received: the therapeutic ultrasound (n=15, group I), placebo therapeutic ultrasound combined with the suboccipital release (n=15, group II), and therapeutic ultrasound combined with suboccipital release (n=15, group III) groups. The intragroup and intergroup differences in muscle mechanical properties, neck disability index, and headache impact test were compared and analyzed. Result : The comparison and analyses of the changes in muscle tone and post hoc analysis revealed statistically significant intragroup decreases in the suboccipitalis and upper trapezius in groups I and III. Furthermore, statistically significant intergroup differences in the upper trapezius with a greater change in group III than in group II and suboccipitalis with greater changes in group III than in groups I and II were observed. The comparison and analyses of the changes in muscle stiffness and post hoc analysis revealed statistically significant intragroup decreases in the upper trapezius in group I and suboccipitalis in group III. Moreover, statistically significant intergroup differences in both muscles with greater changes in group III than in group II for both cases were observed. The comparison and analyses of the changes in the neck disability index and post hoc analysis revealed statistically significant intragroup decreases in all three groups and statistically significant intergroup differences with greater changes in group III than in groups I and II. The comparison and analyses of the changes in the headache impact test and post hoc analysis revealed statistically significant intragroup decreases in all three groups and a statistically significant intergroup difference with greater changes in group III than in groups I and II. Conclusion : The therapeutic ultrasound and suboccipital muscle release exercise effectively increased the flexibility of the muscles around the cervical vertebrae and reduced muscle tension and stiffness in tension-type headache patients.

Effects of Sensory Stimulation Type on Learning in Elderly People

  • The purpose of this study was to examine the effects of sensory stimulation type on the learning of the elderly. The study implemented elderly verbal learning test by evenly dividing a total of 40 elderly people into auditory stimulation followed by visual stimulation group(AVG), visual stimulation followed by auditory stimulation group(VAG), simultaneous visual and auditory stimulation group(SG), and auditory stimulation group(AG). The result are as follows. SSG showed highest results in most of the test, followed by AVG, VAG, and AG. In particular, SG showed a statistically significant difference in immediate recall total score and short delay free recall score(p<.05). In the post-hoc test results of immediate recall total score, SG showed highest score with a statistical significance. AVG showed the second highest score, followed by VAG and AG and all showed a statistic significance(p<.05).In the post-hoc test results of short delay free recall score, SG and AVG showed a statistically significantly higher score than AG(p<.05). The results suggest that simultaneous stimulation of visual and auditory sense is more effective for improving the learning of the elderly people than visual stimulation followed by auditory stimulation or auditory stimulation followed by visual stimulation.

PNF의 어깨뼈 골반 대칭 상호 패턴이 노인의 호흡기능과 삶의 질에 미치는 효과 (The Effects of Scapula and Pelvis Symmetrical Reciprocal Pattern of PNF Concept on Respiratory Function and Quality of Life in the Elderly)

  • Purpose : The purpose of this study is to determine the effects of scapula and pelvis symmetrical reciprocal pattern exercise of PNF concept on respiratory function and quality of life in elderly subjects. Methods : Nineteen elderly subjects with healthy were recruited. Subjects performed scapula and pelvis symmetrical reciprocal pattern exercise of PNF concept. Exercise was appied 30 minutes three times per week for four weeks. The respiratory function and quality of life (QOL) test three times (before, two weeks, and four weeks). Respiratory function includes forced vital capacity (FVC), forced expiratory volume at one second (FEV1), Peak Expiratory Flow (PEF) and chest cage expansion test (CCET). The QOL test was measured Korean WHOQOL-BRIEF. The analysis method was analyzed through the one-way ANOVA repeated methods, and the statistical significance is α=.05. It was analyzed through the post test Bonferroni test. Results : After the 4 week scapula and pelvis symmetrical reciprocal pattern exercise of PNF concept showed statistically significant differences in the respiratory function (FVC, FEV1, PEF, and CCET), and QOL (p<.05). As a result of the post-hoc test, FVC showed a significant increase in the mid test and post test compared to the pre test (p<.05), FEV1 showed a significant increase in the post test compared to the pre test (p<.05). PEF showed a significant increase in all pre-test, mid-test, and post-test sections (p<.05), CCET showed a significant increase in all pre-test, mid-test, and post-test sections (p<.05). QOL showed a significant increase in all pre-test, mid-test, and post-test sections (p<.05). Conclusion : In this study, the scapula and pelvis symmetrical reciprocal pattern exercise of PNF concept improved respiratory function and QOL. The findings suggest that this intervention could be beneficial in improving respiratory function and QOL in the elderly.
