  • Immunizations are among the most cost-effective and widely used public health interventions. This is a report a revision of recommendation of immunization for children by Korean Pediatric Society. Immunization. Vaccines were divided into 4 groups. 1) Vaccines that are recommended to all infants and children (BCG, hepatitis B vaccine, DTaP, Td, Polio vaccine, Japanese encephalitis vaccine, MMR, varicella vaccine, influenza vaccine [6-23 months of age], H. influenzae type b vaccine), 2) those that can be administered to all infants and children, but decision of administration is made by parents (pneumococcal conjugate vaccine, hepatitis A vaccine, influenza vaccine [healthy children ${\geq}24$ months of age], rotavirus vaccine, human papilloma virus vaccine), 3) those that should be given to high risk group (pneumococcal polysaccharide vaccine [high risk patients ${\geq}24$ months of age], influenza vaccine [high risk patients ${\geq}24$ months of age], typhoid vaccine), and 4) those administered for control of outbreaks or prevention of emerging infectious diseases. Immunization schedule recommended by Korean Pediatric Society in 2008 is presented.

  • Vaccination has been considered to be the most effective way to control infectious diseases. Currently, many vaccines used in humans are live-attenuated or killed microorganisms. Polio, mumps, and measles vaccines are live-attenuated. Killed vaccines include cholera and pertussis vaccines, These conventional vaccines, however, suffer from some problems. In the case of live-attenuated vaccines, reversion to virulence is observed in a small but significant number of clinical cases each year. In killed vaccines, due to the possible hazard to employees working with live pathogens, the cost of preparation is high. Killed vaccines also need to be given in multiple doses, Furthermore, both live-attenuated and killed vaccines have possible presence of cellular materials leading to side effects. Moreover, there are diseases such as malaria and hepatitis for which conventional attenuated and killed vaccines are not available because the pathogens cannot be grown in sufficient amounts to allow the classical methods to be used.

  • Bioactivity-directed isolation has led to the isolation of (-)-ent-costunolide (1) as the major active compound from Hepatostolonophora paucistipula. This compound (1) was determined by spectroscopic data interpretation. This sesquiterpene lactone (1) inhibited the growth of the dermatophytic fungus Trichophyton mentagrophytes ATCC 28185, (4 mm inhibition zone at $15{\mu}g$/disc), cytotoxic activity to murine leukaemia cell lines ATCC CCL 46 P 388D1 ($IC_{50}$ 687 ng/ml, at $0.075{\mu}g$/disk), BSC monkey kidney cell lines (100% of well at $15{\mu}g$/disk) and antiviral activity to Herpes simplex virus (0.25 mg/ml, 100% of well at $7.5{\mu}g$/disk) and Polio virus (0.125 mg/ml, 100% of well at $3.75{\mu}g$/disk). These results suggest that (-)-ent-costunolide (1) has potential antimicrobial and cytotoxic agents.

  • Hand-Foot-Mouth disease, which has a various enanthem-exanthem complex at the tongue, buccal mucosa, hands and feets and buttock area with febrile illness, is usually caused by Coxscakie virus type A(16). Generally, this disease shows self limited course and good prognosis without neurologic manifestations. However, enterovirus 71, which was newly discovered and reported in 1974, can cause the striking features of Hand-Foot-Mouth disease outbreaks and has neuropathogenic potentials of polio-like paralytic illness including aseptic meningitis, meningoencephalitis and respiratory disease. We experienced a case of Hand-Foot-Mouth disease with polyradiculitis manifestations, and a case of Hand-Foot-Mouth disease with meningoencephalitis. Therfore, we report these cases with brief review of related literatures.

A Comparative Study of Childhood Immunizational Level between Urban and Rural Areas (도시(都市)와 농촌지역(農村地域)의 영유아 예방접종률(豫防接種率) 비교조사(比較調査))

  • To assess the childhood immunization level of urban and rural areas, 250 mothers of $6{\sim}23$ months old children residing in Namsan 1 Dong, Taegu, and 264 mothers of the same age children residing in five areas of Kyungsan Gun where community health practitioners are assigned were interviewed in March, 1984. Immunization rate for BCG was 98.0% in urban area and 95.8% in rural area. Among children who had BCG vaccination 91.4% of them were immunized within 1 month after birth in urban area and 88.1% in rural area. The percentage of children who received three doses of DPT vaccine was 83.2% in urban, and 87.5% in rural area ana that for the polio vaccine was 80.8% in urban and 87.9% in rural area. Only a few children have never been immunized with either BCG or DPT or polio vaccine. Overall immunization rate for measles was 64.4% in urban area while it was 55.3% in rural area and that for mumps and rubella was 50.4% in urban area as compared with 36.0% in rural area. However, among children of 15 months old and above the percentage of measles vaccination was 85.3% in urban area and 73.7% in rural area. Mumps and rubella vaccination rate was 77.6% in urban area and 62.4% in rural area. These differences in measles, mumps and rubella vaccination rates between urban and rural areas were statistically significant (p<0.05). Such findings as improved immunization level, no significant differences in BCG, DPT and polio vaccination rate between urban and rural areas, and fewer mothers in rural area have not vaccinated their children than mothers in urban area because of their ignorance may be attributed to the general improvement of living standard and implementation of the maternal and child health services of the government. Nevertheless many of the mothers in rural as well as urban area have not immunized their children on time and measles, mumps and rubella vaccination rates are substantially lower than other childhood immunizations. Future immunization activity should be geared to reinforcing these areas.

Up-to-date or Complete Immunization Coverage and Their Related Factors (영유아의 예방접종 및 그 관련요인)

  • Objectives: Understanding the predictor of immunization status in childhood is critical issue to improve National Immunization Program (NIP). The aims of this study were to verify the status of up-to-date or complete immunization coverage and to investigate its related factors. Methods: As of 2005, according to local residence registry data, there were 2,188 children who aged 12 to 35 months in Nonsan city, Korea. We conducted household survey for aged 12 to 35 months children, using questionnaires to obtain data on the status of immunization such as BCG, DTaP (diphtheria, tetanus, and pertussis), Polio, and MMR (mump, measles, rubella). Finally 1,472 participated in the survey. The operating definitions used in this study were following; "Complete immunization rate" refers to the rate of children who received all immunization within recommended age intervals fully "on-time"; "The 4:3:1 series" means status of receiving the fourth diphtheria-tetanus-pertussis (4 DTP), the third Polio (3 Polio), and the first measles-mumps-rubella (1 MMR) doses. Multivariate logistic regression analyses were used to determine factors affecting complete vaccination coverage of children. Results: Immunization rates of vaccine based on the vaccination card were from 92.7% to 96.4% except 4th DTaP (79.3%). Complete immunization rate of Korea NIP was 74.0% and that of the 4:3:1 series was 77.1%. A parent as primary caregiver (OR 0.59, 95% CI 0.39-0.87 at 19-35 months of children's age) and first-born children (OR 1.79, 95% CI 1.05-3.03 at 24-35 months of children's age) were significantly related to complete immunization coverage of Korea NIP. And a parent as primary care giver (OR 0.58, 95% CI 0.38-0.88 at 19-35 months of children's age) and first-born children (OR 1.94, 95% CI 1.21-3.14 at 19-35 months, OR 2.23, 95% CI 1.27-3.91 at 24-35 months of children's age) were significantly related to complete immunization rate of 4:3:1 series. Conclusions: Government should take actions to increase complete immunization rate. In particular, intervention on the secondary caregiver and non-first-born children should be needed.

Foot Drop of Contralateral Limb after Deformity Correction in a Polio Patient: A Case Report

  • Postpoliomyelitis syndrome is a common neurological disorder that occurs in patients who have experienced paralytic poliomyelitis. Recently, as a result of vaccination against poliovirus, incidence of poliomyelitis is exceedingly low. However, many patients with postpolio syndrome may encounter anesthesia when undergoing surgery, such as for correction of foot deformity and other operations. We report on a 45-year-old woman who experienced paralysis of her contralateral limb after operation on the left foot under spinal anesthesia. Postoperative electromyography/nerve conduction study (EMG/NCS) was performed in order to determine the cause of paralysis. Motor power of the sequelae involved leg showed improvement with time and recovered fully to the preoperative level at six months after the index operation. A precise evaluation, including a physical examination and EMG/NCS, should be performed preoperatively when spinal anesthesia is planned for postpolio syndrome patients.

바이러스 백신의 전망

  • 백신이란 학문의 원조는 고금 동서를 불문하고 영국의 Jenner라는 것은 우리가 잘아는 사실이다. 이것은 약 200년전의 일이며 그후 각종 전염병예방 백신이 개발되어 오늘날 우리인간은 예전ㅇ 비해 각종 전염병에 대해 큰 은혜를 받고 있는 셈이다.특히 1949년 미국의 Enders박사 일맥의 조직 배양의 개량법에 의한 바이러스의 대량 배양과 정제및 농축이 용이하게 된것은 백신생산 개발문제뿐만아니라 바이러스학과 전염병 예방학의 발전의 일대혁신을 가져왔으며 세균학에 있어서 Koch의 고형배제를 사용하여 많은 병원성세균의 발견을 하게된 것과 비슷한 느낌을 준다. Enders의 조직배양의 modern technique는 아직까지 원인불명인 여러 종류의 바이러스 전염병 병원체가 구명되었으며 지금도 계속 새로운 바이러스가 발견되고 있는 중이다. 그 대표적인 예를 들면 폴리오(Polio)를 중심으로 Coxsackie virus의 A및 B군과 ECHO virus등 장관계바이러스들의 발견과 그 체계적정비 그리고 Adeno virus를 위시한 호흡기계 바이러스들의 계속적인 발견과 그 실험실진단법의 확립등을 들수 있다. 한편 백신개발에 대한 확립 등을 들수있다. 한편 백신개발에 대한 발자취를 보면 폴리오를 위시하여 홍역(Measles)백신, 멈프스(Mumps)및 풍진 (Rubella virus) 백신등의 개발은 근래 선,후진국을 막론하고 이러한 전염병의 예방효과가 눈에 띠게 덕을 보고있는 것이다. 지금 세계적으로 많은 전염병의 병원체가 바이러스라는 것이 알려져 있고 또한 이에 대한 백신의 개발연구가 활발하게 진행되고 있다. 이번 기회에 바이러스백신의 전망이라는 제목을 가지고 논하게 된것을 기쁘게 생가하며 동시에 어려운 과제라고도 생각된다. 왜냐하면 아직 우리나라에서는 생산을 못하고 있는것이 많은데 그 전망을 말하기가 어려운 까닭이며 또한 제한된 지면에서 충분히 고찰하기는 어렵다. 우리나라에서 자주 거론되는 백신 및 종류에 국한하여 그 문제점과 앞으로의 전망을 고찰해 보기로 한다.

Immunization Statuses of Children in Childcare Centers and Sociodemographic Factors Associated with Delayed DTaP4 Immunization

  • Purpose: Immunization is one of the simplest and most effective approaches for protecting the health of children from preventable infectious disease. This study assessed the immunization statuses of children presenting to childcare centers in Korea and compared the sociodemographic factors of children with age-appropriate immunization for DTaP4 with those with delayed immunization for DTaP4. Method: A national mail survey was used to collect data on the children's immunization history and sociodemographic information. A total of 248 mothers returned completed questionnaires for their children. Results: The overall immunization coverage for HepB, a series of DTaP and polio and varicella was above 95%. However, the percentage of age-appropriate immunizations of vaccines ranged from 45.3% to 69.0% depending on the vaccines. Only the age of the child was a significant factor for delayed immunization for DTaP4. Conclusion: To fully protect children from vaccine-preventable disease, childcare providers can playa role in monitoring the immunization statuses of children in their care centers to ensure that they all receive all childhood immunizations on time.

  • Objectives : Many studies are being done in the method of experimental chuna science in China. The aim of this review is to understand and study the methods of experimental chuna science, focusing on the effects of chuna on nervous system. Methods : We reviewed "實驗推拿學"(experimental tuina science) and Chinese journals were searched using China National Knowledge Infrastructure(CNKI). Korean journals were searched using 3 Korean web databases(OASIS, NDSL, RISS). Results : By experimental chuna science, analgesic effect of chuna, therapeutic action of chuna on stroke, therapeutic action on polio, effect of modulating autonomic nerves of chuna and effect of modulating thermoregulatory centers of chuna were proved. Conclusions : Experimental chuna science is necessary for development of chuna to a higher level. In Korea, not a lot experimental studies are being done, especially in chuna. For this reason we need to learn and adapt the methods of experimental chuna science of China.
