Objectives: The objective of this study was to analyze factors affecting depression in pneumoconiosis patients. Methods: The subjects were 200 pneumoconiosis patients hospitalized in Pneumoconiosis Hospitals in An-san and Tae-beck. Collected 114 surveys were used in this study among 200 surveys since 86 surveys offered insufficient data. SAS for Windows 8.01 was used to analyse the data Results: Percentage of normal and mild depression is 2.6%, Percentage of moderate depression is 14.9%, Percentage of severe depression is 79.8%. In order to analyze the factors affecting depression rate, the factors with p-values less than 0.1, such as age, education, religion, work department, hospitalized period, physical pain existence, daily living ability, family supports and internal locus of control, were selected as independent factors and analyzed using a stepwise multiple regression. As results, the factor that affected the rate of depression were of the age, education, ADL(Activities of Daily Living), the internal locus of control, family support. Conclusion: It is necessary to develop health program which can improve the patient's ADL(Activities of Daily Living), enforce internal locus of control to minimize the depression rate in pneumoconiosis patients.
Background : Pneumoconiosis is a parenchymal lung disease that results from the accumulation of coal dust in the lungs and the consequent tissue reaction. To evaluate the role of various personal factors in pneumoconiosis and the significance of some serologic markers for assessing the disease activity related to pneumoconiosis, the Rheumatoid Factor(RF), ${\alpha}_1$-AT, C-Reactive Protein(CRP), ceruloplasmin and fibrinogen levels were measured. Method : All the patients were males, 45-76 years old, and the mean duration of coal dust exposure was 23.2 years. 51 patients were classified as having Simple Pneumoconiosis (SP), 59 had Progressive Massive Fibrosis (PMF). Fifty eight men with ages ranging from 26-70 years were used as normal controls. The serum RF and CRP were titrated using an Autochemistry analyzer (HITACHI 7150 : Japan) and the ${\alpha}_1$-AT and ceruloplasmin levels were measured using a Nephelometer (Behring Nephelometer : Germany) and the fibrinogen levels were estimated by using an Autoanalyzer for hematologic coagulation. Result : There was a higher RF level in the SP, and PMF groups than in the control groups but there was no statistical difference. The CRP, ${\alpha}_1$-AT, and ceruloplasmin levels were also higher in the SP, and PMF groups. However, the fibrinogen concentration was within the normal ranges in both the SP and PMF groups. Conclusion : The CWP (Ed note : Define CWP) patients had significantly higher CRP, ${\alpha}_1$-AT, and ceruloplasmin levels compared to the control group. It is believed that these serologic changes could be used as a marker of the disease activity.
Purpose: The purpose of this study was to determine the effects of foot reflexology on fatigue and insomnia in patients suffering from coal workers' pneumoconiosis. Method: This study was a quasi-experimental study of pre-test and post-test design in a non-equivalent control group. The subjects of this study consisted of both the experimental group of twenty-nine and the control group of thirty coal workers' pneumoconiosis patients. Data was collected from December 10, 2002 to February 15, 2003. Foot reflexology was performed for 60 minutes twice a week through five weeks in the experimental group, but none in the control group. To evaluate the effects of foot reflexology, the scores of fatigue and insomnia were measured before and after the experiment in both groups. Fatigue was evaluated by Fatigue Symptoms Inventory. Insomnia was measured with the visual analogue scale (VAS). Data of this experiment was analyzed by Chi-square test, t-test, unpaired t-test and Repeated Measures ANOVA with the SAS Program. Results: The scores of fatigue and insomnia decreased in the experimental group but not in the control group. There was a significant difference of fatigue and insomnia between the two groups. Conclusion: It is suggested that foot reflexology might have beneficial effects on reducing fatigue and insomnia in patients suffering from coal workers' pneumoconiosis, and can be recommended as a nursing intervention program for patients with coal workers' pneumoconiosis.
Purpose: Pneumoconiosis is a chronic lung disease in which the lungs become hardened due to the accumulation of fine mineral dust that has been breathed in at industrial sites. The lungs are unable to function properly, resulting in complications of several respiratory diseases. Methods: The subjects were 36 elderly adults (pneumoconiosis patients 18, healthy elderly 18) over the age of 65 years. The respiratory function was assessed using a spirometer to measure forced vital capacity (FVC), first second of forced expiration (FEV1), FEV1/FVC, and peak expiratory flow n(PEF). The static balance function test was determined by the center of mass (COM), ankle angle range, hip angle range, sway parameters, and reciprocal compensatory index (RCI). Cognitive function was measured by applying MoCA-k. Results: Comparison of respiration functions between the two groups showed statistically significant differences in FVC, FEV1, FEV1/FVC, and PEFs (p<0.05). Cognitive abilities showed statistically significant differences due to higher cognitive skills in the control group as compared to the patient group (p<0.05). Most results of the static balance capability measurements showed statistically significant differences between the groups (p<0.05). However, no statistically significant differences were obtained for COM AP Range, Angle AP Range, RCI ML Range, and AP Range (p>0.05). Conclusion: The results of the current study indicate that patients suffering from pneumoconiosis have less cognitive ability and balance function as compared to healthy elderly people. Therefore, we expect an improvement in the balance ability and cognitive function through systematic management and strengthening lung function training to improve the quality of life of pneumoconiosis patients.
Authors studied on the prevalence of 94 cases of pneumoconiosis who were found out through the screening test on 1,062 workers engaged in welding process at 36 shipyard in Pusan area from March 1st, 1986 to November 30th, 1986. The result were as follows; 1. Dust concentration was measured $4.49{\pm}0.54mg/m^3$ in the small scale shipyard while it was $6.25{\pm}1.08mg/m^3$ in the large one. 2. The prevalence of welder's lung was 8.9% (male:8.5%, female:12.0%) and this is consist of 4.9% suspected pneumoconiosis and 4.0% pneumoconiosis more than category 1/0. 3. The prevalence was significantly increased according to the duration of dust exposure(p<0.05), and it showed the increasing tendency by the age group. 4. The prevalence was significantly higher in the large scale shipyard than is the small scale one(p<0.01). 5. The type and shape of opacities were 71.4% of p type and 28.6% of q type, however no pneumoconiosis with r type observed in this study. 6. The main subjective symptoms were the sputum(29.8%), coughing(25.5%), shortness of breath(20.2%), fatique(6.4%), and chest pain(5.3%). In other hand, 57.4% of pneumoconiosis were asymptomatic. 7. The prevalence of pulmonary tuberculosis was radiologically 1.7% in all subjects including 5.3% in pneumoconiosis and 1.3% in no pneumoconiosis(p<0.01).

Background: Chest X-ray of coal workers' pneumoconiosis and shipyard welders' lung show similar and regular opacities mostly, it is very difficult that we distinguish the former from the latter by only chest X-ray. so we performed this study to understand the progression of the disease and to provide the disease by considering pulmonary function and other factors in proportion to the profusion of small regular opacities of chest X-ray in both groups. Method: 430 coal workers' pneumoconiosis were compared with 311 shipyard welders' lung by the number, the age, the duration of dust exposure, %vital capacity(%VC), %FEV1.0, the type of ventilatory impairment, the combined pulmonary disease according to the profusion of small regular opacities on the chest radiographs, which were classified into category 0/1, category 1, and category 2. Result: 1) the percent of category 2 in coal workers' pneumoconiosis was 54.4%. the percent of category 1, and category 2 in welders' lung were 60.0%, 7.4%. the progression to the category 2 was higher in coal workers' pneumoconiosis than in welders' lung 2) The mean age was higher in coal workers' pneumoconiosis than in welders' lung, significantly increased in proportion to the progresion of profusion of small regular opacities in both groups 3) There was no difference in the duration of dust exposure by category 1/0, but the duration of dust exposure by category 1, 2 in coal workers' pneumoconiosis more significantly increased than in welders' lung. the duration in the proportion of category 2 to category 0/1, 1 significantly increased in the proportion to small regular opacities in coal workers' pneumoconiosis. but there was no significant difference in the proportion to small regular opacities in duration of dust exposure in welders' lung. 4) There was no significant difference of mean values of %VC(%vital capacity)in both groups except for category 1. the mean values of %VC had no relationship between the progression of small regular opacities in both groups. 5) The mean values of %FEV1.0 decreased more significantly in coal workers' pneumoconiosis than in welders' lung except for category 0/1. and decreased significantly in proportion to the profusion of small regular opacities in coal workers' pneumoconiosis. there was significant difference of %FEV1.0 in the proportion of category 2 to category 011 in welders' lung. 6) there were no significant difference of %FEV1.0, %VC in smoker and nonsmoker in both groups. 7) With regard to the type of ventilation in both groups in coal workers' pneumoconiosis 21.4 percent of patients belonged to the restrictive type, 11.6 percent to the obstructive type, 5.6 percent to the combined type, but in welders' lung 21.8 percent to the restrictive type, 2.9 percent to the obstructive type, 1.9 percent to the combined type. 8. in the pulmonary disease, the incidence of the pulmonary tuberculosis was the most in both groups, was more in coal workers' pneumoconiosis than welders' lung Conclusion: If we compare coal workers' pneumoconiosis with electric arc welders'lung by considering pulmonary function and other factors in proportion to the profusion of chest X-ray, I think that we will have the better result in understanding the progression of the disease and provision of the disease in both groups.

Early recognition of coalescence in pneumoconiotic lesions is important because such coalescence is associated with the respiratory symptoms and deterioration of lung function. This complicated form of pneumoconiosis also has worse prognosis than does simple pneumoconiosis. High resolution computerized tomography(HRCT) provides significant additional information on the stage of the pneumoconiosis because it easily detects coalescence of nodules and emphysema that may not be apparent on the simple radiograph. The purpose of this study is to clarify the role of HRCT in detection of large opacity and the relationship of change between the coalescence of nodules or emphysema and lung function in dust exposed workers. 1. There was good correlation between the HRCT grade of pneumoconiosis and ILO category of profusion. 5(9.09%) in 55 study population had confluent nodule extending eve, two o, more cuts on HRCT. HRCT could identify the pneumoconiotic nodules which was not found by simple radiogrphy in 6 workers with category 0/0. 2. No significant difference was observed coalescence of nodules and emphysema by dust type. 3. There was no significant difference in pulmonary function according to ILO and HRCT classification. 4. HRCT could detect the significant reduction in $FEV_1,\;FEV_1/FVC$, PEFR, $FEF_{25},\;FEF_{50},\;and\;FEF_{75}$ and remarkable increase in RV and TLC in study persons with emphysema compared with non-emphysema group. 5. Emphysema was found more often in nodules-coalescence group than small opacity group by HRCT. We found that HRCT could easily detect areas of coalescence and complicated emphysema compared to plain chest X-ray. Also our data suggest that it is primarily the degree of emphysema rather than the degree of pneumoconiosis that determines the level of pulmonary function.
Digital radiography has been replacing rapidly the analog radiography for diagnosis of pneumoconiosis. The purpose of this study is to compare quality control of digital radiography (DR) and analog radiography (AR) for chest radiography in medical institution for pneumoconiosis (MIP) For the first time, we visited MIP to evaluate the chest radiography which is used for patients with pneumoconiosis, including equipment, technical parameters and reading environment. There were 33 institutions. DR and AR were installed in 24 and 9 institutions, respectively. Between DR and AR, we compared the radiological technique (RT), image quality (IQ) and reading environment (RE) to use the guideline published by Occupational Safety and Health Research Institute (OSHRI). The image quality was rated by two experienced chest radiologists for pneumoconiosis with certified from OSHRI. The chest radiography equipment was not significantly difference between AR and DR, but there were significantly difference in tube voltage and grid ratio used for chest radiography except to tube current, exposure time. Statistically, DR is significantly higher in RT(70.3 vs. 43.8, p = 0.009), RE(77.7 vs. 33.3, p = 0.004) than AR, but it's not significantly difference in IQ (65.6 vs. 52.8, p = 0.050). AR and DR in RT were passed 33.3%, 75.0% respectively (p = 0.044) and 44.4%, 79.2% (p = 0.090) in IQ and 44.4%, 91.7% (p = 0.009) in RE. In MIP, DR needs to replace AR in diagnosis of pneumoconiosis.
The digital dust indicator (Sibata P-5), one of the direct·reading instruments was evaluated for the respirable dust in the underground coal mine environments. As a reference, respirable dust was determined using three cyclones and/or impactors. All the tests were performed on aerosol in twenty underground coal mines. The coefficients of mass-relative concentration were $0.067{\pm}0.054$ (Mean$\pm$Standard deviation) (range: 0.006-0.172). The relationship between relative concentration and temperature was not significant statistically. Also, the relationship of relative concentration and relative humidity was not significant. Mass concentration and relative concentration were $5.31{\pm}5.22mg/m^3$ and $162{\pm}163$ CPM ($Mean{\pm}Standard$ deviation) respectively. The range of mass concentration was $1.22-22.69mg/m^3$; relative concentration 16-628 CPM. The relationship of mass concentration and relative concentration was not significant in these ranges.
Lead, iron, and zinc concentrations in whole blood were determined by atomic absorption spectrophotometry, using a simple one-step dilution procedure, which were measured in 3 groups, 98 officers unexposed to dust or to metal, 58 coal miners without pneumoconiosis, and 113 coal workers' pneumoconiosis (CWP) patients. The results were as follows : 1. The precisions (C. V.%) of lead, iron, and zinc in blood were $12.65{\pm}6.95%,\;1.47{\pm}1.25%\;and\;6.35{\pm}3.34%$, respectively. 2. Lead and zinc in blood showed the log-normal distribution unlike iron in blood which showed normal distribution. 3. Lead, iron, and zinc concentrations in blood of 3 groups were follows : There was significant difference of concentration for zinc in blood by groups statistically. 4. The difference of lead, iron, and zinc concentrations in blood was not significant (p>0.05) by profusion on chest radiographs.
