Usefulness of Awake Anesthesia in Flexor Tendon Surgery (굴곡건 수술에서 각성마취의 유용성)

  • Purpose: According to various medical publications, it is believed that epinephrine should not be injected in fingers. However numerous articles show the successful use of local anesthetic with epinephrine in the digits. Epinephrine-mixed lidocaine solution enables to maintain a bloodless field for operation and provides long duration of local anesthesia when patient was wide awake. Methods: From May 2009 to December 2009, ten patients underwent flexor tendon reconstruction with local anesthesia using epinephrine. No tourniquet was necessary. Before operation, all patients were injected with local anesthetics using 1% lidocaine 20 mL and 0.1% epinephrine 0.1 mL. Results: There was no case of digital necrosis nor gangrene in the epinephrine injection. All 10 patients actively could move the finger through a full range of motion. All procedures were performed without sedation nor tourniquet and we could obtain a good vision of operative field and patients were comfortable. The patient make his or her fingers move through a full range of active motion before the skin is closed. Phentolamine was not required to reverse the vasoconstriction in any patients. Conclusion: The assertation that epinephrine should not be injected into the fingers is clearly no longer valid. The epinephrine injection allowed the authors to adjust flexor tendon surgery without risks associated with general anesthesia. It also enables to ensure longer anesthetic duration and bloodless operative field, and prevent post operative complications. In case of flexor tendon surgery, the use of epinephrine injection is recommended because of the advantages of local anesthesia.

Development of a new injection mold structure for internal gears (새로운 내측기어 성형용 사출성형 금형구조의 개발)

  • As a rotating machine element, plastic gears are more and more widely used in such as industrial machine element, since plastic gear is lighter, higher wear-resistance, and higher vibration absorbing ability than metal gears. When operating plastic parts, tooth breakage and fatigue life shortened due to increasing number of applying load and tooth flank temperature rising, such that accuracy of plastic gears is divided from allowable range to cause vibration and noise. On this study, a internal plastic gears are developed which improved the filling balance molding process by a new injection mold structure. The new mold structure called HR3P(hot runner type 3plate mold). As the result from this studies, we obtained a very accurate roundness internal gears by using design of experiment.

  • Based on three-dimensional (3-D) FE limit analyses, this paper provides plastic limit, collapse and instability load solutions for pipe bends under combined pressure and in-plane bending. The plastic limit loads are determined from FE limit analyses based on elastic-perfectly plastic materials using the small geometry change option, and the FE limit analyses using the large geometry change option provide plastic collapse loads (using the twice-elastic-slope method) and instability loads. For the bending mode, both closing bending and opening bending are considered, and a wide range of parameters related to the bend geometry is considered. Based on the FE results, closed-form approximations of plastic limit and collapse load solutions for pipe bends under combined pressure and bending are proposed.

  • The authors performed rigid reconstruction using the sandwich technique for full-thickness chest wall defects by using two layers of acellular dermal matrix and bone cement. We assessed six patients who underwent chest wall reconstruction. Reconstruction was performed by sandwiching bone cement between two layers of acellular dermal matrix. In all patients, there was no defect of the overlying soft tissue, and primary closure was performed for external wounds. The average follow-up period was 4 years (range, 2-8 years). No major complications were noted. The sandwich technique can serve as an efficient and safe option for chest wall reconstruction.

Comparison of single-span plastic greenhouse in Korea and high tunnel in North America (우리나라 단동 비닐하우스와 북미지역 하이터널의 비교)

  • Structural characteristics for standard models of single-span plastic greenhouse in Korea and high tunnels in North America were analyzed, and comparative analysis for greenhouse environments measuring in Korean farmhouse and Rutgers high tunnel was carried out to find structural and environmental improvements of single-span plastic greenhouses that occupy most of Korean greenhouse. Widths of high tunnels are similar to single-span plastic greenhouses but their heights are high comparatively and their side heights are fairly higher than single-span plastic greenhouses specially. Rafters, which are main frames, section sizes of high tunnels are bigger and their intervals are wider than single-span plastic greenhouses. Relative bending resistances compared with representative Korean greenhouse were analyzed by 0.92 to 1.42 in single-span plastic greenhouses, and 1.38 to 2.96 in high tunnels. Frame ratios of single-span plastic greenhouses were 6.8 to 8.6%, and those of high tunnels were 5.5 to 8.7%. We analyzed air temperatures and solar radiations measured in single-span plastic greenhouse and high tunnel on clear days in late March. There were outside temperatures in generally similar range, and judging by rise of indoor temperatures, ventilation performance of high tunnel is more excellent than single-span plastic greenhouse. Solar radiations of two areas were no big difference but light transmittance of high tunnel was a little bit higher than single-span plastic greenhouse. Single-span plastic greenhouses are disadvantageous in environmental managements such as ventilation performance and light transmittance because distance between greenhouses is too narrow and length of greenhouse is too long compared to high tunnels. To get the environmental improvement effects as well as to increase the structural resistance of single-span plastic greenhouses are achievable by widening the width of greenhouse in possible range, widening the space between rafters, and enlarging the section size of rafters. Also, we need to secure enough distance between greenhouses and to restrict the length of greenhouse by maximum 50 m in order to improve the ventilation performance and the light transmittance.

Rate-dependent shearing response of Toyoura sand addressing influence of initial density and confinement: A visco-plastic constitutive approach

  • Rate-dependent mechanical response of sand, subjected to loading of medium to high strain rate range, is of interest for several civilian and military applications. Such rate-dependent response can vary significantly based on the initial density state of the sand, applied confining pressure, considered strain rate range, drainage condition and sand morphology. A numerical study has been carried out employing a recently proposed visco-plastic constitutive model to explore the rate-dependent mechanical behaviour of Toyoura sand under drained triaxial loading condition. The model parameters have been calibrated using the experimental data on Toyoura sand available in published literature. Under strain rates higher than a reference strain rate, the simulation results are found to be in good agreement with the experimentally observed characteristic shearing behaviour of sand, which includes increased shear strength, pronounced post-peak softening and suppressed compression. The rate-dependent response, subjected to intermediate strain rate range, has further been assessed in terms of enhancement of peak shear strength and peak friction angle over varying initial density and confining pressure. The simulation results indicate that the rate-induced strength increase is highest for the dense state and such strength enhancements remain nearly independent of the applied confinement level.

  • The primary objective of this paper is to trace the post-buckling behavior of space trusses in the inelastic range. Modeled member material behavior characteristics of struts in the post-critical elasto-plastic stage are determined and three types of idealized hardening rules are described. To perform this analysis, the present work is used the current stiffness parameter method combined with the cylinderical arc-length method. Numerical examples are presented to illustrate the accuracy and the application of the numerical solutions introduced above.

Tensile Deformation Behavior of Zr-based Bulk Metallic Glass Composite with Different Strain Rate (Zr계 벌크 비정질 복합재의 변형률 속도에 따른 인장 변형 거동 연구)

  • Tensile deformation behavior with different strain rate was investigated. $Zr_{56.2}Ti_{13.8}Nb_{5.0}Cu_{6.9}Ni_{5.6}Be_{12.5}$(bulk metallic glass alloy possessed crystal phase which was called $\beta$-phase of dendrite shape, mean size of $20{\sim}30{\mu}m$ and occupied 25% of the total volume) was used in this study. Maximum tensile strength was obtained as 1.74GPa at strain rate $10^2s^{-1}$ and minimum strength was found to be 1.6GPa at $10^{-1}s^{-1}$. And then, maximum plastic deformation occurred at the strain rate of $5{\times}10^{-2}s^{-1}$ and represented 1.75%, though minimum plastic deformation showed 0%. In the specific range of strain rate, relatively higher plastic deformation and lower ultimate tensile strength were found with lots of shear bands. The fractographical observation after tensile test indicated that vein like pattern on the fracture surface was well developed especially in the above range of strain rate.
