Successful treatment of a bowel fistula in the open abdomen by perforator flaps and an aponeurosis plug

  • In this report, we present a case of successful treatment of a bowel fistula in the open abdomen by perforator flaps and an aponeurosis plug. A 70-year-old man underwent total gastrectomy and developed anastomotic leakage and dehiscence of the abdominal wound a week later. He was dependent upon extracorporeal membrane oxygenation, continuous hemodiafiltration, and a respirator. Bowel fluids contaminated the open abdomen. Two months after the gastric operation, a plastic surgery team, in consultation with general surgeons, performed perforator flaps on both sides and constructed, as it were, a bridge of skin sealing the orifice of the fistula. The aponeurosis of the external oblique muscle was elevated with the flap to be used as a plug. The perforators of the flaps were identified on preoperative and intraoperative ultrasonography. This modality allowed us to locate the perforators precisely and to evaluate the perforators by assessing their diameters and performing a waveform analysis. The contamination decreased dramatically afterwards. The bare areas were gradually covered by skin grafts. The fistula was closed completely 18 days after the perforator flap. An ultrasound-guided perforator flap with an aponeurosis plug can be an option for patients suffering from an open abdomen with a bowel fistula.

  • Numerical Calculations for dimensionless pressure drop (friction factor times Reynolds number) have been obtained for fully developed laminar flow of MPL(Modified Power Law) fluid in isosceles triangle pipes. The solutions are valid for Pseudoplastic fluids over a wide range from Newtonian behavior at low shear rates through transition region to power law behavior at higher shear rates. The analysis identified a dimensionless shear rate parameter which for a given set of operating conditions specifies where in the shear rate range a particular system is operating, i.e., Newtonian, transition or power law region. The numerical calculation data of the dimensionless pressure drop for the Newtonian and power law regions are compared with previously published asymptotic results presenting within 0.16 % in Newtonian region and 2.98 % in power law region.

Study on Heat Transfer Characteristics according to Flowing Particles in a Cold Water Tube

  • This experiment was conducted to investigate the effect of particles on the heat transfer characteristics of fluids flowing in a cold water tube. Plastic beads with 3 different sizes were used as flowing particles with cold water. An experimental test section was composed of concentric double tubes having diameters of 25mm for the inner tube and 50mm for the outer tube. The materials for the inner and outer tubes are copper and PVC respectively. It was found that the particles enhanced the heat transfer coefficient by random and vortex motion in the fluid. Hence the heat transfer coefficients for the fluid with 2mm, 5mm and $2{\times}6mm$ particles were $7%{\sim}37%$ higher than the fluid without the particles.

Numerical simulation of dynamic segregation of self-consolidating concrete (SCC) in T-box set-up

  • A CFD software was used to simulate free surface flow of SCC in the T-Box test. In total, seven simulations were developed to study the effect of rheological parameters on the non-restricted flow performance of SCC in both horizontal and vertical directions. Different suspending fluids having five plastic viscosity values between 10 and 50 Pa.s, three yield stress values between 14 and 75 Pa, one density of $2500kg/m^3$, and one shear elasticity modulus of 100 Pa were considered for suspension of 178 spherical particles of 20-mm diameter and $2500kg/m^3$ density. The results of the simulations are found to correlate well to changes in rheological parameters of the suspending fluid. Plastic viscosity was shown to be the most dominant parameter affecting flowability and dynamic stability compared to the yield stress. A new approach was proposed to evaluate performability of SCC based on a trade-off between flowability and dynamic stability.

  • Electrorheological(HER) and magnetorheologica(MR) fluids have a unique ability to increase the dynamic yield stress of the fluid substantially when electric or magnetic field is applied. ER and MR fluid-based dampers are typically analyzed using Bingham-plastic shear flow analysis under Quasi-steady fully developed flow conditions. An alternative perspective, supported by measurements reported in the literature, is to allow for post-yield shear thinning and shear thickening. To model these, the constant post-yield plastic viscosity in Bingham model can be replaced with a power-law model dependent on shear strain rate that is known as the Herschel-Bulkley fluid model. The objective of this paper is to predict the damping forces analytically in a typical ER bypass damper for variable electric field, or yield stress using Herschel-Bulkley analysis.

  • 선박에 사용되는 TEMA 열교환기 전열관의 확관 공정에서 발생되는 탄소성 응력과 변형량의 거동을 유한요소법으로 해석하였다. 열교환기의 관판 구명의 홈 깊이와 롤러 익스팬더의 작용압력을 변화시켜 해석한 결과, 전열관의 관판 구멍의 홈 깊이가 커지면 탄소성 압축응력은 감소하였고, 롤러 익스팬더 압력이 높을수록 관판 구멍의 홈 모서리 부분의 탄소성 응력이 증가되었다.

  • In various manufacturing industries, an in-mold decoration (IMD) process for plastic objects is widely utilized because a film forming and an injection molding processes run simultaneously. In the present study, the deformation of polymer film and filling of resin in the IMD process were numerically investigated to evaluate the quality of the plastic object formed by the IMD process, which consists of thermoforming and injection molding processes. To obtain the initial shape of the polymer film during the injection molding process, the deformation of the polymer film in the thermoforming process was pre-formed using the vacuum conditions to attach the film to a cavity. Since the properties and deformation of polymer film are greatly affected by the behavior of polymer resin being injected into a mold cavity, numerical simulations for the injection molding and film forming were performed with one-way coupling method. The results showed that the injected resin could lead to the tearing of the polymer film in local regions near the corners. In order to verify the proposed numerical methodology, the numerical results of the deformation patterns printed on the initial polymer film were compared with the experimental data. The proposed methodology to couple film forming analysis with injection molding analysis can be used to predict the deformation of film in IMD process.

  • The steady shear flow properties of six kinds of commercial toothpastes were measured using a concentric cylinder type rheometer. In this paper, the shear rate and temperature dependencies of their flow behavior were investigated and the validity of the Casson and Herschel-Bulkley models was examined. Further, the flow properties over a wide temperature range were quantitatively evaluated by calculating the various material parameters. Main results obtained from this study can be summarized as follows: (1) Toothpastes are plastic fluids with a yield stress and their flow behavior shows shear-thinning characteristics. (2) With increasing temperature, the degree of shear-thinning becomes weaker and the Newtonian flow behavior occurs at a lower shear rate range. (3) The Herschel-Bulkley model is more effective than the Casson model in predicting their flow behavior. (4) As the temperature increases, the yield stress, plastic viscosity and consistency index become smaller, on the contrary, the flow behavior index becomes larger.

유리공장피판을 이용한 식도재건술 (Reconstruction of Esophagus by Free Jejunal Graft)

  • Despite of technical advances in surgery & other therapeutic modalities five-year survival rates in patients with carcinoma of hypopharynx have remained low. Many techniques have been used to create a structure capable of allowing the passage of food and fluids in an attempt to maintain the anatomy and physiology of the upper digestive system. The development of microsurgical techniques and the concept of mucocutaneous unit has brought about important changes in the reconstruction of cervical esophagus following tumor resection. The one-stage procedure using microvascular anastomosis of free jejunal graft provides physiologic reconstruction of cervical esophagus and has a low morbidity rate as well as a short recuperation time. With free jejunal graft, there is marked improvement in the quality of life and numerous advantages over the previous methods of reconstruction. Reconstruction of esophageal defect after resection of carcinomas of the hypopharynx, and cervical esophagus has traditionally been carried out with deltopectoral, or musculocutaneous skin-lined flaps. A second approach is to reconstruct the defect with the colon or stomach. A more ideal mettled is to repair these defects with mucosa-lined flaps. The authors experienced 35 cases of reconstruction of cervical esophagus after resection of carcinoma of the hypopharynx with free jejunal autograft and one case of secondary repair with radial forearm free flap after failure of initial free Jejunal autograft. Postoperative results were satisfactory in most patients and two patients expired in 8 days postoperatively because of carotid blow out by chronic inflammation.

  • Coronavirus is an enveloped virus with positive-sense single-stranded RNA. Coronavirus infection in humans mainly affects the upper respiratory tract and to a lesser extent the gastrointestinal tract. Clinical symptoms of coronavirus infections can range from relatively mild (similar to the common cold) to severe (bronchitis, pneumonia, and renal involvement). The disease caused by the 2019 novel coronavirus (2019-nCoV) was called Covid-19 by the World Health Organization in February 2020. Face-to-face communication and consistent exposure to body fluids such as blood and saliva predispose dental care workers at serious risk for 2019-nCoV infection. As demonstrated by the recent coronavirus outbreak, information is not enough. During dental practice, blood and saliva can be scattered. Accordingly, dental practice can be a potential risk for dental staff, and there is a high risk of cross-infection. This article addresses all information collected to date on the virus, in accordance with the guidelines of international health care institutions, and provides a comprehensive protocol for managing possible exposure to patients or those suspected of having coronavirus.
