Effects of Cheunhwasangamibang on serum glucose concentration and lipid composition in high fat diet induced diabetic rat (천화산가미방(天花散加味方)이 고지방식이에 의한 당뇨병 유발 쥐의 혈청 Glucose농도와 지질구성에 미치는 영향)

  • Effects of Cheunhwasangamibang on serum glucose and lipid composition were investigated in high fat diet induced diabetic rat. Plasma glucose and free fatty acids concentration showed a high reduction in Cheunhwasangamibang groups compared to those of control group and these values decreased to increasing Cheunhwasangamibang concentration. Plasma ${\beta}$-lipoprotein and triglyceride concentration showed a low values in Cheunhwasangamibang groups, however these values showed no significantly different in 4 ml and 6 ml treatment groups. Total cholesterol concentration showed no significantly different in control group and 2 ml Cheunhwasangamibang group, however these values in 4 ml and 6 ml treatment groups showed a reduction compared to those of control group. Plasma LDL-cholesterol concentration showed a reduction in Cheunhwasangamibang group and HDL-cholesterol showed no significantly different in all treatment groups.

Effect of Preoperative Intravenous Morphine on Postoperative Pain, Plasma Cortisol and Serum Glucose Levels (술전 Morphine 정주가 술후통증과 혈장 Cortisol 및 혈당치에 미치는 영향)

  • Background: Preoperative blocking of surgical nociceptive inputs may prevent sensitization of CNS and reduce postoperative pain. The stress responses to surgical trauma consist of increase in catabolic hormones and decrease in anabolic hormones. We studied whether preoperative intravenous morphine could affect postoperative pain and change plasma cortisol and serum glucose levels. Methods: Thirty eight patients undergoing total abdominal hysterectomy were randomly assigned to one of three groups. Control group (n=11) did not received intravenous morphine, preoperative group (n=13) received intravenous morphine (0.1 mg/kg as a bolus 10 min before operation and followed by 1.5 mg/hr for 10 hours), postoperative group (n=14) received the same doses and method of intravenous morphine of preoperative group postoperatively. Postoperative pain relief was provided with i.v. fentanyl through Patient-Controlled-Analgesia Pump. Postoperative visual analogue scores (VAS), analgesic requirement (first request time, total amounts used), side effects, plasma cortisol and serum glucose levels were compared. Results: VAS were different between control group and the other two goups, but were not different between preoperative and postoperative group. Total amounts of used fentanyl were not different among groups, but first request time were significantly delayed in the preoperative group compared with the other two groups ($66.2{\pm}33.9$ vs $39.0{\pm}15.4$ and $45.0{\pm}14.9$ min respectively, p<0.05). Plasma cortisol and serum glucose levels were not different among groups. Conclusions: Above dosage of preoperative and postoperative morphine has analgesic effect, but could not block surgical stress induced plasma cortisol and serum glucose increase.

Intermediary Metabolism of Plasma Acetic Acid, Glucose and Protein in Sheep Fed a Rice Straw-based Diet

  • The present study was conducted to determine plasma acetate, glucose and protein metabolism using dilution of isotopes [[1-$^{13}C$]Na acetate, [U-$^{13}C$]glucose and [1-$^{13}C$]leucine (Leu)] in sheep fed rice straw (Oriza japonica L.). Four sheep were assigned to either rice straw (RS-diet) or mixed hay (MH-diet) with a crossover design. Nitrogen (N) intake and N digestibility were lower (p = 0.002 and p = 0.02, respectively) for RS-diet than MH-diet, but N retention did not differ (p>0.10) between the diets. Concentrations of rumen acetate tended to be lower (p = 0.07), and propionate was higher (p = 0.02) for RS-diet than MH-diet. Concentrations of plasma lactate, non-esterified fatty acids, Leu and ${\alpha}$-ketoisocaproic acid did not differ (p>0.10) between the diets, but plasma glucose and urea concentrations were lower (p = 0.01 and p = 0.003, respectively) for RS-diet than MH-diet. Turnover rate of plasma acetate did not differ (p = 0.39) between the diets, and plasma glucose and Leu turnover rates were numerically lower (p = 0.15 and p = 0.14, respectively) for RS-diet than MH-diet. Whole body protein synthesis and degradation did not differ (p>0.10) between the diets. Thus it can be concluded that the intermediary metabolism of acetate, glucose and protein on rice straw is comparable to mixed hay in sheep.

Effects of Preoperative Epidural Block with Low Dose Bupivacaine and Morphine on Postoperative Pain, Plasma Cortisol and Serum Glucose in Total Abdominal Hysterectomy (복식전자궁적출술에서 술전 저용량 국소마취제와 Morphine을 이용한 경막외차단이 술후통증, 혈장 Cortisol, 혈당에 미치는 영향)

We studied whether preoperative low dose epidural bupivacaine and morphine could affect postoperative pain, changes plasma cortisol, and serum glucose. Methods: Thirty patients undergoing total abdominal hysterectomy were randomly assigned to one of three groups. General anesthesia was induced in all patients and after that, epidural blocks were done except the control group (n=10) patients. Preoperative block group (n=10) received 0.5% bupivacaine 50 mg and morphine 2 mg epidurally as a bolus before operation and followed by 0.1% bupivacaine $5\;mghr^{-1}$ and morphine $0.2\;mghr^{-1}$ for 10 hours. Postoperative block group (n=10) received the same doses of bupivacaine and morphine under the same method postoperatively. Postoperative pain relief was provided with i.v. fentanyl through Patient-Controlled-Analgesia Pump. Postoperative pain by visual analogue scores (VAS), analgesic requirement (first requirement time, total amounts used), side effects, plasma cortisol level and serum glucose level were compared. Results: Until postoperative 6 hrs, VAS of control group was higher than those of the epidural groups. No difference was observed in VAS between the two epidural groups. First analgesics requirement time and total amounts of used analgesics were not different between the two epidural groups, but first analgesic requirement time of preoperative block group was significantly prolonged compared with control group. Plasma cortisol and serum glucose levels were not different among groups. Conclusions: Low dose preoperative epidural bupivacaine and morphine could not reduce postoperative pain, plasma cortisol level and serum glucose level compared with postoperative block group.

Pharmacokinetic-Pharmacodynamic Modeling for the Relationship between Glucose-Lowering Effect and Plasma Concentration of Metformin in Volunteers

  • Metformin is a biguanide antihyperglycemic agent often used for the treatment of non-insulin dependent diabetics (NIDDM). In this study, the pharmacokinetics and pharmacodynamics of metformin were investigated in Korean healthy volunteers during a fasting state for over 10 h. In order to evaluate the amount of glucose-lowering effect of metformin, the plasma concentrations of glucose were measured for a period of 10 h followed by the administration of metformin (oral 500 mg) or placebo. In addition, the concentration of metformin in blood samples was determined by HPLC assay for the drug. All volunteers were consumed with 12 g of white sugar 10 minutes after drug intake to maintain initial plasma glucose concentration. The time courses of the plasma concentration of metformin and the glucose-lowering effect were analyzed by nonlinear regression analysis. The estimated $C_{max}$, $T_{max}$, $CL_{t}$/F (apparent clearance), V/F(apparent volume of distribution), and half-life of metformin were 1.42$\{pm}$0.07 $\mu\textrm{g}$/mL, 2.59$\{pm}$0.18h, 66.12$\{pm}$4.6 L/h, 26.63 L, and 1.54 h respectively. Since a significant counterclock-wise hysteresis was found for the metformin concentration in the plasma-effect relationship, indirect response model was used to evaluate pharmacodynamic parameters for metformin. The mean concentration at half-maximum inhibition $IC_{50}$, $k_{in}$, $k_{out}$ were 2.26 $\mu\textrm{g}$/mL, 83.26 $H^{-1}$, and 0.68 $H^{-1}$, respectively. Therefore, the pharmacokinetic-pharmacodynamic model may be useful in the description for the relationship between plasma concentration of metformin and its glucose-lowering effect.

Quercetin attenuates fasting and postprandial hyperglycemia in animal models of diabetes mellitus

  • The objective of this study was to investigate the hypoglycemic effects of quercetin (QE) in animal models of diabetes mellitus (DM). A starch solution (1 g/kg) with and without QE (100 mg/kg) or acarbose (40 mg/kg) was orally administered to streptozotocin (STZ)-induced diabetic rats after an overnight fast. Postprandial plasma glucose levels were measured and incremental areas under the response curve were calculated. To study the effects of chronic feeding of QE, five-week-old db/db mice were fed an AIN-93G diet, a diet containing QE at 0.08%, or a diet containing acarbose at 0.03% for 7 weeks after 1 week of adaptation. Plasma glucose and insulin, blood glycated hemoglobin, and maltase activity of the small intestine were measured. Oral administration of QE (100 mg/kg) or acarbose (40 mg/kg) to STZ-treated rats significantly decreased incremental plasma glucose levels 30-180 min after a single oral dose of starch and the area under the postprandial glucose response, compared with the control group. QE (0.08% of diet) or acarbose (0.03% of diet) offered to db/db mice significantly reduced both plasma glucose and blood glycated hemoglobin compared to controls without significant influence on plasma insulin. Small intestine maltase activities were significantly reduced by consumption of QE or acarbose. Thus, QE could be effective in controlling fasting and postprandial blood glucose levels in animal models of DM.

Effect of CHCl$_3$fraction of Alisma canaliculatum with selenium on the plasma glucose and lipids levels in streptozotocin-induced diabetic rats (택사 CHCl$_3$분획물과 Selenium 보충이 당뇨 흰쥐의 혈당과 지질함량에 미치는 영향)

  • The purpose of this study was to investigate the effect of chloroform(CHC1$_3$) fraction of Alisma canaliculatum All. Braun et Bouche(Ac) with selenium(Se) on the plasma glucose and lipid levels in streptozotocin(STZ)-induced diabetic rats. Male Sprague-Dawley rats were divided into five groups. The normal and diabetic rats were separated into four groups: the STZ-control group, the Ac group, the Ac-Se group and the Se group. Diabetes was induced in the male rats by an injection of STZ into the tail vein at a dose of 45 mg/kg. The CHCl$_3$fraction of Ac(250 mg/kg) was administered orally for 14 days. The supplementation was achieved with the AIN-93 recommended diet by adding 2 mg/kg diet of selenium as Na$_2$SeO$_3$. which was prepared freshly everyday. The body weight, hematocrit(Hct), glucose, insulin, cholesterol, HDL-cholesterol, triglyceride(TG) and free fatty acids(FFA) concentrations in plasma were measured. The aminotransferase activities were also analyzed. The changes of body weight in the experimental groups were not significantly different from that of the STZ-control group, but diabetes hyperphagia accompanied changes with body weight loss in Ac-Se group. The levels of Plasma cholesterol were not significantly different among the experimental groups. The concentrations of FFA in the Ac-Se group increased significantly compared with the STZ-control group. The effect of Se alone significantly increased aspartate aminotransferase activity and alanine aminotransferase activity. The results showed that the treatment of CHCl$_3$fraction of Ac in combination with Se has no synergistic effect. There was a tendency for the plasma glucose levels to decrease when Se was administered into diabetic rats. Supplementation of Se in diabetic rats did not elicit a significant increase in plasma insulin levels and exhibited hypotriglyceridemic effect.

Relationships of Obesity in Childhood to Plasma Lipids, Blood Pressure and Blood Glucose (아동기 비만이 혈장 지질, 혈압 및 혈당에 미치는 영향)

  • In order to investigate the relationships of obesity in childhood to plasma lipid, blood pressure and blood glucose concentration, we selected 21 subjects for the moderate obese group(MO), 9 for mild obese group(MI), and 19 for the control group(C) among children aged 10~12. While the level of plasma triglyceride and VLDL-cholesterol of the MO group was much higher than that of group C, a significantly lower percentage of HDL-cholesterol was found in the MO compared to the percentage found in group C. Also the level of the total cholesterol and LDL-cholesterol of the MI group as well as the MO was much higher than that of C. The elevated total-cholesterol level of the Mi group was due to increased LDL-cholesterol and that of the MO was due to increases in both VLDL-cholesterol and LDL-cholesterol. As the result of these differences, the atherogenic index of the MO was significantly higher than that of C. The incidence of hypercholesterolemia( 200mg/dl) of the MI and MO was 60.0% and 77.8% respectively, All of the physical parameters and indexes except height were positively correlated with plasma lipid levels, systolic blood pressure, and blood glucose concentration. The analyses of the correlated with plasma lipid levels, systolic blood pressure, and blood glucose concentration. The analyses of the correlation indicated that central fat to peripheral fat ratio and waist to thigh girth ratio seemed to be closely associated to plasma lipid levels and atherogenic index. The MO had significantly higher systolic blood pressure than C and significantly higher blood glucose concentration was found in both MI and MO than in C. These results confirmed that obesity in childhood may be relevant to chronic metabolic diseases such as abnormal lipid metabolism, atherosclerosis, high blood pressure and diabetes mellitus.

Molecular Association of Glucose Transporter in the Plasma Membrane of Rat Adipocyte

  • Molecular association of glucose transporters with the other proteins in the plasma membrane was assessed by gel electrophoresis and immunoblot techniques. Approximately $31.5{\pm}5.1%$ of GLUT-4, $64.8{\pm}2.7%$ of clathrin, 48.7% of total protein in the plasma membrane (PM) were found insoluble upon extraction with 1% Tx-100. Sodium dodecyl sulfate polyacrylamide gel electrophoresis revealed that the Tx-100 insoluble PM fraction contained about 4 major polypeptides with apparent molecular weight of above 200, 100-120, 80 and 30-35 KDa that were readily removed upon wash with a high pH buffer which is known to remove clathrin and 0.5 M Tris-buffer which is known to remove assembly proteins (AP). Immunoblotting of GLUT4 and clathrin against specific antibodies showed that GLUT-4 and clathrin were co-solubilized up to 84.6% and 82.7% respectively by wash with a high pH buffer and 1% Tx-100. When the membrane was pre-washed with a high pH buffer and 0.5 M Tris solution, GLUT4 and clathrin were not solubilized further suggesting that GLUT4 molecules are in molecular association with clathrin, AP and/or other extrinsic membrane proteins in plasma membrane and the formation of clathrin-coated structures might be involved in insulin stimulated glucose transporter translocation mechanism.

  • The blood glucose and plasma immunoreactive insulin(IRI) levels were measured glucose tolerance test in 7 healthy subjects and 6 patients with chronic liver diseases. The glucose tolerance was impaired in 5 of the 6 patients and normal in 1. Plasma IRI responses were markedly increased and delayed in all patients, suggesting insulin resistance. Patients with more glucose intolerance showed less increase in plasma IRI than the group with less intolerance. It is suggested that some insulin antagonists may decrease the peripheral insulin sensitivity and stimulate compensatory hyperactivity of pancreatic islets. If the compensatory hyperactivity is inadequate due to genetic predisposition to diabetes mellitus or exhaustion of ${\beta}$-cells of pancreatic islsts, the glucose intolerance and overt diabetes mellitus may ensue.
