Building an Ontology for Structured Data Entry of Signs and Symptoms in Oriental Medicine (Protege를 이용한 한의학의 구조화된 증상 입력을 위한 온톨로지 개발)

  • To obtain both of the fast and complete data entry and the acquisition of reusable data in a Computer-based Patient Record system (CPR), we are building the ontology that is used by the entry supporting agents. Our application domain is Traditional Chinese Medicine. As the tool for the implementation, we used protege 2000 which is ontology building tool and provides frame knowledge representation language. In this paper, the construction methodology of our ontology is reported.

  • Lee je-ma emphasis human constitution and write DongYeuSuSebowon(東醫壽世保元). He describe Sasang constitution diseases with Treatise on Febrile Diseases(傷寒論). Exogenous febrile disease(傷寒) is a pyrexia disease. Therefor this paper is focus on the pyrexia at Soyinin(少陰人) among DongYeuSuSebowon. Soyinin(少陰人) is consider that body water loss is more important than fever in pyrexia disease. And body water loss is happens easily at exothermic reaction (sweating, diarrhea) and sequela of Purgative Prescription(瀉下法), Inducing Diaphoresis(發汗法), Diuresis(利小便法).

Clinical Report of Two Patients with Abnormal Uterine Bleeding Treated by Kungkwikyoae-tang (궁귀교애탕(芎歸膠艾湯) 투여로 호전된 비정상 자궁출혈환자 2례에 대한 증례보고)

  • The purpose of this study is to report the hemostatic effects of Kungkwikyoae-tang for abnormal uterine bleeding. Kungkwikyoae-tang has been known as a useful herbal medication for uterine bleeding. We treated patients who had been suffering from abnormal uterine bleeding with herb-medication, acupuncture therapy, pharmacopuncture therapy and moxa therapy. We prescribed Kungkwikyoae-tang three times a day. The improvement of symptoms were measured by checking patient bleeding volume, frequency and other physical signs. After Kungkwikyoae-tang was given, abnormal uterine bleeding volume and frequency was reduced and other physical symptoms were improved. This case study shows that Kungkwikyoae-tang has a hemostatic effects against the abnormal uterine bleeding.

  • The Stagnation Syndrome of Qi is the depressed and suppressed mental state caused by anxiety, unaccomplished desire, maintained thought, and emotional problems, resulting in a concomitant physiological somatic dysfunction. Although blood test, urine test, EKG, and gastric endoscopy showed normal finding. a patient complained of a group of symptoms, so we considered it as an unidentified clinical syndrome. This syndrome could be interpreted as the disconnection of Fire (Heart) and Water (Kidney) in Oriental medicine, and treated with the therapeutic method of 'Ascending Water-Descending Fire'. After the application of Gyogam-dan and Ganggi-tang for 18 days, symptoms and signs improved.

  • The purpose of this case study is to report the effect of oriental medical treatment on chronic tension-type headache. Despite the patient was treated by cervical nerve block for headache, headache was not improved. We diagnosed chronic tension-type headache according to ICHD-II(The International Classification of Headache Disorders) and Qi deficiency, dampness and phlegm by oriental differential diagnosis of symptom and signs. We applied herbal medicine, acupuncture, moxibustion and cupping therapy for hospitalization(7 days). Oriental medical treatment may have effective results in treating chronic tension-type headache that was not improved by cervical nerve block treatment. But this is a single case study, so further case-series research should be compiled.

  • Cushing syndrome (CS) is rare in pregnancy, and few cases have been reported to date. Women with untreated CS rarely become pregnant because of the ovulatory dysfunction induced by hypercortisolism. It is difficult to diagnose CS in pregnancy because of its very low incidence, the overlap between the clinical signs of hypercortisolism and the physiological changes that occur during pregnancy and the changes in hypothalamus-pituitary-adrenal axis activity that occur during pregnancy and limit the value of standard diagnostic testing. However, CS in pregnancy is associated with poor maternal and fetal outcomes; therefore, its early diagnosis and treatment are important. Here, we report two patients with CS that was not diagnosed during pregnancy, in whom maternal and fetal morbidity developed because of hypercortisolism.

Antioxidant Activity and Safety Evaluation of Juice Containing Protaetia brevitarsis (흰점박이꽃무지(Protaetia brevitarsis) 함유 음료의 in vitro 항산화 관련 생리활성효능 및 안전성 검증)

  • The purpose of this study was to assess the antioxidant activity of vegetable extracts (pumpkin, aloe, and artichoke) containing Protaetia brevitarsis (PB) and the clinical and pathological changes in ICR mice after a single oral administration. The total polyphenol (TP) and 2,2-diphenyl-1-picrylhydrazyl (DPPH) radical scavenging ability, total radical trapping antioxidant potential (TRAP), oxygen radical absorbance activity (ORAC), and single cell gel electrophoresis assay were done to measure their antioxidant activities. The effect of vegetable extracts containing PB in TP and the ORAC value was significantly higher than those without PB. In addition, all extracts had effective $DPPH{\cdot}$ scavenging and $ABTS{\cdot}+$ scavenging activities. The protective effect of vegetable extracts with/without PB on $H_2O_2$-induced DNA damage was found. In a single-dose toxicity study, mortality, body weight, physiological signs, and biochemical analysis were analyzed. Seventy mice were randomly assigned to 7 experimental groups and were administered three vegetable extracts with and without PB (2 g/kg). A full 14 days after administration, no mice mortality was observed in any group. Body weight, physiological signs, and biochemical analysis were never significantly different from those of the control group. Taken together, these findings indicate that vegetable extracts containing PB with antioxidant activities and safety could be applied as medicinal and edible resources in an industrial area.

Physiologic changes on the rescuer and efficiency of CPR in the increased chest compression (흉부압박의 횟수증가가 구조자에게 미치는 생리학적 변화와 심폐소생술 정확도에 미치는 영향)

  • Purpose : This study was designed to examine physiological changes in the body of rescuers conduct CPR according to the 2005 new guideline from American Heart Association. The ratio of artificial respiration has changed from 15 : 2 into 30 : 2 in 2005. The researcher tried to know the correlation between the physiological changes and the accuracy of CPR. Method : The examinees of this study were 26 students (Dept. of Emergency Medical Service). After the training, participants conducted 10 minute CPR and soon after the CPR, their vital signs were checked, and lactic acid and concentration of ammonia were analysed from their blood samples. Questionnaires to ask their subjective fatigue level were filled out after blood samples and 10 minute - CPR was performed. Results : 1) After the CPR, concentrations of ammonia were $149.71{\mu}{\ell}/d{\ell}$ and $162.17{\mu}{\ell}/d{\ell}$ in 15 : 2 and 30 : 2, respectively. The number was higher in 30 : 2 but it wan not statistically meaningful (p = .493). Log value of lactic acid was a little higher in 30 : 2 with 42 log($mmol/{\ell}$) and 54 log($mmol/{\ell}$) in 15 : 2 and 30 : 2, respectively but it was not statistically meaningful (p = .113). 2) Blood pressure in 15 : 2 and 30 : 2 were 118.50 mmHg and 125.08 mmHg while pulse in two different cases were 96.14 and 97.25, showing no statistically significant differences (blood pressure : p = .155, pulse : p = .841). 3) Subjective fatigue was a bit high in 30 : 2 with 5.93 and 6.92 points in 15 : 2 and 30 : 2 respectively but it was not statistically meaningful (p = .142). 4) In the 10 minute CPR, respiration accuracy was 96.21% in 15 : 2 and 94.79% in 30 : 2. There was no statistical significances between the two(p = .225). In the meanwhile, chest compression accuracy was 92.57% in 15 : 2 and 91.83% in 30 : 2. From the beginning to the end of chest compression, there showed no difference(p = .425). the type of CPR did not influence upon the accuracy of chest compression(p = .756). Conclusion : In the CPR conducted by skilled rescuers for 10 minutes, there were no statistically meaningful differences between 15 : 2 and 30 : 2 in the concentration of fatigue element in a blood, subjective fatigue, vital signs and accuracy of CPR. Therefore, 30 : 2 CPR recommended by American Heart Association need to be recommended and performed in scene size up.

  • Stroke is the second leading cause of death in Korea and the leading cause of long-term disability. To develop the Korean Standard Differentiation of the Symptoms and signs, we investigate symptoms frequency and professional's symptoms weight in 248 stroke patients within 1 month of onset. We take professional advice by e-mail.

Acute Oral Toxicity of Taeumjowi-tang in SD Rats (SD계 흰쥐에 대한 태음조위탕(太陰調胃湯)의 급성 경구 독성 시험 연구)

  • The objective of this study was to evaluate the acute toxicity of Taeumjowi-tang in rats. SPF Sprague-Dawley male and female rats were administered orally with Taeumjowi-tang extract of 2,500 mg/kg(low dosage group), and 5,000 mg/kg(high dosage group). We daily examined number of deaths, clinical signs, body weights and gross findings for 14 days. No dead animal and no significant changes of body weights were found during the experimental period. In addition, no differences were found between control and treated groups in clinical signs, hematology, serum biochemistry, and other findings. In conclusion, Taeumjowi-tang extract did not show any toxic effects, and oral LD50 value was over 5,000 mg/kg in SD rats.
