Varicose Veins and the Diagnosis of Chronic Venous Disease in the Lower Extremities

  • Varicose veins usually present in the superficial veins of the lower extremities and are one of the main clinical presentations of chronic venous disease (CVD). Patients' symptoms may vary according to the pathophysiology, location, and severity of CVD. The prevalence of CVD in Korea has been increasing gradually. However, due to its broad clinical spectrum and the subjective nature of its diagnosis using ultrasound, discrepancies in diagnostic and treatment quality may exist among treating physicians. There have been recent efforts to improve the quality of the diagnosis and treatment of varicose veins in Korea by standardizing the diagnostic criteria and the indications for treatment. This study is a comprehensive review of the clinical manifestations and diagnostic criteria of CVD based on the most recent international and domestic guidelines and reports.

  • Depressive disorder is one of well-known major public health problems in Korea. The socioeconomic burden of depression is growing bigger and bigger. Therefore, the Korean government decided to establish the clinical research center for depression for better care of patients with depression. One of the major task of the center is the development of Korean clinical practice guideline for depressive disorders. This guideline should reflect the clinical situation in Korea and meet Korean physicians' needs. In addition, it should be feasible and easy to use. We reviewed the present status of the guideline development in Korea and other countries, and introduces the scope, method, and goals of Korean clinical practice guideline for depression.

Factors Influencing Pain Medication Preference for Breakthrough Cancer Patients and Their Application to Treatments: Survey on Physicians (돌발성 암성 통증 약물 선택 요인과 사용 경험: 의사 대상 설문조사)

  • Purpose: The purpose of this study was to assess the factors influencing the rescue medication decisions for breakthrough cancer patients and evaluate treatments using the factors. Methods: Based on the results of an online survey conducted by the Korean Society of Hospice and Palliative Care from September 2014 through December 2014, we assessed the level of agreement on nine factors influencing rescue medication preference. The same factors were used to evaluate oral transmucosal fentanyl lozenge, oral oxycodone and intravenous morphine. Results: Agreed by 77 physicians, a rapid onset of action was the most important factor for their decision of rescue medication. Other important factors were easy administration, strong efficacy, predictable efficacy and less adverse effects. Participants agreed that intravenous morphine produced a rapid onset of action and strong and predictable efficacy and cited difficulty of administration and adverse effects as negative factors. Oral oxycodone was desirable in terms of easy administration and less adverse effects. However, its onset of action was slower than intravenous morphine. While many agreed to easy administration of oral transmucosal fentanyl lozenge, the level of agreement was low for strength and predictability of its efficacy, long-term durability and sleep improvement. Conclusion: Rapid onset of action is one of the important factors that influence physicians' selection of rescue medication. Physicians' assessment of rescue medication differed by medication.

Measuring the Seriousness of Adverse Drug Reactions : Comparisons between Korean Experts and WHO Working Group Members (의약품 부작용의 심각도 조사 비교분석)

  • This study measures seriousness of adverse drug reactions (ADRs) among Korean physicians and pharmacists using two times surveys based on Delphi technique. Each participant scored 20 ADR terms on a scale of one to ten (10 being the most serious). We repeated the exercise for the 49 first survey respondents and 32 re-evaluated score. We compared the results of our survey with those of WHO CIOMS (Council for International Organization of Medical Sciences) working groups members conducted in 1995. The overall mean ADR seriousness score was 6.49 for Koreans and 5.12 for WHO CIOMS members, presenting Korean experts perceived more seriously for each ADR. Mean score changes for the same respondents showed similar trends regardless of access to the first survey results. There were no statistically significant score differences between the physicians and the pharmacists. The high consensus of seriousness for each ADR between the Korean experts and the WHO CIOMS members implies that the similar results are reproducible, suggesting the possibility of developing standardized tools for measuring the seriousness of individual ADRs in the future.

Effects of Health Care Expenditure on the Infant Mortality Rate and Life Expectancy at Birth in Korea

  • This study examines whether the infant mortality rate and life expectancy at birth are affected by health care expenditure in Korea. It can be provisionally concluded that the infant mortality rate tends to be affected by the health system itself in the long-run, whereas life expectancy at birth is immediately affected by health-related facilities such as the number of physicians and number of hospital beds in the short-run. Therefore, the health-related system should be well established to improve the infant mortality rate. On the contrary, physical capital such as life-prolonging medical technologies has to be accumulated to improve life expectancy at birth.

  • Most of all countries have a governmental system for regulating the certification of nursing. In Korea, certification of advanced practice nurse(APN) is statuary regulation under supervision of government. In this article, the writers discuss the legal characters and effects of certification and regulatory body for certification and suggest the new regulatory system. The advanced practitioner may be recognize "new" health provider in the future. The nursing specialist have right to use title and practice expanded role. It's different things with physicians certification. The regulatory body is important because certification protected the title and empower APN authorize of expanded role. The certification of nursing is closely connected with public interest. The Korea government delegate power of national examination to private institute. To improve utility and publicity of certification system, we suggest that statutory empower the certification authority to public institute which composed with nursing professionals.

  • Single-balloon kyphoplasty via an extrapedicular approach has been reported to be effective because it requires less time than conventional two-balloon kyphoplasty and has comparable therapeutic efficacy. However, single-balloon kyphoplasty is not popular because the extrapedicular approach is believed to be complicated and unsuitable for the thoracolumbar and lumbar spine. The authors describe a standardized surgical technique that utilizes a far-lateral extrapedicular approach for single-balloon kyphoplasty, which can be performed in any part of the spine by physicians without substantial difficulty.

  • Purpose : Asian Quality Assurance Survey program(AQuAS) is and activity of Asian Network for Clinical Laboratory Standardization and Harmonization(ANCLS) which had begun her first colloquium in 1999 in Jakarta, Indonesia by laboratory physicians led by Professors Noriyuki Tatsumi and Yoshinori Funahara with themes of standardization and harmonization of clinical laboratories in Asia. Methods : AQuAS had begun its first survey in July. 2001 and done November and March, three times per year in the fields of Chemistry, Hematology. Coagulation and Urinalysis. As of June 2006 the 15th survey have been completed. Korean Association of Health Promotion(KAHP) had participated in it since July, 2004 in the fields of chemistry, hematology and urinalysis.ゝ⨀

  • Bioterrorism events have worldwide impacts, not only in terms of security and public health policy, but also in other related sectors. Many countries, including Korea, have set up new administrative and operational structures and adapted their preparedness and response plans in order to deal with new kinds of threats. Korea has dual surveillance systems for the early detection of bioterrorism. The first is syndromic surveillance that typically monitors non-specific clinical information that may indicate possible bioterrorism-associated diseases before specific diagnoses are made. The other is infectious disease specialist network that diagnoses and responds to specific illnesses caused by intentional release of biologic agents. Infectious disease physicians, clinical microbiologists, and infection control professionals play critical and complementary roles in these networks. Infectious disease specialists should develop practical and realistic response plans for their institutions in partnership with local and state health departments, in preparation for a real or suspected bioterrorism attack.

Development of Quantitative Diagnostic Technique for Low-Back Pain Patients via Three Dimensional Dynamic Motion Analysis (3차원 동작분석에 의한 요통환자의 정량적 진단기법 개발에 관한 연구)

  • Dynamic motion difference between normal subjects and low-back pain (LBP) patients has been investigated in terms of kinematic variables such as range of motion, velocity and acceleration of the back and hip. Ten healthy subjects and ten LBP patients were recruited in this study. Electro-goniometer such as Lumbar Motion Monitor and Hip Monitor have been used for quantitative measurement of the trunk motion during repetitive flexion and extension for ten seconds. Results indicated that the velocity and acceleration of the back and hip were important parameters to quantitatively identify LBP patients. The consistency of cyclic trunk motion and the relationship between the back and hip were measured in terms of Variance Ratio and Phase Angle in order to accurately assess the motion characteristics of LBP patients. In particular, the hip motion has been proven to be a very important factor in describing the kinematics of damaged lower back. The functional evaluation technique suggested in this study will be a tool to assist physicians for an accurate diagnosis and timely rehabilitation along with current image diagnosis techniques.
